In Italy, more than 2000 new male BC cases were registered between 2000 and 2014. The survival rate was lower in males than in females. Delayed diagnosis may be the reason for poorer outcome observed in male BC patients compared with female patients. Due to lack of substantial evidence and low availability of published data on male BC, the current treatment recommendations are based on evidence derived from trials on female patients. In Italy, most of the male BC patients are estrogen and progesterone receptor–positive. Targeted therapy in combination with endocrine therapy provides a clinically meaningful outcome in patients with hormone receptor–positive (HR-positive), human epidermal growth factor receptor 2–negative (HER2-negative) advanced BC. CompLEEment-1 is a single-arm, open-label, multicenter, phase 3b trial investigating the safety and efficacy of a CDK4/6 inhibitor, ribociclib, in combination with letrozole in men and women. Herein, we report the results from a retrospective analysis of five Italian male patients who completed the core phase. In this case series, the combination of ribociclib and letrozole was well tolerated and appeared to be effective in the male cohort with HR-positive, HER2-negative advanced BC in Italy. To compare the frequency of patients’ oral health problems and prevention needs among Slovenian and international dentists with the aim to validate the four oral health-related quality of life (OHRQoL) dimensions across six clinical dental fields in all World Health Organization (WHO) regions.

Abnormal elongation of mandibular coronoid process, often defined as coronoid hyperplasia, is a rare condition, which is frequently associated with limited mouth opening. In some cases, the enlarged coronoid pushes the zygoma forward causing facial asymmetry. This case report describes a 16-year-old boy whose chief complaint was a progressive difficulty and deviation in mouth opening, together with a deformity appearing at maximum opening at the zygomatic area. The diagnosis was Unilateral Accessory Mandibular Condyle at coronoid process, without reduction of the mouth opening capacity. A coronoidectomy was carried out by means of piezoelectric surgery, instead of a coronoidotomy which is usually performed in these cases, due to a suspect of ramus neoplasm. Abstract Background and aim Some studies suggested an association between Temporomandibular Disorders (TMD) and psychosocial status, but most of them are focused on samples of patients looking for treatment or present limits of sample representativeness. The aim of the present study was to evaluate the psychosocial status in a large sample of adult population, further than to assess its association to TMD symptoms, oral behaviours, and self-reported facial trauma. Results the study sample included 4299 subjects older than 18 years randomly recruited from general population in public spaces during their daily life (1700 Males, 2599 Females mean ± SD age = 40.4 ± 18.1). Psychosocial status and pain-related disability were assessed by means of Patient Health Questionnaire 4 (PHQ-4) and Graded Chronic Pain Scale (GCPS). TMD symptoms were assessed by RDC/TMD and validated screening tools for TMD pain. Oral Behaviours Checklist was used to investigate on oral behaviours. Logistic regression model was used to evaluate the association of the psychosocial status, TMD symptoms, trauma, and oral behaviours. The association was tested using both univariate and multivariate models. The PHQ4 evaluation showed a severe impairment in 4.6% of our sample, moderate in 18.8% and mild in 32.5%. We found a Characteristic Pain Intensity (CPI) level and Interference Score greater that 30 respectively in 36.2% and 22.2% of the study sample. The GCPS status revealed a high disability with severe limitation in 2.5% of the sample, high disability with moderate limitation in 7.0%, low disability high pain intensity in 7.4% and low disability low pain intensity in 37.8%. Anxiety and depression’s levels were significantly associated with gender, TMD pain, coexistence of TMD Pain and sound, and oral behaviours. GCPS status was significantly associated with age, TMD Pain, coexistence of TMD pain and sound, trauma, and oral behaviours. Conclusions In the general population, psychosocial impairment is associated to TMD pain, female gender, and report of oral behaviours. Hence, in adults with TMD accompanied by pain, psychosocial status should also be evaluated.

