Abstract Background Inflammatory bowel disease (IBD) commonly refers to ulcerative colitis (UC) and Crohn disease (CD), which are chronic inflammatory diseases of the gastrointestinal (GI) tract of unknown etiology. This study has been conducted to examine whether there are different components in the data, and if these components related to the treatment and the Inflammatory bowel disease (IBD) score at baseline. Methodology This is a clinical study which consisted of 291 subjects, who divided over four treatment arms and were measured during a seven-week period. The number of weeks in the period Week 0ne through Week seven was considered as the outcome of interest, as well treatment and IBD score at baseline were considered as predictors. Different statistical methods such as explanatory data analysis and finite mixture model were employed to explore the outcome of interest. Results From the finite mixture model, two components were obtained. Most of the patients, 196(67.4%), were classified in the first component (P1). The deviance for single component of the mixture model corrected for the covariates was 1049.3 and that of the two components was 948.8. The effect of ibdsc0 was significant in both subpopulations with p-value = 0.0001 for subpopulation1, and p-value = 0.0422 for subpopulation2, and Exp (0.01) = 1.01 and exp(0.087) = 1.09 are the amounts by which the mean count (µ) is multiplied per unit change in the ibdsc0 for subpopulations 1 and 2, respectively. Conclusions The two components are not related to the treatment, and as a result, the treatment does not completely explain the presence of potential clusters in the outcome. Ibdsc0 partially explains the presence of potential clusters in the outcome.

Abstract Polymeric nanoparticles (NPs) are promising tools used for immunomodulation and drug delivery in various disease contexts. The interaction between NP surfaces and plasma-resident biomolecules results in the formation of a biomolecular corona, which varies patient-to-patient and as a function of disease state. This study investigates how the progression of acute systemic inflammatory disease influences NP corona compositions and the corresponding effects on innate immune cell interactions, phenotypes, and cytokine responses. NP coronas alter cell associations in a disease-dependent manner, induce differential co-stimulatory and co-inhibitory molecule expression, and influence cytokine release. Integrated multi-omics analysis of proteomics, lipidomics, metabolomics, and cytokine datasets highlight a set of differentially enriched TLR4 ligands that correlate with dynamic NP corona-mediated immune activation. Pharmacological inhibition and genetic knockout studies validate that NP coronas mediate this response through TLR4/MyD88/NF-κB signaling. Our findings illuminate the personalized nature of corona formation under a dynamic inflammatory condition and its impact on NP-mediated immune activation profiles and inflammation, suggesting that disease progression-related alterations in plasma composition can manifest in the corona to cause unintended toxicity and altered therapeutic efficacy.

Abstract Background Studies of the world health organization indicated that Diabetes is on the rise. The occurrence of diabetes is steadily increasing everywhere, most markedly in the world’s middle and low-income countries. The aim of this study is to explore the consequence of war on the sugar level of diabetic mellitus patients. Methods A retrospective longitudinal study with a sample of 67 diabetic mellitus patients was used. As a result, longitudinal different models, which are generalized linear mixed effects and nonlinear mixed effect models were fitted on the continuous response variable, the Sugar Level of the diabetic patients. Results The results depicted that Blood Sugar Level of the patients increases over time. Moreover, as age, weight, medication, total cholesterol, high density lipoprotein (HDL), creatinine and linear time effect increase, Blood Sugar Level increases significantly, whereas triglyceride and low density lipoprotein increase, Blood Sugar Level of the adult Diabetes mellitus patients decreases. Conclusion The war has significant effect on the poor control of blood glucose level of the adult diabetic patients in Tigray region, Ethiopia. Due to the war, siege or blockages, in return there were rare of medicine, the patients were not taking their medicines on time, and they did not get enough insulin as well.

