Bangladesh experiences unique challenges in publishing medical research in peer reviewed and high-impact international and national journals. These hurdles come from a combination of factors, including lack of healthcare professional to meet healthcare demand of large number of populations, limited research funding, inadequate research training, and a lack of mentorship for aspiring researchers. Additionally, the language barrier, particularly with English as the predominant language of scientific communication, poses a significant obstacle. Consequently, Bangladeshi researchers often struggle to meet the rigorous standards and competitive landscape of international journals. TMSS Medical College Journal (TMCJ) has also been experiencing some challenges since it has been started in July 2013. Therefore, based on the TMCJ own experience, this study was designed to explore the challenges of medical journal publication. The aim of this study was to explore the challenges of medical journal publication of a medical college in Northern Bangladesh. This qualitative study was conducted in Bogura, Northern Bangladesh. Study participants were journal editors, advisors and editorial board members and authors of TMCJ. The journal and participants were selected purposively. An interview guideline was used to collect in-depth data from eleven providers. This study found lack of quality research papers for publishing each and every issue of the journal. Quality research paper includes well written methodology, analysis, findings, proper citation and language. One of the journal editors (KII-01) mentioned that “We are not receiving a sufficient number of manuscripts, and those we do receive are not publishable.” Associated factors of receiving less paper for publications were more emphasis given on serving patients and students, lack of interest in research, limited research funding, inadequate research training, and a lack of mentorship for young researchers. The potential interventions for removing barriers were enhance research funding, strengthen research training programs, promoting English language proficiency and foster mentorship opportunities. Lack of quality research papers is one of the major challenges for publishing medical journal. By overcoming these barriers, Bangladesh can enhance its contribution to the global body of medical knowledge and improve healthcare outcomes for its population.

The Journal of Karnali Academy of Health Sciences (JKAHS) is an official publication of Karnali Academy of Health Sciences (KAHS), a government organization founded on October 20, 2011, by an act of the Parliament of Nepal [1]. KAHS was established with the objective of enhancing the quality of healthcare services, medical education, and research in remote and underserved areas of Nepal. The establishment of JKAHS in 2018 is a reflection of commitment of KAHS to these goals, providing a dedicated platform for the dissemination of scientific knowledge and clinical experiences in the health sciences [2]. JKAHS is a peer-reviewed, international medical journal that operates under a diamond open access model, that means all its contents are freely accessible to readers without any subscription or paywall barriers [2]. This model not only broadens the journal’s audience, including researchers, students, policymakers, and the general public, but also aligns with the mission of KAHS to disseminate knowledge widely and equitably [1]. Importantly, JKAHS does not charge any fees for the submission, processing, or publication of manuscripts, thus removing financial barriers for authors and promoting inclusivity in scientific publishing. Additionally, to ensure the long-term preservation and accessibility of its content in the internet, JKAHS archives all its articles in LOCKSS (Lots of Copies Keep Stuff Safe) and CLOCKSS (Controlled Lots of Copies Keep Stuff Safe) [2]. These archiving systems safeguard the integrity and usability of the published work, ensuring that it remains available to future generations. It is triannual journal publishing three issues per year, and is available both online and in print with pISSN 2616-0064 and eISSN 2676-1327 [2]. This regular publication schedule ensures a steady flow of new research and insights, contributing to the continuous advancement of medical knowledge. Achievements and milestones One of our significant achievements has been in indexing of JKAHS in two national databases namely Nepal JournalOnline (NepJol) in 2019 [3] and NepalMedLine (NepMed) in 2020 [4]. The NepJol is hosted by Central Library (TUCL) and it is hosted by Ubiquity Press [3] whereas NepMed is hosted my Nepal Health Research Council (NHRC) [4]. A part from these national databases, we have been indexed in DOAJ (Directory of Open Access Journal) [5] and Hinari [6] in 2023. In the earlier days after indexing in NepJol, individual articles were assigned an unique DOI (digital object identifier) from NepJol (10.3126/jkahs). Later on we have taken our own DOI (10.61814/jkahs) directly from CrossRef under the sponsroship under GEM (Global Equitable Membership (GEM) Program from CrossRef [7]. This recognition is a testament to the quality and impact of our published research. Being indexed has enhanced the visibility and accessibility of our articles, attracted a broader readership and fostered greater academic collaboration.

