Background The Danish health care system is facing major challenges as the prevalence of chronic diseases increases. There is a need for new approaches and strategies to prevent chronic disease and promote health and well-being among citizens. The aim is to describe the development of a new model for coordinated, integrated and evidence-based health promotion and disease prevention in Danish municipalities. The model builds on the supersetting approach, intersectoral collaboration and community engagement and applies a broad bio-psychosocial concept of health. Methods Two Danish municipalities were included in the initial development and testing of the model from 2019 to 2021. This involved the following steps in each municipality: 1) Analyzing the health status, lifestyles and socio-economy at municipality level. 2) Mobilizing lead municipal administrators and politicians for intersectoral action including jointly defining thematic focus areas and target populations. 3) Mapping community-based stakeholders, physical environments and existing evidence to qualify relevant action 4) Mobilizing professional stakeholders from the public, private and civic sectors for co-creation of intervention ideas and joint action. 5) Co-creating and implementing interventions together with professional stakeholders and citizens. Results The strategic model and results from the development process will be presented from one of the involved municipalities: The municipal administration chose physical activity and well-being among children and young people as their key focus area. Community-based stakeholders from non-profit organizations and public institutions, including sports clubs, leisure clubs, primary schools, and public departments jointly developed and implemented specific interventions. One specific intervention aimed to engage more children in local clubs. Coaches from three local sport clubs introduced 1st and 4th grade students at two schools to their sport (a course of eight times) during students' time in their local after-school club. Overall, the process fostered broad engagement of stakeholders from the public sector, the private sector, and civil society. Conclusion The model developed in Our Healthy Community builds on contextual analyses, dialogues, workshops, and co-creation processes with a wide range of stakeholders to promote local relevance, integration and sustainability of developed actions and interventions. The model will be pilot tested in two other Danish municipalities (2022-2025)

This paper introduces the conceptual framework and intervention model of Our Healthy Community (OHC), a new, coordinated, and integrated approach towards health promotion and disease prevention in municipalities. The model is inspired by systems-based approaches and employs a supersetting approach for engaging stakeholders across sectors in the development and implementation of interventions to increase health and well-being among citizens. The conceptual model includes a combination of a bottom-up approach emphasizing involvement of citizens and other community-based stakeholders combined with a top-down approach emphasizing political, legal, administrative, and technical support from a variety of councils and departments in local municipality government. The model operates bidirectionally: (1) by pushing political and administrative processes to promote the establishment of conducive structural environments for making healthy choices, and (2) by involving citizens and professional stakeholders at all levels in co-creating processes of shaping their own community and municipality. An operational intervention model was further developed by the OHC project while working with the OHC in two Danish municipalities. The operational intervention model of OHC comprises three main phases and key actions to be implemented at the levels of local government and community: (1) Local government: Situational analysis, dialogue, and political priorities; (2) Community: Thematic co-creation among professional stakeholders; and (3) Target area: Intervention development and implementation. The OHC model will provide municipalities with new tools to improve the citizens’ health and well-being with available resources. Health promotion and disease prevention interventions are developed, implemented, and anchored in the local community by citizens and local stakeholders at municipal and local community levels using collaboration and partnerships as leverage points.

Background: The family is an important setting in the promotion of child health. The parent–child relationship affects the social and health development of children, and children’s healthy behaviors are associated with positive parenting strategies. The parent–child relationship is bi-directional and the connection between parenting and child health is complex. However, few parenting interventions work with parents and children together, and more knowledge is needed on how to develop and implement interventions promoting healthy parent–child relationships. Focusing on a family cooking class program, this study addresses how community initiatives engaging parents and children together can contribute to integrating parenting support with local health promotion. Methods: Participant-driven photo-elicited interviews (nine families), focus group evaluations (nine parents/14 children) and observations during cooking classes (10 classes) were applied to analyze the tools and mechanisms that can support positive parenting. Results: The study found that visual, practical and sensory learning techniques, applied in a context-sensitive learning environment that ensured guidance, safety and a friendly social atmosphere, contributed to positive parent–child interaction and bonding. Conclusion: The cooking program facilitated parenting practices that support child involvement and autonomy. Thus, the program constituted an effective intervention to strengthen parent–child relationships and positive parenting.

Aims: This paper examines the importance of recruitment site in relation to the recruitment of ethnic minorities into health research. It presents a synthesis of experiences drawn from six interlinked Danish studies which applied different methods and used healthcare facilities and educational settings as sites for recruitment. Methods: Inspired by interpretive reviewing, data on recruitment methods from the different studies were synthesized with a focus on the various levels of recruitment success achieved. This involved an iterative process of comparison, analysis and discussion of experiences among the researchers involved. Results: Success in recruitment seemed to depend partly on recruitment site. Using healthcare facilities as the recruitment site and healthcare professionals as gatekeepers was less efficient than using schools and employees from educational institutions. Successful study designs also depended on the possibility of singling out specific locations with a high proportion of the relevant ethnic minority target population. Conclusions:The findings, though based on a small number of cases, indicate that health professionals and healthcare institutions, despite their interest in high-quality health research into all population groups, fail to facilitate research access to some of the most disadvantaged groups, who need to be included in order to understand the mechanisms behind health disparities. This happens despite the genuine wish of many healthcare professionals to help facilitate such research. In this way, the findings indirectly emphasize the specific challenge of accessing more vulnerable and sick groups in research studies.

