The objective of this study was to identify how value assessment frameworks may need to be adapted to measure the value to society of DMD therapies.Patients and Methods: Three stakeholder groups (6 patient advocates, 4 clinicians, 3 health economists; N = 13) participated in semi-structured interviews around the International Society for Pharmacoeconomics and Outcomes Research’s Value Flower, which includes elements to consider within value assessments of healthcare technologies.Results: All stakeholders agreed that traditional value assessment frameworks based on the quality-adjusted life year (QALY) are narrow and will undervalue new DMD therapies. All stakeholders expressed some level of concern that using the QALY as a key metric of value discriminates against patients with severe progressive diseases and disabilities. Some stakeholders saw value in using the QALY for cross-disease comparisons in resource-constrained environments if the methodology was appropriate. All stakeholders recommended considering additional elements of value in decision-making around new DMD therapies. These elements reflect: economic and humanistic costs incurred by patients, caregivers, and families with Duchenne, such as indirect out-of-pocket costs, lost productivity, and family spillovers; meaningful attributes for individuals with disabilities and high unmet need, such as severity of disease, value of hope, and real option value; and factors that contribute to improvements in population health, such as insurance value, equity, and scientific spillovers.Conclusion: These findings highlight the need to expand traditional value assessment frameworks and take a holistic approach that incorporates the perspectives of individuals with Duchenne, caregivers, clinicians, and health economists when assessing the societal value of new DMD therapies. Broadening value assessment will prevent restricted or delayed access to therapies for individuals with Duchenne.Keywords: value assessment framework, qualitative research, rare diseases, ISPOR value flower

4. Thermal design and on-orbit performance of the ECOSTRESS instrument

Abstract

The ECOSystem Spaceborne Thermal Radiometer Experiment on Space Station (ECOSTRESS) instrument, launched on June 29, 2018, has completed nine months of operation in space. ECOSTRESS is a multispectral thermal infrared imaging radiometer and its primary mission is to investigate and understand how climate change affects water and carbon usage on Earth. The thermal control subsystem consists of a combination of active and passive components to maintain instrument components within the allowable flight temperature limits. Its focal plane detector is cooled to 65K by a pair of mechanical cryocoolers and a third mechanical cryocooler cools an intermediate cold shield to 130K. The heat dissipation generated by cryocoolers and electronics, is removed through non-planar cold plates and tube-on plate heat exchanger, which are cooled by a circulating fluid -EF module. The heat collected by the JEM-EF fluid is exchanged with a fluid loop that circulates through radiators located on the exterior of the Space Station. This paper provides an overview of the thermal and cryogenic system design, thermal analysis results and reviews the on-orbit thermal performance.

Abstract

BACKGROUND: The cholesteryl ester transfer protein inhibitor evacetrapib substantially raises the high-density lipoprotein (HDL) cholesterol level, reduces the low-density lipoprotein (LDL) cholesterol level, and enhances cellular cholesterol efflux capacity. We sought to determine the effect of evacetrapib on major adverse cardiovascular outcomes in patients with high-risk vascular disease. METHODS: In a multicenter, randomized, double-blind, placebo-controlled phase 3 trial, we enrolled 12,092 patients who had at least one of the following conditions: an acute coronary syndrome within the previous 30 to 365 days, cerebrovascular atherosclerotic disease, peripheral vascular arterial disease, or diabetes mellitus with coronary artery disease. Patients were randomly assigned to receive either evacetrapib at a dose of 130 mg or matching placebo, administered daily, in addition to standard medical therapy. The primary efficacy end point was the first occurrence of any component of the composite of death from cardiovascular causes, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina. RESULTS: At 3 months, a 31.1% decrease in the mean LDL cholesterol level was observed with evacetrapib versus a 6.0% increase with placebo, and a 133.2% increase in the mean HDL cholesterol level was seen with evacetrapib versus a 1.6% increase with placebo. After 1363 of the planned 1670 primary end-point events had occurred, the data and safety monitoring board recommended that the trial be terminated early because of a lack of efficacy. After a median of 26 months of evacetrapib or placebo, a primary end-point event occurred in 12.9% of the patients in the evacetrapib group and in 12.8% of those in the placebo group (hazard ratio, 1.01; 95% confidence interval, 0.91 to 1.11; P=0.91). CONCLUSIONS: Although the cholesteryl ester transfer protein inhibitor evacetrapib had favorable effects on established lipid biomarkers, treatment with evacetrapib did not result in a lower rate of cardiovascular events than placebo among patients with high-risk vascular disease. Abstract

Organizations that regulate civil engineering have been pressing for integration of 'global responsibility' into higher education curricula since around 2006, with a goal of achieving environmental sustainability and social justice. In an effort led by the American Society of Civil Engineers (ASCE, 2007, 2009), a global vision for civil engineering was identified. Within the UK, the Institution of Civil Engineers (ICE) has been leading the way alongside non-governmental organizations (Bourn & Neal, 2008). Via the in-progress study reported here, a UK-based research team is now studying the effects of ACSE and ICE initiatives. The team seeks to benchmark how global responsibility is perceived and enacted in civil engineering in the UK today and how engineering graduates have learned about and experienced globally responsible decision-making. Findings will hold value for the global community, as achieving sustainability is crucial to humanity, and indeed all life on Earth.

