To assess the individual and community health effects of task shifting for emergency care in low-resource settings and underserved populations worldwide.We systematically searched 13 databases and additional grey literature for studies published between 1984 and 2019. Eligible studies involved emergency care training for laypeople in underserved or low-resource populations, and any quantitative assessment of effects on the health of individuals or communities. We conducted duplicate assessments of study eligibility, data abstraction and quality. We synthesized findings in narrative and tabular format.Of 19 308 papers retrieved, 34 studies met the inclusion criteria from low- and middle-income countries (21 studies) and underserved populations in high-income countries (13 studies). Targeted emergency conditions included trauma, burns, cardiac arrest, opioid poisoning, malaria, paediatric communicable diseases and malnutrition. Trainees included the general public, non-health-care professionals, volunteers and close contacts of at-risk populations, all trained through in-class, peer and multimodal education and public awareness campaigns. Important clinical and policy outcomes included improvements in community capacity to manage emergencies (14 studies), patient outcomes (13 studies) and community health (seven studies). While substantial effects were observed for programmes to address paediatric malaria, trauma and opioid poisoning, most studies reported modest effect sizes and two reported null results. Most studies were of weak (24 studies) or moderate quality (nine studies).First aid education and task shifting to laypeople for emergency care may reduce patient morbidity and mortality and build community capacity to manage health emergencies for a variety of emergency conditions in underserved and low-resource settings.Évaluer l'impact, sur la santé individuelle et collective, du transfert des interventions de premiers secours dans les endroits disposant de ressources limitées et au sein des populations défavorisées à travers le monde.Nous avons analysé systématiquement 13 bases de données ainsi que toute littérature grise complémentaire pour y trouver des études publiées entre 1984 et 2019. Les études retenues devaient faire mention d'une formation aux premiers secours pour les non-professionnels au sein des populations défavorisées ou dotées de peu de ressources, mais aussi d'une évaluation quantitative de l'impact sur la santé individuelle et collective. Nous avons dupliqué les appréciations d'admissibilité de l'étude, de qualité et d'abstraction des données. Enfin, nous avons synthétisé les résultats sous forme de textes et de tableaux.Sur 19 308 articles récupérés, 34 études correspondaient aux critères d'inclusion propres aux pays à faibles et moyens revenus (21 études) et aux populations défavorisées dans les pays à hauts revenus (13 études). Plusieurs situations d'urgence étaient ciblées: traumatismes, brûlures, arrêts cardiaques, intoxications aux opiacés, malaria, maladies infantiles contagieuses et malnutrition. Les stagiaires étaient des individus issus du grand public, des non-professionnels de la santé, des bénévoles et des contacts proches de populations à risque, tous formés dans le cadre de cours multimodaux organisés par des pairs et de campagnes de sensibilisation de l'opinion publique. Diverses retombées politiques et cliniques d'envergure ont été constatées: amélioration de la capacité de gestion des urgences dans les communautés (14 études), conséquences positives pour les patients (13 études) et santé collective (7 études). Los estudios elegibles involucraron la formación en atención de emergencia para personas no profesionales en poblaciones subatendidas o de bajos recursos, y cualquier evaluación cuantitativa de los efectos en la salud de los individuos o las comunidades. Se realizaron evaluaciones duplicadas de la elegibilidad de los estudios, la abstracción de datos y la calidad. Se sintetizaron los resultados en formato narrativo y tabular.De los 19.308 documentos recuperados, 34 estudios cumplían los criterios de inclusión de países con ingresos bajos y medios (21 estudios) y de poblaciones desatendidas de países con ingresos altos (13 estudios). Background: Regional medical campuses are often challenged with providing effective interprofessional education (IPE) opportunities for medical students that are comparable to those at main campuses. At distributed teaching sites, there is often less IPE infrastructure and fewer learners of other health professions. On the other hand, distributed medical education (DME) settings often have community-based clinical environments and fewer medical students, which can provide unique opportunities for IPE curriculum innovation.Methods: At the Niagara Regional Campus (NRC) of McMaster University, the Horizontal Elective for Interprofessional Growth & Healthcare Team ENhancement (HEIGHTEN) was developed to provide first-year medical students the opportunity to learn from and work alongside nurses in a community hospital. This study assesses HEIGHTEN’s impact on students’ knowledge, confidence, and attitudes towards interprofessional care, as well as student satisfaction with the learning experience using a mixed methods evaluation.Results: Findings suggest that HEIGHTEN provided an enjoyable learning experience, fostered positive interprofessional attitudes and an appreciation for the nursing role. Voluntary participation by medical students was high and increased both within the regional campus and with students from other campuses travelling to participate.Conclusion: This model for IPE can be feasibly replicated by distributed teaching sites to provide medical students with hands-on, experiential learning early in training, leading to positive attitudes and behaviours supporting interprofessional collaboration (IPC).

