Abstract

Surface pattern formation of Smart Cut™ silicon-on-insulator (SOI) substrates is investigated using a new process derived from this technology. The local control of SOI surface roughness is achieved by using hetero-temperature surface activated bonding to introduce locally bespoke shear stresses into bonded silicon wafers. The finite element method is used to back up experimental measurements of these large deformations to determine the amount of shear stresses introduced into the structure, and to determine its impact on the fracture mechanism. Subjects were assessed during 2 visits. The following tests were performed: visual acuity, tonometry, optical coherence tomography (OCT) optic nerve scan, frequency doubling time (FDT) and threshold (SAP) VF, ERG, and Online Spectro-reflectometry Oxygenation Measurement in the Eye (OSOME). Results are compared across visits and, when indicated, interpreted against those collected on non-Fabry population matched for age and sex.Results: The study population was composed of 3 males (34.3 ± 8.9 y.o.) and 5 females (46.4 ± 6.5 y.o). For all subjects, BCVA remained 6/6 OU throughout the study and OCT optic nerve scans were normal. FDT showed a defect in at least 1 quadrant for all participants, in contrast with SAP. FDT PSD value was found different vs SAP. For ERG, the i-wave (52.1 + 2.7 ms) and B-waves (31.6 ± 2.1 ms) peak times were significantly longer compared to a non-Fabry population (p < 0.05). Overall blood oxygenation varied from 61.3% ± 4% to 68.1% ± 4% at the second visit, suggesting a loss of capillary perfusion. Blood volume varied based on location (superior/inferior), eye tested (OD/OS) and time (visit 1/2). The range of values exceeds normal subjects findings (p < 0.05). Blood volume was correlated to FDT PSD value for the superior area of the optic nerve.Conclusion: The results suggest that Fabry subjects present FDT deficits and abnormal ERG patterns that may be explained by a retinal dysfunction affecting retinal ganglion cells (RGCs), second to vascular alterations.Keywords: Fabry disease, retinal ganglion cells, electroretinography, OSOME

Abstract

The visitor instrument PIONIER provides VLTI with improved imaging capabilities and sensitivity. The instrument started routinely delivering scientific data in November 2010, that is less than 12 months after being approved by the ESO Science and Technical Committee. We recall the challenges that had to be tackled to design, built and commission PIONIER. We summarize the typical performances and some astrophysical results obtained so far. Abstract

PIONIER stands for Precision Integrated-Optics Near-infrared Imaging ExpeRiment. It combines four 1.8m Auxilliary Telescopes or four 8m Unit Telescopes of the Very Large Telescope Interferometer (ESO, Chile) using an integrated optics combiner. The instrument has been integrated at IPAG starting in December 2009 and commissioned at the Paranal Observatory in October 2010. It provides scientific observations since November 2010. In this paper, we detail the instrumental concept, we describe the standard operational modes and the data reduction strategy. We present the typical performance and discuss how to improve them. This paper is based on laboratory data obtained during the integrations at IPAG, as well as on-sky data gathered during the commissioning at VLTI. We illustrate the imaging capability of PIONIER on the binaries deltaSco and HIP11231. PIONIER provides 6 visibilities and 3 independent closure phases in the H band, either in a broadband mode or with a low spectral dispersion (R=40), using natural light (i.e. unpolarized). The limiting magnitude is Hmag=7 in dispersed mode under median atmospheric conditions (seeing<1", tau0>3ms) with the 1.8m Auxiliary Telescopes. We demonstrate a precision of 0.5deg on the closure phases. The precision on the calibrated visibilities ranges from 3 to 15% depending on the atmospheric conditions. PIONIER has been installed and successfully tested as a visitor instrument for the VLTI. It permits high angular resolution imaging studies at an unprecedented level of sensitivity. The successful combination of the four 8m Unit Telescopes in March 2011 demonstrates that VLTI is ready for 4-telescope operation.

Abstract

PIONIER is a 4-telescope visitor instrument for the VLTI, planned to see its first fringes in 2010. It combines four ATs or four UTs using a pairwise ABCD integrated optics combiner that can also be used in scanning mode. It provides low spectral resolution in H and K band. PIONIER is designed for imaging with a specific emphasis on fast fringe recording to allow closure-phases and visibilities to be precisely measured. Abstract

In Switzerland, altruistic assisted suicide is permitted. We present here the federal regulations, the deontological rules, the provisions adopted by the cantons and other requirements applicable to assisted suicide. Given the complexity of these different rules and the legal questions that remain open, we recommend the preparation of brochures for patients, as well as better training and support for people faced with requests for assisted suicide.