Abstract

Objective Arhalofenate is a novel antiinflammatory uricosuric agent. The objective of this study was to evaluate its antiflare activity in patients with gout. Methods This was a 12-week, randomized, double-blind, controlled phase IIb study. Eligible patients had had ≥3 flares of gout during the previous year, had discontinued urate-lowering therapy and colchicine, and had a serum uric acid (UA) level of 7.5–12 mg/dl. Patients were randomly assigned at a 2:2:2:2:1 ratio to receive 600 mg arhalofenate, 800 mg arhalofenate, 300 mg allopurinol, 300 mg allopurinol plus 0.6 mg colchicine, or placebo once a day. The primary outcome measure was the flare incidence (number of flares divided by time of exposure). The serum UA level was a secondary outcome measure. Results A total of 239 gout patients were randomized and took at least 1 dose of study medication. The primary outcome measure comparing flare incidence between 800 mg arhalofenate and 300 mg allopurinol was achieved, with a 46% decrease in the 800 mg arhalofenate group (0.66 versus 1.24; P = 0.0056). Treatment with 800 mg arhalofenate was also significantly better than placebo (P = 0.049) and not significantly different from treatment with 300 mg allopurinol plus 0.6 mg colchicine (P = 0.091). Mean changes in serum UA level were −12.5% with 600 mg arhalofenate and −16.5% with 800 mg arhalofenate (P = 0.001 and P = 0.0001, respectively, versus −0.9% with placebo). There were no meaningful differences in adverse events (AEs) between groups, and there were no serious AEs related to arhalofenate. Urinary calculus occurred in 1 patient receiving 300 mg allopurinol. No abnormal serum creatinine values >1.5-fold the baseline value were observed in the arhalofenate-treated groups. Conclusion Arhalofenate at a dosage of 800 mg decreased gout flares significantly compared to allopurinol at a dosage of 300 mg. Arhalofenate was well tolerated and appeared safe. Arhalofenate is the first urate-lowering antiflare therapy.

Abstract

A detailed analysis of the vibrational spectra of CH3, CH2D, and CD3 adsorbed on Ni(111) and the products of their reactions is presented. The synthesis of adsorbed methyl radicals from CH4, CH3D, or CD4 is effected by molecular beam techniques. The ability to measure these spectra by high-resolution electron energy loss spectroscopy (HREELS) at higher resolution (35 cm-1) and higher sensitivity (5×106 counts/s) has allowed new features to be observed and a symmetry analysis to be carried out. It is concluded that the CH3 radical is adsorbed with C3v symmetry on a threefold hollow site. The symmetric C–H stretch mode of CH3 and the overtone of the antisymmetric deformation mode are observed to be in Fermi resonance. At temperatures above 150 K, CH3 dissociates to form adsorbed CH. Confirmation for the assignment to a CH species is found in the observation that the spectrum measured after thermal decomposition of CH2D is a superposition of those from the decomposition of CH3 and CD3. The adsorption site of the CH species is concluded to be a threefold hollow site and the geometry of the Ni3–C–H is concluded to be pyramidal. At temperatures above 250 K, carbon–carbon bond formation between the CH species is observed to yield C2H2. Low coverages of C2H2 are shown to dehydrogenate at 400 K. High coverages of C2H2 are shown conclusively to trimerize to form adsorbed benzene in contrast to a literature report of C2H2 dissociation to adsorbed CH at these temperatures. 27. Simulations of the Earth's magnetosphere embedded in sub-Alfvenic solar wind on 24 and 25 May 2002

Abstract

During 24 and 25 May 2002, the solar wind conditions at Earth's orbit were very unusual: the density was extremely low (below 0.1/cc) and, as a result, the flow was subfast and sub-Alfvenic (the Alfven Mach number was as low as 0.4 in the rest frame of the Earth). Consequently, the Earth's bow shock disappeared and two Alfven wings formed on the flanks of the magnetosphere. These two long structures (estimated extension of 600R(E) for this event) affect the incoming plasma as follows: the velocity is reduced and the magnetic field rotates. In the present study, global magnetohydrodynamical simulations of the magnetosphere are performed for such upstream solar wind conditions. The simulations show how the magnetosphere configuration dramatically changes when the sub-Alfvenic solar wind reaches the magnetosphere: the dayside magnetopause expands up to 20R(E), and on the nightside the position of the last closed magnetic field line diminishes to 20R(E). As a result the closed magnetic field line region becomes very symmetric. The open field line configuration also changes such that field lines emanating from the Northern Hemisphere all point in the direction of the dawn Alfven wing (around 8:00LT), while the field lines from the Southern Hemisphere all point in direction of the other wing (around 22:00LT). During the formation of the Alfven wings, the tail lobes completely disappeared and the auroral activity greatly diminished, i.e., the magnetosphere becomes geomagnetically quiet.

