Abstract

Summary: Background: Although immunomodulators have established benefit against the new coronavirus disease (COVID-19) in general, it is uncertain whether such agents improve outcomes without increasing the risk of secondary infections in the specific subgroup of previously immunocompromised patients. We assessed the effect of immunomodulators on outcomes of immunocompromised patients hospitalized for COVID-19. Methods: The protocol was prospectively registered with PROSPERO (CRD42022335397). MEDLINE, Cochrane Central Register of Controlled Trials and references of relevant articles were searched up to 01-06-2022. Authors of potentially eligible randomized controlled trials were contacted to provide data on immunocompromised patients randomized to immunomodulators vs control (i.e., placebo or standard-of-care). Findings: Eleven randomized controlled trials involving 397 immunocompromised patients hospitalized for COVID-19 were included. Ten trials had low risk of bias. There was no difference between immunocompromised patients randomized to immunomodulators vs control regarding mortality [30/182 (16.5%) vs 41/215 (19.1%); RR 0.93, 95% CI 0.61–1.41; p = 0.74], secondary infections (RR 1.00, 95% CI 0.64–1.58; p = 0.99) and change in World Health Organization ordinal scale from baseline to day 15 (weighed mean difference 0.27, 95% CI -0.09–0.63; p = 0.15). In subgroup analyses including only patients with hematologic malignancy, only trials with low risk of bias, only trials administering IL-6 inhibitors, or only trials administering immunosuppressants, there was no difference between comparators regarding mortality. Interpretation: Immunomodulators, compared to control, were not associated with harmful or beneficial outcomes, including mortality, secondary infections, and change in ordinal scale, when administered to immunocompromised patients hospitalized for COVID-19. Abstract

Acute aortic dissections are unpredictable aortic catastrophes. Although significant recent improvements have been accomplished in the treatment and management of aortic dissections, a significant amount is still not fully understood regarding this deadly condition. Comprehension of aortic dissections is especially important due to the impact of the disease, since a lot of patients die before reaching the hospital and a significant number of the ones who do survive the initial event end up dying during follow-up. The epidemiology and disease characteristics of these conditions are still under investigation and not completely clear. In this article we aim to review the current knowledge on the epidemiology and clinical characteristics of aortic dissections.

Abstract

INTRODUCTION: Out of Hospital Cardiac Arrest (OHCA) is one of the main causes of death worldwide. Most of the intrinsic causes of OHCA tend to be of cardiac origin, however, non-cardiac etiologies such as acute aortic dissection (AAD) may be more common than previously thought. The aim of this focused review is to summarize current knowledge on the association between OHCA and AAD. METHODS: A systematic review was previously performed on the incidence of AAD in the context of OHCA. For this publication, the selected references were reviewed to address three pre-determined questions: 1) How prevalent is Acute Aortic Dissection in Patients presenting with Out of Hospital Cardiac Arrest? 2) What clinical signs are associated with OHCA due to Acute Aortic Dissection? 3) How can we treat these patients and what is their prognosis? RESULTS: AAD may cause OHCA due to several reasons, such as retrograde involvement of the coronary arteries, aortic valve insufficiency, pericardium tamponade, aortic rupture, massive stroke, visceral malperfusion or hypertensive induced heart failure, for example. Since both the treatment and diagnosis of acute aortic dissections have improved, a growing number of OHCA due to AAD patients have been diagnosed and managed, however, the epidemiology and outcomes of these patients are still not fully understood.

Abstract

Abstract Background Antiphospholipid Syndrome (APS) is a multisystemic autoimmune disease characterized by arterial and venous thrombosis and / or obstetric morbidity in the presence of at least one circulating anti-phospholipid antibody. The spectrum of vascular events varies from deep venous thrombosis to catastrophic APS, a rare form characterized by acute multiorgan thrombosis and high mortality. Case report We present the case of a 32-week pregnant woman arriving in the hospital emergency room with bilateral acute lower limb ischemia. In the obstetric evaluation, fetal death was declared. Computerized Tomography angiography showed pulmonary embolism of both pulmonary arteries, areas of splenic and right renal infarction and multiple arterial and venous thrombosis. The patient underwent urgent caesarean section and axillary-bifemoral bypass. No events registered. In the postoperative period, in an intensive care unit, treatment with rituximab and plasmapheresis were added to anticoagulant therapy. The laboratorial investigation was negative for thrombophilia and autoimmune diseases. Conclusion Catastrophic APS develops quickly, with multiorgan involvement and high mortality rate. The presented case poses a multidisciplinary challenge, with the surgical approach of extra-anatomical revascularization being less invasive and guaranteeing immediate perfusion of the lower limbs. Although the serological tests were negative for anti-phospholipid antibodies, this case hardly fits into another diagnosis. Therefore, it was treated as a catastrophic APS, having shown a favorable evolution.

