Zusammenfassung Einleitung Die Polycythaemia vera (PV) ist eine seltene Ursache einer Pulmonalen Hypertonie (PH) Klasse 5 nach Nizza-2018. Anhand von Kasuistiken werden die unterschiedlichen Verläufe, zum einen mit primärer Manifestation einer PH und nachfolgend einer PV, zum anderen mit Entwicklung einer PH im Rahmen einer PV, dargestellt. Kasuistiken 1) Bei Erstkontakt 75-jährige Patientin mit progredienter Dyspnoe und Nachweis einer Belastungs-PH im RHK. Im Verlauf Entwicklung einer Ruhe-PH bis 70 mmHg systolisch trotz initialer Mono- und nachfolgend dualer Therapie der PH. Nach 5 Jahren Diagnose einer Polycythaemia vera und Einleitung einer Hydroxycarbamid-Therapie und nachfolgend auch Aderlässe. Im weiteren Verlauf zunehmende kardiale Dekompensation und Versterben. 2) 74-jährige Patientin bei Diagnosestellung einer chronisch megakaryozytären-granulozytären Myelose. Nach 7 Jahren Nachweis einer Polycythaemia vera (V617F-Mutation im JAK2-Gen) und einer monoklonalen Gammopathie. Bei Splenomegalie Milzbestrahlung und nach 1 Jahr Therapie mit Ruxolitinib. 2 Jahre später, bei zunehmender Dyspnoe, Nachweis einer pulmonalen Hypertonie (CTEPH) mit einem PA-mean von 43 mmHg und PVR von 4,5 WE. Unter Antikoagulation und Riociguattherapie nur passager Besserung der Belastbarkeit und der PA-Drucke. Innerhalb eines Jahres Entwicklung einer zunehmend restriktiven Ventilationsstörung mit Hypoxämie und einer Herzinsuffizienz (EF 45 %), mit Rechtsherzdekompensation und kardio-renalem Syndrom. Unter Dialyse nur kurzfristige Rekompensation, sodass die Patientin verstarb. Diskussion Die Kasuistiken zeichnen sich durch eine Kombination aus PV und PH, bei unterschiedlicher zeitlicher Sequenz, sowie eine nur geringe passagere Beeinflussbarkeit durch eine PH-spezifische Therapie, mit nachfolgend progredienter kardialer Dekompensation aus. Sie spiegeln somit die verschiedenen Ätiologien, klinischen Manifestationsformen und die geringe therapeutische Beeinflussbarkeit einer PH bei myeloproliferativen Erkrankungen wider. Unklar ist weiterhin der Stellenwert einer PH-spezifischen Therapie insbesondere in Anbetracht unterschiedlicher Pathomechanismen in der Genese der PH. Schlussfolgerung Patienten mit myeloproliferativen Erkrankungen bedürfen eines Screenings auf eine PH. Auch im Verlauf einer PH kann sich eine myeloproliferative Erkrankung demaskieren oder entwickeln. Die therapeutische Beeinflussbarkeit der PH ist eingeschränkt.

Purpose This executive summary of a national living guideline aims to provide rapid evidence based recommendations on the role of drug interventions in the treatment of hospitalized patients with COVID-19. Methods The guideline makes use of a systematic assessment and decision process using an evidence to decision framework (GRADE) as recommended standard WHO (2021). Recommendations are consented by an interdisciplinary panel. Evidence analysis and interpretation is supported by the CEOsys project providing extensive literature searches and living (meta-) analyses. For this executive summary, selected key recommendations on drug therapy are presented including the quality of the evidence and rationale for the level of recommendation. Results The guideline contains 11 key recommendations for COVID-19 drug therapy, eight of which are based on systematic review and/or meta-analysis, while three recommendations represent consensus expert opinion. Based on current evidence, the panel makes strong recommendations for corticosteroids (WHO scale 5–9) and prophylactic anticoagulation (all hospitalized patients with COVID-19) as standard of care. Intensified anticoagulation may be considered for patients with additional risk factors for venous thromboembolisms (VTE) and a low bleeding risk. The IL-6 antagonist tocilizumab may be added in case of high supplemental oxygen requirement and progressive disease (WHO scale 5–6). Treatment with nMABs may be considered for selected inpatients with an early SARS-CoV-2 infection that are not hospitalized for COVID-19. Convalescent plasma, azithromycin, ivermectin or vitamin D3 should not be used in COVID-19 routine care. Conclusion For COVID-19 drug therapy, there are several options that are sufficiently supported by evidence. The living guidance will be updated as new evidence emerges.

ZusammenfassungSeit Dezember 2019 verbreitet sich das neuartige Coronavirus SARS-CoV-2 (Severe Acute Respiratory Syndrome – Corona Virus-2) rasch im Sinne einer weltweiten Pandemie. Dies stellt Kliniker und Krankenhäuser vor große Herausforderungen und belastet die Gesundheitssysteme vieler Länder in einem nie dagewesenen Ausmaß. Die Mehrheit der Patienten mit Coronavirus Disease 2019 (COVID-19) zeigt lediglich milde Symptome wie Husten und Fieber. Allerdings benötigen etwa 8 % eine stationäre Behandlung. Der frühzeitigen Klärung, ob eine stationäre und ggfs. intensivmedizinische Behandlung medizinisch sinnvoll und vom Patienten gewollt ist, kommt in der Pandemie eine besondere Bedeutung zu. Die akute hypoxämische respiratorische Insuffizienz mit Dyspnoe und hoher Atemfrequenz (> 30/min) führt i. d. R. zur Aufnahme auf die Intensivstation. Oft finden sich dann bereits bilaterale pulmonale Infiltrate/Konsolidierungen oder auch Lungenembolien in der Bildgebung. Im weiteren Verlauf entwickeln einige dieser Patienten ein akutes Lungenversagen (Acute Respiratory Distress Syndrome; ARDS). Eine Sterblichkeitsreduktion einer verfügbaren medikamentösen Therapie bei schwerer COVID-19-Erkrankung ist bisher lediglich für Dexamethason in randomisiert, kontrollierten Studien nachgewiesen. Das Hauptziel der supportiven Therapie besteht in der Sicherstellung einer ausreichenden Oxygenierung. Die invasive Beatmung und wiederholte Bauchlagerung sind dabei wichtige Elemente in der Behandlung von schwer hypoxämischen COVID-19-Patienten. Die strikte Einhaltung der Basishygiene, einschließlich der Händehygiene, sowie das korrekte Tragen von adäquater persönlicher Schutzausrüstung sind im Umgang mit den Patienten unabdingbar. Medizinisch notwendige Handlungen am Patienten, die zur Aerosolbildung führen könnten, sollten mit äußerster Sorgfalt und Vorbereitung durchgeführt werden.

