Abstract

Human papillomavirus (HPV) is a recently identified causative agent for a subset of head and neck cancers, primarily in the oropharynx, and is largely responsible for the rising worldwide incidence of oropharyngeal cancer (OPC). Patients with HPV-positive OPC have distinct risk factor profiles and generally have a better prognosis than patients with traditional, HPV-negative, head and neck cancer. Concurrent chemotherapy and radiation is a widely accepted primary treatment modality for many patients with HPV-positive OPC. However, recent advances in surgical modalities, including transoral laser and robotic surgery, have led to the reemergence of primary surgical treatment for HPV-positive patients. Clinical trials are under way to determine optimal treatment strategies for the growing subset of patients with HPV-positive OPC. Similarly, identifying those patients with HPV-positive cancer who are at risk for recurrence and poor survival is critical in order to tailor individual treatment regimens and avoid potential undertreatment.

Abstract

We report on the identification of the optical/IR counterpart of the 29.5-s transient X-ray pulsar GS 1843+009. We re-analysed an archival ROSAT HRI observation of GS 1843+009, obtaining a new refined position. The optical and IR follow-up observations carried out for the new error circle allowed us to find a relatively faint (V=20.9) and variable early type reddened star (V-R=2.1). The optical spectra show the Balmer and Paschen series in emission, while the IR observations confirm the presence of a flux excess (H=13.2, J-H=0.54), suggesting that the star is surrounded by a circumstellar envelope. Spectroscopic and photometric data together indicate a B0-B2 IV-Ve spectral-type star located at a distance of > 10kpc confirming the Be-star/X-ray binary nature of GS 1843+009., Comment: 6 pages (3 figures included). Abstract

OAO1657-415 is only the seventh eclipsing X-ray pulsar known and therefore has the potential to yield only the seventh mass of a neutron star in an X-ray binary. Here we report photometric and spectroscopic observations of candidates for the optical counterpart to the system and identify a B6V or B5III star as a possible companion to the neutron star. 21. Regulation of the erythropoietin gene

Abstract

Erythropoietin (Epo) is a circulating hormone which regulates red blood cell production, and hence oxygen-carrying capacity. Three striking features of Epo gene expression are that it shows high level induction in response to hypoxia, that it is tightly tissue-restricted and that it is deficient in most forms of renal damage. Each of these aspects has been investigated in this thesis. Chapter 3 concerns the hypoxic-sensing system which induces Epo gene expression via an enhancer element in cultured hepatoma cells. Transient transfection was used to examine a range of cell-lines, which did not produce Epo, for the presence of this oxygen-sensing system. It was demonstrated that such a system is widespread in mammalian cells. It is probable that it is involved in regulating the expression of other genes in response to hypoxia. This widespread oxygen-sensing mechanism contrasts with the tightly tissue-restricted expression of the Epo gene. Transgenic experiments described in Chapters 4 and 5 provide data concerning the DNA sequences involved in this tissue specificity. Sequence from the mouse Epo locus was then used to direct expression of a marker protein, SV40 T antigen, to Epo-expressing cells. This led to identification of the Epo-producing cells in the kidney (Chapter 5) and liver (Chapter 6). In both organs a fibroblast-like population expresses the gene; the Ito cells in the liver and the Type 1 interstitial cells in the kidney. In the liver hepatocytes also produce Epo. Further studies of Epo gene expression would be facilitated by the availability of Epo-producing cell lines. SV40 T antigen is a viral oncogene, and expression of this protein was used as a proliferative stimulus in vivo. The effects of this, and attempts to isolate these cells form the subject of Chapter 7. Chapter 7 also describes experiments in which renal injury was combined with visualisation of individual Epo-producing cells. A homologous recombination at the Epo locus resulted in an allele which expressed the hormone at a greatly reduced level. The phenotype of animals homozygous for this allele is described in Chapter 8. Experiments designed to produce further modification of the Epo locus by this method, and to assess its utility in modifying the mouse genome are also described.

Abstract

Studying tissue composition and function in non-human primates (NHPs) is crucial to understand the nature of our own species. Here we present a large-scale cell transcriptomic atlas that encompasses over 1 million cells from 45 tissues of the adult NHP Macaca fascicularis. This dataset provides a vast annotated resource to study a species phylogenetically close to humans. To demonstrate the utility of the atlas, we have reconstructed the cell–cell interaction networks that drive Wnt signalling across the body, mapped the distribution of receptors and co-receptors for viruses causing human infectious diseases, and intersected our data with human genetic disease orthologues to establish potential clinical associations. 27. Safety and Durability of Accelerated Infliximab Dosing Strategies in Pediatric IBD

Abstract

Infliximab (IFX) is commonly used to treat children with inflammatory bowel disease (IBD). We previously reported that patients with extensive disease started on IFX at a dose of 10?mg/kg had greater treatment durability at year one. The aim of this follow-up study is to assess the long-term safety and durability of this dosing strategy in pediatric IBD. We performed a retrospective single-center study of pediatric IBD patients started on IFX over a 10-year period. Two hundred ninety-one patients were included (mean age = 12.61, 38% female) with a follow-up range of 0.1–9.7 years from IFX induction. One hundred fifty-five (53%) were started at a dose of 10?mg/kg. Only 35 patients (12%) discontinued IFX. The median duration of treatment was 2.9 years. Patients with ulcerative colitis (P= 0.01) and patients with extensive disease (P= 0.01) had lower durability, despite a higher starting dose of IFX (P= 0.03). Adverse events (AEs) were observed to occur at a rate of 234 per 1000 patient-years. Patients with a higher serum IFX trough level (=20 µg/mL) had a higher rate of AEs (P= 0.01). Use of combination therapy had no impact on risk of AEs (P= 0.78). We observed an excellent IFX treatment durability, with only 12% of patients discontinuing therapy over the observed timeframe. The overall rate of AEs was low, the majority being infusion reactions and dermatologic conditions. Higher IFX dose and serum trough level> 20 µg/mL were associated with higher risk of AEs, the majority being mild and not resulting in cessation of therapy.
