Introduction: This study examined the difference in overall survival (OS) between peritoneal metastatic gastric cancer patients who underwent neoadjuvant chemotherapy followed by cytoreductive surgery±hyperthermic intraperitoneal chemotherapy (CRS±HIPEC) and those who did not have surgery but instead received palliative chemotherapy. Methods:This retrospective study included 80 patients who were followed up with the diagnosis of peritoneal metastatic gastric cancer, those undergoing neoadjuvant chemotherapy followed by CRS±HIPEC (CRS±HIPEC group) and those receiving chemotherapy only (non-surgical group), in the medical oncology clinic between April 2011 and December 2021. Clinicopathological features, treatments and OS of the patients were compared. Results:There were 32 patients in the SRC CRS±HIPEC group and 48 in the non-surgical group. In the CRS±HIPEC group, CRS+HIPEC was performed in 20 patients and only CRS was performed on 12 patients. All of the patients who underwent CRS+HIPEC, and 5 of the patients who underwent only CRS received neoadjuvant chemotherapy. WOS: 000388782200027 PubMed ID: 27837629 Purpose: Small bowel adenocarcinoma (SBA) is a rare tumor of the gastrointestinal system with poor prognosis. Since these are rarely encountered tumors, there are limited numbers of studies investigating systemic treatment in advanced SBA. The purpose of this study was to evaluate the prognostic factors and systemic treatments in patients with advance SBA. Methods: Seventy-one patients from 18 Centers with advanced SBA were included in the study. Fifty-six patients received one of the four different chemotherapy regimens as first-line therapy and 15 patients were treated with best supportive care (BSC). Results: Of the 71 patients, 42 (59%) were male and 29 (41%) female with a median age of 56 years. Median follow-up duration was 14.3 months. The median progression free survival (PFS) and overall survival (OS) were 7 and 13 months, respectively (N=71). In patients treated with FOLFOX (N=18), FOLFIRI (N=11), cisplatin-5-fluoroura-cil/5-FU (N=17) and gemcitabine alone (N=10), median PFS was 7, 8, 8 and 5 months, respectively, while median OS was 15, 16, 15 and 11 months, respectively. No significant differences between chemotherapy groups were noticed in terms of PFS and OS. Univariate analysis revealed that chemotherapy administration, de novo metastatic disease, ECOG PS 0 and 1, and overall response to therapy were significantly related to improved outcome. Only overall response to treatment was found to be significantly prognostic in multivariate analysis (p = 0.001). Conclusions: In this study, overall response to chemotherapy emerged as the single significant prognostic factor for advanced SBAs. Platin and irinotecan based regimens achieved similar survival outcomes in advanced SBA patients.

To the Editor: Antonarakis and colleagues (Sept. 11 issue) 1 reported a significant association between the presence of androgen-receptor splice variant 7 messenger RNA (AR-V7) in circulating tumor cells and shorter prostate-specific antigen (PSA) progression–free survival, clinical or radiographic progression–free survival, and overall survival in men with metastatic castration-resistant prostate cancer who were treated with enzalutamide or abiraterone. Several treatment options (abiraterone, enzalutamide, sipuleucel-T, and radium-223) have recently been shown to prolong survival in men with metastatic castration-resistant prostate cancer who have not previously been treated with chemotherapy; the identification of reliable biomarkers that can predict response should optimize treatment with these approaches. A study by Loriot and colleagues 2 suggests that the duration of sensitivity to previous androgen-deprivation therapy (ADT) is predictive of the efficacy of subsequent endocrine manipulations in patients with castrationresistant prostate cancer. Loriot et al. found that 58% of patients with a duration of sensitivity to previous therapy of at least 16 months had a PSA response to subsequent treatment, as compared with 18% of patients with a duration of sensitivity of less than 16 months (P = 0.01). Antonarakis et al. observed no PSA response to abiraterone or enzalutamide in men who tested positive for AR-V7. We wonder about the relationship between AR-V7 positivity and the duration of response to previous ADT.