Background: Combined pulmonary fibrosis and emphysema (CPFE) is a distinct entity among fibrosing lung diseases with a high risk for lung cancer and pulmonary hypertension (PH). Notably, concomitant PH was identified as a negative prognostic indicator that could help with early diagnosis to provide important information regarding prognosis. Objectives: The current study aimed to determine whether cardiopulmonary exercise testing (CPET) can be helpful in differentiating patients having CPFE with and without PH. Methods: Patients diagnosed with CPFE in 2 German cities (Hemer and Greifswald) over a period of 10 years were included herein. CPET parameters, such as peak oxygen uptake (peak VO2), functional dead space ventilation (VDf/VT), alveolar-arterial oxygen difference (AaDO2), arterial-end-tidal CO2 difference [P(a-ET)CO2] at peak exercise, and the minute ventilation-carbon dioxide production relationship (VE/VCO2 slope), were compared between patients with and without PH. Results: A total of 41 patients with CPET (22 with PH, 19 without PH) were analyzed. Right heart catheterization was performed in 15 of 41 patients without clinically relevant complications. Significant differences in peak VO2 (861 ± 190 vs. 1,397 ± 439 mL), VO2/kg body weight/min (10.8 ± 2.6 vs. 17.4 ± 5.2 mL), peak AaDO2 (72.3 ± 7.3 vs. 46.3 ± 14.2 mm Hg), VE/VCO2 slope (70.1 ± 31.5 vs. 39.6 ± 9.6), and peak P(a-ET)tCO2 (13.9 ± 3.5 vs. 8.1 ± 3.6 mm Hg) were observed between patients with and without PH (p < 0.001). Patients with PH had significantly higher VDf/VT at rest, VT1, and at peak exercise (65.6 ± 16.8% vs. 47.2 ± 11.6%; p < 0.001) than those without PH. A cutoff value of 44 for VE/VCO2 slope had a sensitivity and specificity of 94.7 and 72.7%, while a cutoff value of 11 mm Hg for P(a-ET)CO2 in combination with peak AaDO2 >60 mm Hg had a specificity and sensitivity of 95.5 and 84.2%, respectively. Combining peak AaDO2 >60 mm Hg with peak VO2/body weight/min Conclusion: This study provided initial data on CPET among patients having CPFE with and without PH. CPET can help noninvasively detect PH and identify patients at risk. AaDO2 at peak exercise, VE/VCO2 slope, peak P(a-ET)CO2, and peak VO2 were parameters that had high sensitivity and, when combined, high specificity.

Highlights • Diffuse angiosarcoma of the heart cause non-specific symptoms fever and chest pain. • It is difficult to diagnose because no solid tumor can be detected. • A high level of suspicion is a premise to initiate biopsies of the pericardium and myocardium., Introduction We present a very rare case with diffuse cardiac angiosarcoma. Because all symptoms are often non-specific, this diagnosis is difficult to establish. To our knowledge this is the first clinical description of this rare disease. Presentation of case A 47-year-old female presented with bilateral pulmonary infiltrates and non-specific symptoms as fever, chest pain and dyspnoea on exertion. She was treated with antibiotics for suspected lung infection but deteriorated developing rapid recurrent pleural effusion. Her transthoracic- and transoesophageal-echocardiography as well as the thoracentesis and endobronchial ultrasound findings were normal. A minimally invasive pulmonary wedge resection, partial pleurectomy and pericardial fenestration was performed. The pathologic interpretation of these specimen was very difficult and a correct diagnosis could be made only by the second reference pathologist. While awaiting reference histology report she was administered high-flow oxygen therapy for hypoxia, antibiotics, catecholamines and corticosteroids. The patient deteriorated very rapidly and died in the ICU. Discussion As in earlier studies, misdiagnosis delayed the actual diagnosis, especially because there was no clinical suspicion for angiosarcoma. Pathologic evaluation may be difficult because different growth patterns may be present in the same tumour and pleural or lung specimen may show only very tiny tumour formations within a fibrosing tissue changes. Conclusion This case report highlights the difficulties to establish a diagnosis of diffuse angiosarcoma in time. An early diagnosis, to initiate oncologic treatment, require a high level of clinical suspicion and a histological proof from pericardial or myocardial biopsy.

ZusammenfassungDie logistischen und infektiologischen Besonderheiten und Erfordernisse in der Pandemie stellen die intensivmedizinischen Behandlungsteams vor außerordentliche Herausforderungen, die eine erfolgreiche Liberation von Patienten im prolongierten Weaning erschweren können. Gerade in der Pandemie gilt es daher, alle Potenziale zur Beatmungsentwöhnung und Dekanülierung von Patienten im prolongierten Weaning oder in der Langzeitbeatmung auszuschöpfen. Weaning-Zentren stellen Einheiten der intensivmedizinischen Betreuung mit besonderer Spezialisierung auf prolongiertes Weaning dar und sind integraler Bestandteil eines kontinuierlichen Versorgungskonzeptes dieser Patienten. Ein systematisches Weaning-Konzept in der Pandemie setzt strukturelle, personelle, apparative, infektiologische und hygienische Aspekte voraus. Im Mittelpunkt dieses Positionspapiers stehen als Ergänzung zur S2k-Leitlinie „Prolongiertes Weaning“ eine neue Klassifikation im prolongierten Weaning und zukünftig erforderliche Organisationsstrukturen für die Pandemie-Situation. Patienten der Kategorie A mit hohem Weaning-Potenzial bedürfen der strukturierten Respiratorentwöhnung in spezialisierten Weaning-Einheiten, um so die größtmögliche Chance auf erfolgreiches Weaning zu realisieren. Patienten der Kategorie B mit geringem oder aktuell nicht vorhandenem Weaning-Potenzial sollten verzögert nach einer Zwischenphase der weiteren Stabilsierung in einer außerklinischen Beatmungseinrichtung einen Weaning-Versuch erhalten. Patienten der Kategorie C ohne Weaning-Potenzial benötigen eine dauerhafte außerklinische Versorgung, alternativ eine palliativmedizinisch begleitete Beendigung der Beatmung. Die sich aus den 3 Kategorien ergebenden Aufgaben für die Weaning-Zentren werden im Positionspapier vertieft.Schließlich werden unter perspektivischer Betrachtung im Positionspapier folgende zukünftig denkbare Netzwerke und Register benannt: 1. lokal organisierte regionale Netzwerke von zertifizierten Weaning-Zentren, 2. ein zentrales, bundesweites Register der Weaning-Kapazitäten entsprechend dem DIVI-Register und 3. eine Registrierung der Patienten im schwierigen oder prolongierten Weaning.

