10. How digital trust powers the sharing economy

Abstract

Over the centuries, each significant economic expansion has been enabled by the accompanying creation of new trust systems -- from village-based trade and informal trader networks, through contracts, government standards and financial institutions, to today's corporate brands. Emerging digital platforms are catalyzing a new expansion that will reintegrate into our economic interactions the social aspects of commerce that were marginalized by 20th century capitalism, creating a new form of crowd-based capitalism powered by the digital trust grid. This article shares the findings of an ongoing research collaboration between New York University and BlaBlaCar. The authors identify the mechanics of online trust and the resulting level of trust created. They show that, with the right digital tools, many of which are encapsulated in their D.R.E.A.M.S. framework, individuals can achieve high levels of trust without ever having met in person. This signals a radical move toward a friendlier, more personal, more connected, more empowered world of trust.

Abstract

The role of atenolol, a non-vasodilating beta-blocker drug, on long-term mortality in hypertensive older adults is still unclear. The aim of the present study was to evaluate long-term mortality in community-dwelling hypertensive older adults taking atenolol.Long-term mortality after 12-year follow up in isolated hypertensive older adults (n = 972) was analyzed. The patients were stratified in the presence and absence of atenolol use. Systolic, diastolic and pulse arterial pressure were measured.Older adults taking atenolol showed a greater mortality and higher pulse arterial pressure values than those not taking atenolol (73.9% vs 55.0%; P = 0.047 and 74.7 ± 14.1 vs 63.0 ± 14.2 mmHg, P 0.001, respectively). Cox regression analysis showed that atenolol use (hazard risk 1.91; 95% confidence interval 1.04-4.31; P = 0.04) and pulse arterial pressure (hazard risk 1.02; 95% confidence interval 1.01-1.03; P = 0.032) were predictive of long-term mortality.Atenolol use was related to increased mortality in community-dwelling hypertensive Older adults. This increase in mortality risk seems to be related to an increase of pulse arterial pressure.

14. MACARONI SIGN AND TAKAYASU ARTERITIS IN A PATIENT WITH TUBERCOLOSIS: AN EASY TOOL FOR DIAGNOSIS

Abstract

Takayasu arteritis (TKA) is a chronic systemic autoimmune disease characterised by inflammation of the aorta and major branch vessels. Carotid Artery (CA) lesions in TKA, known as the ‘macaroni sign’, are characterised by global thickening of the common CA wall and lumen narrowing. There has been speculation that there is a link between TKA and tuberculosis (TB). Both illnesses cause arterial wall changes. We describe a TKA case diagnosed based on the presence of the ‘macaroni sign’ on CU in a young Asian women 30 years old with a diagnosis of latent TB. She has not previous histiry of disease and visited the hospital with complaints dyspnoea and carotidynia. At admission, body temperature, blood pressure and heart rate were 36.8°C, 112/60mm Hg on her right arm (left, 84/60 mmHg) and 76 beats/min, respectively. The pulse in her left radial artery was not palpable. Physical examination revealed spontaneous pain and tenderness on both sides of the patient’s neck. Blood test results demonstrated an elevated inflammatory response C–reactive protein, 8.0 mg/dL (normal range: less than 0.5 mg/L); erythrocyte sedimentation rate (ESR), 120 mm/hour. ANA, ENA and ASMA profile were negative. Quantiferon TB gold was positive. We suspected TKA and performed a CU, which revealed marked thickening of the intima/media of the bilateral common CAs and a positive macaroni sign (figure 1, Panel A and B). After a detailed evaluation of the systemic vessels the patient was diagnosed with TKA Type I limited to Branches from the aortic arch. The FDG–PET/CT demonstrated an increase in the glucose uptake in thoracic aorta, subclavian and right CA (SUV 4.5). The patient was treated with oral steroids at a dose of 0.8mg/kg and Isoniazid. After 6 months she showed a disappearance of carotidynia and dyspnoea, with a reduction in C–reactive protein, 1.5 mg/ dL and ESR=39 mm/hour. The FDG–PET/CT demonstrated a reduction in the glucose uptake in thoracic aorta, subclavian and right CA (SUV 3.5). Although MRI is usually recommended for the definitive diagnosis, the ‘macaroni sign’ identified by CU is a low–cost, simple and non–invasive method that can be easily performed during initial hospital visits. In addition, a clear correlation between MRI and CT angiography findings and disease activity or progression has not been found for TKA yet. Therefore, methods using ultrasound in combination with ESR should be considered easy and useful for diagnosis and clinical follow–up.

15. A CASE OF A HUGE AORTIC PSEUDO–ANEURYSM FOLLOWING AORTIC BIO PROSTHETIC ENDOCARDITIS: KEY ROLE OF 3D ECHOCARDIOGRAPHY

Abstract

Complications of infective endocarditis (IE) are associated with increased mortality and morbidity and therefore guidelines recommend early surgery [. Although two–dimensional echocardiography (2D) is the first line investigation, three–dimensional (3D) imaging provides incremental diagnostic information by allowing visualisation of structures in multiple planes. We present a cases which demonstrate how 3D imaging provides better morphological assessment of valve leaflets and associated structures, allows greater accuracy in identifying complications of IE. A 47 year old male with previous meccanic prosthetic aortic valve replacement was admitted to our department for evaluation of pyrexia. Lab–work was notable for leukocytosis with a white blood cell count of 13.000/mm3 and a hemoglobin level of 10.8 gr/dl. Blood cultures grew Staphylococcus Epidermidis. A transesophageal echocardiogram was performed showing a pulsatile perivalvular echo free–space with colour Doppler flow inside suggestive of an aortic pseudoaneurysm . 3D–TEE imaging provides better morphological assessment of aortic pseudoaneurysm. 3D TEE clearly demarcates an irregularly shaped of perivalvular cavity and through multiplanar reconstruction allowed a correct evaluation of the area and circumference. Conclusione: Decision making in IE is complex. The most common indications for early surgery are development of heart failure, uncontrolled infection or risk of embolism. In particular uncontrolled infection is commonly due to perivalvular abscess formation. 3D–TOE allows rapid, accurate, unique assessment of IE complication through unlimited multiplanar reconstructions, volume–rendered real–time imaging and colour full–volume regurgitation assessment which may not always possible on 2D–TTE or 2D–TOE. This case demonstrate how traditional 2D TEE may provide incomplete anatomical information. 3D imaging improved identification and characterisation of the complications of IE. The complementary data provided aids in decision making with regard to both requirements for timing and planning of surgery.

