16. Poststroke Care Pathways and Spasticity Treatment: A Retrospective Study in Alberta

Abstract

ABSTRACTBackground:Limited evidence exists regarding care pathways for stroke survivors who do and do not receive poststroke spasticity (PSS) treatment.Methods:Administrative data was used to identify adults who experienced a stroke and sought acute care between 2012 and 2017 in Alberta, Canada. Pathways of stroke care within the health care system were determined among those who initiated PSS treatment (PSS treatment group: outpatient pharmacy dispensation of an anti-spastic medication, focal chemo-denervation injection, or a spasticity tertiary clinic visit) and those who did not (non-PSS treatment group). Time from the stroke event until spasticity treatment initiation, and setting where treatment was initiated were reported. Descriptive statistics were performed.Results:Health care settings within the pathways of stroke care that the PSS (n= 1,079) and non-PSS (n= 22,922) treatment groups encountered were the emergency department (86 and 84%), acute inpatient care (80 and 69%), inpatient rehabilitation (40 and 12%), and long-term care (19 and 13%), respectively. PSS treatment was initiated a median of 291 (interquartile range 625) days after the stroke event, and most often in the community when patients were residing at home (45%), followed by “other” settings (22%), inpatient rehabilitation (18%), long-term care (11%), and acute inpatient care (4%).Conclusions:To our knowledge, this is the first population based cohort study describing pathways of care among adults with stroke who subsequently did or did not initiate spasticity treatment. Areas for improvement in care may include strategies for earlier identification and treatment of PSS.

Abstract

ABSTRACT:Objective:The objective of this study is to determine the physical evaluations and assessment tools used by a group of Canadian healthcare professionals treating adults with spasticity.Methods:A cross-sectional web-based 19-question survey was developed to determine the types of physical evaluations, tone-related impairment measurements, and assessment tools used in the management of adults with spasticity. The survey was distributed to healthcare professionals from the Canadian Advances in Neuro-Orthopedics for Spasticity Congress database.Results:Eighty study participants (61 physiatrists and 19 other healthcare professionals) completed the survey and were included. Nearly half (46.3%, 37/80) of the participants reported having an inter- or trans-disciplinary team managing individuals with spasticity. Visual observation of movement, available range of motion determination, tone during velocity-dependent passive range of motion looking for a spastic catch, spasticity, and clonus, and evaluation of gait were the most frequently used physical evaluations. The most frequently used spasticity tools were the Modified Ashworth Scale, goniometer, and Goal Attainment Scale. Results were similar in brain- and spinal cord-predominant etiologies. To evaluate goals, qualitative description was used most (37.5%).Conclusion:Our findings provide a better understanding of the spasticity management landscape in Canada with respect to staffing, physical evaluations, and outcome measurements used in clinical practice. For all etiologies of spasticity, visual observation of patient movement, Modified Ashworth Scale, and qualitative goal outcomes descriptions were most commonly used to guide treatment and optimize outcomes. Understanding the current practice of spasticity assessment will help provide guidance for clinical evaluation and management of spasticity.

Abstract

An investigation into the system of isolating several Lewis-base adducts of the acidic Fe(NO)(sub 3)(super +) species disclosed that they exhibit distinctly bent Fe-N-O linkages. Experiments carried out show that the bending of the NO ligands results in an increase in the energy of the HOMO relative to the linear case, but at the same time causes a decrease in energy of the HOMO-1 and the HOMO-2 molecular orbitals.

Abstract

The World Administrative Radio Conference (WARC) held in 1992 allocated the bands 19.7-20.2 GHz and 29.5-30.0 GHz to both the Mobile Satellite Service (MSS) and the Fixed Satellite Service (FSS) on a co-primary basis. An economic and flexible solution for the provision of both services is to place both payloads on one spacecraft. Some of the proposed applications of such a hybrid satellite network are described. Abstract

Currently scientists and engineers are using near-infrared (NIR) technology to detect motions of human body parts, and can develop programming for gesture recognition. Software is being developed by researchers for both stationary and mobile NIR cameras to operate as Sign Language Recognition devices. This thesis focus was on adapting an application of a NIR camera to be used on any device with application compatibility, essentially allowing a Hearing Impaired (HI) person to use a highly portable camera to communicate with non-speakers of Sign Language. Some examples of the devices that are able to use this technology include computers, tablets, and smart phones. I have developed a Sign Language recognition technology to establish better techniques, databases, and interactive applications for improved function of communication in the HI culture. The algorithm performed at a 76.67% accuracy across all gestures tested for first-time participants.

Abstract

The citizen Continental-America Telescopic Eclipse (CATE) Experiment was a new type of citizen science experiment designed to capture a time sequence of white-light coronal observations during totality from 17:16 to 18:48 UT on 2017 August 21. Using identical instruments the CATE group imaged the inner corona from 1 to 2.1 RSun with 1.?43 pixels at a cadence of 2.1 s. A slow coronal mass ejection (CME) started on the SW limb of the Sun before the total eclipse began. An analysis of CATE data from 17:22 to 17:39 UT maps the spatial distribution of coronal flow velocities from about 1.2 to 2.1 RSun, and shows the CME material accelerates from about 0 to 200 km s?1across this part of the corona. This CME is observed by LASCO C2 at 3.1-13 RSun with a constant speed of 254 km s?1. The CATE and LASCO observations are not fit by either constant acceleration nor spatially uniform velocity change, and so the CME acceleration mechanism must produce variable acceleration in this region of the corona.