ZusammenfassungVor dem Hintergrund der Pandemie durch Infektionen mit dem SARS-CoV-2 hat die Deutsche Gesellschaft für Pneumologie und Beatmungsmedizin (DGP e.V.) federführend in Kooperation mit weiteren Verbänden ein Expertenteam benannt, um die zur Zeit drängenden Fragen zu Therapiestrategien im Umgang mit COVID-19-Patienten, die an akut respiratorischer Insuffizienz (ARI) leiden, zu beantworten. Das Positionspapier basiert auf dem momentanen aktuellen Wissen, das sich täglich weiterentwickelt. Viele der publizierten und zitierten Studien bedürfen weiterer Überprüfungen, auch weil viele kein übliches Review-Verfahren durchlaufen haben Daher unterliegt auch dieses Positionspapier einer ständigen Überprüfung und wird in Zusammenarbeit in der Zusammenarbeit mit den anderen Fachgesellschaften weiterentwickelt.Dieses Positionspapier wurde in die folgenden fünf Themenfelder gegliedert:1. Pathophysiologie der akuten respiratorischen Insuffizienz bei Infektionen mit SARS-CoV-2 bei Patienten ohne Immunität2. Zeitlicher Verlauf und Prognose der akuten respiratorischen Insuffizienz im Laufe der Erkrankung3. Sauerstoff-Insufflation, High-Flow Sauerstoff, nicht-invasive Beatmung und invasive Beatmung unter besonderer Berücksichtigung der infektiösen Aerosolbildung4. Nicht-Invasive Beatmung bei der ARI5. Versorgungskontinuum zur Behandlung der ARIZentrale Punkte wurden hierbei als Kernaussagen und Feststellungen herausgehoben. Bezüglich pathophysiologischer Aspekte der akuten respiratorischen Insuffizienz (ARI) verläuft die pulmonale Infektion mit SARS-CoV-2 COVID-19 in drei Phasen: Frühe Infektion, pulmonale Manifestation und schwere hyperinflammatorische Phase.Der fortgeschrittene COVID-19-induzierte Lungenschaden weist häufig Unterschiede zu den bekannten Veränderungen entsprechend der Definition des Acute Respiratory Distress Syndrome (ARDS) nach den Berlin-Kriterien auf.In einem pathophysiologisch plausiblen – zur Zeit aber noch nicht histopathologisch untermauerten – Modell wird in zwei Typen (L-Typ und H-Typ) unterschieden, die einer frühen und späten Phase entsprechen. Diese Unterscheidung kann für die apparative Differenzialtherapie der ARI erwogen werden.Die Einschätzung des Ausmaßes der ARI soll durch eine arterielle oder kapilläre Blutgasanalyse bei Raumluft erfolgen und die Errechnung des Sauerstoffangebotes (bemisst sich aus den Variablen der Sauerstoffsättigung, des Hb-Wertes, der Hüfnerʼschen Korrekturzahl sowie des Herzminutenvolumens) beinhalten.Durch Aerosole ist eine Übertragung von infektiösen, Viren-haltigen Partikeln prinzipiell möglich. Offene Systeme bzw. Leckage-Systeme (sog. vented Masken) können die Abgabe von respirablen Partikeln erhöhen. Prozeduren, bei denen das invasive Beatmungssystem geöffnet werden muss, sowie die endotracheale Intubation sind mit einem erhöhten Infektionsrisiko verbunden.Der Schutz des Personals durch persönliche Schutzausrüstung soll sehr hohe Priorität haben, weil die Angst vor Ansteckung kein primärer Intubationsgrund sein darf. Bei Einhaltung der Vorgaben zu Schutzausrüstung (Augenschutz, FFP2- bzw. FFP-3 Maske, Kittel) kann eine Inhalationstherapie, nasale High Flow (NHF) -Therapie, eine CPAP-Therapie oder eine NIV nach jetzigem Kenntnisstand vom Personal ohne erhöhtes Infektionsrisiko durchgeführt werden.Ein signifikanter Anteil der respiratorisch insuffizienten Patienten präsentiert sich mit einer relevanten Hypoxämie, die häufig auch durch eine hohe inspiratorische Sauerstofffraktion (FiO2) inklusive NHF nicht vollständig korrigiert werden kann.In dieser Situation können die CPAP/NIV-Therapie unter Verwendung einer Mund-Nasen-Maske oder eines Beatmungshelms als Therapieeskalation durchgeführt werden, solange die Kriterien für eine endotracheale Intubation nicht erfüllt sind.Die NIV bei akuter hypoxämischer Insuffizienz sollte auf der Intensivstation oder in einer vergleichbaren Struktur mit entsprechender personeller Expertise erfolgen. Unter CPAP/NIV kann es zu einer raschen Verschlechterung kommen. Aus diesem Grund soll ein ständiges Monitoring unter ständiger Intubationsbereitschaft gewährleistet sein. Kommt es unter CPAP/NIV zur weiteren Progression des ARI, sollte ohne zeitliche Verzögerung die Intubation und nachfolgende invasive Beatmung erfolgen, wenn keine DNI-Order vorliegt.Bei Patienten, bei denen eine invasive Beatmung unter Ausschöpfung aller leitliniengerechter Maßnahmen nicht ausreicht, um eine ausreichende Sauerstoffaufnahme und CO2-Abgabe zu gewährleisten, soll ein extrakorporales Lungenersatzverfahren erwogen werden (ECMO).

ZusammenfassungDie enorme Zunahme von Patienten mit schwerer respiratorischer Insuffizienz aufgrund der COVID19-Pandemie erfordert einen systematischen Ansatz zur Optimierung der Betreuung von beatmeten Patienten. Ein standardisierter Algorithmus namens „SAVE“ wurde in Berlin entwickelt, um Patienten mit COVID-19-Infektion, die eine invasive Beatmung benötigen, zu lenken. Um Bettenkapazitäten auf den Intensivstationen sicherzustellen, muss eine konstante Patientenentlassung gewährleistet sein. Ein strukturierter Entlassungsprozess der Patienten nach der Akutbehandlung ist dringend erforderlich. In einem nächsten Schritt haben wir einen Triage-Algorithmus entwickelt, um Patienten auf SAVE-Intensivstationen zu identifizieren, die das Potenzial haben, von der Beatmung entwöhnt zu werden und in dafür spezialisierte Einrichtungen zu verlegen. Dieser Prozess ist das POST-SAVE-Konzept. In der vorliegenden Arbeit werden die Algorithmen dargestellt, einschließlich der Verwendung einer standardisierten digitalen Datenbank, der Verwendung geschulter Lotsen zur Erleichterung der Kommunikation zwischen SAVE-Intensivstationen und Weaningeinheiten und der Etablierung eines prospektiven Datenregisters, um Patienten dauerhaft hinsichtlich ihres Weaningpotenzials reevaluieren zu können.

