Abstract Background Bongkrekic acid is a rare mitochondrial toxin produced by the Burkholderia cocovenenans subsp. Bongkrekic acid poisoning has a case fatality rate of more than 50%, and progresses rapidly to multiple organ failure. However, limited clinical information is available regarding this phenomenon. Case presentation We report the case of a 77-year-old female patient with severe bongkrekic acid poisoning after consuming Auricularia auricula. The patient was treated for sweating, unconsciousness, nausea, vomiting, and acute hepatic failure. The patient died despite receiving supportive care and undergoing plasmapheresis. Conclusions Bongkrekic acid poisoning causes high mortality, but is often mistakenly categorized as a minor foodborne disease. In reporting this case, we aimed to increase the awareness of healthcare providers as well as the public about the risks posed by bongkrekic acid. Medical staff being familiar with the clinical manifestations of bongkrekic acid poisoning is necessary to take corresponding measures to effectively treat patients. Safer fermentation processes and food handling should be emphasized to prevent the occurrence of bongkrekic acid poisoning.

BackgroundTriglycerides and fasting glycemic index are biomarkers used to assess the risk of insulin resistance and metabolic syndrome. It and its derivatives such as the TyG-BMI index are currently able to reflect the degree of insulin resistance and are closely related to a variety of metabolic diseases. The relationship between the TyG-BMI index and hearing loss remains underexplored, particularly in the context of aging and public health. This study investigates the association of the TyG index, BMI index, and TyG-BMI index with hearing loss, with a focus on their potential implications for the aging population.MethodsData from the China Health and Retirement Longitudinal Study (CHARLS) database were analyzed using R software. We applied multi-factor logistic regression, linear regression, restricted cubic splines, and subgroup analyses to assess the impact of the TyG index, BMI index, and TyG-BMI index on hearing loss across different age groups.ResultsThe TyG index was not significantly associated with hearing loss. However, both the BMI index and the TyG-BMI index exhibited a positive correlation with hearing loss, particularly among older individuals. The results suggest that as the population ages, higher BMI and TyG-BMI indices may increase the risk of hearing impairment.ConclusionWhile the TyG index does not show a significant link to hearing loss, higher BMI and TyG-BMI indices are associated with an increased risk of hearing loss, especially in older adults. These findings highlight the importance of considering aging-related factors in public health initiatives aimed at preventing hearing loss. Further research is needed to elucidate the mechanisms underlying these associations and to develop age-inclusive strategies for addressing hearing impairment in the aging population.

Dichloromethane is widely used as an organic solvent in aerospace, electronics, and medicine. Cases of poisoning caused by this substance are rare. Recently, we successfully treated a patient with an acute dichloromethane poisoning. During the production process, owing to pipeline leakage and lack of personal protection, the patient was poisoned by dichloromethane inhalation, fell from a height, and experienced a sudden cardiac arrest. After successful cardiopulmonary resuscitation, the patient was transferred to a local hospital for diagnosis and treatment and then to our hospital. After two visits to our hospital for systematic diagnosis and active treatment with fluid infusion, anti-infection therapy, glucocorticoids, ventilator-assisted respiration, chest strap fixation, and nutritional support, the patient achieved clinical recovery.

Abstract Background Certain studies have indicated a link between obstructive sleep apnea and insulin resistance in specific populations. To gain more clarity, extensive research involving a broad sample of the overall population is essential. The primary objective of this study was to investigate this correlation by utilizing data from the National Health and Nutrition Examination Survey database. Methods The analysis incorporated data from the National Health and Nutrition Examination Survey database spanning the time periods from 2005 to 2008 and from 2015 to 2018, with a focus on American adults aged 18 years and older after applying weight adjustments. Key variables such as obstructive sleep apnea, triglyceride glucose index, and various confounding factors were considered. A generalized linear logistic regression model was used to investigate the association between obstructive sleep apnea and the triglyceride glucose index, with additional exploration of the consistency of the results through hierarchical analysis and other techniques. Results The study included participants aged between 18 and 90 years, with an average age of 46.75 years. Among the total sample, 50.76% were male. The triglyceride glucose index demonstrated a diagnostic capability for obstructive sleep apnea, with an AUC of 0.701 (95% CI: 0.6619–0.688). According to the fully adjusted model, individuals in the fourth quartile of the triglyceride glucose index showed an increased likelihood of having obstructive sleep apnea compared to those in the first quartile (OR: 1.45; 95% CI: 1.02–2.06; P

Xylazine is used in veterinary medicine as a sedative, analgesic, and muscle relaxant. However, in recent decades, it has frequently been detected in illicit drugs. Xylazine poisoning is characterized by depression of the central nervous and cardiovascular systems. Herein, we present a case of a 41-year-old man who not only had severe depression of the central nervous and cardiovascular systems, but also developed hyperpyrexia during the treatment of xylazine poisoning, which led to his death 3 days after poisoning. This case indicates that, in addition to its other effects, xylazine may also cause hyperthermia, which has not yet been reported in humans.

