Introduction Most people with schizophrenia in China are supported by their family members in community. The patient’s family is confronted with severe care burden and pressure, which directly affects the caregiver’s own health and social life, and indirectly affects the patient’s rehabilitation. Adequate family resources can reduce the burden and pressure on families. But there is an absence of systematic family resource indicators for people with schizophrenic disorder in China.Objectives This study aimed to develop a set of family resource indicators for people with schizophrenic disorder in China.Design Preliminary family resource indicators were generated and refined by literature review and an expert consultation meeting. Two rounds of email-based Delphi survey were carried out to identify family resource indicators.Setting Two rounds of email-based Delphi survey were performed from July to September 2021 in Beijing, China.Participants There were 15 mental health doctors from community health service centres and four psychiatrists from tertiary hospitals, and two primary care researchers from universities in the first and second rounds Delphi survey.Results All the 21 experts participated in both rounds of Delphi survey. A total of 46 indicators achieved consensus for inclusion in the final set of indicators after two rounds of Delphi survey. The final set of indicators was grouped into 10 domains: financial support (three indicators), psychological and spiritual support (eight indicators), medical treatment (three indicators), information and education (three indicators), structural support (two indicators), external family resources included social resources (five indicators), cultural resources (two indicators), economic resources (seven indicators), environmental resources (four indicators) and medical resources (nine indicators).Conclusions A set of 46 family resource indicators for people with schizophrenic disorder in community was identified by an iterative Delphi process in Beijing, China. However, the indicators still need to be validated by testing in further studies.

The structural morphology of mesenteric artery vessels is of significant importance for the diagnosis and treatment of colorectal cancer. However, developing automated vessel segmentation methods for this purpose remains challenging. Existing convolution-based segmentation methods have limitations in capturing long-range dependencies, while transformer-based models require large datasets, making them less suitable for tasks with limited training samples. Moreover, over-segmentation, mis-segmentation, and vessel discontinuity are common challenges in vessel segmentation tasks. To address these issues, we propose a parallel encoding architecture that combines transformers and convolutions to retain the advantages of both approaches. The model effectively learns position deviations and enhances robustness for small-scale datasets. Additionally, we introduce a vessel edge capture module to improve vessel continuity and topology. Extensive experimental results demonstrate the improved performance of our model, with Dice Similarity Coefficient and Average Hausdorff Distance scores of 81.64% and 7.7428, respectively.

Vascular images contain a lot of key information, such as length, diameter and distribution. Thus reconstruction of vessels such as the Superior Mesenteric Artery is critical for the diagnosis of some abdominal diseases. However automatic segmentation of abdominal vessels is extremely challenging due to the multi-scale nature of vessels, boundary-blurring, low contrast, artifact disturbance and vascular cracks in Maximum Intensity Projection images. In this work, we propose a dual attention guided method where an adaptive adjustment field is applied to deal with multi-scale vessel information, and a channel feature fusion module is used to refine the extraction of thin vessels, reducing the interference and background noise. In particular, we propose a novel structure that accepts multiple sequential images as input, and successfully introduces spatial-temporal features by contextual information. A further IterUnet step is introduced to connect tiny cracks caused using CT scans. Comparing our proposed model with other state-of-the-art models, our model yields better segmentation and achieves an average F1 metric of 0.812.

Abstract The magnitude and spatial pattern of anomalous net biome exchange (NBE) induced by the 2015/16 El Niño over Amazonian rainforests remain uncertain. We here investigated them using multi‐model posterior NBE products in the Orbiting Carbon Observatory‐2 (OCO‐2) version 10 modeling intercomparison project. Results suggest that relative to the annual NBE average in 2017/18, larger anomalous carbon release occurred over the eastern and northern Amazonian rainforests in 2015/16, with a total flux of approximately 0.4 PgC yr−1 after assimilating satellite‐observed column CO2 concentrations (XCO2) over land. We further find that this anomalous spatial pattern was predominantly determined by soil dryness, while the total positive NBE anomaly was dominated by higher temperature with its contribution of approximately 68～70%. We believe that atmospheric inversions assimilating more satellite‐observed XCO2 in future can provide us more comprehensive understanding how Amazonian rainforests cope with the abiotic stresses induced by El Niño events.