Abstract Background Worldwide, antimicrobial resistance (AMR) has grown to represent a serious threat to the diagnosis, management, and prevention of bacterial diseases. Due to their multidrug resistance attributes, the WHO has classified extended-spectrum-β-lactamase-producing Enterobacteriaceae (ESBL-PE)-associated infections as infections of critical significance, posing a serious risk to human health. Thus, the goal of this systematic review and meta-analysis was to assess the pooled prevalence of ESBL-PE and AMR among strains causing clinical infections in Africa. Methods In this systematic review and meta-analysis, two investigators independently made an electronic search in Google Scholar and PubMed databases using related keywords and corresponding “MeSH.” terms for the PubMed. The accessed studies were screened, assessed for eligibility, and critically evaluated as per the PRISMA guidelines. Abstract Introduction : Depression is a common and debilitating mental health issue among refugees in East Africa, who face numerous challenges. However, there is a lack of systematic reviews and meta-analyses that have explored the pooled prevalence and associated factors of depression among refugees in East Africa. This study aims to investigate the pooled prevalence of depression and its associated factors among refugees living in East Africa. Methods A systematic search was conducted across several databases, including PubMed/MEDLINE, CINAHL, ScienceDirect, African Journals of Online (AJOL), and Google Scholar. The quality of the included studies was assessed using a Joanna Briggs Institute (JBI) quality appraisal tool. Statistical analysis was carried out using STATA-17 software packages, and a meta-analysis was conducted using a random-effects model. Heterogeneity among the studies was assessed using the I2 statistic. Publication bias was evaluated using the DOI plot, Luis Furuya Kanamori (LFK) index, and Egger’s test. For associated factors of depression, effect sizes (odds ratio) with 95% confidence intervals were analyzed. Result A total of eight studies involving 6,388 participants were included in this systematic review and meta-analysis, all of which were assessed to have a low risk of bias. The pooled prevalence of depression was 50.60%, with a 95% CI (35.49%, 65.71). Regarding factors associated with depression; being female [(OR = 2.01; 95% CI (1.06, 3.82)], having poor social support [OR 5.88; 95% CI (2.53, 13.67)], and experienced eight or more traumatic events [OR = 3.31;95% CI (1.74, 6.31) were positively associated factors with depression. Conclusion The pooled prevalence of depression among refugees in East Africa was found to be significantly high. Female participants, poor social support, and experienced eight or more traumatic events were factors affecting depression among refugees in East Africa. Therefore, policymakers and health personnel in East Africa should prioritize addressing the needs of female participants, individuals with poor social support, and those who have experienced eight or more traumatic events.

Abstract Diarrhea is a major health problem in Ethiopia, and the Amhara Regional State is experiencing the highest prevalence rate of diarrhea. Therefore, this study was designed to assess the prevalence and risk factors attributed to diarrhea among children under-five in the Amhara Regional State. The study employed descriptive statistics and multivariable logistic regression analysis to identify the main statistically significant associated factors of diarrhea using Stata version 14. The overall prevalence rate of experiencing diarrhea was 17.7% [95% CI: 15.4, 20.2] in the Amhara Regional State. The multivariable logistic regression model analysis indicated that the risk factors are fourth and above birth order [AOR = 2.95, 95% CI (1.483, 5.85)], gender of child (being male) [AOR = 1.66, 95% CI (1.144, 2.41)], no education level of mother’s [AOR = 4.7, 95% CI (1.01, 22.0)], being age of child 7–11 [AOR = 3.33, 95% CI (1.58, 7.01)] and 12–23 month [AOR = 2.17, 95% CI (1.11, 4.22)], mother’s age at 15–19 years [AOR = 12.2, 95% CI (2.986, 50.2)], mother’s age at 20–24 years [AOR = 6, 95% CI (1.758, 20.8)], and mother’s age at 25–29 years [AOR = 4.48, 95% CI (1.41, 14.2)], and five and greater than family size [AOR = 0.53; 95% CI (0.343, 0.84)] were statistically significant associated with a higher risk of diarrhea. The prevalence of diarrhea in the Amhara Regional State was higher than that at the national level. The study identified gender of a child, education level of the mother’s, birth order, family size, mother’s age, and child’s age as risk factors for diarrhea in children under-five.

