Le ricerche presentate in questa tesi hanno l’obiettivo di approfondire le conoscenze sulla complessità delle infezioni endodontiche e di indagare nuove strategie per la disinfezione del canale radicolare. La prima parte è costituita da una revisione sistematica della letteratura sulla prevalenza di Candida nei canali radicolari infetti. La ricerca è stata condotta utilizzando le più importanti banche dati biomediche e ulteriori studi sono stati selezionati dalle bibliografie degli articoli rilevanti. La qualità degli studi è stata valutata utilizzando una versione modificata della Joanna Briggs Institute Critical Appraisal Checklist. Dagli iniziali 2225 studi selezionati, 2118 sono stati esclusi sulla base della valutazione del titolo e dell’abstract. Dei rimanenti 107 studi, 50 sono stati esclusi dopo l’analisi del full-text e 57 studi sono stati inclusi per l’analisi qualitativa e quantitativa. La prevalenza combinata di Candida spp. nelle infezioni endodontiche è stata 8,20% (intervallo di confidenza al 95%: 5,56-11,21%). La specie più frequentemente isolata è stata Candida albicans. E’ stata osservata una significativa eterogeneità fra gli studi (P < 0.001, I2 = 86.07%). L’analisi dei sottogruppi ha mostrato una prevalenza di Candida spp. significativamente più alta nei campioni provenienti dal continente africano. Considerando complessivamente tutti gli studi, un rischio di bias alto o incerto era prevalente per 6 degli 8 items utilizzati per la valutazione qualitativa. Dalla nostra ricerca è emerso che Candida spp. sono presenti in una piccola proporzione di canali infetti, tuttavia la comprensione del reale contributo di Candida nell’ecologia microbica delle infezioni endodontiche richiederà ulteriori indagini. La seconda parte è costituita da indagini in vitro sulle proprietà antibatteriche di KP e L18R, due peptidi sintetici di derivazione anticorpale, nei confronti di un ceppo di riferimento di Enterococcus faecalis. L’attività battericida dei peptidi nei confronti di E. faecalis ATCC 29212 allo stato planctonico è stata valutata mediante saggi convenzionali di determinazione di unità formanti colonia, stabilendo la concentrazione in grado di inibire il 50% della crescita batterica (EC50). L’effetto dei peptidi sulla riduzione della massa e della vitalità del biofilm è stato valutato mediante saggi con Crystal Violet e Alamar Blue. Mediante microscopia confocale a scansione laser (MCSL) e microscopia elettronica a scansione (SEM) è stato valutato l’effetto di KP, L18R e di una soluzione satura di Ca(OH)2 in un modello sperimentale di infezione endodontica costituito da biofilm di E. faecalis cresciuti su dischetti di idrossiapatite. KP e L18R hanno mostrato un’efficace attività battericida nei confronti di E. faecalis allo stato planctonico e L18R si è mostrato 10 volte più efficace rispetto a KP (valori di EC50 rispettivamente di 4.520 × 10‑6 M e 3.624 × 10-7 M). Entrambi i peptidi sono stati in grado di inibire in vitro sia la crescita che l’attività metabolica di biofilm di E. faecalis con un effetto dose-dipendente. Le immagini di MCSL e di SEM hanno mostrato che i peptidi erano in grado di alterare la vitalità e la conformazione dei biofilm anche nel modello di infezione endodontica analizzato. L’analisi quantitativa delle ricostruzioni 3D ottenute mediante MCSL ha mostrato una maggiore efficacia di L18R rispetto a KP e Ca(OH)2. L'esposizione ai peptidi per 24 h a concentrazioni fino a 400 µg/ml non ha determinato effetti tossici nei confronti di fibroblasti L929. I risultati ottenuti mostrano che i peptidi antimicrobici possono rappresentare una promettente strategia per il controllo delle infezioni endodontiche. The research presented in this thesis aims to give further insight into the complexity of root canal infections and to investigate new root canal disinfection strategies. In the first part a systematic review the literature on the prevalence of Candida species in root canal infections was conducted. Extensive literature research was performed in the most important electronic biomedical databases, and additional studies have been identified from references from relevant articles. Studies were critically appraised using a modified version of the Joanna Briggs Institute Critical Appraisal Checklist. From 2225 unique records, 2118 were excluded on the basis of title and abstract. Of the remaining 107 studies, 50 were excluded after