Abstract Background Family doctor service (FDS) is a scheme oriented to improving the access and continuity of primary care in China. Type 2 diabetes mellitus (T2DM) management is a core component of FDS. However, evidence on the quality of T2DM care is lacking and the potential association between FDS attributes and T2DM care is largely unknown. This study attempted to assess the process quality of T2DM care in general practice and explore the association between patient perceived FDS attributes and process quality of T2DM care. Methods Total 400 patients were recruited from 5 community health service centers in two urban districts in Beijing. Questionnaire survey and extraction of data from electronic health record (EHR) were conducted to collect patient characteristics, patient perceived FDS attributes (accessibility, continuity and team-based care) and process quality indicators (monitoring and health counseling indicators). Chi-square test and a two-level generalized linear mixed model (GLMM) were used to explore the association between FDS attributes and process quality. Results The utilization rate of all the 12 indicators in monitoring, 6 indicators in health counseling and all the 18 process indicators, was 12.8%, 23.8% and 6.0% respectively. Over half of the patients (56.8%) perceived all the 3 FDS attributes. This study aims to investigate the impacts of the spring sensible heat (SH) over the Tibetan Plateau (TP) and the El Niño–Southern Oscillation (ENSO) in the preceding wintertime on midsummer (July–August) precipitation over South China under the different Pacific decadal oscillation (PDO) phases. More specifically, eight classifications are adopted at the demarcation point around 1996 when the spring SH over the TP and the midsummer precipitation in South China occurred as well as the PDO phase transition, including positive and negative SHs and ENSOs under a positive PDO phase (1979–1996) and a negative PDO phase (1997–2019), respectively, based on the Niño-3 index and the spring SH calculated from 48 stations over the central and eastern parts of the TP. The results show that both the spring SH and the ENSO in preceding wintertime have a significant impact on the midsummer precipitation over South China; that is, when the two factors are in their respective positive (negative) phase, the midsummer precipitation in South China is generally less (more). Importantly, the phase change of background field PDO can significantly enhance the effect of the SH and the ENSO on summer precipitation over South China. Moreover, compared with the preceding wintertime ENSO, the spring SH over the TP contributes more to the midsummer precipitation in South China based on analyses of their independent and synergistic effects. The main mechanism responsible for the anomalous midsummer precipitation over South China are the combined effects of the South Asian high (SAH) and the western Pacific subtropical high (WPSH), which are controlled by the spring SH anomaly over the TP and the ENSO, respectively. Deep understanding of the dominant factors of the midsummer precipitation over South China will help understand the local climate change and reduce the losses caused by drought and flood disasters.

Recent years have witnessed contrasting trends in summer total rainfall (STR) over the Tibetan Plateau (TP), with an increase in the northern and a decrease in the southern TP. This study identifies four significant centers of rainfall trends: eastern TP (“region A”), Qiangtang Plateau (“B”), Qaidam Basin (“C”), and the northern foothills of the Himalayas (“D”). Heavy rainfall dominates STR trends in regions A and D, accounting for 55.6% and 52.0%, respectively. In region B, moderate and light rainfall contribute almost equally, accounting for 37.3% and 44.8% of the STR trend, respectively. Region C is primarily influenced by light rainfall, explaining 71.2% of the STR trend. Notably, the contributions of different rainfall intensities to STR in each region vary annually, with region A experiencing more heavy rainfall, region B having moderate dominance but less light rainfall, and region C and D showing reduced and increased light rainfall contributions, respectively. Mechanistically, the strengthening of the upper-level westerly jet and the South Asian High, coupled with changes in moisture transport and convective available potential energy, collectively cause variations in rainfall intensity, characterizing the spatial heterogeneity in STR in the TP.

【Objective】As an important vegetable, Brassica juncea has high economic value due to its edible and processed characters. However, the possible seed dormancy after harvesting is harmful to agricultural production, hence, studies on seed germination and vigor may contribute to understanding the physiological rules and timely sawing of B. juncea seeds.【Method】In order to study the relationship between temperature and seed vigor and dormancy, B. juncea seeds harvested in 2017 and 2020 were used to analyze indexes of seed germination, seedling number, seedling length, mean germination time (MGT), and seedling vigor index (SVI) at 25℃ (CK), 4℃ and -10℃ with different treatment time.【Result】Seeds harvested in 2017 (hereinafter called 2017 seeds) had no dormancy but seeds harvested in 2020 (hereinafter called 2020 seeds) did have instead. And vigor indexes of 2020 seeds were extremely higher than those of 2017 seeds. The germination rate of 2017 seeds was less than 20%, and SVI was less than 1.5, however, the germination rate of 2020 seeds was higher than 60%, and SVI was more than 5. Especially, the germination rate of 2020 seeds was increased to 80% under -10℃ treatment.【Conclusion】Short time (24 h) treatment at 4℃ was adverse to seed germination of B. juncea seeds, but -10℃ treatment under dry condition was beneficial to breaking seed dormancy and raising seed vigor. Finally, seed storage time should be controlled in 1 year.

