Resumen Introduccion La enfermedad COVID-19 se ha convertido en una prioridad para nuestro sistema sanitario. Los MIR de Endocrinologia y Nutricion (MIR-EyN) se han integrado en los equipos COVID-19. Se ha disenado este estudio con el objetivo de analizar el impacto a nivel formativo, laboral y en la salud de los MIR-EyN. Material y metodos Estudio observacional transversal tipo encuesta via web, dirigida a los MIR-EyN socios de la SEEN, realizado en noviembre de 2020. Se han analizado: variables demograficas, tamano del hospital de formacion en numero de camas, alteracion de las rotaciones, integracion en equipos COVID-19, participacion en teleconsulta, actividad cientifica e impacto sobre la salud fisica y emocional. Resultados Se han obtenido 87 respuestas (27% de los MIR-EyN); 67,8% mujeres, edad media 28,1 ± 1,8 anos, 60% MIR-EyN 4.o ano. El 84% ha participado en equipos COVID-19 y el 93% en las consultas telematicas de su servicio. La mayoria ha visto interrumpidas sus rotaciones. El 97,7% ha participado en reuniones cientificas o congresos virtuales y un tercio de ellos ha colaborado en trabajos cientificos sobre la COVID-19 en relacion con la Endocrinologia y Nutricion. La pandemia ha afectado a su estado de animo mucho o bastante (75,8%) y opinan que ha impactado de forma negativa a su formacion (73,8%). Conclusiones La pandemia por SARS-CoV-2 ha comprometido la formacion, la actividad laboral y la salud de los MIR-EyN. Se han integrado tanto en equipos COVID-19 como en la actividad reestructurada de sus servicios. Sin embargo, han conseguido mantener la formacion en formato virtual y participado en trabajos cientificos.

Gastrointestinal adverse events; Obesity; Type 2 diabetes Esdeveniments adversos gastrointestinals; Obesitat; Diabetis tipus 2 Eventos adversos gastrointestinales; Obesidad; Diabetes tipo 2 Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are indicated in type 2 diabetes and obesity for their high efficacy in controlling glycaemia and inducing body weight loss, respectively. Patients may develop gastrointestinal adverse events (GI AEs), namely nausea, vomiting, diarrhoea and/or constipation. To minimize their severity and duration, healthcare providers (HCPs) and patients must be aware of appropriate measures to follow while undergoing treatment. An expert panel comprising endocrinologists, nephrologists, primary care physicians, cardiologists, internists and diabetes nurse educators convened across virtual meetings to reach a consensus regarding these compelling recommendations. Firstly, specific guidelines are provided about how to reach the maintenance dose and how to proceed if GI AEs develop during dose-escalation. Secondly, specific directions are set about how to avoid/minimize nausea, vomiting, diarrhoea and constipation symptoms. Clinical scenarios representing common situations in daily practice, and infographics useful to guide both HCPs and patients, are included. These recommendations may prevent people with T2D and/or obesity from withdrawing from GLP-1 RAs treatment, thus benefitting from their superior effect on glycaemic control and weight loss. IntroductionHyponatremia often occurs during the practice of endurance sports. We evaluated the impact on hyponatremia of the hydration recommendations of the Third International Exercise-Associated Hyponatremia Consensus Development Conference 2015 (3IE-AHCD) during the 2017 Gran Trail de Peñalara marathon (GTP) and the Vitoria Gasteiz Ironman triathlon (VGI).MethodsProspective study of GTP and VGI athletes participating in four information sessions in the months prior to the events, to explain that hydration should only be according to their level of thirst, per the recommendations of the 3IE-AHCD. Consenting event finishers were included in final analysis. Pre- and post-race anthropometric and biochemical parameters were compared.ResultsThirty-six GTP (33 male) and 94 VGI (88 male) finishers were evaluated. GTP race median fluid intake was 800 ml/h, with 900 ml/h in the VGI race. 83.3% GTPfin and 77.6% VGIfin remained eunatremic (blood sodium 135–145 mmol/L). Only 1/36 GTP and 1/94 VGI participant finished in hyponatremia, both with a sodium level of 134 mmol/L. Fourteen percent of GTP, and 21.2% of VGI participants finished in hypernatremia, with no increase in race completion times. No participating athlete required medical attention, except for musculoskeletal complaints. Pro-BNP and Copeptin levels rose significantly. Changes in copeptin levels did not correlate with changes in plasma osmolality, nor total body water content in impedance analysis.ConclusionsRecommending that athletes' fluid intake in endurance events be a function of their thirst almost entirely prevented development of hyponatremia, without induction of clinically significant hypernatremia, or a negative repercussion on race completion times.

