Displacement of dental implants into the maxillary sinus is not an uncommon event in implant dentistry and may lead to serious complications, such as sinusitis. To avoid systemic problems, performing the removal of the foreign body as soon as possible is suggested. Despite the fact that early implants dislocation has been reported several times, late migration into maxillary sinus has been described by just a few studies. The purpose of this study was to report a rare case of dental implant migration into maxillary sinus after 12 years of function. A 61-year-old woman came to our attention in June 2015 after being visited by an otolaryngologist and being diagnosed with sinusitis and presence of a foreign body into the right maxillary sinus. A panoramic radiograph and a CT scan showed the migration of dental implant sited in 1.6 positions into the maxillary sinus. The implant was removed following a Caldwell-Luc procedure under local anesthesia. Postoperative course was uncomplicated and the patient reported no symptoms of sinusitis after 12 months of follow-up.

Aim The aim of this study is to evaluate, both in vitro and in vivo, two different types of implant-abutment connections: screwed connection and cemented connection, analyzing the permeability of the IAI to bacterial colonization and the stability to chewing forces. Materials and Methods In this study were compared two different types of implant-abutment connections: internal hexagon screwed connection (Winsix®, BioSAF IN, Ancona, Italy ) Group 1; and internal hexagon cemented-conical connection (Bone System®, Milano, Italy) Group 2. Group 1 and Group 2 were compared on three levels: impermeability to bacterial penetration an in vitro study, resistance to loading for 5 years a simulated computer model, type of peri-implant bacterial colonization and health of peri-implant soft tissues around the implant for 2 year in an in vivo study. Results The results had showed the lower stability to the screwed implant-abutment connection than the cemented implant-abutment connection both for the permeability to the bacterial colonization than for the stability to the chewing forces. Conclusion Also if the implants long-term failures are consequences of multi-factorial elements the choice of an adequate implant system is fundamental for the long-term success. Also the choice of connection system is very important and would be preferable to choose implants with cemented connection instead of implants with screwed connection.

Numerous factors help to conserve the dentition of elderly patients, such as healthy food habits, a strong physical constitution, and a good quality of life. The aim of this study was to define a model that takes into account the integration of both the structural and functional aspects of a healthy dentition. Twenty-three patients aged at least 75 years were recruited. The patients were required to possess all of their dentition and have no prosthetic rehabilitations and be asymptomatic for temporomandibular joint disorders. Occlusal characteristics were measured and recorded using the criteria adopted by the US National Health and Nutrition Examination Survey: presence or absence of rotation of the upper arches, trend of the occlusal table, and distribution of occlusal contacts during movements. We believe that the following parameters are predictive of a condition of the dental arches' equilibrium: crowding and disalignment of the teeth, derotated position of the upper arches, absence of the curve of Spee, an occlusal plane trend contrary to spherical theory, and presence of group function on the working side and malocclusion on the nonworking side. We consider that these factors are merely the consequence of correct functioning within the framework of favorable environmental factors.