Fine particulate matter (PM2.5) has been widely reported to contribute to the pathogenesis of pulmonary diseases. The direct hazardous effect of PM2.5 on the respiratory system at high concentrations in vitro and in vivo have been well identified. However, its effect on the pre-existing respiratory diseases of patients at environment-related concentrations remains unclear. Diesel exhaust PM2.5 as a primary representative of ambient PM2.5 fine particles were used to investigated the effect of PM2.5 on the fibrosis progression of existing pulmonary fibrosis disease models. This study reported that PM2.5 could result in the enhanced sensitivity to fibrotic response, which may be ascribed to ferroptosis induced by PM2.5 in damaged lung areas. Proteomic analysis revealed that the upregulation of HO-1 as a key mechanism in the ferroptosis and exacerbation of pulmonary fibrosis induced by PM2.5. As a result, HO-1 degraded heme-containing protein and released iron in fibrotic cells, leading to generation of mitochondrial ROS and impaired mitochondrial function. Transmission electron microscopic assay verified that PM2.5 entered the mitochondria of fibrotic cells and was accompanied by significant mitochondrial morphological changes characterized by increased mitochondrial membrane density and reduced mitochondrial size. The HO-1 inhibitor zinc protoporphyrin and mitochondrion-targeted antioxidant Mito-TEMPO significantly attenuated PM2.5-induced ferroptosis and exacerbation of fibrosis. In addition, AMPK-ULK1 axis-triggered autophagy activation and NCOA4-mediated degradation of ferritin by autophagy were found to be related to the PM2.5-induced ferroptosis of fibrotic cells. As evidenced by the inhibition of autophagy with 3-methyladenine or AMPK inhibitor, NCOA4 knockdown decreased intracellular iron accumulation and lipid peroxidation, thereby relieving PM2.5-induced epithelial–mesenchymal transition and cell death in fibrotic cells. Overall, this study provided experimental support for the idea that PM2.5 greatly deteriorates fibrosis process in pre-existing pulmonary fibrosis, and HO-1-mediated mitochondrial dysfunction and NCOA4-mediated ferritinophagy are jointly required for the PM2.5-induced ferroptosis and enhanced fibrosis effects.

With the continuous development of nanobiotechnology in recent years, combining photothermal materials with nanotechnology for tumor photothermal therapy (PTT) has drawn many attentions nanomedicine research. Although nanomaterial-mediated PTT is more specific and targeted than traditional treatment modalities, hyperthermia can also damage normal cells. Therefore, researchers have proposed the concept of low-temperature PTT, in which the expression of heat shock proteins (HSPs) is inhibited. In this article, the research strategies proposed in recent years based on the inhibition of HSPs expression to achieve low-temperature PTT was reviewed. Folowing this, the synthesis, properties, and applications of these nanomaterials were introduced. In addition, we also summarized the problems of nanomaterial-mediated low-temperature PTT at this stage and provided an outlook on future research directions.

Objectives This study aims to investigate the access to family resources by families living with schizophrenia from the perspective of primary care workers in Beijing, and provide evidence for appropriate and effective family resource coordination in primary care.Design Qualitative research using individual in-depth interviews to identify the access to family resources by families living with schizophrenia from the perspective of primary care workers.Setting This study was conducted from September to December 2021 in six urban community health service centres (CHSCs) in Beijing, China.Participants 3 general practitioners and 10 mental health doctors selected by purposive sampling method from 6 CHSCs in urban Beijing were interviewed.Results Five themes emerged from the insights of the primary care workers: most family resources are non-targeted for families living with schizophrenia, the publicity of family resources is difficult, burdensome application process of family resources, limited available community-based treatment options and stigma hindering effective communication between families and society.Conclusions It is necessary to simplify the application process of family resources and provide primary care workers with systematic training regarding family resources. More family resources and improved public attitudes should be promoted for patients with schizophrenia and their caregivers.

Abstract Background The “5 + 3” residency training is the main stream general practitioner training program in China. However, a competency model is absent for evaluating the clinical competence attained by general practitioners after training. This study was conducted to develop a consensus set of competencies for general practitioners after standardized residency training in China. Methods A modified Delphi process was deployed to develop the competency model, including two stages: (1) generation of an initial set of competencies derived from literature review, behavioral observation of GP–patient consultations, and critical incidents interview of GPs; (2) a 2-round, web-based Delphi survey of experts in general practice, selected using purposive sampling, to prioritize and gain consensus on the essential competencies of GPs. Results From literature review, behavioral observation, and critical incidents interview, 46 competencies in 7 domains were identified. After two rounds of Delphi survey of 28 participants (the mean age was 47.9 [9.3] years and 64.3% were women) representing a range of health professionals (GPs, managers, and researchers), a consensus was reached on 50 competencies categorized into 7 domains. Conclusion A consensus-based competency model for general practitioners in China has been identified which may be used to evaluate the general practitioners’ clinical competence after standardized training.

The spring sensible heating (SSH) over the Tibetan Plateau (TP), which can significantly affect the precipitation in China, has experienced three different stages of change, showing significant increasing (1961–1979, Stage I), decreasing (1980–2002, Stage II), and increasing (2003–2014, Stage III) trends. In this study, the impact of these different trends in TP SSH on spring precipitation (SPR) in China and their possible mechanisms are investigated, based on observations and the reanalysis product. In Stage I, the SPR represents a contrasting north-south pattern associated with the increasing TP SSH, showing increasing trends over southern China and decreasing trends over central and northern China. Further, the spatial distribution of SPR trends shows a contrasting east-west pattern in Stage II. That is, persistent weakening TP SSH plays a more crucial role in increasing and decreasing SPR over southwestern and southern China, respectively. However, compared with the significant trend in SPR in Stage III, the regulation of TP SSH on SPR weakens significantly. Dynamically, the increasing TP SSH in Stage I can strengthen the subtropical westerly jet in the upper layer, simultaneously configured with an anomalous cyclone in northeastern China, which deepens the East Asian trough. Thus, anomalous convergence in the upper layer occurs over central and northern China, favoring the downdraft. It then causes more cold and dry air to move southward in the lower troposphere, which then encounters the warm and wet southwest airflows, boosting the updraft over southern China. In Stage II, regression analysis shows that if the TP SSH increases, an anomalous cyclone in the middle and upper troposphere occurs over the western TP, causing the downdraft and less precipitation over southwestern China, while a cyclone in the lower troposphere occurs over the western North Pacific and extends to southern China, promoting the ascending motions and more precipitation in southern China. However, in this stage, TP SSH actually weakens, thus contributing to more precipitation over southwestern China and less precipitation over southern China.

