Objectives The aims of this study were to determine if there was an increased risk of incident cardiovascular disease (CVD) and diabetes and an increase in arterial stiffness in participants who reported working 41–54 h per week and more than 55 h compared to those who worked 40 h or less over a time interval of 5 years. Methods In a subsample of the population-based prospective Gutenberg Health Study (GHS) study, we examined working participants younger than 65 years at baseline (n = 7241) and after 5 years. To test the association of working time at baseline and incident cardiovascular events and diabetes type II, we estimated hazard ratios (HR) using competing risks models. For a change in the arterial stiffness index (SI) based on assessment using a Pulse Trace PCA2 device, we used multivariate linear regression models. Results The SI increased in those working more than 55 h per week (beta coefficiant = 0.32 m/s (95% CI 0.07–0.58) compared to those working 40 h and less after adjustment for sex, age and SES. Due to small numbers there was no significant association of working hours and clinically manifest cardiovascular events and diabetes type II in the 5-year follow-up time. Conclusions Further studies are needed to confirm the results on working hours and arterial stiffness. Analyses of the 10-year follow-up with more events may clarify the results for incident cardiovascular events and metabolic outcomes.

Objectives Following an exploratory approach, we examined cardiovascular disease risk factors at baseline and the 5-year incidence proportion of self-reported doctor-diagnosed cardiovascular diseases (CVD) in teachers and other occupational groups of the Gutenberg Health Study. Methods Study participants lived in the region of Mainz, Germany. Data from 6510 working participants without prevalent CVD at baseline (2007–2012) were analyzed. Participants were teachers (n = 215), other professionals from the health, social or educational (HSE) fields (n = 1061) or worked outside the HSE fields (n = 5234). For occupational comparisons, we estimated prevalence ratios (PR) for each CVD risk factor at baseline with robust Poisson regression analyses. We calculated crude CVD incidence rates based on the observed 5-year CVD cumulative incidence at follow-up and estimated age-weighted incidence proportions. All analyses were stratified by sex. Results Male non-HSE workers showed a higher prevalence of smoking and physical inactivity than male teachers (PR 2.26; 95%-CI: 1.06–4.82/PR 1.89; 95%-CI: 1.24–2.87). In contrast, non-HSE workers and other HSE professionals were less likely to have reported an unhealthy alcohol intake than teachers. Differences were attenuated after SES-adjustment. We did not detect occupational group-specific differences in CVD incidence. However, there were only two cases of CVD among the teachers. Conclusion Particularly male teachers showed a healthier lifestyle regarding physical inactivity and smoking. Nevertheless, occupational-medical care practitioners and researchers need to be aware of the relatively heightened prevalence of unhealthy alcohol intake in female and male teachers, and in absolute terms, the high hypertension prevalence in male teachers.

Zusammenfassung Ziel Für SPGS wurde inhaltsanalytisch und unter Berücksichtigung personen- bzw. tätigkeitsbezogener Belastungs- und Beanspruchungsmerkmale erfasst, welche Maßnahmen aus Sicht von LK zur Verbesserung der gesundheitlichen Situation am Arbeitsplatz beitragen können. Methodik Die Daten stammen aus einer vom Institut für Lehrergesundheit (IFL) von August 2013 bis April 2015 an 14 SPGS in Rheinland-Pfalz durchgeführten schriftlichen Querschnittserhebung. Die Teilnahmequote liegt bei 46,3% (143/309 LK). Antworten auf die offene Frage „Welche konkreten berufsbezogenen Maßnahmen würden zu einer Verbesserung Ihres Wohlbefindens oder Ihrer gesundheitlichen Situation am Arbeitsplatz beitragen?“ wurden kategorisiert und quantifiziert. Es erfolgte eine Item-Non-Responder-Analyse. Logistische Regressionsanalysen dienten dazu, Belastungs- oder Beanspruchungsmerkmale zu identifizieren, die mit der Benennung spezifischer Kategorien in Zusammenhang stehen. Ergebnisse Es wurden die Angaben von 143 LK ausgewertet. 79 LK machten insgesamt 362 Vorschläge zur Verbesserung der gesundheitlichen Situation am Arbeitsplatz. Die Abgabe eines Vorschlags wurde nicht von soziodemografischen Variablen beeinflusst. 33,6% der LK thematisierten die baulichen/physikalischen Arbeitsbedingungen, bspw. klimatische oder akustische, oder die Schaffung bislang fehlender Rückzugsräume. 29,4% der LK machten Vorschläge zu personalstrukturellen Maßnahmen (Stichwort: „Doppelbesetzung“). 9,1% der LK führten Aspekte der kollegialen Zusammenarbeit an. Den Regressionsanalysen zufolge steht die Benennung einzelner Kategorien in Zusammenhang mit spezifischen tätigkeitsbezogenen Belastungsmerkmalen. Zudem weisen LK, die sich stärker beansprucht fühlen, hier: ihre Arbeit als (sehr) stark gesundheitsgefährdend wahrnehmen, eine signifikant höhere Chance (Odd) auf, Vorschläge der Kategorie „KollegInnen“ (aOR 3,71; 95%KI 1,00–13,76) oder „Externe Unterstützung“ (z. B. Supervision) (aOR 3,92; 95%KI 1,15–13,41) zu machen als weniger stark beanspruchte LK. Schlussfolgerungen An SPGS tätige LK zeigen sich bereit, sich aktiv an der gesundheitsbezogenen Organisationsentwicklung zu beteiligen. Die kombinierte Auswertung qualitativer und quantitativer Daten erleichtert die Entwicklung problem- und zielgruppenspezifischer Interventionsansätze.