Nutrition plays an important role in bone health. The aim of our study was to update the evidence regarding dairy intake, osteoporotic fracture (OF) risk, and prospective bone mass density (BMD) evolution assessed by dual-energy X-ray absorptiometry in Europeans and non-Hispanic whites from North America. A systematic search was conducted in MEDLINE, EMBASE, and Scopus for papers published from 1 January, 2000 to 30 April, 2018. The eligibility criteria were as follows: healthy adults; measurable dairy exposure; hip, vertebral, wrist or OF as outcomes; and cohort or case-control studies. Two independent investigators conducted the search and the data extraction. A pooled analysis was conducted with random-effects models. Publication bias and meta-regression were considered. Ten cohort studies relating to OF risk were selected for meta-analysis. Three papers reporting BMD changes associated with dairy intake could not be aggregated in the meta-analysis. The pooled HRs of the highest compared with the lowest levels of dairy intake were 0.95 (95% CI: 0.87, 1.03; I(2) = 82.9%; P-heterogeneity 65 y. In conclusion, in the studied population, the highest consumption of dairy products did not show a clear association with the total OF or hip fracture risks; however, a diminished risk of vertebral fracture could be described. The results regarding BMD change were heterogeneous and did not allow for a definitive conclusion.

Bariatric surgery is the most effective treatment for morbid obesity and its associated comorbidities. To ensure long-term postoperative success and safety, patients and physicians must be informed about the related commitment required, regarding supplementation, follow-up, and lifestyle changes. Hypoabsorptive procedures have demonstrated to be most effective in the long term when compared with other procedures. As morbid obesity is a chronic disease, these techniques should be applied as long as postoperative premises can be responsibly carried out.

One of the most important points for a successful result of bariatric surgery is the adequate nutritional status after surgery to ensure a healthy process of weight loss. The main objectives of the follow-up along the postoperative period will be the prevention of nutritional deficiencies, the early detection of subclinical deficiencies, and, eventually, the adequate treatment of overt deficiencies. For the prevention of deficiencies, we must encourage the right compliance of the postoperative diet and the supplements of vitamins and minerals from the first month after surgery. Depending on the surgical technique, we must take into account some specific and more prevalent deficiencies over others. The screening must be performed by periodical laboratory testing that detects subclinical deficiencies to treat them on time. We must evaluate and treat all the micronutrients globally because there are many potential interactions among them. The frequency of the laboratory testing will also be influenced by the surgical technique but should be evaluated individually.

Aim: Cholecystokinin (CCK) participates in the storage of dietary triglycerides in white adipose tissue (WAT). Our goal was to characterize, both in subcutaneous (Sc-WAT) and visceral WAT (Vis-WAT), the functional expression of the two known CCK receptors, CCK-1 (CCK-1R) and CCK-2 (CCK-2R), as well as of CCK. Main methods: Gene and protein expression was assessed in different cell types of rat and human WAT by means of RT-PCR and western-blot, respectively. The functionality of CCK-Rs was tested by quantifying protein kinase B (Akt) phosphorylation after treatment of pre-adipocytes with the bioactive fragment of CCK, CCK-8. The CCK receptor subtype involved in Akt phosphorylation was investigated by using selective CCK-1R (SR-27,897) and CCK-2R antagonists (L-365,260). Key findings: In rats, CCK-1R (Cckar) and CCK-2R (Cckbr) gene expression was detected in the two types of WAT analyzed as well as in isolated adipocytes, mesenchymal stem cells and pre-adipocytes. CCK-1R and CCK-2R proteins were identified in adipocytes and, to a minor extent, in pre-adipocytes. In addition, CCK-2R were detected in subcutaneous mesenchymal stem cells. Gene expression of the CCK precursor preproCCK as well as CCK immunoreactivity were also found in Sc-WAT and Vis-WAT. In human WAT, CCK gene expression as well as CCK-2Rs and CCK were also identified. CCK-8 evoked Akt phosphorylation in rat pre-adipocytes, and this effect was antagonized by SR-27,897 and L-365,260. Significance: Our data show that both human and rat WAT express a complete CCK system, and suggest that CCK may have an autocrine/paracrine role in regulating adipose tissue biology.