Chronic myeloproliferative diseases are rare causes of PH class 5 according to Nice classification 2018. The present case reports show different courses, on the one hand with a primary manifestation of a PH and subsequently a PV, on the other hand with the development of a PH in the context of a PV. 1) At first contact, a 75-year-old female patient who complained progressive dyspnea and had evidence of stress-PH in the right heart catheter. During the course she developed a resting PH of up to 70 mmHg systolic despite initial monotherapy and subsequent dual therapy for PH. After 5 years she had the diagnosis of polycythemia vera, treated with hydroxycarbamide and subsequent phlebotomies. In the further course increasing cardiac decompensation and death. 2) 74-year-old female patient at the time of diagnosis of chronic megakaryocytic-granulocytic myelosis. After 7 years, evidence of polycythemia vera (V617F mutation in the JAK2 gene), a monoclonal gammopathy. In the case of splenomegaly, irradiation of the spleen was carried out and, after 1 year, therapy with ruxolitinib was started. After another 2 years, with increasing dyspnea, pulmonary hypertension (CTEPH) with a PA-mean of 43 mmHg and a PVR of 4.5 WE were detected. With anticoagulation and riociguat therapy exercise capacity and PA pressures were only temporarily improved. Within 1 year restrictive ventilation, hypoxemia, heart failure (EF 45 %) with leading right heart decompensation and cardiorenal syndrome developed. The case reports are characterized by a combination of PV and PH, with different temporal sequence, as well as only a low influence of PH-specific therapy, with subsequent progressive cardiac decompensation. Thus, they reflect the different etiologies, clinical manifestations, and the low therapeutic influence of PH in myeloproliferative disorders. The value of PH-specific therapy remains unclear, especially in view of different pathomechanisms in the genesis of PH. Patients with myeloproliferative diseases require screening for PH. In the course of PH, myeloproliferative disease can unmask or develop. The therapeutic influence on PH is limited. Bei Splenomegalie Milzbestrahlung und nach 1 Jahr Therapie mit Ruxolitinib. 2 Jahre später, bei zunehmender Dyspnoe, Nachweis einer pulmonalen Hypertonie (CTEPH) mit einem PA-mean von 43 mmHg und PVR von 4,5 WE. Unter Antikoagulation und Riociguattherapie nur passager Besserung der Belastbarkeit und der PA-Drucke. Innerhalb eines Jahres Entwicklung einer zunehmend restriktiven Ventilationsstörung mit Hypoxämie und einer Herzinsuffizienz (EF 45 %), mit Rechtsherzdekompensation und kardio-renalem Syndrom. Unter Dialyse nur kurzfristige Rekompensation, sodass die Patientin verstarb. Die Kasuistiken zeichnen sich durch eine Kombination aus PV und PH, bei unterschiedlicher zeitlicher Sequenz, sowie eine nur geringe passagere Beeinflussbarkeit durch eine PH-spezifische Therapie, mit nachfolgend progredienter kardialer Dekompensation aus. Sie spiegeln somit die verschiedenen Ätiologien, klinischen Manifestationsformen und die geringe therapeutische Beeinflussbarkeit einer PH bei myeloproliferativen Erkrankungen wider. Unklar ist weiterhin der Stellenwert einer PH-spezifischen Therapie insbesondere in Anbetracht unterschiedlicher Pathomechanismen in der Genese der PH. Patienten mit myeloproliferativen Erkrankungen bedürfen eines Screenings auf eine PH. Auch im Verlauf einer PH kann sich eine myeloproliferative Erkrankung demaskieren oder entwickeln. Die therapeutische Beeinflussbarkeit der PH ist eingeschränkt.

Against the background of the pandemic caused by infection with the SARS-CoV-2 virus, the German Respiratory Society has appointed experts to develop therapy strategies for COVID-19 patients with acute respiratory failure (ARF). Here we present key position statements including observations about the pathophysiology of (ARF). In terms of the pathophysiology of pulmonary infection with SARS-CoV-2, COVID-19 can be divided into 3 phases. Pulmonary damage in advanced COVID-19 often differs from the known changes in acute respiratory distress syndrome (ARDS). Two types (type L and type H) are differentiated, corresponding to early- and late-stage lung damage. This differentiation should be taken into consideration in the respiratory support of ARF. The assessment of the extent of ARF should be based on arterial or capillary blood gas analysis under room air conditions, and it needs to include the calculation of oxygen supply (measured from the variables of oxygen saturation, hemoglobin level, the corrected values of Hüfner’s factor, and cardiac output). Aerosols can cause transmission of infectious, virus-laden particles. Open systems or vented systems can increase the release of respirable particles. Procedures in which the invasive ventilation system must be opened and endotracheal intubation carried out are associated with an increased risk of infection. Personal protective equipment (PPE) should have top priority because fear of contagion should not be a primary reason for intubation. Based on the current knowledge, inhalation therapy, nasal high-flow therapy (NHF), continuous positive airway pressure (CPAP), or noninvasive ventilation (NIV) can be performed without an increased risk of infection to staff if PPE is provided. A significant proportion of patients with ARF present with relevant hypoxemia, which often cannot be fully corrected, even with a high inspired oxygen fraction (FiO2) under NHF. In this situation, the oxygen therapy can be escalated to CPAP or NIV when the criteria for endotracheal intubation are not met. In ARF, NIV should be carried out in an intensive care unit or a comparable setting by experienced staff. Under CPAP/NIV, a patient can deteriorate rapidly. For this reason, continuous monitoring and readiness for intubation are to be ensured at all times. If the ARF progresses under CPAP/NIV, intubation should be implemented without delay in patients who do not have a “do not intubate” order.