Abstract Background Academic performance is the extent to which a student, teacher, or institution has attained their short- or long-term educational goals and is measured either by continuous assessment or cumulative grade point average (CGPA). Good academic performance is defined as the students who scored a cumulative GPA of 3 and above. Even though few studies were done in Ethiopia, there was no clear evidence regarding the role of cognitive and learning issues such as academic competence, test competence, time management, strategic studying, and test anxiety on the academic performance of students. Objective To assess the magnitude academic performance and its associated factors among health science students at Eastern Ethiopian universities. Method An institution based cross sectional study design was implemented on a total of 924 regular undergraduate students of eastern Ethiopian universities. Good academic performance was measured as the students who scored a cumulative GPA of three and above. The study participants were selected using simple random sampling techniques, and a self-administered questionnaire was used to collect data. Abstract Background Hepatitis virus infection remains a serious public health concern worldwide. Percutaneous, mucosal, or non-intact skin exposure to infectious blood, semen, and other body fluids are the major routes of infection in endemic areas, including Ethiopia, where little is known about the community’s knowledge, attitudes, and practice toward hepatitis infection. Thus, this study aimed to determine the knowledge, attitude, and practices towards hepatitis infection among adults in Gondar town. Methods A community-based cross-sectional study was conducted from July to August 2021 on a total of 390 participants. Proportional sample allocation was used in 3 randomly selected sub-cities. Then, study participants from randomly selected households were recruited by using a systematic random sampling technique. Abstract Soybean, one of the most important oil seed crops known for its high-quality oil and protein content, is widely grown and consumed worldwide. However, the crop is severely affected by various biotic and abiotic factors, of which diseases are by far the most important, resulting in significant yield losses each year. The major diseases affecting soybean production include soybean rust, soybean mosaic virus, anthracnose, charcoal rot, Sclerotinia stem rot (white mold), seedling diseases, sudden death syndrome, frogeye leaf spot, and Stem canker. Development of soybean varieties resistant to these diseases using modern biotechnology techniques is a viable option to increase genetic potential and boost soybean production. This paper, hence, aims to explore the application of functional genomics in improving resistance to various diseases in soybean. Therefore, this paper provides a comprehensive overview of the progress made in improving soybean resistance to major diseases by using the various approaches of modern biotechnological tools such as molecular markers, QTL/gene mapping, omics technology, freely available online databases, genome editing, genetic modification, and marker-assisted breeding. This review also highlights future directions that may be important for genomics-based research programs to improve disease resistance in soybean while increasing production.