full-text review, and 57 were included for qualitative and quantitative analysis. The overall prevalence of Candida spp. in root canal infections was 8.20% (95% confidence interval, 5.56%–11.21%). Candida albicans was the most frequently isolated species. Significant heterogeneity among studies was observed (P < .001, I2 = 86.07%). Subgroup analyses revealed a higher prevalence of Candida spp. from African samples. All studies considered, a high or unclear risk of bias was prevalent regarding 6 out of the 8 items considered in the critical appraisal. According to our study, Candida spp. occurred in a small proportion of root canal infections and further research is needed to investigate the real contribution of Candida spp. to the microbial ecology in infected root canals. In the second part we assessed the antibacterial properties of two synthetic peptides, namely KP and L18R, against planktonic cells and biofilms of the endodontic pathogen Enterococcus faecalis and their cytotoxicity against fibroblast cells. The KP and L18R bactericidal activity against E. faecalis ATCC 29212 was evaluated by the colony forming unit assay and the half maximal effective concentration (EC50) was calculated. The peptide inhibition of E. faecalis biofilm mass and metabolic activity was assessed. Peptide effects on 2-day-old E. faecalis biofilms grown on hydroxyapatite discs were compared to a Ca(OH)2 saturated solution by confocal scanning laser microscopy (CLSM) and scanning electron microscopy (SEM). In order to test peptide cytotoxicity, L929 fibroblasts were exposed to KP and L18R for 24 hours and cell metabolism was evaluated by MTT assay. Both KP and L18R showed an effective bactericidal activity against planktonic E. faecalis and L18R proved to be 10-folds more effective compared to KP (EC50 value of 4.520 × 10‑6 M versus 3.624 × 10-7 M). The EC50 values, calculated with reference to the biomass and metabolic activity reduction, were 1.607 × 10-4 M and 1.235 × 10-4 M for KP and 1.411 × 10-5 M and 1.585 × 10-5 M for L18R, respectively. CLSM and SEM images showed that Ca(OH)2, KP and L18R remarkably impaired E. faecalis biofilms grown on hydroxyapatite discs and L18R was associated to a higher dead bacteria percentage compared to the other treatments. Toxicity studies demonstrated that 24-hour incubation with the two peptides at concentrations between 50 µg/ml and 400 µg/ml had no adverse effect on L929 fibroblast viability. KP and L18R effectively inhibited E. faecalis, both in planktonic and biofilm state and showed no cytotoxic effects in our experimental settings. L18R demonstrated a more potent antibacterial activity compared to KP. The obtained results show that antimicrobial peptides may represent a promising new strategy for endodontic infection control.

The aim of this study is to evaluate the possible intraoperative advantages of Nd:YAG laser and quantic molecular resonance (QMR) scalpel in oral soft tissue surgery. One hundred sixty-three interventions were evaluated. Group 1 (G1) included 77 interventions performed with Nd:YAG laser, group 2 (G2) 45 with QMR scalpel, and group 3 (G3) 41 with cold blade. Parameters analyzed were as follows: speed of incision, time of intervention, intraoperative bleeding, number of stitches, patient compliance, and operator comfort. Data were analyzed using software STATA 12 (StataCorp LP, College Station, TX, USA). Mean speed of incision was 0.54 mm/s in G1, 2.83 mm/s in G2, and 1.58 mm/s in G3, Nd:YAG laser being the slowest. However, no significant differences among times of intervention were found. In particular, interventions in G1 (221.15 ± 220.89 s) have a mean duration lower than G2 (280.56 ± 248.31 s) and G3 (316.10 ± 248.69 s). Intraoperative bleeding occurred in 29.9 % (n = 23/77) of interventions in G1, 97.8 % (n = 44/45) in G2, and 97.6 % (n = 40/41) in G3 (p < 0.0001). Mean number of stitches in G1 was statistically lower (G1, 0.10; G2, 2.07; G3, 2.29; p < 0.0001). No differences with regard to patient compliance were detected. Operator comfort was higher in G1 (p < 0.0003). Nd:YAG laser and QMR scalpel give several advantages in oral mucosal surgery: Nd:YAG laser cuts tissue slowly, but it provides a good visibility and excellent hemostasis. QMR scalpel allows a very rapid cutting with no considerable temperature increase, but there is major risk of bleeding and need for sutures with lower operator comfort.