Abstract

International audience; Background The identification of patients at high risk of a disabling disease course would be invaluable in guiding initial therapy in Crohn’s disease (CD). Our objective was to evaluate a combination of clinical, serological, and genetic factors to predict complicated disease course in pediatric-onset CD. Methods Data for pediatric-onset CD patients, diagnosed before 17 years of age between 1988 and 2004 and followed more than 5 years, were extracted from the population-based EPIMAD registry. The main outcome was defined by the occurrence of complicated behavior (stricturing or penetrating) and/or intestinal resection within the 5 years following diagnosis. Lasso logistic regression models were used to build a predictive model based on clinical data at diagnosis, serological data (ASCA, pANCA, anti-OmpC, anti-Cbir1, anti-Fla2, anti-Flax), and 369 candidate single nucleotide polymorphisms. Results In total, 156 children with an inflammatory (B1) disease at diagnosis were included. Among them, 35% (n = 54) progressed to a complicated behavior or an intestinal resection within the 5 years following diagnosis. The best predictive model (PREDICT-EPIMAD) included the location at diagnosis, pANCA, and 6 single nucleotide polymorphisms. This model showed good discrimination and good calibration, with an area under the curve of 0.80 after correction for optimism bias (sensitivity, 79%, specificity, 74%, positive predictive value, 61%, negative predictive value, 87%). Decision curve analysis confirmed the clinical utility of the model. Conclusions A combination of clinical, serotypic, and genotypic variables can predict disease progression in this population-based pediatric-onset CD cohort. Independent validation is needed before it can be used in clinical practice.

Abstract

Health services and systems research examines the match between individual and population health needs and clinical services. In the context of the ambulatory shift in psychiatry, services research is important to ensure the quality of clinical developments, their appropriateness to patient and societal needs, their accessibility, efficiency, implementation and real-world outcomes. The purpose of this article is to present the principles that guide mental health services and systems research and to illustrate them with three examples related to the ambulatory shift in psychiatry. Research on services must be based on both epidemiological and clinical research as well as on partnerships with expert patients.

In June 2021, the Swiss parliament accepted a legislative proposal calling for the introduction of a fee to fight emergency department (ED) overcrowding. Although this issue remains a major challenge for health policies, the introduction of such a fee raises many questions, notably regarding health equity. However, other more equitable solutions exist: improving the case management of ED frequent users and improving coordination between ED and primary care.

Abstract

Training provided by liaison psychiatry has been considerably developed over the last decades. This article describes how " communication skills" training in oncology evolved to now address interpersonal dimensions of the medical encounter, the clinician's "inner" world and the context. We further present a so-called clinician-centered supervision, which aims to address the intra psychic issues of the clinician; the opportunities and precautions with this specific type of supervision are discussed. Focusing on the clinician facilitates and enriches the clinical encounter and improves medical care.

Abstract

Adolescents aged 10 to 19 live a period of their life marked by opportunities and vulnerabilities during which the issue of mental health is of prime importance. Since several decades, and especially since the start of the COVID pandemic, mental health problems and disorders among adolescents have increased around the world. Depression, self-harm and suicidal behavior are common during this period of life, and pediatricians can play a pivotal role in identifying affected or vulnerable youngsters. This article describes risk factors for self-harm and suicidal behavior and reviews how primary care pediatricians and health professionals can respond to such situations. This scoping review is based on existing evidences as well as the authors clinical experience. It suggests concrete actions that can be taken to secure the life of at risk teenagers, and discusses how to organize the transfer to mental health professionals when needed.

Abstract

Men die more by suicide than women, but women attempt suicide more frequently. Gender does indeed shape suicidal ideas and behaviors. Due to differences in the way men express their suffering, men's depression may be underdiagnosed and undertreated. Furthermore, regarding suicidal behaviors, men use more lethal suicide methods than women. Their suicidal process seems also to progress more rapidly, which is probably related to their frequent substance use. Clinicians and researchers should consider these differences in their approach, without reducing men and women to their gender.