Abstract Background Evidence generated on utilization of maternal health services during times of conflict can inform the development of targeted interventions. The deadly war in Tigray caused the health system to collapse. However, utilization of maternal health services is not well documented that might help in design effective interventions applicable during armed conflict. Thus, this study aimed to assess maternal health service utilization and bridge the gap in healthcare provision and support during conflict. . Methods A community-based cross-sectional survey was conducted among mothers of children under the age of 1 year in six accessible zones of Tigray from August 4–20, 2021. The study participants were selected through a random sampling method. The study was conducted during the armed conflict where the access to health services was limited due to the damaged and looted health facilities. A descriptive study was used to characterize the study population and data were presented using proportions and percentages. A Chi-square test was used for categorical variables and a P-value of 0.05 was considered significant. . Results A total of 4,381 participants were included in the study. Modern contraceptives were used by 1002 (22.9%) of women, and injectable contraceptives were the most common method used by 472 (47.1%) women. Likewise, only 830 (36.5%),1956 (47.1%), and 623 (15.0%) respondents reported the use of optimal antenatal care (ANC), skilled delivery, and postnatal care (PNC) in the first 24 h by mothers, respectively. About 34% of women received comprehensive abortion care services. Most delivery services took place in hospitals. Abstract Introduction The majority of pregnancy-related complications were preventable. However, the magnitude of complications during pregnancy and childbirth was high in sub-Saharan Africa. There was limited evidence on the magnitude of obstetric complications during childbirth and its predictors among postpartum women in Ethiopia. Methods and materials The research used secondary data sources based on performance monitoring for action in Ethiopia’s second cohort longitudinal survey. It was a national survey that was conducted in three large, predominantly agrarian regions (Oromia, Amhara, and SNNP) and one urban region (Addis Ababa) of Ethiopia between 2021 and 2023. It was conducted to track pregnant women’s use of essential maternal and newborn health services. It involved the enrollment of pregnant women and tracking them at six weeks, six months, and one year after giving birth. The data collected at the beginning of the survey and six weeks after childbirth were used for the analysis. Sample weights were applied to account for differences in sample allocation and response rates across regions and urban/rural areas. Multi-collinearity and intra-cluster correlation were assessed before fitting the multilevel models to ensure the accuracy of the estimates. A multi-level logistic regression model was used to assess the magnitude and predictors for the occurrence of obstetric complications during delivery. The model fit was evaluated using Akaike’s Information Criterion and Bayesian Information Criterion. Adjusted odds ratio with its 95% confidence interval was used to measure the strength of association for this study. Results The magnitude of obstetric complications during delivery was found to be 33.86% with 95% CI (31.56, 36.24. Among the obstetric complications that occurred during delivery, about 15.73%with 95% CI(14.02, 17.61) of women experienced bleeding, 4.14% with 95% CI (3.30, 5.17) had their membrane rupture but labor did not start within 24 h, 2.29% with 95% CI(1.67, 3.13) had their membrane rupture before 9 months, 3.95% with 95% CI(3.16, 4.93) had faced malpresentation or malposition of the baby, 12.70% with 95% CI (11.18, 14.39) had prolonged labor lasting more than 12 h, and 12.40% with 95% CI (10.83,14.17) had convulsions. The odds of occurrence of obstetric complication during delivery among women from severely food insecure households were 1.88 times [AOR = 1.88; 95% CI (1.22, 2.90)] more likely to occur than women from food secure households. Moreover, the odds of occurrence for obstetric complications during delivery among women who had complications during their pregnancy were 2.39 times [AOR = 2.39; 95% CI (1.81, 3.16)] more likely to occur as compared to those women who had no complication during their pregnancy. On the other hand, women’s who had 1–4 live births given before this delivery were 0.61 times [AOR = 0.61; 95% (0.43, 0.88)] less likely to develop complications as compared to those women’s who had no prior live birth. Conclusion and recommendation Delivery-related obstetric complications during delivery in Ethiopia were high. Approximately one-third of postpartum women in Ethiopia experienced obstetric complications during delivery. Based on this study, women’s from severely food insecure households, those women’s who had no prior live birth, and women’s who had complications during their pregnancy were more likely to develop delivery-related obstetric complications. Thus, policy makers and program implementers who were working on maternal and newborn health should give special attention for women’s from severely food insecure households, women’s who had no prior live births, and those women’s who encountered obstetric complications during their pregnancy to decrease the occurrence of obstetric complications during delivery. In Ethiopia, various governmental organizations, such as the Ministry of Health and the Ministry of Agriculture, are dedicated to addressing food insecurity and improving nutritional access. The country has launched several effective nutritional programs, including the Productive Safety Net Program, the Targeted Supplementary Feeding Program, and the National Nutrition Program, aimed at alleviating food insecurity and enhancing nutrition. Ongoing efforts are crucial to tackle the food insecurity experienced by women, which can help to reduce obstetric related complications of women’s during delivery. It is crucial for everyone involved in maternal and newborn health to prioritize addressing the factors that lead to obstetric complications during delivery in women in order to reach the goal of ending all preventable maternal and newborn deaths by 2030.

BackgroundPreeclampsia is the leading cause of maternal and fetal morbidity and mortality. Calcium supplementation has been considered a potential intervention to reduce the risk of preeclampsia. This umbrella review aims to summarize the effects of calcium supplementation in the prevention of preeclampsia based on existing systematic reviews and meta-analyses studies.MethodsA systematic search of electronic databases, such as MEDLINE, Web of Science, SCOPUS, and the Cochrane Library, was conducted from inception to 30 December 2023. The methodological quality of the included studies was assessed using the revised version of the Assessing the Methodological Quality of Systematic Reviews (AMSTAR 2) tool. A random-effects model was used to estimate the effect of calcium supplementation on preeclampsia. Heterogeneity among included studies and publication bias were assessed using the I2 statistic and the Egger’s test, respectively.ResultsCalcium supplementation reduced the risk of preeclampsia by 47% (RR: 0.53, 95% CI: 0.42, 0.68) with a considerable level of heterogeneity (I2 = 84.39%). Our subgroup analyses revealed that the risk of preeclampsia was significantly lower in high-risk pregnancies that received calcium supplementation (RR: 0.35, 95% CI: 0.26, 0.47), indicating a 65% risk reduction. In comparison, low-risk pregnant women who received calcium supplementation experienced a 33% risk reduction (RR: 0.67, 95% CI: 0.59, 0.77). Furthermore, the effects of calcium supplementation were more pronounced in women from developing countries compared to those from developed countries.ConclusionThis umbrella review provides a summary of the evidence supporting the use of calcium supplementation to reduce preeclampsia. Incorporating calcium supplementation into antenatal care interventions may help to reduce the burden of preeclampsia and improve maternal and fetal outcomes. Further studies are needed to explore the impact of baseline calcium levels, optimal dosage, timing, and routes of supplementation to effectively decrease the incidence of preeclampsia.