The British Journal of Anaesthesia (BJA) celebrates its centenary in 2023, and with it 100 yr of continuous anaesthesia research publication. As an editorially and financially independent journal, the BJA faced a rapidly changing anaesthesia profession, health system, and publishing world without the security of institutional support. In its early days, the Journal was vocal about the challenging conditions faced by anaesthetists before the National Health System was established, and was essential in advocating for the specialty. Although the years after World War II brought improving fortunes for the specialty, the BJA found itself struggling to publish. As the Journal's fortunes began to improve, a new research and healthcare context emerged, radically changing the face of anaesthesia research and practice, to which the Journal needed to adapt. In spite of a range of challenges throughout the years, the BJA has developed into an international, future-focused, well-respected publication. Giving certain elements more significance than others in the same context is called prominence. This study aims at identifying and comparing markers of focal and thematic prominence in Persian general practitioners’ books (GPBs) and Persian online medical journals (OMJs) based on Levinsohn’s model. The data were gathered from 100 texts from two written Persian GPBs and 100 texts from two Persian OMJs. SPSS software version 24 was used to evaluate the data. Results demonstrated “emphatic markers” such as fæqæt ‘just’ were used for focal prominence much more frequently in OMJs than in GPBs. In addition, significant differences were observed between GPBs and OMJs with regard to the markers of focal prominence and prominence in general, but not with regard to markers of thematic prominence. The process of medical scientific journal publishing merits further explanation for authors and readers. Prospective authors need to understand the scope of the journal and the article types that are published. We give an overview of the editorial process, including selection of reviewers, peer review and decisions regarding revision, acceptance and rejection of papers for Australasian Psychiatry. RAD Croatian Academy Sciences and Arts (RAD CASA) is one of the oldest Croatian scientific journals, published in continuity from 1867. In this paper, we will present the historical context relevant to the establishment of the first Croatian scientific journals and explore their importance for Croatia’s nation-building process. Our emphasis will be on medical journals, among which an important place belongs to RAD CASA – Medical Sciences. "is journal, which was established as a separate series within RAD CASA in 1951, had issues with continuity until 2006, when new editorial board was elected. Since then, the journal has been included as an open access journal on HRČAK, the portal of Croatian scientific and professional journals, and is being published in English, which contributes to its international visibility and availability. Since 2019 the journal is being published in two double issues per year and has an expanded number of different sections and an increasing number of contributions. In the journal’s 70-year history, RAD CASA – Medical Sciences published 582 articles authored by 1012 researchers and occupying 8010 pages. During this time, nine members of the Academy’s Department of Medical Sciences served as editors of the journal. At the end of 2022, RAD CASA – Medical Sciences has more than 500.000 visits on the HRČAK portal, and its international visibility is also rising, especially on Google Scholar. Analysis of the content of medical journals enables us to frame the shifting scientific, material, ethical, and epistemic underpinnings of medicine over time, including today. Leveraging a dataset comprised of nearly half-a-million articles published in the Journal of the American Medical Association (JAMA) and the New England Journal of Medicine (NEJM) over the past 200 years, we (a) highlight the evolution of medical language, and its manifestations in shifts of usage and meaning, (b) examine traces of the medical profession’s changing self-identity over time, reflected in its shifting ethical and epistemic underpinnings, (c) analyze medicine’s material underpinnings and how we describe where medicine is practiced, (d) demonstrate how the occurrence of specific disease terms within the journals reflects the changing burden of disease itself over time and the interests and perspectives of authors and editors, and (e) showcase how this dataset can allow us to explore the evolution of modern medical ideas and further our understanding of how modern disease concepts came to be, and of the retained legacies of prior embedded values.