18. Determinants of prolonged intensive care unit stay after cardiac surgery in the elderly

Abstract

Objectives: In the last decade, there has been a rapid increase in the number of elderly patients referred for cardiac surgery. Recent studies have identified risk factors for prolonged intensive care unit (ICU) stay in cardiac surgery patients. The aims of this study was to evaluate pre-operative risk factors for ICU stay longer than 3 days in a cardiac surgery elderly population, and whether prolonged ICU stay may influence disability, functional recovery and length of rehabilitation. Methods: Two hundred and fifty elderly (≥65 years) cardiac surgery patients were consecutively evaluated at enter in cardiac rehabilitation after ICU dismissal from January 2008 to July 2009. Univariate and multivariate analyses for risk factors were performed for ICU stay longer than 3 days. Thereafter, 6-minute walking test (6MWT), Barthel Index (BI), BI percent recovery and length of stay (LOS) in rehabilitation were evaluated. Results: Mean age was 72.9±4.8 yrs, 170 (68%) patients underwent cardiac surgery for coronary artery by-pass grafting (CABG), 56 (22.4%) for valve replacement and 24 (9.6%) for both CABG and valve replacement. Mean ICU stay was 1.9±1.5 days and 72 patients (28.8%) spent more than 3 days in ICU. Age, New York Heart Association class ≥3, Cumulative Illness Rating Scale (CIRS) score, prevalence of stroke and renal failure were significantly higher in patients with than in those without ICU stay ≥3 days. Off-pump CABG, Physical Activity Scale for the Elderly (PASE), BI and 6MWT were significantly lower in patients with than in those without ICU stay ≥3 days. Multivariate analysis shows that female sex, a NYHA class ≥3, CIRS and PASE score are predictors of ICU stay ≥3 days independently of age, off-pump CABG, stroke and renal failure. Multiple linear regression shows that ICU stay ≥3 days is negatively associated with 6MWT, BI at entry and BI percent recovery, whereas it is positively associated with a longer rehabilitation LOS. Conclusions: Pre-operative comprehensive assessment in the elderly could help to identify predictors of long ICU stay after cardiac surgery. This approach could help to better define the elderly cardiac surgery patients and their needs throughout the cardiac rehabilitation program in order to maximize functional capacity recovery, reducing disability and rehabilitation LOS.

Abstract

Background Chronic heart failure (CHF) is characterized by a high mortality in the elderly. Moreover, depression status is also related to poor prognosis in advancing age. Thus, we sought to determine whether depressive status predicts long-term mortality in subject with or without CHF. Methods and results Long-term mortality after 12-year follow-up in 125 elderly subjects with CHF and 1143 elderly subjects without CHF was studied. Depression was evaluated using the Geriatric Depression Scale (GDS), and all subjects were stratified in tertiles according to GDS score (0–10, 11–20 and 21–30). With increasing GDS score, long-term mortality at the end of follow-up increased from 43Æ4% to 72Æ0% in subjects without (P < 0Æ001) and from 52Æ6% to 83Æ9% in subjects with (P < 0Æ007) CHF. In multivariate analysis, GDS appeared to be predictive of long-term mortality in the absence (Hazard ratio = 1Æ01; confidence interval 95% 1Æ00–1Æ05; P = 0Æ04) and, even more, in the presence of CHF (Hazard ratio = 1Æ08; confidence interval 95% 1Æ01–1Æ15; P < 0Æ01). Conclusions Depression symptoms predict long-term mortality in elderly subjects without and, even more, with CHF. Thus, depression can be considered a strong predictor of death in CHF elderly subjects.

Abstract

Objectives: To evaluate whether a training intervention can improve the ability of geriatricians to recognize depression in older persons. Design: Multicenter, cluster randomized clinical trial. Setting: Fourteen geriatric outpatient clinics in Italy, each representing the unit of randomization. Participants: After training, a total of 1,914 outpatients aged 65 years and older in both arms, not on antidepressant at entry, were blindly evaluated by the clinic geriatrician, in charge of routine clinical management, and by a field researcher, who formally diagnosed depression by the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV), taken as the criterion standard. Intervention: After randomization, geriatricians belonging to the intervention arm were assigned to receive a residential 3-day educational program on depression. Those in the control arm received a generic course on disease management in elderly people. Measurements: Sensitivity and specificity of the diagnosis of depression made by geriatricians, compared with the DSM-IV diagnosis. Results: Sensitivity and specificity were significantly higher in trained than in untrained geriatricians (49 vs 35% and 91 vs 88%, respectively; P=.002 in marginal regression models). Effectiveness of training was confirmed, adjusting for age, sex, and cognitive performance (P=.02). Conclusion: The ability of geriatricians to diagnose depression in older outpatients can be improved with a specific training intervention. Improvement of diagnostic performance might translate into more-appropriate clinical management.