Objective: Accidental oral imidacloprid poisoning occurred in a family in Shandong, China, in May 2023. This study aimed to analyze the clinical characteristics of this imidacloprid poisoning event and investigated the detection of toxicants.Methods: Clinical data of four patients with oral imidacloprid poisoning were collected and retrospectively analyzed. The relevant literature was then reviewed.Results: Four patients from the same family received different oral doses of imidacloprid. The main clinical manifestations were digestive and neurological symptoms, including nausea, vomiting, and varying degrees of consciousness. Laboratory tests showed an increased white blood cell count, neutrophil proportion, and mild elevation of transaminase and urea nitrogen levels in some patients. Following comprehensive treatment, which included hemoperfusion, gastric lavage, total gastrointestinal decontamination, and drug symptomatic treatment, the patient’s symptoms were quickly relieved, and the concentration of imidacloprid in the blood rapidly decreased.Conclusion: Toxicant detection is an important criterion for the differential diagnosis of poisoning and is helpful for disease assessment, treatment plan formulation, and in determining patient prognosis.

Bearings are crucial in electromechanical equipment, and the state significantly affects operational efficiency and industrial income. Therefore, efficient and accurate monitoring of bearing health and fault diagnosis are imperative for intelligent equipment operation and maintenance. The Kurtogram is widely used for bearing fault diagnosis as it adeptly extracts faulty information from the frequency domain. However, this approach imposes limitations on the center frequency and bandwidth, preventing the association of specific frequency groups with the spectrum. Furthermore, the Kurtogram exhibits poor robustness and cannot support its use with new equipment or under extreme working conditions. To address these shortcomings, this paper introduces a novel technique known as Cepsogram. First, a new cepstrum reconstruction spectral trend estimation method is designed to distinguish different modal information in the frequency domain. In this method, the number of reconstruction iterations is increased to improves the complexity of spectrum trends and expand the diversity of modular segmentation. Additionally, to capture the cyclic transient characteristics of the signal, spectral negentropy is employed to reduce interference and obtain more obvious fault characteristics compared to steepness. The use of engineering data is evaluated to substantiate the effectiveness and stability of this proposed method.

Pyroptosis is a type of programmed cell death that is mediated by both typical and atypical pathways and ultimately leads to the lysis and rupture of cell membranes and the release of proinflammatory factors, triggering an intense inflammatory response. Heart failure (HF) is a serious and terminal stage of various heart diseases. Myocardial hypertrophy, myocardial fibrosis, ventricular remodeling, oxidative stress, the inflammatory response and cardiomyocyte ionic disorders caused by various cardiac diseases are all risk factors for and aggravate HF. Numerous studies have shown that pyroptosis can induce and exacerbate these reactions, causing progression to HF. Therefore, targeting pyroptosis is a promising strategy to treat HF. This paper summarizes the role of pyroptosis in the development of HF and the underlying mechanism involved. Recent research progress on the ability of traditional Chinese medicine (TCM) extracts and formulas to inhibit pyroptosis and treat HF was summarized, and some traditional Chinese medicine extracts and formulas can alleviate different types of HF, including heart failure with preserved ejection fraction (HFpEF), heart failure with reduced ejection fraction (HFrEF), and heart failure with midrange ejection fraction (HFmrEF), by targeting pyroptosis. These findings may provide new ideas and evidence for the treatment or adjuvant treatment of HF by targeting pyroptosis.