5. ENDOVASCULAR MANAGEMENT OF TRANSPLANT RENAL ARTERY STENOSIS: EARLY AND MID-TERM RESULTS

Abstract

OBJECTIVE: Renal graft dysfunction or worsened hypertension in renal transplanted patients may be a manifestation of graft hypoperfusion due to transplant renal artery stenosis (TRAS) or stenosis of iliac arteries proximal to renal transplant (pseudo-TRAS). Endovascular management of TRAS has been increasingly used to preserve renal graft function. With this study, we aim to evaluate the impact of endovascular treatment of TRAS on renal function in the short to medium term. MATERIAL AND METHODS: This is an observational, retrospective, single-center study that included all adult renal transplant patients who underwent endovascular intervention on TRAS between September 2017 and June 2020. Renal graft function was monitored by serum Creatinine (sCr) levels. RESULTS: Thirteen patients were included (53.8% female), with a median age of 57 (21-70) years. Eleven patients (84.6%) presented with graft dysfunction. Ten subjects (76.9%) underwent transluminal angioplasty and stenting of renal artery and three (23,1%) of donor iliac arteries. Most cases (69.2%) were interventioned in the first-year post-transplant. Overall technical success was 100%, with no periprocedural deaths. Overall 30-day morbidity was 15.4%. Median follow-up time was 20.2 (1,3 – 36,3) months. One patient died during follow up and other worsened graft dysfunction, requiring hemodi- alysis and nephrectomy. Reduction in sCr levels was statistically significant in the first postoperative month, compared to preoperative values, but sCR levels were still increased when compared to baseline levels (pre-TRAS diagnosis). CONCLUSIONS: The majority (12/13) of patients showed improvement or stabilization of renal graft function compared to the preoperative period, during the follow-up period, supporting the procedure’s safety. Despite this, most patients did not recover baseline sCr levels, reinforcing the importance of prompt graft revascularization. Delayed diagnosis of TRAS may compromise the benefit of revascularization and prevent full recovery of renal function.

Abstract

Aorto-iliac occlusive disease sometimes extends proximally with involvement of the renal arteries and visceral aorta, with loss of renal functional mass and intestinal ischemia. In this article we report the case of a patient presenting with progressive disabling intermittent claudication caused by chronic juxtarenal aortic thrombosis with proximal progression and involvement of the left renal artery, with a functional kidney preserved by the collateral circulation. This imagiologic finding led to a change in the usual surgical procedure, needing a supraceliac aortic clamp. Simultaneously, we choose to revascularize the left renal artery while perfunding the right renal artery with cold Ringer’s lactate solution during clamping period. No complications occurred during the surgery and post-operative period. Renal involvement in aortic juxtarenal thrombosis occurs in 3 to 15% of the cases but demands a more complex surgical approach with more proximal aortic clamping level as well as the use of intraoperative strategies to protect the visceral and renal territories.

Abstract

O tratamento endovascular dos aneurismas da aorta torácica descendente (AATD) está associado a baixas taxas de morbi- -mortalidade e é atualmente a primeira linha de intervenção terapêutica.. Os endoleaks são uma complicação bem conhecida desta técnica, sendo consensual que a identificação de endoleak tipo I implica a sua correção celere. Apresentamos o caso de uma doente submetida a TEVAR para tratamento de AATD, com exclusão do aneurisma no controlo angiográfico final. A angioTC de controlo mostrou endoleak tipo Ib com crescimento do saco aneurismático pelo que foi proposto tratamento, que a doente recusou. Admitida posteriormente de urgência, em rotura do aneurisma, a doente foi submetida de imediato a debranching dos troncos viscerais, seguido de extensão distal do TEVAR. O tratamento híbrido dos aneurismas torácicos sem landing zone distal através do debranching dos troncos viscerais e de TEVAR é uma alternativa à cirurgia convencional e à cirurgia endovascular complexa que apresenta bons resultados em doentes com risco cirúrgico elevado e permite ainda tratar doentes em regime urgente uma vez que não dependem da manufatura de endoprótese fenestradas e/ou ramificadas.

