Abstract

International audience; Bizmoune cave is located in southwestern Morocco on the south-facing slopes of JebelHadid, about 15 km northeast of the modern city of Essaouira. The cave was first identifiedas a Paleolithic site in 2008. The current project, a collaboration between Moroccan, French,German and American researchers and institutions, began in 2014. Archaeological deposits atthe site span the Neolithic, Upper Paleolithic/LSA (Ibero-Maurusian) and MSA (Aterian):the majority of the stratigraphic sequence contains Aterian assemblages. Bizmoune caveholds the potential to provide much unique information about Aterian adaptations in alittle-explored area of western North Africa. Current chronometric information places theAterian deposts in MIS 4 and 5, but it is likely that the earliest archaeological depositspredate the last Interglacial. Aterian layers document changing occupational intensity, withsparse remains at the top of the sequence and deposits very rich in organics and archaeologicalmaterials in the lower part of the sequence. Typical Aterian shaped tools such as tangedpieces and bifacial foliates are relatively abundant, and shell beads have been recoveredfrom the earliest Aterian layers as well. The faunal assemblages, which consist mainly ofAfrican bovids, appear to be primarily or entirely anthropogenic in origin. The presence ofshells of typical marine food species such as Mytilus is noteworthy given the site’s distancefrom the sea during the Pleistocene. The density of land snail shells is very high in theNeolithic, Ibero-Maurusian but also in the earliest MSA deposits. Further study is requiredto determine whether the Aterian snail shells were accumulated by hominins or were theresults of natural deaths within the cave.

Abstract

International audience; Elands Bay Cave (EBC) is one of the key sites for the analysis of the Late Pleistocene/Holocene record in southern Africa. It typifies an area of study, the West Coast of South Africa, which benefits from a long history of research, from the 1960s until today. The 2011 project of EBC was initiated within the framework of the Middle Stone Age (MSA) research at Diepkloof Rock Shelter (DRS). The objective was to build a local synthesis and a complementary picture on the basis of these two sites located 14 km apart from one another, on the left bank of the Verlorenvlei. The excavation at EBC took place during May 2011 with the aim of clarifying the site formation processes, the chronology of the Late Pleistocene occupations as well as the nature of the technological sequence. Our excavation focused on a 1.2 m deep profile that records two main occupational phases separated by a significant hiatus: (1) the initial phase represents an early MSA technology (previously called ‘MSA 1’ by T. Volman 1981) within deposits that started accumulating ca. 250 ka years ago; (2) the second phase documents (late) MSA, Early Later Stone Age (ELSA) and Robberg occupations. The present synthesis is part of a series of several papers that take a multidisciplinary perspective. In this paper, we introduce our 2011 excavation, present our main results and discuss the succession from the late MSA to the LSA at EBC. In an epilogue, we provide a comparison between the archaeological records of EBC and DRS and further explore the reasons why these two sites do not represent similar occupational sequences.

Abstract

Asthmatic airways are infiltrated with inflammatory cells that release mediators and cytokines into the microenvironment. In this study, we evaluated the distribution of CD45-positive leukocytes and eosinophils in lung tissue from five patients who died with severe asthma compared with five patients with cystic fibrosis. For morphometric analysis, the airway wall was partitioned into an 'inner' area (between basement membrane and smooth muscle) and an 'outer' area (between smooth muscle and alveolar attachments). Large airways (with a perimeter greater than 3.0 mm) from patients with asthma or cystic fibrosis had a greater density of CD45-positive cells (p < 0.05) and eosinophils (p < 0.001) in the inner airway region compared with the same airway region in small airways. Furthermore, in small airways, asthmatic lungs showed a greater density of CD45-positive cells (p < 0.01) and eosinophils (p < 0.01) in the outer compared with the inner airway wall region. These observations indicate that there are regional variations in inflammatory cell distribution within the airway wall in patients with asthma that are not observed in airways from patients with cystic fibrosis. We speculate that this inflammatory cell density in peripheral airways in severe asthma may relate to the peripheral airway obstruction characteristic of this condition.

44. Lung resection in patients with preoperative FEV1 < 35% predicted.

Abstract

OBJECTIVES: To determine the morbidity, mortality, and feasibility of lung resection in patients with tumors and preoperative FEV1 < 35% predicted. DESIGN: Retrospective review. SETTING: A 734-bed, tertiary care, academic hospital with a dedicated general thoracic surgery unit performing > 2,000 operations per year. PATIENTS: One hundred consecutive patients with discrete lung tumors and with preoperative FEV1 < 35% predicted undergoing lung resection between September 1997 and May 2003. Only operations with curative intent were included. Average preoperative predicted FEV1 was 26%. Sixteen percent of the patients were oxygen dependent prior to the operation. RESULTS: Open and thoracoscopic wedge resections, segmentectomies, lobectomies, and combined lung resections with lung volume reduction were performed. Sixty-six of the lesions were malignant, and 57 were primary lung cancers. Only one patient left the operating room with positive margins. There was one in-hospital or 30-day mortality. Thirty-six percent of the patients had one or more complications. Twenty-two percent of the patients had prolonged air leaks requiring a chest tube for > 7 days. One patient left the hospital ventilator dependent, 3 additional patients required intubation > 48 h, and 11 patients were discharged with a new oxygen requirement. There were four pneumonias, one myocardial infarction, and two reoperations for bleeding. Male gender (p = 0.003), preoperative oxygen dependence (p = 0.03), and pack-year history (p = 0.006) were associated with a higher overall incidence of complications, while age, incision, diabetes, coronary artery disease, duration of smoking cessation, amount of lung resected, size of lesion, and preoperative percentage of predicted FEV1 did not correlate with the overall incidence of complications. CONCLUSIONS: In a large academic center, minimally invasive surgical techniques, intensive pulmonary care, and advanced anesthetic techniques allow for curative lung tumor resections in patients with very low preoperative FEV1 with a very low mortality and very low incidence of ventilator dependence. Other serious complications such as pneumonia, myocardial infarction, and bleeding are uncommon. An extended hospital stay and a high incidence of prolonged air leak should be expected, especially in patients with preoperative FEV1 < or = 20% predicted. [ABSTRACT FROM AUTHOR]