Abstract Background In an era of an increasingly ageing society, part of healthcare for older adults can be provided in patients’ homes, and the need for home health care services (HHCSs) is increasing. This study sought to determine whether a gap exists between the views of community health professionals and the caregivers of disabled older adults towards HHCSs in Beijing, China. Methods A cross-sectional study with two comparative questionnaire surveys was conducted in Beijing, China. One survey was administered to the caregivers of disabled older adults, and the other was administered to health professionals in community health service institutions (CHSIs). T-tests and Wilcoxon signed-rank tests were used to explore potential differences between the views of caregivers and community health professionals towards HHCSs. Results We received 370 valid questionnaires from caregivers and 224 questionnaires from health professionals. Of the 370 caregivers, 314 (84.9%) were willing to apply for HHCSs for the older adults, but only 20.5% (N = 76) received HHCSs. Over 80% of the caregivers accepted out-of-pocket costs less than 100 yuan per visit. Abstract Background Domestic migration poses a challenge for China as migrants have little access to preventive healthcare services and are vulnerable to certain risks and diseases. This research sought to unveil and explore the determinant factors associated with health education utilization as a key aspect in basic public health services for migrants in Beijing, China. Methods A sample of 863 inter-provincial migrants, 18 years old and above, was selected by three-stage stratified cluster sampling method in urban-rural fringe areas of Beijing during 2016 to 2017. Face-to-face structured interviews were conducted in the questionnaire survey. The effects of the explanatory variables on health education utilization from predisposing, enabling, health behaviors and need variables were used to demonstrate by Anderson health service utilization model. Results The study revealed that 61.6% migrants desired to receive health education, while only 53.8% of them received in the past year. There were differences in the utilization and needs of health education among the migrants in different ages and genders. Many migrants desired to gain access to various types of health education information from the internet. Chi-square independence test lists such major determinant factors in migrants whole health education as age, “Hukou” registration system, marital status, education level, long-term residence plan in Beijing, one or more children in Beijing, employment status, housing source, average daily working time, exercises, health knowledge, smoking, self-rated health. The binary logistic regression indicates that the migrants with younger age, high education level, one or more children in Beijing, exercises and good self-rated health were more likely to receive whole health education. The results also show that average daily working time of enabling variables and exercise of health behavior variables were the strong and consistent determinants of three types of health education utilization, including communicable, non-communicable and occupational diseases. Conclusion Gaps exist between the needs and utilization in health education and more attention should be given to the migrants with heavy workload and low education level. Feasible policies and measures, such as multiple health information channels, should be vigorously implemented to ensure equitable and easy access to health education for migrants.

Edema toxin (ET), one of the main toxic factors of Bacillus anthracis (B. anthracis), is a kind of potent adenylate cyclase (AC). B. anthracis has adapted to resist macrophage microbicidal mechanisms in part by secreting ET. To date, there is limited information on the pathogenic mechanisms used by ET to manipulate macrophage function, especially at the transcriptome level. We used RNA sequencing to study transcriptional changes in RAW264.7 cells treated with ET. We aimed to identify molecular events associated with the establishment of infection and followed changes in cellular proteins. Our results indicate that ET inhibited TNF-α expression in the RAW264.7 mouse macrophage cell line by activating the cAMP/PKA pathway. ET challenge of macrophages induced a differential expression of genes that participate in multiple macrophage effector functions such as cytokine production, cell adhesion, and the inflammatory response. Furthermore, ET influenced the expression of components of the ERK1/2, as well as the NF-αB signaling pathways. We also showed that ET treatments inhibit the phosphorylation of the ERK1/2 protein. ET also attenuated NF-αB subunit p65 phosphorylation and transcriptional activity of NF-αB via the cAMP/PKA pathway in macrophages. Since the observed modulatory effects were characteristic only of the bacterial exotoxin ET, we propose this may be a mechanism used by B. anthracis to manipulate macrophages and establish systemic infection.