Purpose To assess the associations of occupational heat and cold-related illnesses presenting in emergency departments in south western Ontario, Canada, with daily meteorological conditions using Bayesian inference. Methodology Meteorological and air pollution data for the south western economic region of Ontario were gathered from Environment Canada and the Ministry of Environment. Daily heat and cold-related emergency department visits clinically attributed to work from 2004 to 2010 were tabulated. A novel application of Bayesian inference on a flexible Poisson time series model was undertaken to examine linear and non-linear associations between average, regional meteorological conditions and daily morbidity rates, to adjust for relevant confounders and temporal trends, and to consider potential interactions. Results Bilinear associations were observed between regional temperatures and morbidities resulting from extreme temperature exposures. The median increase in the daily rate of emergency department visits for heat illness was 75% for each degree above 22 °C (posterior 95% credible interval (CI) relative rate=1.56–1.99) in the daily maximum temperature. Below 0 °C, rates of occupational cold illness increased by a median of 15% for each degree decrease in the minimum temperature (posterior 95% CI 0.80–0.91); wind speed also had a significant effect. Conclusions The observed associations can inform occupational surveillance and injury prevention programming, as well as public health efforts targeting vulnerable populations. Methodologically, the use of Bayesian inference in time series analyses of meteorological exposures is feasible and conducive to providing accurate advice for policy and practice.

OBJECTIVE: To describe the incidence of occupational heat illness in Ontario. METHODS: Heat illness events were identified in two population-based data sources: work-related emergency department (ED) records and lost time claims for the period 2004-2010 in Ontario, Canada. Incidence rates were calculated using denominator estimates from national labour market surveys and estimates were adjusted for workers’ compensation insurance coverage. Proportional morbidity ratios were estimated for industry, occupation and tenure of employment. RESULTS: There were 785 heat illness events identified in the ED encounter records (incidence rate 1.6 per 1,000,000 full-time equivalent (FTE) months) and 612 heat illness events identified in the lost time claim records (incidence rate 1.7 per 1,000,000 FTE months) in the seven-year observation period with peak incidence observed in the summer months. The risk of heat illness was elevated for men, young workers, manual workers and those with shorter employment tenure. A higher proportion of lost time claims attributed to heat illness were observed in the government services, agriculture and construction sectors relative to all lost time claims. CONCLUSIONS: Occupational heat illnesses are experienced in Ontario’s population and are observed in ED records and lost time claims. The variation of heat illness incidence observed with worker and industry characteristics, and over time, can inform prevention efforts by occupational health services in Ontario.

Background: Approximately 24,000 Ontarians live in remote Indigenous communities with no road access. These communities are a subset of Nishnawbe Aski Nation (NAN), a political grouping of 49 First Nations communities in Northern Ontario, Canada. Limited information is available regarding the status of emergency care in these communities. Objective: We aimed to understand emergency response systems, services, and training in remote NAN communities. Design: We used an environmental scan approach to compile information from multiple sources including community-based participatory research. This included the analysis of data collected from key informant interviews (n=10) with First Nations community health leaders and a multi-stakeholder roundtable meeting (n=33) in October 2013. Results: Qualitative analysis of the interview data revealed four issues related to emergency response systems and training: (1) inequity in response capacity and services, (2) lack of formalised dispatch systems, (3) turnover and burnout in volunteer emergency services, and (4) challenges related to first aid training. Roundtable stakeholders supported the development of a community-based emergency care system to address gaps. Conclusions: Existing first response, paramedical, and ambulance service models do not meet the unique geographical, epidemiological and cultural needs in most NAN communities. Sustainable, context-appropriate, and culturally relevant emergency care systems are needed.

Introduction The Disease Control Priorities Project recommends emergency care training for laypersons in low-resource settings, but evidence for these interventions has not yet been systematically reviewed. This review will identify the individual and community health effects of educating laypeople to deliver prehospital emergency care interventions in low-resource settings. Methods and analysis This systematic review addresses the following question: in underserviced populations and low-resource settings (P), does first aid or emergency care training or education for laypeople (I) confer any individual or community health benefit for emergency health conditions (O), in comparison with no training or other forms of education (C)? We restrict this review to studies reporting quantitatively measurable outcomes, and search 12 electronic bibliographic databases and grey literature sources. A team of expert content and methodology reviewers will conduct title and abstract screening and full-text review, using a custom-built online platform. Two investigators will independently extract methodological variables and outcomes related to patient-level morbidity and mortality and community-level effects on resilience or emergency care capacity. Two investigators will independently assess external validity, selection bias, performance bias, measurement bias, attrition bias and confounding. We will summarise the findings using a narrative approach to highlight similarities and differences between the gathered studies. Ethics and dissemination Formal ethical approval is not required. Results The results will be disseminated through a peer-reviewed publication and knowledge translation strategy. Review registration number CRD42014009685.
