Abstract

Atrial fibrillation (AF) is the most common arrhythmia worldwide. Thromboembolism from the left atrial appendage (LAA) is the most feared complication in patients with AF. The cornerstone for the management of AF is oral anticoagulation to reduce the incidence of cardioembolic stroke. There is, however, a significant proportion of patients who cannot undergo long-term oral anticoagulation. Transcatheter LAA occlusion is an evolving technology with proven benefits in terms of AF-related stroke prevention, representing a valid alternative to anticoagulation for high-risk patients with contraindications for long-term oral anticoagulation. This has resulted in the development of a plethora of transcatheter devices to achieve endocardial occlusion or epicardial exclusion of the LAA.A panel of expert Italian cardiologists gathered under the aegis of the Italian Society of Interventional Cardiology (SICI-GISE) with the aim of reviewing the most relevant aspects of LAA occlusion, underlying anatomy and pathophysiology, summarizing current clinical knowledge, and discussing the practicalities of available devices and imaging techniques. Finally, the position paper highlights the importance of an adequate environment and of an appropriate organization in order to optimize all steps of the procedure.

21. Peripheral intravascular lithotripsy to facilitate transfemoral TAVR: a multicentric prospective registry

Abstract

Background The presence of severe calcific atherosclerotic disease at iliac artery level is a contraindication to transfemoral (TF) TAVI procedures, challenging TF delivery in 15–20% of cases. Many case reports described the efficacy of Intravascular lithotripsy (IVL) to facilitate TF access but only one prospective registry has been reported in literature and dates back 2018. For this reason a new multicenter prospective registry was ideated to confirm the role of IVL technology in facilitating TF TAVR. Aims The aims of this study were 1)to analyze the trend of TF TAVR compared to alternative approaches in the last 5 years and 2)to evaluate the prevalence of IVLassistedTAVR since it was introduced in this specific setting in 2018 3) to evaluate the success rate in terms of valve performance and procedural success of TF-TAVR system delivery after IVL lesion preparation. Materials and methods We prospectively collected data from all consecutive TAVRprocedures performed between Jan2016 andDec2020 at 4Italian and 1 Denmark centres. All patients underwent CT angiography of lower extremity before the procedure in order to assess the severity of aorto-iliac-femoral calcification and to select those patients who required specific lesion preparation to preserve TF access. For each target calcified lesion we measured length, diameter and %of stenosis, circumferential extension and minimal cross-sectional area. Angiographic IVL-related and access site complications (dissection, perforation, major bleeding) were examined. Results Between 2016 and 2020, a total of 3710 TAVR were performed, 3428 (92%) via TF route while the remaining 240 (8%) included alternative approaches mainly represented by subclavian and transapical access. IVL-assisted TAVR were 0 in 2016 and 2017, 13 (2%) in 2018 when the first IVL-assisted aortic valve implantation was performed and doubled in 2019, reaching a total of 112 in the whole 5-year period considered. The number of IVL-facilitated TAVR has been increased in the 5-year period, achieving 7% of the TF procedures while non-TF TAVR decreased considerably from 10% to 6%. Common and external iliac axis was the target lesion in the majority of cases (54%) followed by common iliac artery alone. lesion minimum diameter 4.7mm, with average stenosis of 50%. The maximum calcium angle was 332°. The majority of IVL was performed with a 7-mm catheter (78.6%). 1 balloon per lesion was employed. 1 perforation and 2 severe dissections occurred, that required stent placement. In 55% of cases the aortic regurgitation was absent or minimal. Conclusions TF approach remained the first choice for TAVR procedure in the majority of cases compared to non-TF thanks to the progressive increase of IVLassistedTAVR that allowed operators to preserve TF route. Peripheral IVL appeared feasible, safe and effective in patients with severe peripheral artery disease, with high success rate in terms of valve performance and low rate of complications. Abstract

La scelta di analizzare l’argomento oggetto della presente trattazione, pur deri-vando da un interesse di ricerca personale, sviluppato in particolare negli ultimi anni, presenta alcuni elementi di collegamento con i temi trattati dal Prof. Lucia-no Marchi nella suaLectio Magistralis che si è tenuta a ottobre 2019 presso l’Università di Pisa . L’informazione aziendale non può essere limitata alla di-mensione economico-finanziaria: tale esigenza ormai diventata un “imperativo categorico, è avvertita in misura crescente sia dal business che dalla finanza e ciò si riflette indubbiamente anche nell’ambito dei sistemi di controllo e di revisione. L’analisi, per quanto sommaria, della tematica dell’informativa non finanzia-ria e del controllo richiama inoltre la contrapposizione fra dimensione interna e esterna. Il Prof. Marchi era solito, nelle sue lezioni e spesso anche nei suoi di-scorsi introduttivi in sede di apertura di molti convegni, richiamare e spesso enfa-tizzare la contrapposizione fra controllo “interno” e controllo “esterno” e in gene-rale fra dimensione “interna” ed “esterna” in ambito aziendale. Devo ammettere che tale distinzione mi appariva un po' semplicistica: negli ultimi anni mi sono invece resa conto che stavo commettendo un grande errore non soltanto per l’effettiva rilevanza della distinzione ma anche perché, in riferimento all’informativa non finanziaria, tale contrapposizione assume una particolare ri-levanza tanto da stimolare molteplici ricerche, anche a livello internazionale. Si assiste infatti ad un costante richiamo negli studi, sia di reportistica che di con-trollo aziendale, alla contrapposizione fra dimensione “interna” ed “esterna”, do-ve indubbiamente la crescente importanza degli interessi degli stakeholder esterni ha svolto un ruolo determinante. Richiamando la sua Lectio Magistralis infatti il Prof. Marchi sottolinea quanto l’evoluzione del sistema di controllo interno “nella direzione del risk management, da un lato, e della

Abstract

Purpose: The aim of this study is to compare two different methods for auto-contouring for prostate radiation therapy. On the one hand Atlas-Based segmentation, on the other a deep-learning Artificial Intelligence (AI) based method by means of the recently developed software module Contour ProtégéAI ofMIM Maestro (MIM Software Inc., Cleveland, OH, USA). Methods: Ten patients with prostate cancer treated at the Radiotherapy Unit of S.Stefano Hospital of Prato (IT) were selected retrospectively by specific inclusion criteria. To make a comparison between the auto-segmentation methods in prostate radiation therapy the manual contouring was used as a reference, called Gold Standard. Similarity indices, like as Dice Similarity Coefficient (DSC) and Mean Distance to Agreement (MDA), are used to compare AI and Atlas-based contours with Gold Standard contours. Results: Data analysis show a significant difference in results obtained by Atlas based segmentation and AI. Significant differences in DSC and MDA (in terms of mean and SD) values between the two automatic methods of segmentation are present in the prostate (Mean DSC AI 0.78 ± 0.07 vs Atlas-based 0.64 ± 0.10; Mean MDA AI 1.42 ± 0.34 vs Atlas-based 6.56 ± 2.85) and rectum (Mean DSC AI 0.86 ± 0.05 vs Atlas-based 0.58 ± 0.13; Mean MDA AI 1.89 ± 0.53 vs Atlas-based 3.75 ± 1.51). There is not a significant difference in segmentation of both femurs. Even for empty bladder both methods give good results. Conclusion: In summary in the case of prostate treatment the use of software Contour ProtégéAI is extremely valid and the superiority in terms of accuracy of this method in comparison with the Atlas-based one has been shown.