The characteristics of diurnal variation of the surface sensible heat flux (SH) over the Tibetan Plateau (TP) are comprehensively investigated by using the long-term dataset of integrated land–atmosphere interaction observations (2006–2016) on the TP. Results show that the diurnal variation of SH shows obvious seasonal variabilities in terms of amplitude, duration, and peak time. At the Muztagh Ata Westerly Observation and Research Station (MAWORS), the Ngari Desert Observation and Research Station (NADORS), and the Qomolangma Atmospheric and Environmental Observation and Research Station (QOMS), the SH diurnal amplitude is consistently the largest in spring, followed by summer and autumn, and the smallest in winter, with a peak at 15:00. However, for the Southeast Tibet Observation and Research Station (SETORS), the amplitude in winter is rather violent with the peak at 12:00. We find that positive SH at most stations has the longest duration from May to August. Moreover, the peak time fluctuates from month to month, even showing a shift at the QOMS before and after 2015, and the double-peak phenomenon of SH mainly occurs in spring and autumn. Additionally, magnitudes of calculated SH with the conventional heat transfer coefficient (CDH) of 0.004 are about 64–100% larger than those of directly observed SH at the QOMS and the Nam Co Monitoring and Research Station (NAMORS). We here additionally recommend a new CDH values of about 2.24 × 10−3 in spring and 2.78 × 10−3 in summer, respectively, to more accurately calculate the TP SH.

Abstract Background Most schizophrenia patients are supported by main informal caregivers at home in China. This study aims to investigate the further needs of social supports for main informal caregivers of schizophrenia patients and to analyze influencing factors on the awareness and utilization of social supports in Beijing. The results of this study could potentially act as reference for health professionals to implement appropriate and effective support programs. Methods A mixed-methods approach was used in this study. Awareness, utilization and influencing factors of social supports for main informal caregivers were investigated using questionnaires in 6 urban districts in Beijing. Meanwhile, individual in-depth interviews with 10 main informal caregivers from the urban districts of Beijing were conducted to identify the caregivers’ perspective on social supports and their further needs. Results Quantitative results showed that although the government provided multi-channel resources for schizophrenia patients and their families, awareness and utilization of the resources such as rehabilitation and relevant subsidies were less than 10.0 and 5.0% respectively. Most caregivers in in-depth interviews expressed that they had negative experiences with respect to obtaining social supports, and they emphasized that more support would be needed in terms of financial support, respect, and rehabilitation institutions. Conclusions The awareness and utilization of social supports are low for main informal caregivers of schizophrenia patients. More services and improved public attitudes are needed for schizophrenia patients and their caregivers.

Abstract Background The service capacity of primary care has improved in China. General practice also takes growing responsibility in the management of type 2 diabetes mellitus, but there are concerns about the paucity of evidence of the quality of care delivered. And there is an absence of systematic quality indicators of type 2 diabetes mellitus in general practice in China. This study aimed to develop a set of type 2 diabetes mellitus quality indicators to facilitate quality measurement in general practice in China. Methods Preliminary quality indicators were generated and refined by literature review and an expert consultation meeting. Two rounds of email-based Delphi survey and a consensus meeting were carried out to identify quality indicators. Delphi questionnaires with 43 indicators were sent to 30 participants in the first round. There were 16 general practitioners and 10 community health service center leaders from primary care, 3 endocrinologists and a primary care researcher in the first round. And 27 out of the 30 participants participated in the second round. The consensus meeting was held among 9 participants to refine the indicators and a last round of rating was carried out in the meeting. The indicators were rated in terms of importance and feasibility. The agreement criteria were defined as median ≥ 7.0 and ≥ 85.0% of ratings in the 7–9 tertile for importance; median ≥ 7.0 and ≥ 65.0, 70.0, 75.0% of ratings in the 7–9 tertile for feasibility respectively in the three rounds of rating. Results After 2 rounds of Delphi survey and the consensus meeting, total 38 indicators achieved consensus for inclusion in the final set of indicators. The final set of indicators were grouped into 7 domains: access (5 indicators), monitoring (12 indicators), health counseling (7 indicators), records (2 indicators), health status (7 indicators), patient satisfaction (2 indicators) and self-management (3 indicators). Conclusions A set of 38 potential quality indicators of type 2 diabetes mellitus in general practice were identified by an iterative Delphi process in Beijing, China. Preliminary approach for measurement and data collection were described. However, the indicators still need to be validated by testing in a further study.

Abstract Background The human myxovirus resistance 2 (Mx2/MxB) protein was originally found to regulate cytoplasmic-nuclear transport but was recently reported to restrict HIV-1 replication by binding to HIV-1 capsid (CA), preventing uncoating, the nuclear import of pre-integration complex (PIC) and viral DNA integration. This work explores the mechanisms of MxB-mediated HIV-1 inhibition. Results We demonstrated that MxB represses NUP358-mediated PIC nuclear import and HIV-1 replication. Moreover, MxB’s effects on PIC nuclear import and HIV-1 replication depend critically on cofactor cleavage and polyadenylation specificity factor subunit 6 (CPSF6). MxB binds nucleoporin NUP358, blocks NUP358-CA interaction, thereby impeding the nuclear import of HIV-1 PIC with CPSF6 binding to PIC. More intriguingly, CPSF6’s role in nuclear import depends on MxB, being a facilitator of HIV-1 nuclear import on its own, but becoming an inhibitor when MxB is present. Conclusions Our work establishes that MxB impedes the NUP358-mediated HIV-1 nuclear import and viral replication cooperatively with CPSF6.