Abstract

Com o surgir da pandemia a SARS-CoV-2 no início de 2020, os serviços de saúde sofreram várias adaptações de forma a mitigar e controlar a progressão do surto a nível europeu e nacional. Estes ajustes repercutiram-se inevitavelmente na dinâmica da maioria dos serviços hospitalares, nomeadamente no nosso serviço de Cirurgia Vascular. Perante a previsível chegada de uma segunda vaga na Europa e em Portugal, com desfechos e repercussões imprevisíveis nos serviços de saúde, é fundamental aprender com as experiências anteriores e partilhar estratégias para prestar o melhor cuidado aos nossos doentes, apesar das restrições impostas pela pandemia. Através deste artigo, analisamos as adaptações no Centro Hospitalar Universitário Lisboa Norte para superar a pandemia e, em particular, no serviço de cirurgia vascular. Avaliamos ainda o impacto dessas mudanças na nossa atividade global, em comparação com a experiência vivenciada noutros centros nacionais e internacionais. Perante uma segunda vaga iminente, é crucial tirar lições desta pandemia por forma a evitar uma potencial crise na saúde.

Abstract

Summary Background Patients with COVID-19 pneumonia can have increased inflammation and elevated cytokines, including interleukin (IL)-6, which might be deleterious. Thus, sarilumab, a high-affinity anti-IL-6 receptor antibody, might improve the outcome of patients with moderate-to-severe COVID-19 pneumonia. Methods We did a multicentric, open-label, Bayesian randomised, adaptive, phase 2/3 clinical trial, nested within the CORIMUNO-19 cohort, to test a superiority hypothesis. Patients 18 years or older hospitalised with COVID-19 in six French centres, requiring at least 3L/min of oxygen but without ventilation assistance and a WHO Clinical Progression Scale [CPS] score of 5 were enrolled. Patients were randomly assigned (1:1) via a web-based system, according to a randomisation list stratified on centre and with blocks randomly selected among 2 and 4, to receive usual care plus 400 mg of sarilumab intravenously on day 1 and on day 3 if clinically indicated (sarilumab group) or usual care alone (usual care group). Primary outcomes were the proportion of patients with WHO-CPS scores greater than 5 on the 10-point scale on day 4 and survival without invasive or non-invasive ventilation at day 14. This completed trial is closed to new participants and is registered with ClinicalTrials.gov , NCT04324073 . Findings 165 patients were recruited from March 27 to April 6, 2020, and 148 patients were randomised (68 patients to the sarilumab group and 80 to the usual care group) and followed up for 90 days. Median age was 61·7 years [IQR 53·0–71·1] in the sarilumab group and 62·8 years [56·0–71·7] in the usual care group. In the sarilumab group 49 (72%) of 68 were men and in the usual care group 59 (78%) of 76 were men. Four patients in the usual care group withdrew consent and were not analysed. 18 (26%) of 68 patients in the sarilumab group had a WHO-CPS score greater than 5 at day 4 versus 20 (26%) of 76 in the usual care group (median posterior absolute risk difference 0·2%; 90% credible interval [CrI] −11·7 to 12·2), with a posterior probability of absolute risk difference greater than 0 of 48·9%. At day 14, 25 (37%) patients in the sarilumab and 26 (34%) patients in the usual care group needed ventilation or died, (median posterior hazard ratio [HR] 1·10; 90% CrI 0·69–1·74) with a posterior probability HR greater than 1 of 37·4%. Serious adverse events occurred in 27 (40%) patients in the sarilumab group and 28 (37%) patients in the usual care group (p=0·73). Interpretation Sarilumab treatment did not improve early outcomes in patients with moderate-to-severe COVID-19 pneumonia. Abstract

