2. Identification of Priorities for Colorectal Cancer Screening Interventions Among US Hispanic/Latino Populations.

Abstract

Objectives. To identify nationwide census tract‒level areas where improving colorectal cancer (CRC) screening uptake via targeted local preventive intervention may benefit Hispanic or Latino/a (H/L) groups defined by region or country of origin. Methods. Using 2021 Centers for Disease Control and Prevention PLACES and American Community Survey data, we applied geographically weighted regression and Getis-Ord Gi* hot spot procedures to identify CRC screening priority zones for H/L groups in the United States. Priority zones can be conceptualized as census tracts with strong inverse associations between percentage of a particular H/L group in the population and CRC screening rate, after adjusting for socioeconomic deprivation and lack of insurance. Results. We identified 6519, 3477, 3522, 1069, and 1424 census tract CRC screening priority zones for H/L communities of Mexican, Puerto Rican, Central/South American, Dominican, and Cuban heritage, respectively. Priority zones for H/L groups had strong spatial heterogeneity, and overlap of geographic patterns among H/L groups varied by region. Conclusions. Our findings and interactive web map may serve as a translational tool for public health authorities, policymakers, clinicians, and other stakeholders to target investment and interventions to increase guideline-concordant CRC screening uptake benefitting specific H/L communities in the United States. Abstract

Background: High red meat and/or processed meat consumption are established colorectal cancer risk factors. We conducted a genome-wide gene-environment (GxE) interaction analysis to identify genetic variants that may modify these associations. Methods: A pooled sample of 29,842 colorectal cancer cases and 39,635 controls of European ancestry from 27 studies were included. Quantiles for red meat and processed meat intake were constructed from harmonized questionnaire data. Genotyping arrays were imputed to the Haplotype Reference Consortium. Two-step EDGE and joint tests of GxE interaction were utilized in our genome-wide scan. Results: Meta-analyses confirmed positive associations between increased consumption of red meat and processed meat with colorectal cancer risk [per quartile red meat OR = 1.30; 95% confidence interval (CI) = 1.21-1.41; processed meat OR = 1.40; 95% CI = 1.20-1.63]. Two significant genome-wide GxE interactions for red meat consumption were found. Joint GxE tests revealed the rs4871179 SNP in chromosome 8 (downstream of HAS2); greater than median of consumption ORs = 1.38 (95% CI = 1.29-1.46), 1.20 (95% CI = 1.12-1.27), and 1.07 (95% CI = 0.95-1.19) for CC, CG, and GG, respectively. The two-step EDGE method identified the rs35352860 SNP in chromosome 18 (SMAD7 intron); greater than median of consumption ORs = 1.18 (95% CI = 1.11-1.24), 1.35 (95% CI = 1.26-1.44), and 1.46 (95% CI = 1.26-1.69) for CC, CT, and TT, respectively. 17. Continuous A-Mode Ultrasound-Based Prediction of Transfemoral Amputee Prosthesis Kinematics Across Different Ambulation Tasks

Abstract

Objective: Volitional control systems for powered prostheses require the detection of user intent to operate in real life scenarios. Ambulation mode classification has been proposed to address this issue. However, these approaches introduce discrete labels to the otherwise continuous task that is ambulation. An alternative approach is to provide users with direct, voluntary control of the powered prosthesis motion. Surface electromyography (EMG) sensors have been proposed for this task, but poor signal-to-noise ratios and crosstalk from neighboring muscles limit performance. B-mode ultrasound can address some of these issues at the cost of reduced clinical viability due to the substantial increase in size, weight, and cost. Thus, there is an unmet need for a lightweight, portable neural system that can effectively detect the movement intention of individuals with lower-limb amputation. Methods: In this study, we show that a small and lightweight A-mode ultrasound system can continuously predict prosthesis joint kinematics in seven individuals with transfemoral amputation across different ambulation tasks. Features from the A-mode ultrasound signals were mapped to the user's prosthesis kinematics via an artificial neural network. Results: Predictions on testing ambulation circuit trials resulted in a mean normalized RMSE across different ambulation modes of 8.7 ± 3.1%, 4.6 ± 2.5%, 7.2 ± 1.8%, and 4.6 ± 2.4% for knee position, knee velocity, ankle position, and ankle velocity, respectively. Conclusion and Significance: This study lays the foundation for future applications of A-mode ultrasound for volitional control of powered prostheses during a variety of daily ambulation tasks.