Food tolerance has been related to quality of life after bariatric surgery. However, long-term results about this issue are quite limited. The aim of this study was to evaluate the long-term food tolerance in obese patients submitted to surgery, comparing the results between gastric bypass (GBP), long alimentary modified biliopancreatic diversion (MBPD), and long alimentary modified duodenal switch (MDS). A cross-sectional analytic study was performed. Food tolerance was studied with a questionnaire based on subjective alimentary satisfaction, tolerance to different foods, and frequency of vomiting and regurgitation. A food tolerance score was obtained (1 point being the worst possible tolerance and 27 points being a perfect one). Information was obtained with a telephone interview. One hundred ninety-six patients submitted to bariatric surgery were included. Ninety-nine patients were submitted to GBP, 54 to MBPD, and 43 to MDS. One hundred and sixty-one patients (82.1%) were not lost during a mean follow-up time of 87.9 months. Mean food tolerance score was 24.2. Tolerance satisfaction was good or excellent in 73.3% of the patients. Red meat was the worst tolerated food, but nearly 80% of the patients could tolerate it without any problem. Mean food tolerance score was 24.6, 24.0, and 23.7 for GBP, MBPD, and MDS, respectively. There were no significant differences between these procedures in food tolerance score, alimentary satisfaction, or frequency of vomiting. Long-term food tolerance after bariatric surgery is good. No differences between GBP, MBPD, and MDS were found.

The beneficial effects of fish consumption in both children and adults are well known. However, the intake of methylmercury, mainly from contaminated fish and shellfish, can have adverse health effects. The study group on the prevention of exposure to methylmercury (GEPREM-Hg), made up of representatives from different Spanish scientific societies, has prepared a consensus document in a question and answer format, containing the group's main conclusions, recommendations and proposals. The objective of the document is to provide broader knowledge of factors associated with methylmercury exposure, its possible effects on health amongst the Spanish population, methods of analysis, interpretation of the results and economic costs, and to then set recommendations for fish and shellfish consumption. The group sees the merit of all initiatives aimed at reducing or prohibiting the use of mercury as well as the need to be aware of the results of contaminant analyses performed on fish and shellfish marketed in Spain. In addition, the group believes that biomonitoring systems should be set up in order to follow the evolution of methylmercury exposure in children and adults and perform studies designed to learn more about the possible health effects of concentrations found in the Spanish population, taking into account the lifestyle, eating patterns and the Mediterranean diet.

In Spain, certain population-based studies have shown high blood mercury (Hg) levels due to the high consumption of fish. Some studies have stated that one of the most consumed fish in Spain is canned tuna. Different Spanish organisms consider that it is safe to consume canned tuna as it supposedly has a low mercury content, particularly in so-called light tuna. However, in Spain light tuna is mainly yellowfin and bigeye tuna, while in other countries it is mainly skipjack tuna. This study analyzed 36 cans of the most popular brands in Spain and examined the influence of the type of tuna, packaging medium (olive oil, sunflower seed oil, water or marinade), different brands, prices and expiration dates. Mercury concentrations (mg/kg) were measured by atomic absorption spectrometry and thermal decomposition amalgamation. The medians observed were (mg/kg): light tuna: 0.314; IQR: 0.205 - 0.594, white tuna: 0.338; IQR: 0.276 - 0.558, skipjack: 0.311; IQR: 0.299 - 0.322, frigate tuna: 0.219; IQR 0.182 - 0.257 and mackerel: 0.042; IQR 0.029 - 0.074. We found statistically significant differences between white tuna, light tuna and mackerel (p = 0.004); light tuna and mackerel (p = 0.002) and white tuna and mackerel (p = 0.006). However, we found no differences between white tuna and light tuna, or among packaging medium, brands, prices or expiration dates. The limit of 0.500 mg/kg of mercury in canned tuna was exceeded by the following percentages of the cans: 33.3% of light tuna, 16.7% of white tuna, and 0% of Skipjack, frigate tuna and mackerel. The mercury content of the cans of Spanish light tuna that were analyzed was variable and high. The results of this study indicate that stricter regulation of Hg in canned tuna is necessary. Until then, it is safer to recommend that vulnerable populations such as children and pregnant women consume canned mackerel, which has a markedly lower mercury content.