ObjectiveIn January 2023, a rare event of collective inhalation paraquat poisoning occurred in Shandong, China. To analyze the clinical characteristics of an event of respiratory tract paraquat poisoning through inhalation.MethodsClinical data from eight patients with paraquat inhalation poisoning were retrospectively analyzed.ResultsThe patients were mainly exposed to paraquat via the respiratory tract. The main clinical manifestations were ocular and respiratory irritation. Lung computed tomography (CT) showed that all eight patients had varying degrees of lung injury, mainly manifesting as exudative lesions. Laboratory tests revealed arterial blood gas hypoxemia, abnormal white blood cell count, and increased neutrophil ratio. Sufficient glucocorticoid impact therapy was effective, and all eight patients survived.ConclusionEight patients experienced chest tightness, shortness of breath, and varying degrees of lung injury due to inhalation of paraquat through the respiratory tract. The early use of glucocorticoids and other comprehensive treatment measures, active prevention and treatment of lung infections, and protection of organ function have beneficial effects in such cases.

This paper summarizes and analyzes the clinical data of a patient with Occupational hydrogen sulfide poisoning admitted to our hospital on March 4, 2023. On the morning of March 2, 2023, the patient worked at an environmental energy company (waste treatment plant) in Shandong Province for the first time, The job was to flush the sludge from the walls of the sludge treatment tank (anaerobic tank) with a water gun, which can release hydrogen sulfide gas. When the patient was about to start work after entering the tank for about 1 min, he suddenly smelled a harsh and pungent odor, felt dizzy and weak, and then the patient suddenly fainted. After hearing the sound of the patient fainting, the workman waiting at the entrance of the tank immediately called someone to go into the tank and quickly pull the patient out, and sent to the local hospital. In the local hospital, the patient was confused, accompanied by irritability, convulsion and other manifestations, and was treated with sedation and nutritional support. Two days later, the patient’s condition did not improve. For further diagnosis and treatment, the patient was transferred to the Department of Poisoning and Occupational Diseases in our hospital. After comprehensive treatment in our hospital, the patient got better and was discharged. Subsequent reexamination and follow-up showed that the patient recovered well. The work unit of the patient did not provide any personal protective equipment. According to the field investigation after the accident, the pipeline around the sludge treatment tank was blocked by sludge, resulting in a large amount of high concentration of H2S accumulated in the tank, causing the patient to faint soon after entering the tank, and his worker should be in the tank for a short time, and no health abnormalities were found. Hydrogen sulfide has a strong irritation to the human body, which can lead to asphyxia or even death in severe cases. The safety prevention and prevention knowledge of hydrogen sulfide poisoning should be popularized among enterprises and workers to reduce the occurrence of such incidents.

Chloroacetyl chloride is a potent acylation agent that decomposes violently in water to produce chloroacetic acid and irritant hydrogen chloride. It and its decomposition products are corrosive to the eyes, skin, and respiratory system and can cause multiple organ failure. Herein, we report cases of poisoning by chloroacetyl chloride and its decomposition products in the skin and respiratory system. After exposure, one patient developed vomiting, irritability, coma, hypoxemia, hypotension, acidosis, and hypokalemia. Another patient developed bronchiolitis, pneumonia, and decreased vision. One patient died and two recovered. Chloroacetyl chloride and its decomposition products are corrosive and can damage multiple organs after absorption through the skin and respiratory tract, leading to severe heart failure. Cardiogenic shock may be the primary cause of early mortality.

Abstract Background Agrobacterium rhizogenes-mediated hairy root transformation provides a powerful tool for investigating the functions of plant genes involved in rhizobia-legume symbiosis. However, in the traditional identification methods of transgenic hairy roots based on reporter genes, an expensive chemical substrate or equipment is required. Results Here, we report a novel, low cost, and robust reporter for convenient, non-destructive, and directly visual selection of transgenic hairy roots by naked eye, which can be used in the study of rhizobia-legume symbiosis. The reporter gene AtMyb75 in Arabidopsis, encoding an R2R3 type MYB transcription factor, was ectopically expressed in hairy roots-mediated by A. rhizogenes, which induced purple/red colored anthocyanin accumulation in crop species like soybean (Glycine max (L.) Merr.) and two model legume species, Lotus japonicas and Medicago truncatula. Transgenic hairy roots of legumes containing anthocyanin can establish effective symbiosis with rhizobia. We also demonstrated the reliability of AtMyb75 as a reporter gene by CRISPR/Cas9-targeted mutagenesis of the soybean resistance to nodulation Rfg1 gene in the soybean PI377578 (Nod-) inoculated with Sinorhizobium fredii USDA193. Without exception, mature nitrogen-fixation nodules, were formed on purple transgenic hairy roots containing anthocyanin. Conclusions Anthocyanin is a reliable, user-friendly, convenient, non-destructive, low cost, directly visual reporter for studying symbiotic nitrogen-fixing nodule development and could be widely applied in broad leguminous plants.