Abstract Background General practitioners (GPs) play a significant role in dementia care. However, the knowledge and attitudes of them towards dementia care are poorly characterized. The present study aimed to investigate GPs’ knowledge, attitudes and skills of dementia care in primary health settings in Beijing. Methods A cross-sectional survey was conducted in 27 community health service centers (CHSCs) in Beijing. The GPs’ knowledge, attitudes and skills were assessed utilizing the Alzheimer’s Disease Knowledge Scale (ADKS), Dementia Care Attitude Scale (DCAS) and self-designed questionnaire, respectively. Results A total of 341 participants returned the questionnaire. The overall mean score of GPs’ dementia knowledge measured by the ADKS was 21.42 (SD = 2.73) out of 30 (71.4%), GPs’ attitudes to dementia care was 36.25 (SD = 5.12) out of 50 (72.5%), and GPs’ self-confidence on dementia care skills was 53.93 (SD = 9.57) out of 75 (71.9%). GPs’ overall knowledge towards dementia care was limited and the attitudes were generally positive. They had low level recognition of their roles towards dementia care. The majority of GPs believed that dementia care was within a specialist’s domain not that of general practice. Conclusion GPs demonstrate low levels of dementia knowledge and skills, but express generally positive attitudes towards dementia in this study. It is much needed to translate detailed dementia care handbook, and adequate dementia knowledge training for GPs into practice to improve care outcomes for people with dementia in China. In addition, dementia management should be covered in the national basic package of public health services in primary care.

For the purpose of minimizing the damage to apples during the operation of an apple-picking robot, improving the stability of an apple-harvesting mechanical hand and reducing the use and operation cost of the robot, a novel biomimetic apple mechanical hand based on negative pressure air suction is purposely designed based on the physical characteristics of apples by imitating octopus predation. The mechanical hand has four suction cups evenly distributed in the hand, which produce concave deformation under negative vacuum pressure to fit the apple surface and adsorb the apple; the fruit stalk is separated by wrist rotation and dragging in a compound way to complete the picking operation. This paper firstly determines the key parameters of the mechanical hand based on the characteristics of silicone material and the three growth postures of apples, tries to make a physical prototype of the mechanical hand and conducts relevant performance tests on the picking platform built in the laboratory to explore the best combination of parameters and further optimize the manipulator. The results show that the success rate of the biomimetic nondestructive apple picker with the best combination of parameters is 100%, 76% and 68% for three different apple-growth postures and the operation is smooth, reliable, safe and non-invasive.

Abstract Indian Ocean Dipole (IOD), a major climate variability in the tropics which drives the abiotic stress associated with heavy rainfalls and severe droughts, is not much understood in terms of its role in the carbon cycle, while El Niño‐Southern Oscillation (ENSO)‐related terrestrial carbon cycle variation has been intensively studied. Here, we investigate IOD's impact on land photosynthesis over the Indian ocean rim countries during austral spring using satellite‐based gross primary productivity (GPP) and Earth System Model simulations produced in the Coupled Model Intercomparison Project Phase 6 (CMIP6). IOD independently affects GPP with significant positive partial correlation coefficients (pcor) over most of Africa and India, and negative pcor over southern China, Indo‐China peninsula, maritime continent, and Australia, mostly driven by precipitation variations; this obviously differs from the widespread significant negative pcor pattern induced by ENSO. The recent extremely positive IOD in 2019 caused the canonical IOD‐affected GPP patterns, however, with its extreme impacts. Furthermore, though large inter‐model spreads exist, the CMIP6 multimodel median can basically capture the main characteristics of IOD‐affected precipitation and GPP patterns. Importantly, IOD is predicted to occur more frequently in future warming scenarios. Model future projections suggest that it will exert larger impacts on GPP variations over central and eastern Africa, Sumatra, western and southeastern Australia with stronger pcor and enhanced explained variance, but less impacts over southern Africa, east India, Indo‐China peninsula, and northeastern Australia. Therefore, besides ENSO, understanding the IOD impacts can provide us new insights into regional and global carbon cycle interannual variability.

The Tibetan Plateau (TP) vortex (TPV), one of the crucial weather systems triggering rainfall, plays a key role in modulating precipitation over TP and downstream regions. The role of atmospheric heat source in TPV development is explored by a case study in June 2016, using high-resolution ERA5 reanalysis, black-body temperature (TBB) obtained from the Fengyun-2E (FY-2E) satellite, and precipitation amount from the Tropical Rainfall Measurement Mission (TRMM). The evolutions of TPV can be split into three stages, i.e., generation, development, and pre-moving-off stage. The intensity of TPV increases with fluctuations, with weaker and shallower TPV in the generation stage, strongest in the development stage and deepest in the pre-moving-off stage. Importantly, the genesis of TPV is related to the surface warming center driven by surface sensible heating while its development is primarily dependent on the latent heat of condensation. The main contributor of the latent heat of condensation is further analyzed as a vertical transport of the water vapor that promotes TPV development.