Aims: Adult non-Western immigrants in Scandinavia tend to be worse off in terms of health than native-born populations, which cannot be fully ascribed to their often lower socioeconomic status (SES). This review examines if differences in health status are also present between non-Western immigrant and majority children in Denmark, Norway and Sweden, and if SES explains the differences. Methods: Following PRISMA guidelines, relevant Scandinavian peer-reviewed quantitative publications since 1990 were identified through a systematic search of PubMed, EMBASE, Scopus, Web of Science and SveMed. Of 1197 identified publications, 27 remained relevant after applying inclusion criteria: 3 Danish, 6 Norwegian and 18 Swedish studies. Results: Non-western immigrant children had overall poorer outcomes compared with ethnic majority children in Denmark, Norway and Sweden in health issues covered by the included studies: diabetes, obesity, oral and mental health, and well-being. However, in diabetes, obesity and mental health, non-Western immigrant children from certain countries and regions, and descendants of non-Western immigrants had similar/more favourable outcomes than majority children. In mental health and well-being, ethnic inequalities were strongly associated with SES, while for diabetes, obesity and oral health, differences remained significant after adjusting for SES. Conclusions: Overall poorer health outcomes in non-Western immigrant compared with majority children in Scandinavia cannot be fully explained by SES. Evidence points to additional mechanisms at individual, household, societal or policy levels, including reasons for migration, culture and societal discrimination. Finally, methodological issues may influence study outcomes, e.g. heterogeneity of populations studied and socioeconomic variables included.

This article explores how health responsibility in relation to body weight management is institutionally enacted in three welfare institutions in the interplay between traditional ‘social interventionist’ welfare and ‘non-interfering’ neoliberal ideology. The analysis asks how citizens of non-Western origin at different life stages are positioned within, and navigate, opposing ideological value systems, and adapt to, or resist, institutionally constructed ‘health subjectivities’. The cross-case analysis shows that as people grow older, the institutional requirement to adapt to neoliberal norms of individual responsibility increases, but that in all three settings health responsibility is ambiguously distributed and enacted among welfare state agencies and citizens, and that this renders children, youths and parents vulnerable in different ways. We identify how marginalised citizens are constituted by, but also resist, neoliberal health promotion policies as welfare policy moves away from universalism and towards targeting in Denmark, which appears to increase health inequalities along socio-economic and ethnic lines.

This article addresses race and ethnicity as social practices among young students at a Danish public sports school and explores how these practices engage with emotional well-being in the institutional context. The study is based on ethnographic fieldwork carried out in two school classes in 2012–2013 using multiple qualitative methods. Taking a phenomenological practice approach, the article addresses how racial (and ethnic) practices affect everyday school life. The analysis shows how a common-sense, habitual background of whiteness positions non-white bodies as different and ‘non-belonging’, thus shaping experiences of being ‘out of place’. These experiences are stressful to students in the study and foster a selfawareness that restrains the body from engaging habitually in the world and that obstructs emotional well-being. The article argues that a reluctance to acknowledge social practices as racial enables everyday racism while blocking the positions available to speak out against ethnic and racial discriminatory experiences.

This article explores how everyday school life interacts with students’ practices of ‘becoming teenagers’ at a Danish school, analysing how age and ethnicity intersect with emotional well-being. The article builds on an ethnographic study at a public sports school following ethnic minority and majority students in two school classes from the fifth to seventh grades. Taking a practice approach, the article first analyses school as a social site before turning phenomenological attention to experiences and expectations of becoming teenagers, focusing on the experiences of ethnic minority students. The article addresses how school as social site constituted by discursive, material and social arrangements shapes a normative linear process of becoming at school, that is, becoming a responsible, healthy, Danish citizen. Consequently, dissonance between embodied being and expected normality affects the emotional well-being of ethnic minority students, whose transnational practices are constrained within a national practice architecture.

Promoting healthy eating among children has high priority in Nordic countries but remains complex. With the purpose of contributing knowledge to inform efforts to promote healthy eating environments in schools, this article explores how children feel and reflect about eating at school and seeks to nuance understandings of how food and eating are situated in school life. The article draws on ethnographic studies carried out at a Danish public sports school following two classes from fifth to seventh grades (age 11–14). By adopting a practice perspective and the analytical concepts of foodscapes and emotional spaces, the article analyses how emotions (affects and feelings), discourses, materialities, and social relations within the school interact. The findings show that many students find eating at school unpleasant. Students want to eat in an un-stressful place away from the gaze of others. They want to eat in “a safe space”, which is difficult to find at school. Students’ accounts reveal how eating at school, intersected by the transitional life-phase of youth, is affected by normativity, control and (self-)discipline, which shape and constrain their eating habits. The article points to the importance of addressing the emotional dimensions of eating in efforts to promote school health.