Abstract

Powered prostheses aim to mimic the missing biological limb with controllers that are finely tuned to replicate the nominal gait pattern of non-amputee individuals. Unfortunately, this control approach poses a problem with real-world ambulation, which includes tasks such as crossing over obstacles, where the prosthesis trajectory must be modified to provide adequate foot clearance and ensure timely foot placement. Here, we show an indirect volitional control approach that enables prosthesis users to walk at different speeds while smoothly and continuously crossing over obstacles of different sizes without explicit classification of the environment. At the high level, the proposed controller relies on a heuristic algorithm to continuously change the maximum knee flexion angle and the swing duration in harmony with the user's residual limb. At the low level, minimum-jerk planning is used to continuously adapt the swing trajectory while maximizing smoothness. Experiments with three individuals with above-knee amputation show that the proposed control approach allows for volitional control of foot clearance, which is necessary to negotiate environmental barriers. Our study suggests that a powered prosthesis controller with intrinsic, volitional adaptability may provide prosthesis users with functionality that is not currently available, facilitating real-world ambulation.

Abstract

This papers details the experimental methods used to measure various properties relevant to the performance of ablative materials. The materials tested were fine grain graphite, and two glass-phenolic ablatives, including MXBE-350 (rubber modified glass-phenolic) and MXB-360 (glass-phenolic) manufactured by Cytec-Solvay. Such properties observed in this paper includes mass loss, erosion rate, heat-soaked temperature, compression char strength, and shear char strength. High definition images and infrared images were also recorded to observe the changes undergone during experimentation. The results from the experiment showed that graphite is superior in terms of structural integrity, however it fails to provide the same level of heat protection as the two glass-phenolic materials. MXBE-350 proved to be the best material in terms of insulation, although it does not hold up structurally like MXB-360. [ABSTRACT FROM AUTHOR]

42. Prostate cancer disparities in clinical and survival patterns among Black and Latino patients considering nativity: findings from the California Cancer Registry.

Abstract

Background: We investigated clinical characteristics and prostate cancer (PCa) survival patterns among Latino patients considering nativity compared to non-Latino Black (NLB) and non-Latino White (NLW) patients., Methods: We used data from the California Cancer Registry (1995-2021), which included 347,540 NLW, 50,032 NLB, and 75,238 Latino PCa patients. Frequencies of sociodemographic and clinical variables were assessed with Chi-square tests. Multivariable regression models were fitted to evaluate determinants of treatment reception, Gleason upgrade, and survival differences. Exploratory analyses were conducted grouping Latino cases into US-born and non-US-born by country-of-origin., Results: Compared to NLW, NLB cases had the greatest proportion of younger patients, whereas non-US-born Latino patients had the greatest proportion of low socio-economic status and uninsured patients. Non-US-born Latinos showed greater proportion of diagnoses completed with <6 core biopsies, Gleason >8, stage IV tumors and metastasis. Multivariable analyses showed that compared to NLW, Latino patients were as likely to receive treatment, whereas NLB cases were less likely (OR = 0.81, 95% CI: 0.67-0.98, p = 0.029). Compared to NLW, Non-US-born Latino cases were less likely to die of PCa (HR = 0.78; 95% CI = 0.64-0.94, p=0.011), with no difference reported for NLB cases., Conclusions: Considering sociodemographic and clinical characteristics, non-US-born Latino PCa patients had better survival than NLW. This highlights the need to identify key determinants of these survival differences, and the importance of sociodemographic and clinical determinants in survival disparities., Impact: Our study emphasizes the importance of considering nativity among Latino patients to understand PCa disparities and outcomes in this population.