Hintergrund Sonderpadagogische Forderung ist und bleibt ein wesentlicher Bestandteil des deutschen Schul- und Bildungssystems. Da verschiedene Schulsysteme mit unterschiedlichen Anforderungsprofilen einhergehen, kann erwartet werden, dass sich Belastungen und Beanspruchungen fur Beschaftigte in Abhangigkeit von der Schulform unterscheiden.

Teachers' suggestions to enhance their health at inclusive primary schools ("Schwerpunktgrundschulen") were analyzed by quantitative content analysis taking personal or job-related parameters of effort and strain into consideration.The paper and pencil cross-sectional study was conducted by Institut für Lehrergesundheit (IFL) between August 2013 and April 2015 at 14 schools in Rhineland-Palatinate. Overall response rate was 46.3% (143/309 teachers). The answers to the open-ended question "Which concrete job-related measures would enhance your wellbeing or health situation at your work place?" was categorized and quantified. An analysis of the item-non-response was undertaken. The aim was to identify by logistic regression, personal or job-related parameters of effort or strain associated with a specific category.Data from 143 teachers were analyzed. 79 teachers made 362 suggestions to enhance the health situation at their work place. Making a suggestion was not related to socio-demographic parameters. 33.6% of the teachers addressed physical environmental conditions of work, e. g., climatic or acoustic conditions or the availability of space for recreation. 29.4% of the teachers made suggestions concerning human resource strategy (e. g., "double teaching"). 9.1% of the teachers referred to aspects of relationships with colleagues. After regression analysis, there were relationships between suggestions of single categories and specific job-related parameters of effort. Furthermore, teachers under (high) strain - here: teachers who (very) strongly experienced their work as hazardous to health - showed significantly higher odds to make suggestions of the categories "Colleagues" (aOR 3.71; 95%CI 1.00-13.76) or "External Support" (e. g., "supervision") (aOR 3.92; 95%CI 1.15-13.41) than teachers experiencing less stress.Teachers of inclusive primary schools are ready to take active part in health-related organizational development. Introduction Since 2011 the Institute for Teachers’ Health (IfL) is responsible for the occupational health of more than 40.000 German teachers. Analysing teachers’ accidents is an important part of everyday work. The aim of the study was to describe accident black spots and type of injuries. We further wanted to identify factors influencing accident severity. Methods We included accident reports of teachers between August 2011 and July 2014 (three school-years) which occurred at schools in Rhineland-Palatinate (Germany). In case of an accident, teachers fill out an accident report which is forwarded either to the statutory accident insurance or to the IfL. For the present analysis, anonymised data from the two sources were combined. Accidents leading to hospitalisation or ≥4 days of work inability were classified as severe. Logistic regression analysis was applied to identify influencing factors on severity. Results Altogether, 2,212 accidents were reported in three school-years. Victims were on average 45 years old and mainly female (70.6%). Commuting accidents accounted for 23.5% of all accidents. Most accidents occurred in the gym (18.2%), on the street (17.1%) and in the classroom (14.9%). Accidents were mainly caused by falls (42.6%) and collisions/hits (38.5%). Frequent types of injuries were bruises/concussions (30.4%), strains (26.9%), and ruptures (22.4%). Most affected parts of the body were foot (18.1%), neck/spine (16.0%), and head (15.6%). 34.6% of accidents were classified as severe. Factors influencing severity included age, type of school, commuting accident, place and mechanism of accident, and injury type. Discussion Prevention of accidents is an important task, since they cause suffering to victims and relatives. Furthermore school lessons are cancelled if substitute teachers are unavailable. Analysing accidents is an important first step, as black spots can be identified and targeted preventive measures can be implemented. Note The abstract is part of the dissertation of M. Claus