Despite increased availability of safe abortion following legal reform in Ethiopia, one-half of public sector abortion services are treatment of postabortion complications, indicating challenges meeting women's needs. Self-managed abortion (SMA)—the ability of pregnant people to manage their unwanted pregnancies with or without the support of a health care provider—is a safe, feasible, and acceptable option for women at gestational ages up to 12 weeks. Seeing the potential of SMA to vastly expand access to safe abortion and reduce postabortion complications, a pilot initiative targeting private pharmacies was implemented by the Ethiopian Ministry of Health, the Oromia Regional Health Bureau, and a non-profit organization. From December 2021 to March 2023, implementers trained and supported 41 pharmacies to provide SMA counseling and medical abortion drugs, with and without prescriptions, which was considered to contradict the legal framework at the time. Pharmacy clients' SMA experiences were documented in logbooks and via 21-day follow-up phone surveys. Thirty-two pharmacies (78%) supported 1,457 self-managed abortions during the pilot. Among clients with complete follow-up surveys (n = 1,233), 98.3% had a complete abortion without needing additional treatment. Only four clients (0.3%) reported a complication. The pilot demonstrated high demand for and feasibility of increasing access to quality SMA through private pharmacies in Ethiopia, but challenges remain due to the lack of a legal framework. We recommend providing multi-sector support on SMA to private pharmacies so they can in turn improve safe abortion accessibility by bringing safe, acceptable services closer to the people that need them.

IntroductionStroke is a leading cause of adult neurologic disability, cognitive decline, and death worldwide, and particularly in Africa. Stroke research in Africa has exposed challenges militating against the translation of research evidence into practice and policy. The evidence-based, context-sensitive multilevel strategies required to surmount these challenges are presented in this report on the first African Stroke Leaders' Summit (ASLS) organized to tackle the burden of stroke in Africa.MethodsThe Africa–UK Stroke Partnership (AUKSP) Project had a Steering Committee (SC) and four theme-based Working Groups (WGs): stroke services, stroke training/capacity building, research and stroke advocacy, each with defined terms of reference. These groups generated 20 priorities (five per thematic area) during breakout sessions at the first ASLS which were further refined into four topmost priorities (one per thematic area) at the general consensus session.ResultsThe topmost priorities included promoting the development of acute stroke services (stroke services), strengthening population-based stroke education focusing on prevention and symptom recognition (stroke training), research on hypertension control to reduce stroke risk (stroke research), and developing national stroke action plans (advocacy).ConclusionSustained reduction of stroke burden in Africa requires the adaptation of best practices to the African context, building the capacity of African stroke care professionals and using available resources with political support. Improving stroke literacy in African communities is a complementary strategy to reinforce healthy lifestyle choices and improve screening and detection of hypertension and other modifiable stroke risk factors. This process will culminate in a strategic African Stroke Action Plan (ASAP), the blueprint for the control of stroke in Africa.

