We present a case of a 65-year-old male with chronic obstructive pulmonary disease who developed multiple BPFs postresection of the right lower lobe due to lung cancer and with tumor recurrence and metastasis. We employed a comprehensive management strategy comprising rigorous infection control, meticulous chest drainage, antitumor treatment, and targeted deployment of bronchial stents. This approach not only resolved the BPFs but also achieved a complete response of the lung cancer, extending progression-free survival to over 3 years. In this prospective (Clinical Trials Registry-India [CTRI] registered) observational study, we present the comparative oncological outcomes of ADT alone and ADT + abiraterone in Indian patients, which is not available currently. Methods: This study (CTRI-number-CTRI/2020/07/026545) included newly diagnosed mHSPC patients from January 2020 to June 2023 in a tertiary care hospital, urology department. Patients fulfilling inclusion criteria were advised ADT with abiraterone (A + ADT), and those not affording received ADT monotherapy (ADT). The primary endpoint was overall survival (OS). Secondary outcomes included prostate-specific antigen (PSA) decline >90, radiographic progression-free survival (rPFS), and PSA progression-free survival (pPFS). Results: Out of 278 patients with mHSPC, 163 patients were excluded and 115 were analyzed (ADT = 40 vs. A + ADT = 75). After a median follow-up of 20.3 months, 11 of 40 (27.5) in ADT-only arm and 15 of 75 (20) in ADT + abiraterone arm had died (Hazard-ratio of death 0.72; 95 confidence interval 0.68-0.88; P 90 was seen in 85 in the ADT alone group and 93.3 in the ADT + abiraterone group. Significantly better outcomes of the ADT + abiraterone were seen in the secondary endpoints of rPFS (P < 0.001) and pPFS (P < 0.001). The OS benefit was 28 reduction in risk of death in our study versus 37 and 38 in STAMPEDE and LATITUDE, respectively. pPFS and rPFS were also poorer in Indian subsets. Conclusions: Abiraterone with ADT improves OS, PSA response, rPFS, and pPFS in the Indian population akin to the Western data but with poorer OS, rPFS, and PSA progression-free survival on comparison.

Methods: We retrospectively reviewed the clinicopathologic data and follow-up information of 266 patients who underwent LN dissection for apparent early-stage EOC between January 2018 and September 2022 at the Obstetrics and Gynecology Hospital of Fudan University. Results: Among 266 patients, 44 (16.5) showed LN metastasis, of which 65.9 and 59.1 presented in the pelvic region and para-aortic region, respectively. Univariate analysis revealed higher LN positivity in patients with high-grade serous carcinoma (HGSC), preoperative imaging suggestive of LN metastasis, bilateral adnexal involvement, lymphovascular space invasion (LVSI), positive peritoneal cytology, and clinical stage IIA. LN metastases were identified in 7.9, 10.2, and 39.7 of clinical stage IA/B, IC, and IIA disease cases, respectively. Multivariate analysis confirmed significantly higher LN positivity rates in patients with HGSC, LVSI, and clinical stage IIA. In clinical stage IIA EOC, the 3-year progression-free survival (PFS) rates were 65.8 and 77.4 (P = 0.360) for LN-negative and LN-positive groups, respectively. In clinical stage I EOC, the 3-year PFS rates were 93.5 and 59.4 (P < 0.001) for LN-negative and LN-positive groups, respectively. Conclusions: High-grade serous histology, LVSI, and clinical stage IIA disease are predictive factors for LN involvement in early-stage EOC. In addition, LN metastasis appears to be associated with worse PFS in clinical stage I EOC compared with clinical stage IIA EOC. Introduction Clear cell sarcoma (CCS) is commonly seen in adolescents and young adults, with most patients aged 20 to 40 years at the time of diagnosis. (1) It commonly develops [...], BACKGROUND Clear cell sarcoma (CCS) is a rare but aggressive malignancy that typically occurs in young adults and is characterized by soft tissue tumors of the extremities. CCS can be difficult to distinguish from metastatic melanoma based solely on histology and immunohistochemistry (IHC) because of the significant overlap between them. However, it is imperative to get an accurate clinical diagnosis, as it informs disease staging and treatment options for patient care. Present in approximately 75% of CCS cases, the EWSR1 gene rearrangement detected by fluorescence in situ hybridization (FISH) can help with establishing a diagnosis; the underlying reciprocal translocation has never been reported in cutaneous melanoma. CASE DESCRIPTION We reviewed a case of a young woman who presented with a confusing picture of widespread lymphadenopathy, cutaneous metastases, and electrolyte derangements and was subsequently diagnosed with metastatic CCS. CONCLUSIONS This case suggests possible value in performing molecular testing when a clinical picture does not correspond with what is expected for melanoma. It also raises the question of whether CCS cases may be underreported. This case highlights an uncommon presentation that may not be recognized as a manifestation of CCS by an oncologist who is not a sarcoma specialist. It is unclear how COVID-19 vaccination contributed to her clinical presentation, and it is also unclear whether an early diagnosis would have changed her clinical outcome.

Abstract

Introduction Metastasis accounts for over 90% of cancer-related deaths, calling for new strategies to prevent and treat metastasis (1). Recent studies using single-cell lineage tracing and single-cell RNA-Seq (scRNA-Seq) technologies [...], Cancer cell plasticity contributes to therapy resistance and metastasis, which represent the main causes of cancer-related death, including in breast cancer. The tumor microenvironment drives cancer cell plasticity and metastasis, and unraveling the underlying cues may provide novel strategies for managing metastatic disease. Using breast cancer experimental models and transcriptomic analyses, we show that stem cell antigen-1 positive ([SCA1.sup.+]) murine breast cancer cells enriched during tumor progression and metastasis had higher in vitro cancer stem cell-like properties, enhanced in vivo metastatic ability, and generated tumors rich in [Gr1.sup.hi][Ly6G.sup.+][CD11b.sup.+] cells. In turn, tumor-educated [Gr1.sup.+][CD11b.sup.+] (Tu-[Gr1.sup.+][CD11b.sup.+]) cells rapidly and transiently converted low metastatic [SCA1.sup.-] cells into highly metastatic [SCA1.sup.+] cells via secreted oncostatin M (OSM) and IL-6. JAK inhibition prevented OSM/IL-6-induced [SCA1.sup.+] population enrichment, while OSM/IL-6 depletion suppressed Tu-[Gr1.sup.+][CD11b.sup.+]-induced [SCA1.sup.+] population enrichment in vitro and metastasis in vivo. Moreover, chemotherapy-selected highly metastatic 4T1 cells maintained high [SCA1.sup.+] positivity through autocrine IL-6 production, and in vitro JAK inhibition blunted SCA1 positivity and metastatic capacity. Importantly, Tu-[Gr1.sup.+][CD11b.sup.+] cells invoked a gene signature in tumor cells predicting shorter overall survival (OS), relapse-free survival (RFS), and lung metastasis in breast cancer patients. Collectively, our data identified OSM/IL-6/JAK as a clinically relevant paracrine/autocrine axis instigating breast cancer cell plasticity and triggering metastasis.