Abstract Background Mandibular second molar (M2M) impaction is a clinically significant manifestation of eruption disturbance in dental development. The primary aim of this study was to investigate the impact of the three-dimensional (3D) characterization on clinical and therapeutic decisions for M2M impaction. The secondary aim was to introduce a validated 3D classification system incorporating both surgical and orthodontic parameters. Methods Bidimensional (2D) and 3D radiological records of 15 impacted M2M were collected and deidentified. Ten experienced clinicians (5 oral surgeons;5 orthodontists) categorized each case, first based on 2D records and then with 3D scans. The degree of orthodontic and surgical difficulty in treating impacted M2M was evaluated using a novel classification system based on anatomical and radiological features. The primary outcome variable was the assessment of differences in diagnosis and decision-making protocol using 2D or 3D records, where clinical relevance ranged from 0 to 4. The secondary outcome variable was the validation analysis of the proposed 3D classification system to determine the concordance among the clinicians. Descriptive statistics and multivariable inferential analysis based on Akaike information criterion (AIC) were performed (α = 0.05). Results 3D examination allowed a better visualization of M2M impaction with higher clinical relevance for diagnosis of M2M root relationship to alveolar nerve and lingual plate, height to alveolar crest, depth, and inclination relative to the first molar and position relative to the third molar (range:2.69–3.43). The proposed 3D classification of M2M impaction changed clinical decisions regarding surgical-orthodontic approach, biomechanics, patient education, and treatment time estimate (range:2.59–3.33). In the validation analysis of the classification, no evidence of inter- or intra-group (surgeon/orthodontist) bias in score attribution occurred: the model with the minimum AIC was the null model (AIC = 718.04). Conclusion 3D evaluation of impacted M2Ms could enhance diagnostic accuracy, and a classification system was proposed and validated by a group of experienced surgeons and orthodontists with high concordance.

ABSTRACT: Researchers have reported the benefits of feeding rumen-protected methionine (RPM) during the peripartum on the health parameters of dairy cows. Rumen-protected Met has reportedly improved milk yield, milk components, and liver health, but the literature is scarce on its effects in commercial herds. Therefore, we aimed to determine the effects of feeding RPM (Smartamine M, Adisseo Inc., Antony, France) prepartum (8 g/cow per day) and postpartum (15 g/cow per day) on performance, metabolic profile, and culling rate of Holstein cows in a commercial herd. One hundred sixty-six (n = 166) Holstein cows, 58 nulliparous and 108 parous, were randomly assigned to 1 of 2 dietary treatments, consisting of TMR top-dressed with RPM (RPMet; 2.35% and 2.24% Met of MP for close-up and fresh cows, respectively) or without (control [CON] 2.03% and 1.89% Met of MP for close-up and fresh cows, respectively), fed from 21 ± 6 d prepartum until 16 ± 5 d postpartum. From 17 DIM until dry-off, all cows received RPMet. Daily milk yield was recorded, and milk samples were collected in the first and second weeks after calving to determine their composition. Blood samples were collected before the morning feeding on −14, −7, +1, +7, and +14 d relative to calving. Mortality and morbidity were recorded during the first 60 DIM. Cows supplemented with RPMet had greater milk yield during the first 16 DIM (31.76 vs. 30.37 kg/d; SEM = 1.04, respectively), and had greater milk fat content (4.45 vs. 4.10%; SEM = 0.11, respectively), but not milk total protein (3.47 vs. 3.39%; SEM = 0.04, respectively) and casein contents (2.74 vs. 2.66%; SEM = 0.04, respectively) than CON cows. Cows in RPMet had increased plasma Met concentrations than cows in CON (24.9 vs. 21.0 µmol/L; SEM = 1.2, respectively). Although morbidity was similar between treatments, the culling rate from calving until 60 DIM was lower for RPMet cows than for CON cows (2.4% vs. 12.1%; SEM = 0.02). In conclusion, cows receiving RPMet have greater milk yield, improved milk fat content, and a lower culling rate at 60 DIM than CON cows.