ZusammenfassungBeatmungstherapie stellt einen zentralen und wesentlichen Bestandteil der modernen Intensivmedizin dar. Sie kommt bei Patienten mit schwerer respiratorischer Insuffizienz infolge Versagens der muskulären Atempumpe oder bei direkter oder indirekter Schädigung des Lungenparenchyms mit nachfolgendem Oxygenierungsversagen zum Einsatz, wenn mit anderen nicht-medikamentösen Maßnahmen, Sauerstoffgabe, Sekretmobilisation, kontinuierlicher positiver Atemwegsdruck – Continuous Positive Airway Pressure (CPAP) oder Nasal-High-Flow-Therapie, keine ausreichende Stabilisierung erreicht werden kann.Die maschinelle Beatmung dient der direkten Behandlung der Atmungsinsuffizienz und schafft Zeit für die Behandlung der zugrundeliegenden Ursache. Der überwiegende Anteil beatmeter Patienten kann nach kurzzeitiger Beatmungstherapie und kausaler Behandlung unproblematisch von der Beatmung entwöhnt werden. Allerdings muss die Beatmung bei ca. 20 % der Patienten auch noch dann fortgesetzt werden, wenn die ursprüngliche Indikation (z. B. eine schwere Pneumonie) längst behoben ist, sodass sich die Phase des Weanings (Entwöhnung von der maschinellen Beatmung) deutlich verlängert. Ungefähr 40 – 50 % der gesamten Beatmungszeit eines Intensivpatienten entfallen aufgrund einer prolongierten Atmungsinsuffizienz auf den Prozess, den Patienten von der Beatmung zu trennen. Neben der respiratorischen Funktionsstörung tragen häufig hohes Alter und Komorbiditäten der Patienten zum prolongierten Weaning-Prozess bei.Nach internationalem Konsens liegt ein prolongiertes Weaning dann vor, wenn es erst nach 3 erfolglosen Spontanatmungsversuchen (spontaneous breathing trial = SBT) oder nach über 7 Tagen Beatmung nach dem ersten erfolglosen SBT gelingt, den Patienten von der Beatmung zu trennen.Das Patientenkollektiv mit prolongiertem Weaning stellt das behandelnde Team vor eine besondere Herausforderung. Ganz wesentlich für den Therapieerfolg ist die eng verzahnte interdisziplinäre Behandlung der Patienten im prolongierten Weaning. Nicht selten sind es der fehlende multidisziplinäre Ansatz und die unzureichende Beachtung der multifaktoriellen Ursachen, die ein erfolgreiches Weaning verhindern. Dieses erfolgreich durchzuführen, setzt eine hohe Expertise in der modernen Intensivmedizin, der Anwendung invasiver und nichtinvasiver Beatmungsverfahren, ein klares Weaning-Konzept, und eine enge, fachübergreifende interdisziplinäre Zusammenarbeit voraus.Im komplexen prolongierten Weaning-Prozess gelingt es in spezialisierten Weaning-Zentren/-Einheiten nach Verlegung der invasiv beatmeten Patienten in ca. 50 % der Fälle doch noch, ein Weaning-Versagen abzuwenden. Bei einem Teil der Patienten schlagen auch wiederholte Weaning-Versuche fehl, sodass gegebenenfalls eine dauerhafte invasive Beatmung in außerklinischer Umgebung erforderlich ist.Vor dem Hintergrund der wachsenden Bedeutung des prolongierten Weanings, insbesondere der medizinischen, psychosozialen und ökonomischen Folgen des Weaning-Versagens, wurde erstmals 2014 diese Leitlinie auf Initiative der Deutschen Gesellschaft für Pneumologie und Beatmungsmedizin e. V. (DGP) gemeinsam mit anderen wissenschaftlichen Fachgesellschaften, die sich zum Thema prolongiertes Weaning engagieren, publiziert. Aktuelle Forschungs- und Studienergebnisse, Registerdaten und die Erfahrungen in der täglichen Praxis machten die Revision dieser Leitlinie erforderlich.In der revidierten Leitlinie werden Definitionen, Epidemiologie und Weaning-Kategorien, die zugrundeliegende Pathophysiologie, Strategien zur Prävenion von prolongiertem Weaning, das gesamte Spektrum der verfügbaren Therapiestrategien, die Weaning-Einheit, die Überleitung in eine außerklinische Beatmung und schließlich Empfehlungen zu Therapieentscheidungen am Ende des Lebens bei prolongiertem bzw. erfolglosem Weaning abgehandelt.Besondere Schwerpunkte in der Revision der Leitlinie sind folgende Themenfelder:– Eine neue Klassifikation der Untergruppen der Patienten im prolongieren Weaning– Wichtige Aspekte der pneumologischen Rehabilitation und Neurorehabilitation im prolongieren Weaning– Infrastruktur und Prozessorganisation in der Versorgung von Patienten im prolongierten Weaning im Sinne eines kontinuierlichen Behandlungskonzeptes– Therapiezieländerung und Kommunikation mit AngehörigenDie Besonderheiten bei pädiatrischen Patienten werden innerhalb der einzelnen Kapitel jeweils gesondert behandelt.Wichtige Adressaten dieser Leitlinie sind Intensivmediziner, Pneumologen, Anästhesisten, Internisten, Kardiologen, Chirurgen, Neurologen, Pädiater, Geriater, Palliativmediziner, Rehabilitationsmediziner, Pflegekräfte, Logopäden, Physiotherapeuten, Atmungstherapeuten, der medizinische Dienst der Krankenkassen und die Hersteller von Beatmungstechnik.Die wesentlichen Ziele der revidierten Leitlinie sind es, den aktuellen Wissensstand zum Thema „Prolongiertes Weaning“ wissenschaftlich zu bewerten und auf Basis der Evidenz und der Erfahrung von Experten Empfehlungen hinsichtlich des prolongierten Weanings nicht nur für den Bereich der Akutmedizin, sondern auch für den Bereich „Chronic critical care“ zu geben.