Abstract Background Informal caregivers of people with dementia in Beijing are increasingly called upon to provide home-based care for their patients due to the increasing number of dementia patients and the shortage of standardized institutional solutions of care for patients in China. This study aimed to clarify the needs of informal caregivers and barriers of primary care workers toward dementia management in primary care in Beijing to provide references that may help to improve the care and services provided to individuals with dementia and their family caregivers residing in urban China. Methods A mixed-methods approach was used in this study. We performed individual in-depth interviews with 10 informal caregivers. Moreover, we carried out focus group interviews with 29 primary care workers. Content analysis was used to separately identify themes and codes. Discrepancies were discussed until final agreement was achieved. Results Three themes representing the core attitudes of informal caregivers and primary care workers were identified: care knowledge and skills, psychological counseling, and collaborative management. Most primary care workers believed that the management of dementia patients in primary care was necessary. However, due to the heavy work load and different medical specialties involved, these workers were unable to manage it. Conclusions Professional training focused on dementia for primary care workers should be strengthened. At the same time, the establishment of a community-based dementia team management model that includes specialists, community health service centers (CHSCs), and community committees should be explored.

Correctly calculating the vertical velocity of the Asian summer monsoon anticyclone (ASMA) region is helpful for accurately knowing the ozone stratosphere–troposphere exchange, so as to explore the variation of ozone in the ASMA region. Therefore, the vertical velocity over the ASMA in June, July, August, and September 2012 and 2016 was calculated using the thermodynamic method, which may avoid the deviations produced by the kinematics method using the mass continuity equation. In order to improve the accuracy, we used high-resolution heating rate datasets obtained via the radiation model in Canadian Atmospheric Global Climate Model called CanAM4.3_RAD based on in situ observations and revised satellite data from MLS/AIRS. The vertical velocity calculated by the thermodynamic method (VT) is then compared with the data from ERA-Interim (VERA–I). In the daytime, values of VT were similar to VERA–I and were dominated by ascending motion, although VT showed descending motion at the western edge of the ASMA below 100 hPa. The intensity of VT was slightly smaller than that of VERA–I at lower levels (200–100 hPa) over the ASMA region and significantly weaker above 100 hPa. The situation was more complex at night. Both VT and VERA–I showed the convergence of vertical wind at 150 hPa and the divergence at 80 hPa, but VT had a smaller standard deviation. VT showed descending in the western and northern ASMA, but VERA–I only descended in the west. The descending motion in the west, seen in both VT and VERA–I, is produced by the heating difference between the Qinghai–Tibet Plateau and the Iranian Plateau. The difference of the two vertical velocities in the northern ASMA may indicate the different understandings of the local Hadley Circulation and local Brewer-Dobson Circulation.

Abstract Background Migrants are the unique production of China’s urbanization process. They are often excluded from social welfare and security systems of cities, and often exposed to high health risk related closely to their health problems. This research sought to unveil and explore the influencing factors on health services utilization of migrants in Beijing. Methods A sample of 2014 inter-provincial migrants and 4578 residents with Beijing “Hukou” who were 15 years old and above was chosen by three-stage stratified cluster sampling method. A structured questionnaire survey was conducted via face-to-face interviews. Anderson health service utilization model was used to demonstrate the effects of the explanatory variables on health seeking behavior from predisposing, enabling and need variables. Results The study reveals that the rate of ‘having symptoms’ of migrants was lower than that of residents with “Hukou” only in the group of 25 to 34 years old in the past month. 503 migrants (25.0%) and 1441 (31.5%) residents with “Hukou” reported at least one episode of discomfort in the past month, and the rate of health service seeking behavior among migrants (46.8%) was lower than residents with “Hukou” (62.6%) (P

In this paper, synthetic metakaolin with fixed composition (Al2O3·2SiO2) was produced by a simple chemosynthetic route. The chemosynthetic metakaolin can eliminate the influence of impurities in metakaolin from natural kaolin minerals. The synthetic metakaolin together with NaOH and SiO2-sol were used to prepare Na-based geopolymer precursors with various molar ratios of Si/Al. The molar ratios of Si/Al from 1 to 2 were tailored by adding different contents of SiO2-sol. Zeolite/geopolymer composites or monolith-type zeolite were successfully fabricated from synthetic metakaolin-based geopolymer through a hydrothermal process. The effects of Si/Al ratios on the phase composition and microstructure of the produced zeolite/geopolymer composites or zeolites were studied. The results proved that the composition of synthetic metakaolin and geopolymer precursors can be facilely tuned, and the monolithic geopolymer precursors can be mostly, or even totally, transformed into zeolite after hydrothermal treatment.