Abstract Background Vitamin A is essential for physiological processes like vision and immunity. Vitamin A’s effect on gut microbiome composition, which affects absorption and metabolism of other vitamins, is still unknown. Here we examined the relationship between gut metagenome composition and six vitamin A-related metabolites (two retinoid: -retinol, 4 oxoretinoic acid (oxoRA) and four carotenoid metabolites, including beta-cryptoxanthin and three carotene diols). Methods We included 1053 individuals from the TwinsUK cohort with vitamin A-related metabolites measured in serum and faeces, diet history, and gut microbiome composition assessed by shotgun metagenome sequencing. Results were replicated in 327 women from the ZOE PREDICT-1 study. Abstract Background During targeted treatment, HER2-positive breast cancers invariably lose HER2 DNA amplification. In contrast, and interestingly, HER2 proteins may be either lost or gained. To longitudinally and systematically appreciate complex/discordant changes in HER2 DNA/protein stoichiometry, HER2 DNA copy numbers and soluble blood proteins (aHER2/sHER2) were tested in parallel, non-invasively (by liquid biopsy), and in two-dimensions, hence HER2-2D. Methods aHER2 and sHER2 were assessed by digital PCR and ELISA before and after standard-of-care treatment of advanced HER2-positive breast cancer patients (n=37) with the antibody-drug conjugate (ADC) Trastuzumab-emtansine (T-DM1). Results As expected, aHER2 was invariably suppressed by T-DM1, but this loss was surprisingly mirrored by sHER2 gain, sometimes of considerable entity, in most (30/37; 81%) patients. This unorthodox split in HER2 oncogenic dosage was supported by reciprocal aHER2/sHER2 kinetics in two representative cases, and an immunohistochemistry-high status despite copy-number-neutrality in 4/5 available post-T-DM1 tumor re-biopsies from sHER2-gain patients. Moreover, sHER2 was preferentially released by dying breast cancer cell lines treated in vitro by T-DM1. Finally, sHER2 gain was associated with a longer PFS than sHER2 loss (mean PFS 282 vs 133 days, 95% CI [210-354] vs [56-209], log-rank test p=0.047), particularly when cases (n=11) developing circulating HER2-bypass alterations during T-DM1 treatment were excluded (mean PFS 349 vs 139 days, 95% CI [255-444] vs [45-232], log-rank test p=0.009). Conclusions HER2 gain is adaptively selected in tumor tissues and recapitulated in blood by sHER2 gain. Possibly, an increased oncogenic dosage is beneficial to the tumor during anti-HER2 treatment with naked antibodies, but favorable to the host during treatment with a strongly cytotoxic ADC such as T-DM1. In the latter case, HER2-gain tumors may be kept transiently in check until alternative oncogenic drivers, revealed by liquid biopsy, bypass HER2. Whichever the interpretation, HER2-2D might help to tailor/prioritize anti-HER2 treatments, particularly ADCs active on aHER2-low/sHER2-low tumors. Abstract Background Knowledge about patient safety in orthodontics is scarce. Lack of standardisation and a common terminology hinders research and limits our understanding of the discipline. This study aims to 1) summarise current knowledge about patient safety incidents (PSI) in orthodontic care by conducting a systematic literature search, 2) propose a new standardisation of PSI terminology and 3) propose a future research agenda on patient safety in the field of orthodontics. Methods A systematic literature search was performed in the main online sources of PubMed, Web of Science, Scopus and OpenGrey from their inception to 1 July 2023. Inclusion criteria were based on the World Health Organization´s (WHO) research cycle on patient safety. Studies providing information about the cycle’s steps related to orthodontics were included. Study selection and data extraction were performed by two of the authors. Results A total of 3,923 articles were retrieved. After review of titles and abstracts, 41 articles were selected for full-text review and 25 articles were eligible for inclusion. Seven provided information on the WHO’s research cycle step 1 (“measuring harm”), twenty-one on “understanding causes” (step 2) and twelve on “identifying solutions” (step 3). No study provided information on Steps 4 and 5 (“evaluating impact” or “translating evidence into safer care”). Conclusion Current evidence on patient safety in orthodontics is scarce due to a lack of standardised reporting and probably also under-reporting of PSIs. Current literature on orthodontic patient safety deals primarily with “measuring harms” and “understanding causes of patient safety”, whereas less attention has been devoted to initiatives “identifying solutions”, “evaluating impact” and “translating evidence into safer care”. The present project holds a proposal for a new categorisation, terminology and future research agenda that may serve as a framework to support future research and clinical initiatives to improve patient safety in orthodontic care. Paclobutrazol (PBZ) is a growth regulator that widely used in horticulture and in the tomato seedling growth to compact the shoots, increase the stem diameter and, root biomass, allowing more tolerance of the seedlings against adverse weather conditions. The objective of this work was to evaluate the rates of paclobutrazol (0, 4, 7, 10 and 13 mg L-1) applied 15 days after sowing by foliar spray on the growth, chemical composition and xylem vessel number of tomato seedlings cultivated in two periods. The PBZ regardless of the application rate reduced the height of tomato seedlings in both growth periods. The basal stem diameter and leaf area were increased with 13 mg L-1 of PBZ. The lignin percentage also increased with 10 and 13 mg L-1 of PBZ as compared to control for both periods. The number of xylem vessel was not affected by PBZ application on the seedlings in the first period. PBZ application at rates of 7 and 10 mg L-1 increased the xylem vessel number in the second period. In general, the application of 13 mg L-1 of PBZ generated seedling more robust to overcome climate adversities.