The interpretation of exhaled breath analysis needs to address to the influence of exogenous factors, especially to a transfer of confounding analytes by the test persons. A test person who was exposed to a disinfectant had exhaled breath analysis by MCC/IMS (Bioscout®) after different time intervals. Additionally, a new sampling method with inhalation of synthetic air before breath analysis was tested. After exposure to the disinfectant, 3-Pentanone monomer, 3-Pentanone dimer, Hexanal, 3-Pentanone trimer, 2-Propanamine, 1-Propanol, Benzene, Nonanal showed significantly higher intensities, in exhaled breath and air of the examination room, compared to the corresponding baseline measurements. Only one ingredient of the disinfectant (1-Propanol) was identical to the 8 analytes. Prolonging the time intervals between exposure and breath analysis showed a decrease of their intensities. However, the half-time of the decrease was different. The inhalation of synthetic air –more than consequently airing the examination room with fresh air –reduced the exogenous and also relevant endogenous analytes, leading to a reduction and even changing polarity of the alveolar gradient. The interpretation of exhaled breath needs further knowledge about the former residence of the proband and the likelihood and relevance of the inhalation of local, site-specific and confounding exogenous analytes by him. Their inhalation facilitates a transfer to the examination room and a detection of high concentrations in room air and exhaled breath, but also the exhalation of new analytes. This may lead to a misinterpretation of these analytes as endogenous resp. disease-specific ones.

The high sensitivity of the methods applied in breath analysis entails a high risk of detecting analytes that do not derive from endogenous production. Consequentially, it appears useful to have knowledge about the composition of inhaled air and to include alveolar gradients into interpretation.The current study aimed to standardise sampling procedures in breath analysis, especially with multicapillary column ion-mobility spectrometry (MCC-IMS), by applying a simultaneous registration of inhaled air and exhaled breath.A “double MCC-IMS” device, which for the first time allows simultaneous analysis of inhaled air and exhaled breath, was developed and tested in 18 healthy individuals. For this, two BreathDiscovery instruments were coupled with each other.Measurements of inhaled air and exhaled breath in 18 healthy individuals (mean age 46±10.9 years; nine men, nine women) identified 35 different volatile organic compounds (VOCs) for further analysis. Not all of these had positive alveolar gradients and could be regarded as endogenous VOCs: 16 VOCs had a positive alveolar gradient in mean; 19 VOCs a negative one. 12 VOCs were positive in >12 of the healthy subjects.For the first time in our understanding, a method is described that enables simultaneous measurement of inhaled air and exhaled breath. This facilitates the calculation of alveolar gradients and selection of endogenous VOCs for exhaled breath analysis. Only a part of VOCs in exhaled breath are truly endogenous VOCs. The observation of different and varying polarities of the alveolar gradients needs further analysis.

With appropriate esteem, we have read the article by Voshaar et al. [1] on the management of hypoxaemia related to COVID-19 pneumonia. The authors have concluded that treatment escalation in the sequence 1) supplemental oxygen; 2) nasal high-flow, continuous positive airway pressure (CPAP) or noninvasive ventilation (NIV); and 3) invasive mechanical ventilation resulted in low overall mortality if guided by the calculation of oxygen content in addition to the clinical presentation. We fully agree with the authors and others [2, 3] that hypoxaemia per se should not serve as the exclusive indication parameter for intubation. However, we disagree that this statement is, in any manner, supported by the data presented by Voshaar et al. RationaleThe role of non-invasive ventilation (NIV) in severe COVID-19 remains a matter of debate.ObjectivesTo determine the utilization and outcome of NIV in COVID-19 in an unbiased cohort.MethodsObservational study of confirmed COVID-19 cases of claims data of the Local Health Care Funds comparing patients with non-invasive and invasive mechanical ventilation (IMV) between spring versus autumn period 2020.Measurements and Main ResultsNationwide cohort of 7490 cases (median/IQR age 70/60–79 years, 66% male) 3851 (51%) patients primarily received IMV without NIV, 1614 (22%) patients received NIV without subsequent intubation, and 1247 (17%) patients had NIV failure (NIV-F), defined by subsequent endotracheal intubation. The proportion of patients who received invasive MV decreased from 74% to 39% during the second period. Accordingly, the proportion of patients with NIV exclusively increased from 10% to 28%, and those failing NIV increased from 9% to 21%. Median length of hospital stay decreased from 26 to 22 days, and duration of MV decreased from 11.6 to 7.6 days. The NIV failure rate decreased from 49% to 42%. Overall mortality remained unchanged (51% versus 53%). Mortality was 39% with NIV-only, 52% with IMV and 66% with NIV-F with mortality rates steadily increasing from 58% in early NIV-F (day 1) to 75% in late NIV-F (>4 days).ConclusionUtilization of NIV rapidly increased during the autumn period, which was associated with a reduced duration of MV, but not with overall mortality. In addition, robust data concerning the influence of the duration and failure of NIV on intubation and mortality rates are still missing, although the time span between initiation of NIV and subsequent intubation in case of respiratory failure progression is suggested to influence patient outcome.What This Study Adds to the FieldThis is the first large observational study describing differences of ventilatory strategies between the spring and autumn period of the SARS-CoV-2 pandemic in Germany and provides the in-hospital mortality rate of 7,490 patients who received mechanical ventilation. The increased utilization of NIV from 10% (first period) to 29% (second period) was associated with overall reduced durations of mechanical ventilation and length of hospital stay, but overall mortality remained comparably high and reached 51%, 53% respectively. Patients succeeding with NIV had lower mortality rates than those getting intubated without preceding NIV attempts, but those failing NIV had higher mortality rates, respectively, and this became even more predominant in late NIV failure. The present observational study shows the increasing role of NIV in the concert of ICU medicine related to COVID-19, but also clearly addresses its risks in addition to its benefits, both impacting on mortality.

Weaning from invasive mechanical ventilation is challenging for the ICU team in terms of shortening time of ventilation via endotracheal tube in order to improve the patient's prognosis by early extubation. Thereby prolonged mechanical ventilation ( 14 days), which is associated with risk of tracheotomy and prolonged weaning, shall be avoided. This article will give an overview about weaning categories, causes for weaning failure and strategies to overcome this problem. In the last part we will cover concepts in the process of prolonged weaning including discharge management with invasive mechanical ventilation.Das Weaning von invasiver maschineller Beatmung stellt das Intensivteam vor die Herausforderung, im Interesse des Patienten die Beatmungsdauer so kurz wie möglich zu halten und eine möglichst rasche Extubation zu erreichen. Hiermit sollen eine Langzeitbeatmung (invasive Beatmung 14 Tage) mit Tracheotomie und ein prolongiertes Weaning sowie eine ggf. notwendige außerklinische invasive Beatmung vermieden werden. Dieser Artikel gibt einen Überblick über die verschiedenen Weaningkategorien, die Ursachen für ein Scheitern des Weanings und Strategien, dieses zu vermeiden. Im letzten Teil stellt der Artikel Konzepte für das prolongierte Weaning und ggf. die Überleitung in die außerklinische invasive Beatmung dar.