Lassa virus (LASV) is a rodent-borne arenavirus circulating in West African regions that causes Lassa fever (LF). LF is normally asymptomatic at the initial infection stage, but can progress to severe disease with multiorgan collapse and hemorrhagic fever. To date, the therapeutic choices are limited, and there is no approved vaccine for avoiding LASV infection. Adenoviral vector-based vaccines represent an effective countermeasure against LASV because of their safety and adequate immunogenicity, as demonstrated in use against other emerging viral infections. Here, we constructed and characterized a novel Ad5 (E1-, E3-) vectored vaccine containing the glycoprotein precursor (GPC) of LASV. Ad5-GPCLASV elicited both humoral and cellular immune responses in BALB/c mice. Moreover, a bioluminescent imaging-based BALB/c mouse model infected with GPC-bearing and luciferase-expressing replication-incompetent LASV pseudovirus was utilized to evaluate the vaccine efficacy. The bioluminescence intensity of immunized mice was significantly lower than that of control mice after being inoculated with LASV pseudovirus. This study suggests that Ad5-GPCLASV represents a potential vaccine candidate against LF.

Multi-robots have shown good application prospects in agricultural production. Studying the synergistic technologies of agricultural multi-robots can not only improve the efficiency of the overall robot system and meet the needs of precision farming but also solve the problems of decreasing effective labor supply and increasing labor costs in agriculture. Therefore, starting from the point of view of an agricultural multiple robot system architectures, this paper reviews the representative research results of five synergistic technologies of agricultural multi-robots in recent years, namely, environment perception, task allocation, path planning, formation control, and communication, and summarizes the technological progress and development characteristics of these five technologies. Finally, because of these development characteristics, it is shown that the trends and research focus for agricultural multi-robots are to optimize the existing technologies and apply them to a variety of agricultural multi-robots, such as building a hybrid architecture of multi-robot systems, SLAM (simultaneous localization and mapping), cooperation learning of robots, hybrid path planning and formation reconstruction. While synergistic technologies of agricultural multi-robots are extremely challenging in production, in combination with previous research results for real agricultural multi-robots and social development demand, we conclude that it is realistic to expect automated multi-robot systems in the future.

The genus Henipavirus (HNVs) includes two fatal viruses, namely Nipah virus (NiV) and Hendra virus (HeV). Since 1994, NiV and HeV have been endemic to the Asia–Pacific region and responsible for more than 600 cases of infections. Two emerging HNVs, Ghana virus (GhV) and Mojiang virus (MojV), are speculated to be associated with unrecognized human diseases in Africa and China, respectively. Despite many efforts to develop vaccines against henipaviral diseases, there is presently no licensed human vaccine. As HNVs are highly pathogenic and diverse, it is necessary to develop universal vaccines to prevent future outbreaks. The attachment enveloped glycoprotein (G protein) of HNVs mediates HNV attachment to the host cell’s surface receptors. G proteins have been used as a protective antigen in many vaccine candidates for HNVs. We performed quantitative studies on the antibody responses elicited by the G proteins of NiV, HeV, GhV, and MojV. We found that the G proteins of NiV and HeV elicited only a limited cross-reactive antibody response. Further, there was no cross-protection between MojV, GhV, and highly pathogenic HNVs. We then constructed a bivalent vaccine where the G proteins of NiV and HeV were fused with the human IgG1 Fc domain. The immunogenicity of the bivalent vaccine was compared with that of monovalent vaccines. Our results revealed that the Fc-based bivalent vaccine elicited a potent antibody response against both NiV and HeV. We also constructed a tetravalent Fc heterodimer fusion protein that contains the G protein domains of four HNVs. Immunization with the tetravalent vaccine elicited broad antibody responses against NiV, HeV, GhV, and MojV in mice, indicating compatibility among the four antigens in the Fc-fusion protein. These data suggest that our novel bivalent and tetravalent Fc-fusion proteins may be efficient candidates to prevent HNV infection.

The spring sensible heating (SH) over the Tibetan Plateau (TP) serves as a huge ‘air pump’, significantly influencing the Asian summer monsoon, has experienced a decreasing trend. However, it remains unclear whether this decline will continue. Therefore, we here examine the long-term trends of spring SH over the central and eastern TP (CETP) based on a meteorological station-based calculated SH dataset, and CMIP6 multi-model simulations. These two sources confirmed the previous finding that the SH peaks in May. Further, we find that the declining SH was replaced by a fast recovery after approximate 2000 in the station-based SH. This is to some extent verified by the historical simulations of CMIP6 models. Importantly, CMIP6 future projections suggest that this increasing trend will continue, and get stronger with higher radiative forcing from SSP126 to SSP585. Mechanism analysis indicates that the previous decreasing trend in SH was mainly caused by the decline of 10 m wind speed, while the recent and future increasing trend results from the rising ground-air temperature difference. We suggest that this increasing trend of spring SH over the CETP may serve as an alternative driver for the enhancement of the East Asian summer monsoon in the future.

The coronavirus disease 2019 (COVID-19) vaccines have been shown to be effective in protecting people from severe disease progression, hospitalisation and death. However, a wide range of side effects have been reported worldwide. New onset or flare-up of autoimmune hepatitis (AIH) is an extremely rare adverse event following COVID-19 vaccination, with the majority of cases presenting with mild symptoms. Unfortunately, there have been cases of fatal complications. In this mini-review, we have summarised the clinical characteristics of a total of 35 currently reported cases of AIH after COVID-19 vaccination and suggest that patients with autoimmune diseases may be at higher risk of developing AIH after vaccination.