Abstract The purpose of this study was to systematically review the randomized and non-randomized clinical trials (RCT; nRCT) concerning the different available osteotomies for surgically assisted rapid maxillary expansion (SARME): pterygomaxillary disjunction (SARME + PD vs SARME-PD) and segmental Le Fort I osteotomy (2-piece vs 3-piece). Outcomes focused on skeletal, dental, upper airway changes, complications, and relapse. Two authors investigated five databases (PubMed, Cochrane Library, Google Scholar, Scopus, Web of Science) until August 2023. The Cochrane Collaboration Tool and the Newcastle–Ottawa scale were used for the quality assessment of the included RCTs and nRCTs, respectively. A total of 554 articles were retrieved and after duplicates removing and full-text reading, 40 studies were included. Two RCTs showed a low risk of bias, one an unclear risk and one a high risk. Among the non-RCTs, 15 studies showed a good quality, while 21 exhibited a fair quality score. SARME + PD resulted in more homogeneous posterior bone expansion, with minimal dental effects. No difference between 2-piece and 3-piece in asymmetric expansion was observed, although 3SO showed 1–2 mm of more transverse increase. The oropharynx minimum cross-sectional area, the nasopharynx and the oropharynx volume were greater in SARME + PD. Both dental and bone relapse can occur but no differences between the groups were observed. All osteotomies guaranteed a correction of transverse maxillary deficiency. Lower side effects were described in SARME + PD. Two-piece and 3-piece segmental Le Fort I osteotomies did not show any differences in the symmetry and amount of expansion.

ObjectivesThe goal of the study was to analyze the eruption time of the maxillary impacted canines treated with the “canine first technique” and evaluate the success rate.Materials and methodsA total of 103 patients with 131 impacted canines were treated. Alpha angle, Erickson–Kurol sectors, and age were studied to assess the difficulty of canine eruption. All the canines were treated with the “canine first” approach. The median follow-up time was evaluated using the Kaplan–Meier inverse procedure. The primary outcome (canine eruption time) was analyzed using Kaplan–Meier curves. The curve comparison between the different known risk factors was made using the log-rank test. The median eruption time (95% confidence interval) was calculated for each result.ResultsThe majority of the canines (66.4%) were placed palatally and positioned in sector 3 (30.16%). The aim of the paper was to study the intellectual profile of 94 gifted elders aged 60-90 years old with an IQ equal to or higher than 130 on at least one of the four primary indexes of the Wechsler Adult Intelligence Scale -- Fourth Edition. Their performances in the 15 subtests, in the Full Scale Intelligence Quotient, in the primary, supplementary and Cattell-Horn-Carroll theory Indexes, were analyzed. This group exhibited a significantly higher level of performance in all subtests and indexes than the control group matched for gender, age, and education. The differences between the two groups appear higher for the Matrix reasoning and Visual puzzle subtests. Additionally, in the gifted group the strengths seem to be represented more by Processing Speed and Perceptual Reasoning Indexes, in the control group are Comprehension Verbal and Working Memory Indexes. Moreover, gifted elders showed lower discrepancies across intellectual domains. These data provide evidence that, as the difference between the gifted group and the control group revealed in the present study is more due to processing speed, this dimension, which normaly is most affected by aging, on the contrary, is maintained/increased as a strength in the gifted elderly group.