Since December 2019 a novel coronavirus (severe acute respiratory syndrome coronavirus 2, SARS-CoV-2) has rapidly spread around the world resulting in an acute respiratory illness pandemic. The immense challenges for clinicians and hospitals as well as the strain on many healthcare systems has been unprecedented. The majority of patients present with mild symptoms of coronavirus disease 2019 (COVID-19); however, 5–8% become critically ill and require intensive care treatment. Acute hypoxemic respiratory failure with severe dyspnea and an increased respiratory rate (>30/min) usually leads to intensive care unit (ICU) admission. At this point bilateral pulmonary infiltrates are typically seen. Patients often develop a severe acute respiratory distress syndrome (ARDS). So far, remdesivir and dexamethasone have shown clinical effectiveness in severe COVID-19 in hospitalized patients. The main goal of supportive treatment is to ascertain adequate oxygenation. Invasive mechanical ventilation and repeated prone positioning are key elements in treating severely hypoxemic COVID-19 patients. Strict adherence to basic infection control measures (including hand hygiene) and correct use of personal protection equipment (PPE) are essential in the care of patients. Procedures that lead to formation of aerosols should be carried out with utmost precaution and preparation.

Standardisation of breath sampling is important for application of breath analysis in clinical settings. By studying the effect of room airing on indoor and breath analytes and by generating time series of room air with different sampling intervals we sought to get further insights into room air metabolism, to detect the relevance of exogenous VOCs and to make conclusions about their consideration for the interpretation of exhaled breath. Room air and exhaled breath of a healthy subject were analysed before and after room airing. Furthermore a time series of room air with doors and windows closed was taken over 84 h by an automatic sampling every 180 min. A second times series studied room air analytes over 70 h with samples taken every 16.5 min. For breath and room air measurements an IMS coupled to a multi-capillary column (IMS/MCC) [Bio-Scout® - BS Pentan-2-one [107-87-9] – Dimer; Hexan-1-al [66-25-1]; Pentan-2-one [107-87-9]) – Monomer showed high intensities in the room air and exhaled breath. They were significantly but not equally reduced by room airing. The time series about 84 h showed a time dependent decrease of analytes (limonen-monomer and -dimer; Decamethylcylopentasiloxane, Butan-1-ol, Butan-1-ol) as well as increase (Pentan-2-one [107-87-9] – Dimer). Shorter sampling intervals exhibited circadian variations of analyte concentrations for many analytes. Breath sampling in the morning needs room airing before starting. Then the variation of the intensity of indoor analytes can be kept small. The time series of indoor analytes show, that their intensities have a different behaviour, with time dependent declines, constant increases and circadian variations, dependent on room airing. This has implications on the breath sampling procedure and the intrepretation of exhaled breath.

Anhand der aktuellen Datenlage und einer Konsensuskonferenz wurden Empfehlungen zu Indikationen, zur Auswahl und Durchführung der geeigneten Belastungsuntersuchungen sowie zu den technischen und personellen Voraussetzungen formuliert. Detaillierte Angaben werden zur Blutgasanalyse unter Belastung, zu Gehtests und Rechtsherzkatheter unter Belastung sowie zur Spiroergometrie und Stressechokardiografie gemacht. Der korrekte Einsatz von Belastungsuntersuchungen bei anstrengungsinduziertem Asthma, bei sport-, arbeits- bzw. rehabilitationsmedizinischen Fragestellungen, bei Adipositas, bei der präoperativen Risikostratifizierung und beim Therapiemonitoring pneumologischer Erkrankungen wird dargestellt.

Mechanical ventilation (MV) is an essential part of modern intensive care medicine. MV is performed in patients with severe respiratory failure caused by respiratory muscle insufficiency and/or lung parenchymal disease; that is, when other treatments such as medication, oxygen administration, secretion management, continuous positive airway pressure (CPAP), or nasal high-flow therapy have failed. MV is required for maintaining gas exchange and allows more time to curatively treat the underlying cause of respiratory failure. In the majority of ventilated patients, liberation or “weaning” from MV is routine, without the occurrence of any major problems. However, approximately 20% of patients require ongoing MV, despite amelioration of the conditions that precipitated the need for it in the first place. Approximately 40–50% of the time spent on MV is required to liberate the patient from the ventilator, a process called “weaning”. In addition to acute respiratory failure, numerous factors can influence the duration and success rate of the weaning process; these include age, comorbidities, and conditions and complications acquired during the ICU stay. According to international consensus, “prolonged weaning” is defined as the weaning process in patients who have failed at least 3 weaning attempts, or require more than 7 days of weaning after the first spontaneous breathing trial (SBT). Given that prolonged weaning is a complex process, an interdisciplinary approach is essential for it to be successful. In specialised weaning centres, approximately 50% of patients with initial weaning failure can be liberated from MV after prolonged weaning. However, the heterogeneity of patients undergoing prolonged weaning precludes the direct comparison of individual centres. Patients with persistent weaning failure either die during the weaning process, or are discharged back to their home or to a long-term care facility with ongoing MV. Urged by the growing importance of prolonged weaning, this Sk2 Guideline was first published in 2014 as an initiative of the German Respiratory Society (DGP), in conjunction with other scientific societies involved in prolonged weaning. The emergence of new research, clinical study findings and registry data, as well as the accumulation of experience in daily practice, have made the revision of this guideline necessary. The following topics are dealt with in the present guideline: Definitions, epidemiology, weaning categories, underlying pathophysiology, prevention of prolonged weaning, treatment strategies in prolonged weaning, the weaning unit, discharge from hospital on MV, and recommendations for end-of-life decisions. Special emphasis was placed on the following themes: (1) A new classification of patient sub-groups in prolonged weaning. (2) Important aspects of pulmonary rehabilitation and neurorehabilitation in prolonged weaning. (3) Infrastructure and process organisation in the care of patients in prolonged weaning based on a continuous treatment concept. (4) Changes in therapeutic goals and communication with relatives. Aspects of paediatric weaning are addressed separately within individual chapters. The main aim of the revised guideline was to summarize both current evidence and expert-based knowledge on the topic of “prolonged weaning”, and to use this information as a foundation for formulating recommendations related to “prolonged weaning”, not only in acute medicine but also in the field of chronic intensive care medicine. The following professionals served as important addressees for this guideline: intensivists, pulmonary medicine specialists, anaesthesiologists, internists, cardiologists, surgeons, neurologists, paediatricians, geriatricians, palliative care clinicians, rehabilitation physicians, intensive/chronic care nurses, physiotherapists, respiratory therapists, speech therapists, medical service of health insurance, and associated ventilator manufacturers.