Abstract

International audience; [1] The motion of mid-latitude surface cyclones relative to the jet streams is of particular interest because of the com-monly observed strong and rapid deepening they undergo when they cross the upper-level jet axis. The purpose of the present study is to validate a recent theory that may explain this motion which is a generalization of the so-called beta drift in the mid-latitude baroclinic context. According to this theory, the key parameter controlling the movement of a surface cyclone across the mean tropospheric jet is the ver-tically averaged potential vorticity (PV) gradient associated with the jet. To test this theoretical result, numerical sensi-tivity experiments are performed using the Météo-France global operational forecast model ARPEGE-IFS for the particular case of the storm Xynthia (26–28 February 2010). The control forecast, starting from the operational analy-sis almost 2 days before the storm hit France, represents the trajectory of the storm quite well, together with the deepening during the crossing of the upper-level jet axis. A PV-inversion tool is used to modify the vertically aver-aged PV gradient of the initial state. As expected from the theory, when the PV gradient is intensified, there is a quicker displacement of the surface cyclone toward the jet axis and the jet-crossing phase occurs earlier than in the control forecast. The opposite occurs for a reduced PV gradient. The interpretation is that an enhanced PV gradient reinforces the dipolar PV anomaly located at upper levels which, in turn, advects the surface cyclone faster towards the jet axis. International audience; Le premier volet du projet Cyprim vise à mieux identifier les précurseurs des cyclogenèses et des précipitations intenses en région méditerranéenne. Cet article en présente les principaux résultats. Il est question des ingrédients météorologiques à l'origine de ces systèmes, de la grande à la mésoéchelle, des causes des fortes intensités, localisations et parfois stationnarité des récipitations associées. Le coeur de l'article présente des méthodes d'identification des anomalies de tourbillon potentiel liées à l'occurrence d'événements précipitants intenses et dresse une climatologie de ces anomalies. Les perspectives d'exploitation de cette information pour améliorer la qualité des prévisions sont évoquées. ENGLISH VERSION: The first part of the CYPRIM project (Cyclogenesis and intense precipitations in the Mediterranean region) aims at a better identification of the precursor structures of cyclogenesis and heavy precipitation in the Mediterranean region. This article presents the main results. It mentions the meteorological ingredients from which these systems originate from the large to themesoscale, the reasons for large intensities, the localization and sometimes stationarity of the associated precipitations. The heart of the article shows methods for identifiying potential vorticity anomalies involved in the occurrence of heavy precipitation events and sets up a climatology of these anomalies.The perspectives of the usefulness of this information for improving forecast quality are discussed.

Abstract

International audience; A new Ertel-potential-vorticity inversion method is proposed and tested in the framework of a low-order model as well as in Météo France's operational primitive-equation spectral model, ARPEGE. The potential-vorticity (PV) inversion procedure is designed so that its result lies within the subspace of model solutions, without the use of any explicit balance condition. The inversion is based on a variational problem in which one minimizes simultaneously the distance to the dynamical initialized solution following in a digital-filter method and the PV to be inverted. Since the inverted solution results from model backward and forward integrations, and not from a set of algebraic equations, it possesses a strong dynamical consistency. In the framework of a low-order model, it is found that for small Rossby number, digital-filter initialization lies between nonlinear normal-mode initialization and quasi-geostrophic methods. The method has also been successfully used to rebuild the non-divergent flow, the temperature, and a significant part of the divergent flow, knowing only the three-dimensional PV field and a boundary condition.

Abstract

On March 26, 1986, Governor Gerald L. Baliles appointed the Commission on Excellence in Education and charged them with the responsibility of bringing him recommendations for specific actions to place Virginia in the top ten states in the Quality of education it offers. The Commission's Report, entitled Excellence in Education: A Plan for Virginia's Future. contained thirty-six recommendations and was submitted to the Governor in October, 1986. Specifically this study was designed to determine how and why the thirty-six recommendations were made by the Commission. Those key actors or decision makers and those key factors that were most influential in developing the thirty-six recommendations made by the Commission were to be identified. A third purpose of this study was to determine whether the impact of the Commission's Report had met or exceeded the expectations of those responsible for the report. Ed. D.