the ultimate cause for the increased incidence of gastric ulcer following Roux-en-Y gastric bypass (RYGB) remains unclear. Treatment of HP infection is recommended before surgery in countries with high prevalence such as Spain in other to diminish the risk. However, the current regimens used might not be adequate in view of the high failure rate for HP eradication.we reviewed 243 patients retrospectively undergoing RYGB and found 111 patients (45%) with HP infection. Therefore, we compared the eradication rate between 2 different regimens.70 patients received OCA(Omeprazole:20 mg/12h, Clarithromycin 500 mg/12h and Amoxicillin 1 gram/12h for 10 days) while 41 patients received OLA (Omeprazole 20 mg/12 hours, Levofloxacin 500 mg/12hours and Amoxicillin 1 gram/12h for 10 days) for HP eradication. In 56/70 (80%) patients receiving OCA therapy HP was eradicated compared to 37/41 (91%) receiving OLA as first line therapy (p = 0.283). When used as second line therapy, in 13/14 (92%) patients receiving OLA HP was eradicated.clarithromycin resistance remains a matter of concern in this population while OLA seems to be a good alternative therapy for HP eradication, especially when OCA regimen fails.Introducción: las causas implicadas en el aumento de incidencia de úlcera gástrica tras el bypass en Y de Roux no son completamente conocidas. El tratamiento de la infección por HP se recomienda antes de la cirugía en países cuya prevalencia sea elevada, como el caso de España, de cara a disminuir dicha complicación. Sin embargo, las pautas actuales de tratamiento pueden no ser adecuadas dados los elevados índices de resistencia. Pacientes y métodos: análisis retrospectivo de 243 pacientes operados de bypass en Y de Roux. De ellos, 111 pacientes (45%) presentaban infección por HP. Objetivo principal: comparación de la eficacia de dos terapias de erradicación de la infección por HP. Resultados: 70 pacientes recibieron OCA( Omeprazol 20 mg/12 h, Claritromicina 500 mg/12 h y Amoxicilina 1 g/12h durante 10 días), mientras que 41 pacientes recibieron OLA (Omeprazol 20 mg/12 h, Levofloxacino 500 mg/12 h y Amoxicilina 1 g/12 h durante 10 días). En 56/70 pacientes (80%) que recibieron OCA HP fue erradicado, comparado con 37/41 (91%) del grupo que recibió OLA como primera terapia (p = 0,283). La terapia con OLA usada de segunda línea fue eficaz en 13/14 pacientes con HP resistente a la terapia OCA. Conclusión: la resistencia de HP a Claritromicina es significativa en nuestra serie de pacientes, siendo la terapia con OLA una alternativa adecuada en las cepas resistentes.

El mercurio es un tóxico ambiental que causa numerosos efectos adversos en la salud humana y en los ecosistemas naturales. Los factores que determinan la aparición de efectos adversos y su severidad son entre otros: la forma química del mercurio (elemental, inorgánico, orgánico), la dosis, la edad, la duración de la exposición, la vía de exposición y los factores ambientales, nutricionales y genéticos. En el ciclo acuático del mercurio, una vez que se ha depositado, se transforma en metilmercurio por la acción de determinadas bacterias sulfato reductoras y se bioacumula en los organismos acuáticos incorporándose a la cadena trófica de alimentos. El contenido de metilmercurio es mayor en las especies depredadoras de mayor tamaño y que viven más años como el emperador, pez espada, tiburón, atún o marlín. El metilmercurio se halla unido a las proteínas del pescado por lo que no se elimina mediante la limpieza ni el cocinado del mismo. El feto en desarrollo y los niños pequeños son los más vulnerables a los efectos neurotóxicos del metilmercurio procedente de la ingesta de pescado contaminado. El metilmercurio se absorbe en el tracto gastrointestinal y atraviesa la barrera hematoencefálica y la placenta. Algunos componentes de la dieta como los ácidos grasos poliinsaturados, el selenio, la fibra, los compuestos tiol, algunos fitoquímicos y otros nutrientes pueden modificar la bioaccesibilidad del mercurio y su toxicidad. Además de los factores ambientales, los factores genéticos pueden influir en la toxicidad del mercurio y explicar parte de la vulnerabilidad individual. Mercury is an environmental toxicant that causes numerous adverse effects on human health and natural ecosystems. The factors that determine the existance of adverse effects, as well as their severity are, among others: the chemical form of mercury (elemental, inorganic, organic), dosis, age, period of exposure, pathways of exposure and environmental, nutritional and genetic factors. In the aquatic cycle of mercury, once it has been deposited, it is transformed into methylmercury due to the action of certain sulphate-reducing bacteria, which bioaccumulates in the aquatic organisms and moves into the food chain. The methylmercury content of large, long-lived fish such as swordfish, shark, tuna or marlin, is higher. Methylmercury binds to protein in fish and is therefore not eliminated by cleaning or cooking the fish. Fetuses and small children are more vulnerable to the neurotoxic effects of methylmercury from the consumption of contaminated fish. Methylmercury is absorbed in the gastrointestinal tract and crosses the blood-brain barrier and the placenta. The intake of certain dietary components such as polyunsaturated fatty acids, selenium, fiber, thiol compounds, certain phytochemicals and other nutrients can modify methylmercury bioaccesibility and its toxicity. Apart from environmental factors, genetic factors can influence mercury toxicity and explain part of the individual vulnerability.