Introduction: The normal diameter values of the aorta are controversial, dictate when aneurysmal disease develops and when it should be treated. They differ between populations implicating that the indications for treatment are not universal. The need arises for regional registries so that treatment may be personalized. However, there are no previous registries in a latin population. Therefore, we performed a population based study on aortic diameters based on CTs measures. Methods: Cross-sectional study based on retrospective analysis of random CTs performed during 2018 in the largest Portuguese hospital. Patients with aneurysms were excluded. Diameter was presented in millimeters using outer to outer wall measurements, in an anterior-posterior or transverse plane, perpendicular to the lumen center line. Diameter was adjusted for age and sex, and measured in the locations specified in table 1. Age was categorized in 3 groups: >=50; 50-75; >75. This project was submitted and approved by the Ethics Committee of our institution (ref336/20). Results: The mean age of the population analyzed was 64 years old (SD 17; min-max 18-98) and 53.3% were male. A total of 396 CTs were analyzed, and of these 45% had thorax, 97% had abdominal and 83% had pelvic imaging. Mean aortic and iliac diameter are presented in table 1 and were adjusted for age and sex. In all locations of the aorta, the diameter was significantly higher in men than in women, except for the ascending aorta in women under 50 (pk : 0.21 and 0.24, respectively). Conclusions: We defined a set of normal values at key aortic locations. Age and sex significantly influenced these measures, while the dimensions follow a similar pattern through the vessel regardless of these factors. Table 1. Abstract

International audience; Background: Patients with COVID-19 pneumonia have an excess of inflammation and increased concentrations of cytokines including interleukin-1 (IL-1). We aimed to determine whether anakinra, a recombinant human IL-1 receptor antagonist, could improve outcomes in patients in hospital with mild-to-moderate COVID-19 pneumonia.Methods: In this multicentre, open-label, Bayesian randomised clinical trial (CORIMUNO-ANA-1), nested within the CORIMUNO-19 cohort, we recruited patients from 16 University hospitals in France with mild-to-moderate COVID-19 pneumonia, severe acute respiratory syndrome coronavirus 2 infection confirmed by real-time RT-PCR, requiring at least 3 L/min of oxygen by mask or nasal cannula but without ventilation assistance, a score of 5 on the WHO Clinical Progression Scale (WHO-CPS), and a C-reactive protein serum concentration of more than 25 mg/L not requiring admission to the intensive care unit at admission to hospital. Eligible patients were randomly assigned (1:1) using a web-based secure centralised system, stratified by centre and blocked with varying block sizes (randomly of size two or four), to either usual care plus anakinra (200 mg twice a day on days 1-3, 100 mg twice on day 4, 100 mg once on day 5) or usual care alone. Usual care was provided at the discretion of the site clinicians. The two coprimary outcomes were the proportion of patients who had died or needed non-invasive or mechanical ventilation by day 4 (ie, a score of >5 on the WHO-CPS) and survival without need for mechanical or non-invasive ventilation (including high-flow oxygen) at day 14. All analyses were done on an intention-to-treat basis. The trial is registered with ClinicalTrials.gov, NCT04341584, and is now closed to accrual.Findings: Between April 8 and April 26, 2020, we screened 153 patients. The study was stopped early following the recommendation of the data and safety monitoring board, after the recruitment of 116 patients: 59 were assigned to the anakinra group, and 57 were assigned to the usual care group. Two patients in the usual care group withdrew consent and were not analysed. In the analysable population, the median age was 66 years (IQR 59 to 76) and 80 (70%) participants were men. In the anakinra group, 21 (36%) of 59 patients had a WHO-CPS score of more than 5 at day 4 versus 21 (38%) of 55 in the usual care group (median posterior absolute risk difference [ARD] -2·5%, 90% credible interval [CrI] -17·1 to 12·0), with a posterior probability of ARD of less than 0 (ie, anakinra better than usual care) of 61·2%. At day 14, 28 (47%; 95% CI 33 to 59) patients in the anakinra group and 28 (51%; 95% CI 36 to 62) in the usual care group needed ventilation or died, with a posterior probability of any efficacy of anakinra (hazard ratio [HR] being less than 1) of 54·5% (median posterior HR 0·97; 90% CrI 0·62 to 1·52). At day 90, 16 (27%) patients in the anakinra group and 15 (27%) in the usual care group had died. Serious adverse events occurred in 27 (46%) patients in the anakinra group and 21 (38%) in the usual care group (p=0·45).Interpretation: Anakinra did not improve outcomes in patients with mild-to-moderate COVID-19 pneumonia. Further studies are needed to assess the efficacy of anakinra in other selected groups of patients with more severe COVID-19.Funding: The Ministry of Health, Programme Hospitalier de Recherche Clinique, Foundation for Medical Research, and AP-HP Foundation.
