Ziel dieser Dissertation war es, die klinische Sicherheit und antirestenotische Effektivität von bioresorbierbaren Stents zu untersuchen. Hierfür wurden angiographische und klinische Langzeitdaten aus dem ISAR-ABSORB Register des deutschen Herzzentrums München erhoben und statistisch ausgewertet. Der primäre Endpunkt war die Kombination aus Tod, Myokardinfarkt und Zielläsion-Revaskularisation. Sekundäre Endpunkte waren sowohl die individuellen Ereignisse des primären Endpunktes, als auch die Rate an definitiven Stentthrombosen. The aim of this dissertation was to investigate the clinical safety and anti-restenotic effectivity of bioresorbable stents. Thus, angiographic and clinical long-term data from the ISAR-ABSORB registry of the German Heart Center in Munich were collected and statistically evaluated. The primary endpoint was the composite of death, myocardial infarction or target lesion revascularization. Secondary endpoints included the individual components of the primary endpoint, as well as the rate of definite stent thrombosis.

Determina los factores de conversión a cirugía abierta en pacientes sometidos a colecistectomía laparoscópica en el Servicio de Cirugía de Emergencias del Hospital Nacional Guillermo AImenara 2016. La colecistectomía laparoscópica es una de las intervenciones quirúrgicas efectuadas con mayor frecuencia en el Hospital Guillermo Almenara como principal alternativa en el tratamiento de la patología vesicular. Aún con la enorme experiencia y capacitación escolarizada en las diversas técnicas laparoscópicas de colecistectomía, existen razones múltiples por las que un porcentaje de las mismas terminan convertidas a cirugía convencional durante el acto quirúrgico. En el presente estudio, se pretende identificar las principales causas de conversión de la colecistectomía laparoscópica, para luego plantear posibles soluciones a mediano y largo plazo.

The genus Notomicrus Sharp, 1882 is recorded from Guadeloupe for the first time. Examination of recently collected material revealed the presence of four species, of which three are described as new species so far endemic to Guadeloupe. Notomicrus sabrouxi sp. n. is akin to the continental Neotropical species N. gracilipes Sharp, 1882 and N. traili Sharp, 1882. Notomicrus chailliei sp. n. and N. femineus sp. n. are sibling species which diverge from each other by 3.4% of observed distance in cytochrome oxidase 1 sequences. The latter species seems to be comprised of only females, and is suspected to reproduce by obligate parthenogenesis. The fourth species is N. sharpi Balfour-Browne, 1939. These species are morphologically described notably through scanning electron microscopy study, and data on their distribution in the Guadeloupe archipelago and habitat preferences are presented and discussed.

Asthma is a significant health issue in the pediatric population with a noteworthy growth over the years. The proposed challenge for this PhD thesis was the development of advanced methodologies to establish metabolomic patterns in urine and exhaled breath associated with asthma whose applicability was subsequently exploited to evaluate the disease state, the therapy adhesion and effect and for diagnostic purposes. The volatile composition of exhaled breath was studied combining headspace solid phase microextraction (HS-SPME) with gas chromatography coupled to mass spectrometry or with comprehensive two-dimensional gas chromatography coupled to mass spectrometry with a high resolution time of flight analyzer (GC×GC–ToFMS). These methodologies allowed the identification of several hundred compounds from different chemical families. Multivariate analysis (MVA) led to the conclusion that the metabolomic profile of asthma individuals is characterized by higher levels of compounds associated with lipid peroxidation, possibly linked to oxidative stress and inflammation (alkanes and aldehydes) known to play an important role in asthma. For future applications in clinical settings a set of nine compounds was defined and the clinical applicability was proven in monitoring the disease status and in the evaluation of the effect and / or adherence to therapy. The global volatile metabolome of urine was also explored using an HSSPME/GC×GC–ToFMS method and c.a. 200 compounds were identified. A targeted analysis was performed, with 78 compounds related with lipid peroxidation and consequently to oxidative stress levels and inflammation. The urinary non-volatile metabolomic pattern of asthma was established using proton nuclear magnetic resonance (1H NMR). This analysis allowed identifying central metabolic pathways such as oxidative stress, amino acid and lipid metabolism, gut microflora alterations, alterations in the tricarboxylic acid (TCA) cycle, histidine metabolism, lactic acidosis, and modification of free tyrosine residues after eosinophil stimulation. The obtained results allowed exploring and demonstrating the potential of analyzing the metabolomic profile of exhaled air and urine in asthma. Besides the successful development of analysis methodologies, it was possible to explore through exhaled air and urine biochemical pathways affected by asthma, observing complementarity between matrices, as well as, verify the clinical applicability.