In acute exacerbations of COPD with acute hypercapnic respiratory failure and a pH 7.25 - 7.35, the initiation of non-invasive ventilation is the gold standard. However, absolute and relative contraindications have to be taken into account. The implementation of non-invasive ventilation in case of a severe respiratory acidosis necessitates a skilled therapeutic team and a close monitoring in order to avoid or perceive a NIV failure in time. In this case, the intubation and invasive mechanical ventilation is recommended. Ventilator settings have to aim at the prevention of an overinflation and increase of intrinsic PEEP. If severe hypercapnia and respiratory acidosis cannot be managed by mechanical ventilation, extracorporeal CO2 removal (ECCO2R) is a new treatment option. There are some reports about its use in awake patients in order to avoid an intubation. However, its general and primary use without optimizing medical therapy and mechanical ventilation is not indicated. ECCO2R is an experimental therapy in COPD with acute hypercapnic respiratory failure, its significance is still ambiguous. Therefore, it should only be applied in individual situations by a specialist team trained in its use.

Die nicht-invasive Beatmung (NIV) ist in der klinischen Medizin weit verbreitet und hat mittlerweile einen hohen Stellenwert im klinischen Alltag erlangt. Der Einsatz der NIV reduziert die Intensiv- und Krankenhausaufenthaltsdauer sowie die Mortalitat von Patienten mit akutem hyperkapnischen Atemversagen. Patienten mit akuter respiratorischer Insuffizienz im Rahmen eines Lungenodems sollten neben den notwendigen kardiologischen Interventionen mit CPAP oder NIV behandelt werden. Bei anderen Formen des akuten hypoxamischen Atemversagens wird empfohlen, den Einsatz der NIV auf lediglich mildes ARDS zu begrenzen, da eine NIV bei schweren Formen mit hoheren Raten von Therapieversagen und Mortalitat assoziiert ist. Im Weaning von invasiver Beatmung reduziert die NIV die Reintubationsraten im Wesentlichen bei hyperkapnischen Patienten. Eine verzogerte Intubation bei Auftreten eines NIV-Versagens fuhrt zu einem Anstieg der Mortalitat und sollte deshalb vermieden werden. Bei entsprechendem Monitoring auf der Intensivstation kann die NIV auch bei padiatrischen Patienten mit akuter respiratorischer Insuffizienz erfolgreich eingesetzt werden. Weiterhin kann im Rahmen der Palliativmedizin die NIV hilfreich bei der Reduktion von Dyspnoe und Verbesserung von Lebensqualitat sein. Ziel des Updates dieser Leitlinie ist es, unter Berucksichtigung der zunehmenden aktuellen wissenschaftlichen Evidenz, sowohl die Vorteile als auch die Limitationen der NIV in der Behandlung der akuten respiratorischen Insuffizienz in der taglichen klinischen Praxis und bei verschiedenen Indikationen aufzuzeigen.

All mechanically ventilated patients must be weaned from the ventilator at some stage. According to an International Consensus Conference the criteria for “prolonged weaning” are fulfilled if patients fail at least 3 weaning attempts (i. e. spontaneous breathing trial, SBT) or require more than 7 days of weaning after the first SBT. This occurs in about 15 – 20 % of patients. Because of the growing number of patients requiring prolonged weaning a German guideline on prolonged weaning has been developed. It is an initiative of the German Respiratory Society (Deutsche Gesellschaft fur Pneumologie und Beatmungsmedizin e. V., DGP) in cooperation with other societies (see acknowledgement) engaged in the field chaired by the Association of Scientific and Medical Societies in Germany (Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften, AWMF). This guideline deals with the definition, epidemiology, weaning categories, underlying pathophysiology, therapeutic strategies, the weaning unit, transition to out-of-hospital ventilation and therapeutic recommendations for end of life care. This short version summarises recommendations on prolonged weaning from the German guideline.

There are only few data about the influence of interfaces on restorative sleep and required CPAP/APAP levels in patients with obstructive sleep apnoea (OSA). Observations of obstructive apnoeas when using oro-nasal masks with normalisation of respiratory disturbance index (RDI) under nasal masks and of non-restorative sleep under oro-nasal masks in spite of normal RDI led to a registration of patients with such findings. This study is a cohort analysis (June 1, 2006 to April 30, 2014) of patients with OSA using an oro-nasal mask and normalisation of the RDI after changing to a nasal mask and of patients complaining about a non-restorative sleep under an oro-nasal mask despite normal RDI. Sixty-five patients (BMI 32.2 ± 8.1 kg/m2; 64.4 ± 12.8 years) with OSA (n = 54) and non-restorative sleep with normal RDI (n = 11) under oro-nasal masks were included. Background: The SERVE-HF study has raised questions concerning the higher mortality under adaptive servoventilation. The ventilatory mode was discussed as a possible aggravating factor. Objectives: We wondered if the data recorded by the adaptive servo-ventilation (ASV)-devices in heart failure patients with CSA-CSR ± OSA are different in terms of respiratory parameters and therapeutic pressures compared to patients with CPAP-resistant/emergent-CSA with normal BNP/NT-pro-BNP. Methods: Patients were included, if ASV had normalized respiratory disturbance index in the first night of application and after at least 6 weeks. ASV-device data were analyzed in terms of respiratory rate (RR), min ventilation (MV), endexpiratory (EEP), peak inspiratory pressure (Ppeak) and median pressure. Results: Compared to CPAP-resistant/emergent-CSA with normal BNP/NT-pro-BNP (n = 25), CSA-CSR- (n = 13) CSA-CSR+OSA-patients (n = 32) with elevated BNP/NT-pro-BNP had higher RR (p < 0.01) in the first night of ASV therapy and during follow-up (15.3 ± 1.3 vs. 17.3 ± 2.4/min) with similar MV (6.5 ± 1.3 vs. 6.6 ± 1.3 L), resulting in significantly lower tidal volumes. EEP (5.6 ± 1.1 vs. 5.5 ± 1.1 hPa), Pmedian and Ppeak (9.8 ± 1.5 vs. 9.7 ± 1.2 hPa) were comparable. Ventilatory parameters were not different between LVEF < 40, 40–49, and ≥50%, neither within the whole group nor the group of CSA-CSR ± OSA and heart failure. Conclusion: Patients with heart failure and CSA-CSR ± OSA have higher RRs but similar MV under ASV-therapy than patients with CSA and normal BNP. This indicates higher dead space ventilation. EF was not found to have an influence on the ventilatory parameters.