Non-O blood groups are associated with decreased insulin sensitivity and risk of type 2 diabetes. A recent study pinpointed the associations between ABO blood groups and gut microbiome, which may serve as potential mediators for the observed increased disease risks. We aimed to characterize associations between ABO haplotypes and insulin-related traits as well as potential mediating pathways. We assessed insulin homeostasis in African Americans (AAs; n = 109) and non-Hispanic whites (n = 210) from the Microbiome and Insulin Longitudinal Evaluation Study. The ABO haplotype was determined by six SNPs located in the ABO gene. Based on prior knowledge, we included 21 gut bacteria and 13 plasma metabolites for mediation analysis. In the white study cohort (60 ± 9 years, 42% male), compared to the O1 haplotype, A1 was associated with a higher Matsuda insulin sensitivity index, while a lower relative abundance of Bacteroides massiliensis and lactate levels. Lactate was a likely mediator of this association but not Bacteroides massiliensis. In the AAs group (57 ± 8 years, 33% male), we found no association between any haplotype and insulin-related traits. In conclusion, the A1 haplotype may promote healthy insulin sensitivity in non-Hispanic whites and lactate likely play a role in this process but not selected gut bacteria.

Background: The aim of this study was to evaluate the accuracy and the repeatability of two reference systems for three-dimensional facial scans (FSs) compared with a reference system based on cone-beam computed tomography (CBCT). Subjects and methods: A total of sixty subjects, thirty growing participants (8–17 years old) and thirty non-growing participants (older than 21 years old), with FSs and full-field-of-view CBCT were included. Two different reference systems on the same FS were constructed. To assess validity, the two reference systems based on FSs were compared with the reference systems built using the CBCT scans. After two weeks, all of the FSs were reassessed to evaluate intra-operator repeatability. Reliability and repeatability were evaluated by means of parametric and non-parametric tests, intra-class correlation, the Dahlberg formula, and Bland–Altman plots (p < 0.05). Results: Both reference systems presented good reliability and showed a small difference with respect to the CBCT-based sagittal plane (Sagittal FS1 = 1.90 ± 0.98°; Sagittal FS2 = 1.80 ± 1.13°; p = 0.232). Between them, the two reference systems showed a small difference in the position of the sagittal plane (Sagittal FS1^Sagittal FS2 = 1.39 ± 1.13°). Conclusions: Both reference systems assessed in this study showed good intra-operator repeatability and their use may be suggested as reliable reference systems for FSs.

Mugil liza juveniles (6.69 ± 0.06 g) were subjected to dietary citral (0-control, 0.5, 1.0 and 2.0 mL per kg feed) for 45 days, and its possible effects on zootechnical and metabolic parameters, digestive enzymes, innate immunity, oxidative status and liver damage were evaluated. At the end of the experiment, fish fed 2.0 mL citral per kg feed showed a greater weight gain and protein retention efficiency, as well as enhanced activities of pepsin (stomach) and amylase (intestine) compared with control fish. Citral supplementation decreased liver lipoperoxidation and increased the activities of glutathione peroxidase, glutathione-S-transferase and superoxide dismutase in the gills, liver and brain. The highest level of citral inclusion augmented non-protein thiol content in the brain and gills. Myeloperoxidase activity was lower in fish offered 1.0 and 2.0 mL citral per kg feed. Dietary citral did not influence the plasma levels of aspartate aminotransferase and alanine aminotransferase or the hepatic histology. As it improved growth, the activity of digestive enzymes and general health, dietary citral may be recommended for M. liza at 2.0 mL per kg feed.