Journal of Bangladesh College of Physicians and Surgeons (JBCPS), all the go by 'BCPS Journal', is the official organ of BCPS inclusive of academic, extra-academic and official matters. The journal started its journey in August 1983, now in the fourth decades is embedded in the fanfare of Golden Jubilee of BCPS. It commensurate with the fellows' desire that a supplement of the JBCPS shall be published as the official organ of the college on behalf of the education, training, research and publication to Showcase the college image in the academic arena. The special editorial board is tasked to marinate a document of the sojourn of the JBCPS, for now and then. The authors have tried to pick up pieces of information from all available local libraries and online resources to build up an edifice to be added on from time to time. It has been tried to fullfill the mandate through expanded and extended search and find the tenets of the emergence and evolution of the JBCPS with an endeavor to bring out a canvas of the paintings of the stories of the Odyssey. Bangladesh Medical Journal (BMJ) published by Bangladesh Medical Association (BMA) is the oldest journal of the country first published 50 years ago in July 1972. Then the Bangladesh Medical Research Council Bulletin (BMRC Bulletin), published in December 1975. JBCPS is the third in this sequence. Prof Golam Rasul was the first editor in chief and so far, fifteen chiefs navigated the journal so far. The first editorial board consisted of five members and present editorial board embracing almost all the subjects including editors from abroad. JBCPS is online since 2004 and is available in the Bangladesh Journals Online (BanglaJOL) since 2006. With Continued relentless untiring efforts of the editorial board and collaboration from the executive committee of BCPS the JBCPS is now one of the top ranked & prestigious peer reviewed journal of the country. Conflict of interest (COI) declaration policies are not commonplace in China and the level of awareness and reporting among Chinese researchers have not been fully investigated. This study sought to discover the presence of COI policies in high‐impact Chinese journals, the compliance of authors with these policies and the awareness of authors with the principles of COI and to compare these findings against comparable international journals. The study included review of publications and policies and a survey of Chinese researchers. It compared 14 Chinese journals and 16 international journals during 2018. Most Chinese journals have a COI policy, but only 0.6% of articles published in 2018 included a statement of potential COI. All international journals required COI statements in all articles, and 15.6% of Chinese articles declared a potential COI (of which most were financial COIs). However, this was lower than COI declarations from non‐Chinese authors (47.8%). When surveyed, 24% of researchers had potential COI in a submitted article, but only 4.2% declared a COI: most of the under‐declaration was due to lack of knowledge about COI, followed by concern that a COI would lead to rejection of their articles. The research shows that there is a knowledge and policy gap in China that needs to be rectified and that international journals can be used as a reference point for good practice. Background: R-CHOP/R-CVP is the only recommended first-line treatment for Non-Hodgkin’s Lymphoma (NHL). Limited treatment alternatives often lead to relapse and refractory NHL, which increases disease progressivity and worsens prognosis. Bendamustine-rituximab is being studied for its potential as a superior first-line therapy for indolent NHL and mantle-cell NHL (MCL); however, it is not in the national guidelines. Evidence-based research is needed to demonstrate the effectivity of bendamustine-rituximab compared to R-CHOP/R-CVP for a complete response of indolent NHL and MCL. Methods: A literature search was conducted using PubMed, Scopus, EBSCOHost, and Cochrane. Studies consistent with clinical question and eligibility criteria were included and critically appraised using the Oxford Centre for Evidence-Based Medicine (CEBM) tool. Results: Two randomized controlled trials (RCTs) were included in this study, both concluding that bendamustine-rituximab is superior to R-CHOP/R-CVP with a complete response, with RR values of 0.90 (95% CI 0.80 – 1.01) and 0.86 (95% CI 0.76 – 0.98). Conclusion: Bendamustine-rituximab is more effective than R-CHOP/R-CVP as a first-line treatment of indolent NHL or MCL.

Portal hypertension is a clinical syndrome that consists of hypersplenism, ascites, gastroesophageal varices, and encephalopathy. This condition is marked by increased portal pressure gradient and may occur with or without liver cirrhosis. To date, portal hypertension remains as the leading cause of severe complications and death of a patient with chronic liver disease, especially liver cirrhosis. Therefore, thorough understanding about management of portal hypertension is strongly required, especially considering that many complications of portal hypertension require early diagnosis and treatment to improve the prognosis of the patients. Additionally, although hepatic venous pressure gradient (HVPG) measurement has become a gold standard procedure for measuring portal pressure in the last twenty years, utilization of this method in Indonesia has been hindered by reluctance of the patients due to its invasiveness, high cost, and limited availability. This consensus is developed with evidence-based medicine principles to provide a guideline for portal hypertension management for general practitioners, specialists, and consultants, to achieve better clinical outcomes of portal hypertension in Indonesia. The International Journal of Medical Students (IJMS) has emerged over the past decade as a critical platform for showcasing medical student innovation and experiences. Though the work of trainees has historically been undervalued and over scrutinized, the IJMS is committed to highlighting the immense capacity for novel and robust research in this cohort. Thus, supporting an upcoming generation of leaders in medicine and academia to gain confidence in their work and contribute positively to the scientific community. In this issue of the IJMS, we are proud to present 16 articles from the Americas, Europe, Asia, and Africa. Original research articles cover a breadth of topics, including medical training, impacts of the COVID-19 pandemic on teaching and communication, pediatric respiratory illness, gender equity in medicine, understudied illnesses, and cardiovascular disease. The IJMS is proud to feature first-hand experiences of medical trainees in each issue. Accordingly, in the present issue perspectives of six medical students are outlined following unique and career-altering experiences. From working in palliative care to international outreach program, local vaccination initiatives, and the creation of a student-oriented research and innovation council in India. The IJMs extends our gratitude to our contributors, team, and readers for another remarkable issue.