Uterine carcinosarcoma is a rare malignancy that has a high recurrence rate after treatment with very poor long term prognosis. Women are diagnosed usually over the age of 50, with most cases occurring between the sixth and seventh decade of life. We present a rare case of uterine carcinosarcoma diagnosed a nineteen year old after having a dilation and curettage for heavy vaginal bleeding. The patient went on to have a total abdominal hysterectomy with transposition of the ovaries as well as adjuvant treatment with chemotherapy.

Ecological developmental biology (eco-devo) explores the mechanistic relationships between the processes of individual development and environmental factors. Recent studies imply that some of these relationships have deep evolutionary origins, and may even pre-date the divergences of the simplest extant animals, including cnidarians and sponges. Development of these early diverging metazoans is often sensitive to environmental factors, and these interactions occur in the context of conserved signaling pathways and mechanisms of tissue homeostasis whose detailed molecular logic remain elusive. Efficient methods for transgenesis in cnidarians together with the ease of experimental manipulation in cnidarians and sponges make them ideal models for understanding causal relationships between environmental factors and developmental mechanisms. Here, we identify major questions at the interface between animal evolution and development and outline a road map for research aimed at identifying the mechanisms that link environmental factors to developmental mechanisms in early diverging metazoans. Hydroporus rifensis sp. n. is described from Central Rif in northern Morocco. The new species is the first member of the Hydroporus tristis-group to be recorded from the African continent. Analyses of cytochrome oxydase 1 (CO1) and 16S rRNA partial sequences indicate unambiguously that H. rifensis sp. n. is a geographical vicariant of the widespread and common European species Hydroporus gyllenhalii Schiödte, 1841, whose closest Iberian populations are known from southern Spain and southern Portugal. Genetically the two species are very close, but in terms of morphology they differ considerably, to the extent that in several respects H. rifensis sp. n. is more similar to some species of the H. striola-group, the sister clade of the H. tristis-group. The new species differs from H. gyllenhalii notably by its larger size, lighter elytra, stronger pubescence, much finer elytral punctation, morphology of the median prosternum area, and marked differentiation of protarsal claws in male. Hydroporus rifensis sp. n. is the first dytiscid species endemic to the Rif and the second Hydroporus species endemic to Morocco.

This book presents the lectures notes of the 34th Saas-Fee Advanced Course "The Sun, Solar Analogs and the Climate" given by leading scientists in the field. Emphasis is on the observed variability of the Sun and the present understanding of the variability’s origin as well as its impact on the Earth's climate. The solar variability is then studied in the broader context of solar-type stars, allowing for better understanding of the solar-activity cycle and the magnetic activity in general. This book provides an accessible and up-to-date introduction to the field for graduate students and serves as modern source of reference for active researchers in this field.

Layered deposits of dusty ice in the martian polar caps have been hypothesized to record climate changes driven by orbitally induced variations in the distribution of incoming solar radiation. Attempts to identify such an orbital signal by tuning a stratigraphic sequence of polar layered deposits (PLDs) to match an assumed forcing introduce a risk of identifying spurious matches between unrelated records. We present an approach for evaluating the significance of matches obtained by orbital tuning, and investigate the utility of this approach for identifying orbital signals in the Mars PLDs. Using a set of simple models for ice and dust accumulation driven by insolation, we generate synthetic PLD stratigraphic sequences with nonlinear time–depth relationships. We then use a dynamic time warping algorithm to attempt to identify an orbital signal in the modeled sequences, and apply a Monte Carlo procedure to determine whether this match is significantly better than a match to a random sequence that contains no orbital signal. For simple deposition mechanisms in which dust deposition rate is constant and ice deposition rate varies linearly with insolation, we find that an orbital signal can be confidently identified if at least 10% of the accumulation time interval is preserved as strata. Addition of noise to our models raises this minimum preservation requirement, and we expect that more complex deposition functions would generally also make identification more difficult. In light of these results, we consider the prospects for identifying an orbital signal in the actual PLD stratigraphy, and conclude that this is feasible even with a strongly nonlinear relationship between stratigraphic depth and time, provided that a sufficient fraction of time is preserved in the record and that ice and dust deposition rates vary predictably with insolation. Independent age constraints from other techniques may be necessary, for example, if an insufficient amount of time is preserved in the stratigraphy., United States.