Background:. Carpal tunnel syndrome affects up to 6% of the general population, and surgical intervention is often required to ameliorate symptoms. Osteoarthritis (OA) is a common condition that often coexists with carpal tunnel syndrome. We hypothesized that patients with preexisting OA use more healthcare resources after carpal tunnel release (CTR) than patients without arthritis. Methods:. This was a retrospective cohort study at a single academic center between January 1, 2018 and November 1, 2021. Patients who underwent CTR were included. Preoperative carpal tunnel symptoms, diagnostic tests, medications, and concomitant OA were abstracted. Hand, wrist, and basal joint arthritis were specified. The primary outcome was healthcare utilization represented by duration and frequency of hand clinic and occupational therapy (OT) follow-up. In total, 312 hands were included. Multivariable analysis was performed. Results:. The average duration of hand clinic follow-up among patients without arthritis was 25.3 days compared with 87.1 days for patients with any arthritis (P = 0.0375) and 172 days for patients with wrist arthritis (P = 0.012). The average number of postoperative surgeon visits was increased in patients with hand arthritis, with an average of 2.3 visits versus 1.34 visits for patients without arthritis (P = 0.003). Both the number of OT visits and the duration of OT follow-up did not differ between cohorts. Conclusion:. After CTR, patients with preexisting OA use more healthcare resources than patients without OA.

According to IPCC projections, if there is no joint action between countries to curb the advance of greenhouse gas emissions, the temperature could rise by up to 4 °C, the worst-case scenario. With rising temperatures come adversities that will be and are being faced worldwide, one of which is rising sea levels. Given these level sea-level fluctuations and future projections, there is a need for research into multi-temporal coastline variations, thus providing a view of the past, present, and future of coastal zones around the globe, given that the world's growing population lives on the coast and that a repositioning of the coastline towards the continent has and will have devastating effects on populous regions. This work aims to analyze the coastline variations for the municipality of São Francisco de Itabapoana/Rio de Janeiro (Brazil). Three time periods were chosen: first, for the past 4773–4400 cal yr BP; second, for the present period between 1984 and 2022; third, to project a future scenario for the year 2100 using the sixth IPCC report on mean sea level. For the analysis of the coastline of the present, the area chosen was the entire length of the coast of the municipality of São Francisco de Itabapoana, and for the past and future projections, the area selected was the stretch from Lagoa Doce beach to the mouth of the Itabapoana river. The methodology used consisted of obtaining and processing high-resolution drone images and using Landsat satellite images (5, 7, and 8) to reconstruct and build sea levels for the three time periods chosen in this study. Based on the drone images, a digital surface model and an orthophoto were produced, which were used to reconstruct the sea level of the past when it was 2.5 m higher than the current level and to construct the future scenario (2100) with the scenario of 1.01 m higher than the current level. In the past, the results showed that the study area was flooded and that the current paleocean was active. The worst-case scenario from the IPCC's sixth report (1.01 m) was chosen for the future simulation. The result indicated that the area near the mouth, which is inhabited, will be flooded; in other words, there will be a social, environmental, and economic risk for the region. For the present time frame, the extraction of the coastline from 1984 to 2022 showed the evolution of the entire coastline of the municipality, both positive and negative, showing that the active cliff, which is on Lagoa Doce beach, is eroding continuously. The results show that in the reconstruction of the past, the paleofault was active, indicating that the ridges developed later. The current rate of change of the coastline indicated that it is generally stable in the municipality. The simulation for 2100 shows that the mouth of the Itabapoana River will suffer severe flooding, and that adaptations will be necessary for the region's population.