Medical Journal of Tabriz University of Medical Sciences was initially published by the School of Medicine and Pharmacy in 1960 under the title of "The Journal of the Medical Faculty of Tabriz" (Fig 1-3) [Revue de la Faculté de Médecine et de Pharmacie from 1963 (Fig 4,5)], focusing on medical and pharmaceutical papers with English and French abstracts. The journal has been regularly issued, except for a short period of time, publishing original articles in the field of medical sciences. Meanwhile, it has been issued bimonthly since 2004 under the title of "Medical Journal of Tabriz University of Medical Sciences". I am pleased to announce that, as the result of the editorial and administrative teams’ efforts, we have recently met the requirements to be indexed in the Scopus database. This happened because of the sincere help and empathetic support provided by all my colleagues, authors, reviewers, the Vice-Chancellor for Research and Technology of TUOMS, and the Director of Medical Information and Scientific Resources. Currently, our vision is to promote the quality of the Journal to meet the requirements of Pubmed and ISI databases. To reach this aim, one of our strategic plans is to publish the Journal bilingually (in both Persian and English languages) to share domestic data globally and invite the researches from other countries to publish their works in our Journal. Hence, we seek to publish evidenced-based studies in several medical scopes, including basic, clinical, and health medicine. More rigorous criteria have been created to promote the quality of the Journal according to the regional and research priorities. Also, many researchers have been invited to review the submitted papers critically. We hope that scientists in different fields participate in broadening the scientific borders through submitting their state-of-the-art studies in our Journal. The editorial board is open to any comments or corrective recommendations. Background: health workers are one of the groups affected physically as well as psychologically from the pandemic. Recent studies showed many of the health workers reported experiencing anxiety, depression, and insomnia. This study aims to identify psychotherapy as a psychological health intervention, for healthcare workers during the COVID-19 pandemic. Methods: an evidence based clinical review of psychotherapy as a psychological health intervention, reported in the literature, which is developed for healthcare workers during the COVID-19 pandemic. The review was conducted following set out for Evidence-based clinical review by searching the following digital libraries: PubMed, ProQuest, Cochrane, and Google Scholar. Results: six publications were selected. The identified psychotherapy used as a mental or psychological intervention for healthcare workers during COVID-19 consists of supportive psychotherapy, psychoeducation, social support, and music therapy. Overall, it shows that psychotherapy, especially supportive psychotherapy, can reduce anxiety, depression, and insomnia. Conclusion: the physiological condition of health workers needs to get attention during the COVID-19 pandemic. Although there is no standard of therapy yet, psychotherapy could be an option as it is proven to be able to reduce anxiety, depression, and insomnia.

The clinical trial registry is an important platform to register clinical trials to be conducted, along with all the related information of the study, the institutions involved and the details about the authors. In the author's opinion, this disclosure of information can effect the blind peer review process. Although medical journal editors try best to evaluate all related information, relationships and conflicts, but the disclosure in trials registry can effect the doubleblind review process. This can also effect the reviewer's decsion and can be a source of bias. The authors of medical journals should consider this to further keep the review process transparent and unbiased. The British Journal of Anaesthesia (BJA) had an eventful 2021, following what was a cataclysmic 2020 for the whole world. Despite the tragic challenges of multiple waves of the COVID-19 pandemic and the unparalleled burdens this created for everyone working in anaesthesia and critical care, the BJA underwent a major transformation during 2021. The BJA strongly supported research and education relevant to the pandemic, and to the broader missions of anaesthesia, critical, and pain medicine. Innovations to the BJA in 2021 included a special section on COVID-19 and the Anaesthetist; a new open access journal in the BJA stable; creation of a new social media editor position; new webinar and author interview series; transition to a new manuscript management system; and a move away from paper to electronic publication. The article is written by the current Editor-in-Chief of the scientific journal Scripta Medica (Banja Luka) on the occasion of publishing the final issue of the fiftieth volume of the journal. Founded in 1966, the Scripta Medica (Banja Luka) started as a local journal of the Banja Luka Medical Centre, its General Hospital, later belonging to the Association of Medical Doctors of the Bosnian Krajina region and the Republic of Srpska and the Faculty of Medicine, University of Banja Luka. Its eight editors were: Professor Andrija Mikeš (1966-1968), Dr Dragomir Todorović (1969-1971), Assistant Professor Zvonimir Klepac (1972-1974), Professor Branko Pikula (1975-1990), Professor Bogdan Žigić (1995-2009), Professor Rajko Igić (2010-2013), Professor Predrag Grubor (2014-2018) and Professor Miloš P Stojiljković (since 6 August 2018). It used to appear twice a year or quarterly. The Scripta Medica (Banja Luka) has a peer-review process and tends to reach a status of well-established scientific journal.

