The case is reported of a 35 year old heroin addict presenting with acute confusion which was later found to be due to meningococcal meningitis. Other than his altered mental state, the only abnormal finding on examination was a mild pyrexia.

An analysis of 266 operations on 63 patients with osteogenesis imperfecta, mostly type III and IV, showed that about half had corrections of skeletal deformities and internal splinting with intramedullary stainless steel rods. The remaining operations were for other orthopaedic procedures as well as non-orthopaedic procedures such as hernia repairs. There were no major anaesthetic or post-operative complications. Mild pyrexia was common but without the features of malignant hyperpyrexia. Correction of long deformities and internal splintage of the bones with non-expanding rods effectively reduced the fracture rate but deformities and fractures recurred as the epiphyses grew off the ends of the rods. Expanding rods provided continuing splintage during growth but could not be used in children with very narrow or very fragile bones.

Canine granulocytic ehrlichial organisms were transmitted from an infected dog from Missouri to two male, 10-month-old dogs by an intravenous injection of whole blood. Physical or behavioral abnormalities were not detected during the 98 days of evaluation other than a mild pyrexia from Day 18 to 20. Ehrlichial morulae were found in blood granulocytes of Dog 1 from Day 13 to 44 and of Dog 2 from Day 14 to 34 with the peak rickettsemia occurring on Day 16 for both dogs. By Day 21 after inoculaiuon, both dogs had positive titers to Ehrlichia canis. The highest titers for both dogs were found 63 days after inoculation, after which the titers decreased. Most of the hematologic abnormalities (i.e., neutropenia, lymphocytosis, thrombocytopenia) and fever occurred between 18 and 24 days after inoculation. The pathologic bases of these abnormalities were not investigated but their concurrent presence suggested an association with the dogs' immunologic responses to the granulocytic ehrlichial agent. Results from the study indicated that the canine granulocytic ehrlichial agent of Missouri may produce subclinical infections and suggested that dogs may be able to clear the organism without antimicrobial therapy.

Young adult cats were given an intravenous injection of Escherichia coli after ligation of one ureter lasting 24 or 48 h. Ten cats injected with a human strain of E. coli (between 2·3 and 35·23 × 10 8 per kg) developed mild pyrexia. Some of the cats examined between 1 and 16 months later had a mild focal non-progressive pyelonephritis in the obstructed kidney. Injection of a feline strain E. coli (between 0·83 and 6·4 × 10 8 per kg) produced an acute bacteraemic illness and 6 of 10 cats died between 1 and 11 days after infection. Cats that survived the acute illness had extensive pyelonephritis and scarring in the obstructed kidney when examined between 17 days and 5 months after infection. The experiments indicate that, after temporary ligation of one ureter, intravenous injection of E. coli is followed by establishment of infection in the obstructed kidney. Renal infection with a feline strain of E. coli can progress to a stage of patchy pyelonephritis that resembles the naturally occurring lesion in the cat.

In nine healthy men, after prolonged exercise (approximately 37 km walk and 2.5 km ascent and descent in 9 hr) deep body temperature at rest stabilized at a higher level than in the absence of previous exercise.2. The increase, approximately 0.36 degrees C, was recorded at both oral and rectal sites. It persisted while the subjects rested in warm and in cool surroundings, and disappeared about 11 hr after the end of exercise.3. The increase in body temperature after exercise was accompanied by a similar increase in the body temperature at which sweating started during body heating.4. After exercise the subjects' metabolic response to cold air (13.6 degrees C) was normal or increased in spite of the elevation in body temperature.5. Blood glucose concentration was never lower than 3.1 mM during these experiments.6. The temperature changes are most easily explained by mild pyrexia due to the release of endogenous pyrogen associated with minor tissue damage.

From among 1,360 stool specimens examined during the 19-month period from March 1982 to September 1983, small round-structured viruses, morphologically indistinguishable from the Norwalk agent, were detected by electron microscopy in 42 specimens from 28 children. Diarrhoea was mild, and vomiting, although frequently observed, was not an invariable feature. Mild pyrexia occurred in three patients. Many cases occurred some time after admission to the ward; this and other features suggested a hospital-acquired infection. These cases in particular tended to have other viral infectious agents detected at the same time. Persistent diarrhoea occurred in only three cases; one with preexisting gastrointestinal pathology, and two with a combined Norwalk-like agent, astrovirus, and rotavirus infection.
