Background The aim was to evaluate efficacy and feasibility of left bundle branch area pacing (LBBAP) with right ventricle outflow tract pacing(RVOP), as well as describing the learning curve. Methods Prospective single-center observational study, including 200 patients that required pacemaker implantation. Two groups: LBBAP and RVOP with 100 patients each. Our aim was to compare safety and efficacy as well as the procedure and fluoroscopy times and to describe the LBBAP-learning curve. Results Success and acute complication rate were similar in both groups(p=0.56,p=0.65). Time for ventricular lead placement was higher in LBBAP group(18 [13-28] vs 11[7-17]min, p Conclusions LBBAP is as safe as RVOP and it achieved a narrower paced-QRS than RVOP. It requires a longer procedure time and a shorter fluoroscopy time, with a rapid learning curve.

Main funding source(s): TED2021-130459B-I00 LMP94_21 Introduction Right ventricular pacing (RVP) has been the treatment of choice for patients with atrioventricular block and sinus node disease for the last decades . It is well known that RVP ventricular may cause ventricular dyssynchrony and could lead to left ventricular dysfunction in 20% of patients, and therefore could increase the risk of heart failure, atrial fibrillation and cardiovascular mortality. Left bundle branch area pacing (LBBAP) has recently emerged as a new physiological pacing modality that can circumvent the limitations of RVP. The purpose of this study was to compare changes induced by RVP and LBBAP in the standard 12-lead ECG by the analysis of QRS complex characteristics in order to asses ventricular dyssynchrony. Methods ECG recordings from 134 patients (82 LBBAP, 52 RVP) with indication for anti-bradycardia therapy were acquired before and after pacemaker implantation. Patients were grouped into narrow and wide baseline QRS using a threshold for the QRS duration of 120 ms. Median beats were calculated at baseline and 24 hours post-implantation states for each patient. ECG markers describing ventricular activation were measured, including: 1) QRS duration (QRSd) from QRS onset to QRS end; 2) QRS area (QRSa), computed from the individual areas in orthogonal X, Y and Z leads calculated using the Kors conversion matrix; 3) a new index QRS50 proposed to estimate the ventricular activation time from the first three components of the singular value decomposition of the median QRS complex. Results Note that the QRS characteristics showed similar values in baseline for both pacing techniques. In patients with narrow QRS, we found that paced QRS characteristics remained similar from the basal QRS in LBBAP group whereas in RVP group they worsened. The three markers, QRSd, QRSa and QRS50, increased after RVP by 34 ms, 68 uVs and 22 ms, respectively (p Conclusion In patients receiving a pacemaker for anti-bradycardia therapy, LBBAP led to more synchronized ventricular activation than RVP measured from QRS duration- and area-related characteristics.

Abstract

El objetivo del estudio es presentar los resultados obtenidos sobre conocimientos, actitudes y prácticas acerca de la enfermedad de Chagas, en 241 escolares de educación primaria en La Tinguiña, Ica, Perú (diciembre 2000 - enero 2001). Menos del 1% de los encuestados reconoce que los triatomas trasmiten la enfermedad de Chagas, y casi la cuarta parte reconoce la enfermedad por la formación de "ronchas" en la piel; el 35,27% sabe que la infestación por el vector se controla con insecticidas. El 26,56% reconoce a los estados adultos del vector y el 21,16% a las ninfas; el 14,11% lo conoce con el nombre de "chirimacha"; el 82,57% aceptaría una encuesta entomológica; el 66,80% permitiría un estudio serológico y el 63,90% participaría en la búsqueda de triatominos. Este estudio revela que la población, a pesar de tener conocimientos muy limitados sobre la enfermedad y su vector, muestra interés en colaborar. Por lo tanto, se recomienda que las estrategias de vigilancia y control de esta enfermedad, incluyan necesariamente programas educativos y de participación comunitaria, en la implantación de futuros programas de control.

24. Eficiencia del Deep Learning para el modelado de la fuerza de reacción durante la marcha

Abstract

La medición de las fuerzas de reacción con el suelo durante la marcha humana de un paciente es una herramienta esencial para la disminución, prevención y rehabilitación de lesiones. El objetivo de este trabajo ha sido el de evaluar la eficiencia del Deep Learning implementado en Matlab 2021a para el modelado de las fuerzas de reacción con el suelo a partir de mediciones de los ángulos de flexión de la rodilla, del tobillo y de la cadera. Las mediciones fueron proporcionadas por el laboratorio de biomecánica de la Universidad de Sevilla, el cual desarrolló un ensayo basado en el protocolo Plug-in-Gait (PiG), el cual está limitado a la medición en un espacio cerrado sobre placas de fuerza. Los resultados del estudio mostraron resultados superiores para el entrenamiento de redes Long-Short-Term-Memory (LSTM) de varias capas. Por ende, la simulación en Deep Learning basada en estructuras LSTM es una herramienta prometedora para el modelado de la dinámica de la marcha eliminando las limitaciones de laboratorio. The measurement of ground reaction forces during human gait is an essential tool for injury reduction, prevention and rehabilitation. The aim of this project has been to evaluate the efficiency of Deep Learning, implemented in Matlab 2021a, for the modeling of ground reaction forces from measurements of knee, ankle and hip flexion angles. The database was taken from the biomechanics laboratory of the University of Seville, which developed a Plug-in-Gait (PiG) based experiment, which is limited to measurement in an indoorspace on force plates. The results of the study showed superior results for the training of multi-layer Long-Short-TermMemory (LSTM) networks. Abstract