Medical journals keep a critical role for reflecting on how clinical trials have evolved while researchers manage to overcome many challenges. 2020 will be the year of coronavirus for medical publications. As of 2020, The Covid-19 has announced as a pandemic viral disease by the World Health Organization. As of october 2020, when you search for ‘’Coronavirus-Covid-19’’ in pubmed; more than 40,000 results have already found their place in literature. Now we will try to review how relevant are the prestigious and popular medical journals on this topic.

Background: Altmetrics is a newly emerging scholarly tool measuring online attention surrounding scientific research outputs. With respect to increasing demand of disseminating research findings on the World Wide Web, this study aims to analyze the altmetric statues of Iranian medical journals. Methods: On February 27, 2019, the list of Iranian medical journals extracted from http://journals. research.ac.ir/ and consequently altmetric data token out from Altmetric database (Altmetric LLP, London, UK). The science mapping done via keyword co-occurrence, co-citation and co-authorship, network analysis using the VOSviewer. The Pearson coefficient was then employed for the correlation analysis using R. Results: Among a total of 104 journals, 7518 articles were mentioned in Altmetric data resources (Mean: 72.28, Confidence Level (95.0%): 16.8), total mentions were 27577 (Mean: 265.16, Confidence Level (95.0%): 79.9). Considering the total mentions of articles, International Journal of Preventive Medicine achieved the first rank, followed by Journal of Research in Medical Sciences and Iranian Journal of Public Health. Notably, Twitter was the most popular altmetric resource followed by Facebook and news outlets. Tweets were generally from the United States and United Kingdom. Among top 5% popular Iranian medical articles multiple sclerosis, cancer, and anxiety was hot topics. Conclusions: Iranian biomedical journal editors and research scientists needs to be more dynamic in World Wide Web using social media, post-publication peer review tools, Stack Exchange (Q and A) sites, research highlight tools, Wikipedia, and etc. In spite, more attention to the concept of evidence-based policymaking, by Iranian government along with the health policymakers seems necessary. Dear Reader, You are receiving the second volume of Archives of Pediatric Neurosurgery. This is an electronic journal from the Brazilian Society of Pediatric Neurosurgery, which is published quarterly. We are dedicating this volume to hydrocephalus-related topics. Hydrocephalus is one of the most important topics in pediatric neurosurgery and is related to up to 40% of the activities performed in many specialized services. With its multiple etiologies and variety of proposed treatment modalities, this disease presents a huge challenge, and many questions have not been answered. When focusing on fetal hydrocephalus, we still cannot determine which fetuses will benefit from treatment during the fetal period. The answers are also unclear as to why surgery for myelomeningocele correction performed under 27 weeks of gestation decreases the prevalence of hydrocephalus. What is the best treatment for intraventricular hemorrhage in premature infants: Brain Washing, external ventricular drainage, ventriculosubgaleal shunt, reservoir placement and repeated punctures or lumbar punctures? Furthermore, what is the best treatment for hydrocephalus in cases of posterior fossa tumor: Placing an external ventricular drain and resecting the tumor, performing a third ventriculostomy or placing a ventriculoperitoneal shunt, considering that in cases of giant tumors in each of these situations, ascending herniations can occur? Of course, the procedure selected will be related to the dynamics, volume and expertise of the treatment center. Regarding valves, we know that the number of shunts implanted annually is enormous and that the market for valves is divided among several companies that manufacture these prostheses. Which valve is the best? An anti-gravitational one, a programmable one, one with an anti-siphon mechanism? Fixed-pressure valves, valves that produce fewer MRI artifacts, those that do not experience pressure changes due to MRI, or those impregnated with antibiotics? How can a simple valve work as well as a valve with a complex system? Why does a valve system cost $1,000 in one country but costs ten times more in another country? Why are some types of valves sold only in underdeveloped countries? How can we avoid or minimize biological and mechanical complications after valve placement? Even neuroendoscopic procedures that have been performed for more than 25 years in Brazil are not free of complications such as bleeding, infection and technical incapacity of the professional. Nevertheless, we believe that all services that treat children with hydrocephalus should be able to provide neuroendoscopic procedures, thus decreasing the number of shunts placed. We hope that this volume can answer some of these questions. I invite you to participate in Archives of Pediatric Neurosurgery by contributing scientific articles focusing on pediatric neurosurgery. This volume was compiled during the coronavirus (COVID-19) pandemic. We hope that this disease will be controlled quickly, allowing us to return to our normal activities. We also hope that pregnant women will not transmit the disease to their fetuses, causing hydrocephalus, as was observed in the last Zika virus epidemic.