Objective Beneficiaries’ satisfaction with health insurance schemes is crucial for the success of these programmes, influencing their effective implementation and reducing dropout rates. This systematic review and meta-analysis aimed to assess the proportion of beneficiaries satisfied with health insurance and identify factors associated with their satisfaction in Sub-Saharan Africa (SSA).Design A systematic review and meta-analysis were conducted in accordance with the Preferred Reporting Items for Systematic Review and Meta-analysis guidelines.Data sources PubMed/MEDLINE, African Journals Online, Cumulative Index to Nursing & Allied Health Literature and Google Scholar were searched up to 23 May 2024.Eligibility criteria Observational and quantitative studies conducted in SSA that reported the proportion of health insurance beneficiaries satisfied with the scheme and/or determined factors associated with satisfaction were included.Data extraction and synthesis Data were extracted using Microsoft Excel and analysed with STATA V.17 software. The quality of studies was assessed using Joanna Briggs Institute checklists. A random-effects model was employed to estimate pooled outcomes. Publication bias was evaluated with a funnel plot and Egger’s regression test, while heterogeneity was assessed using the I² statistic.Result The study included 29 primary studies with a total of 11,488 participants. Among the included studies, 17 (58.62%) were found to have a low risk of bias, while the remaining studies exhibited a moderate risk of bias. The findings suggested publication bias among the included studies. To address this, a trim-and-fill analysis imputed 10 hypothetical missing studies, resulting in a more symmetrical funnel plot. Background The WHO and Ethiopia’s Ministry of Health have developed strategies to expand and integrate services for co-endemic neglected tropical diseases (NTDs) which manifest in the skin. To inform these strategies, we aimed to understand the social, economic and health system context of skin NTD care in Kalu woreda, Amhara region, Ethiopia, where cutaneous leishmaniasis (CL) and leprosy are endemic.Methods Between October 2020 and May 2022, we surveyed and reviewed records of 41 primary healthcare facilities and explored common disease experiences in focus group discussions (n=40) and interviews with people affected by leprosy (n=37) and CL (n=33), health workers (n=23), kebele authorities and opinion leaders (n=33) and traditional healers (n=7). Opportunities for integrated skin NTD service provision were explored through policy document review, interviews with health officials (n=25), and stakeholder meetings.Results Availability of diagnostic supplies and health worker competence to provide skin care was very limited across primary healthcare facilities, particularly for CL. People with leprosy commonly sought care from healthcare facilities, while people with CL administered self-care or sought help from traditional healers. Travel and costs of care at specialised facilities outside the district inhibited timely care-seeking for both diseases. Transmission discourses shaped different understandings of who was affected by leprosy and CL and expectations of behaviour during and after treatment. Many policy actors felt that existing supply chain interventions, decentralised treatment approaches and community engagement initiatives for leprosy could also benefit CL, but others also warned against increasing care-seeking unless CL treatment could be provided on a scale commensurate with the large burden they perceived.Conclusion Our findings demonstrate significant gaps in the provision of care for skin NTDs within primary healthcare, very different health-seeking patterns for leprosy and CL, and a need to develop new models of care, especially for CL.

Diarrheal disease is a significant public health concern and causes the death of approximately 525,000 children under the age of 5 every year. In Ethiopia, studies revealed that the prevalence of diarrhea among children under 5 years is alarming. However, there has been a shortage of studies regarding the predictors of diarrheal disease in Geshiyaro project implementation sites in Ethiopia. Therefore, this study aimed to fill this gap. Introduction: Pain management is crucial for improving patients' quality of care. Persistent pain has been linked to higher depression, anxiety, and work-related difficulties. This study aimed to enhance the time to pain relief in the emergency department through triage nurse-initiated analgesia. It evaluated the impact of nurse-led analgesia on patient satisfaction compared to standard pain management at Tikur Anbessa Specialized Hospital and Kidus Paulos Specialized Hospital. Additionally, it compared the time to analgesia between the two hospitals and assessed the effect of nurse-led analgesia on reducing the length of stay for patients with pain. Data was collected using an open data kit, and after ensuring data completeness, it was exported to SPSS and Excel for analysis. To assess the effectiveness of the intervention, the time to analgesia was compared between the intervention and control groups using an independent samples t-test. This statistical test allowed for a comparison of the mean time to analgesia between the two groups.Patient satisfaction scores were also compared between the intervention and control groups using the Mann-Whitney U test. Kaplan-Meier curves were employed to compare the time to analgesia between the intervention and control groups within both settings. A point bi-serial correlation analysis was performed to examine the association between the length of stay and the intervention of nurse-led analgesia in both hospital settings. Result and discussion: The study enrolled 179 participants, with a median age of 34 years (range: 9–80) and 67% female. The most common events leading to pain were medical conditions (21%), followed by trauma/quarrel/war, fall accidents, and underlying diseases (15%, 13%, and 13%, respectively). There was a significant correlation between the degree of pain on arrival and time to analgesia. Additionally, a significant correlation (p < 0.01) was found between time to analgesia and patient satisfaction. Conclusion and recommendation: Implementing a nurse-led analgesia protocol in the emergency department is crucial for reducing time to analgesia and improving patient satisfaction. It is recommended to scale up this approach to other healthcare facilities by incorporating it into the nursing practice guidelines of the country.