Squamous cell lung cancer (sqCLC) is an aggressive form of cancer that poses many therapeutic challenges. Patients tend to be older, present at a later stage, and have a high incidence of comorbidities, which can compromise treatment delivery and exacerbate toxicity. In addition, certain agents routinely available for nonsquamous cell histologic subtypes, such as bevacizumab and pemetrexed, are contraindicated or lack efficacy in sqCLC. Therapeutic progress has been much slower for advanced sqCLC, with median survival times of approximately 9 to 11 months in most studies. Herein, we discuss the current therapeutic landscape for patients with sqCLC versus with nonsquamous NSCLC. Current evidence indicates that new targeted treatments, notably monoclonal antibodies such as ramucirumab and necitumumab, and immunotherapies such as nivolumab and pembrolizumab can provide survival prolongation, although the benefits are still relatively modest. These incremental improvements, all realized since 2012, in aggregate, will very likely have a clinically meaningful impact for patients with sqCLC. We also discuss recent genomic studies of sqCLC that have identified potentially actionable molecular targets, as well as the relevant targeted agents in clinical development. Finally, we discuss the magnitude of survival benefit and the risk-to-benefit ratio that would prove clinically meaningful in this underserved patient population with unmet needs.

Abstract

In the present work, the environmental damage of two types of molten salts and synthetic oil has been evaluated with the aim of deciding whether the use of molten salts is more suitable than synthetic oil from an environmental point of view. To this end Life Cycle Assessment has been used, since it offers numerous advantages in the assessment of aspects associated with the development of a product and its potential impact throughout the life of a given product from raw material acquisition, processing, manufacturing, use and finally its disposal. According to the results, the replacement of the VP-1 like thermal oil by molten salts is absolutely recommended from an environmental perspective, since the environmental impact of molten salts appear to be reduced in comparison to Therminol®VP-1. Thus, direct systems, in which thermal storage and heat transfer fluid are unified using molten salts, emerge to improve the power cycle performance, these being a good option not only from the known technical point of view, but also from the environmental point of view. Life Cycle Assessment seems to be a suitable and necessary methodology to quantify the environmental impacts of fluids, materials, and O&M in CSP plants. [ABSTRACT FROM AUTHOR]

Abstract

The pottery found in the burials of El Cano is uniform in style to these made in the coclesanos valleys between 700 and 1000 AD. The coefficient of variability of the different pottery forms, evidence diverse standardizations values for polychrome and non-polychrome ceramics. Moreover, data of funerary contexts from the Cano recently excavated, suggest that elite has controlled ceramic production. This control over the production of certain goods reveals that these were important in the support or proper operational of the chiefdoms in Panama and mark the phase of splendour of this culture. La cerámica encontrada en las tumbas de El Caño comparte el mismo estilo que la producida en las llanuras aluviales de la región en los siglos comprendidos entre el 700 y 1000 d.C. Los datos de los coeficientes de variabilidad de las diferentes formas analizadas evidencian distintos valores de estandarización en la producción de la vajilla polícroma y la no polícroma. Los contextos funerarios excavados recientemente en el Caño indican además que la producción de cerámica polícroma era controlada por una élite. Este control sobre la producción de ciertos bienes sugiere que estos fueron clave en el desarrollo de las jefaturas en el istmo de Panamá y marca la fase de esplendor de la cultura coclesana.

Abstract

El Caño, situado en la provincia de Coclé (Panamá), es un yacimiento arqueológico conocido por su singular estilo escultórico y por contener estructuras arqueológicas de piedra. Recientemente se han descubierto en este lugar cuatro tumbas de personas de élite con ricos ajuares, fechadas entre el 700 y el 1000 d. C. Estas son coetáneas a las halladas en el cercano Sitio Conte, una necrópolis en la que se encontraron, hace más de ochenta años, las primeras evidencias arqueológicas de la complejidad de las sociedades precolombinas del istmo. La existencia en Sitio Conte y El Caño de elementos semejantes —basaltos columnarios dispuestos en hileras, calzadas y ricas tumbas— organizados además de forma similar, indica la posibilidad de la existencia de un patrón funerario en el valle de Río Grande. ENGLISH: The Discovery of an Elite Cemetery at El Caño: Traces of a Mortuary Pattern in Río Grande Valley, Coclé, Panama. El Caño, located in the Coclé province, Panama, is known for its unique stone sculptural style and stone structures. Recently, four lavish burials have been discovered. Dated between 700 and 1000 AD, they are broadly coeval with similar mortuary features at nearby Sitio Conte, a cemetery, which provided the first archaeological evidence for the sumptuous wealth of Panamanian chiefdoms more than eighty years ago. The existence in Sitio Conte and El Caño of similar elements—alignments of basalt columns, causeways and rich tombs—with a similar spatial organization, indicates the possibility of the existence of a mortuary pattern in Río Grande.
