Abstract

ImportanceSocially responsible surgery (SRS) integrates surgery and public health, providing a framework for research, advocacy, education, and clinical practice to address the social barriers of health that decrease surgical access and worsen surgical outcomes in underserved patient populations. These patients face disparities in both health and in health care, which can be effectively addressed by surgeons in collaboration with allied health professionals.ObjectiveWe reviewed the current state of surgical access and outcomes of underserved populations in American rural communities, American urban communities, and in low- and middle-income countries.Evidence reviewWe searched PubMed using standardized search terms and reviewed the reference lists of highly relevant articles. We reviewed the reports of two recent global surgery commissions.ConclusionThere is an opportunity for scholarship in rural surgery, urban surgery, and global surgery to be unified under the concept of SRS. The burden of surgical disease and the challenges to management demonstrate that achieving optimal health outcomes requires more than excellent perioperative care. Surgeons can and should regularly address the social determinants of health experienced by their patients. Formalized research and training opportunities are needed to meet the growing enthusiasm among surgeons and trainees to develop their practice as socially responsible surgeons.

11. A randomized feasibility trial evaluating the "My Changed Body" writing activity for people with endometriosis.

Abstract

Objectives: Body image disturbance is prevalent in people with endometriosis. This study aimed to explore the feasibility, acceptability and preliminary efficacy of the My Changed Body (MyCB) writing activity for body image in people with endometriosis. Design: Two‐arm randomized controlled pilot and feasibility study. Methods: Australian adults living with endometriosis who identified as having had a negative endometriosis‐related body image experience were recruited online through Endometriosis Australia. Participants completed a baseline survey capturing demographic and health data before being randomized into the MyCB or control writing conditions. Additional assessment was administered post‐activity and at 1‐week follow‐up. Results: A total of 82 participants were recruited and randomized (Mage = 32 years, SD = 7.8). MyCB participants were less likely to complete the writing tasks compared to the control group (51% vs. 83%); MyCB completers had lower self‐compassion and quality of life, and higher psychological distress, compared to those who dropped out. The MyCB activity received good satisfaction rates (47%–87%). However, there was significant drop‐out in the MyCB group (n = 4 vs. 13 at 1‐week follow‐up). Limited‐efficacy testing showed an increase in the self‐compassion self‐identification for the MyCB group. Conclusions: The MyCB writing activity may be feasible for people with endometriosis. A future larger trial will benefit from several methodology changes to enhance recruitment and retention. 22. ‘My body is out to wreck everything I have’: a qualitative study of how women with endometriosis feel about their bodies.

Abstract Objective Methods and Measures Results Conclusion There is a distinct lack of research regarding the relationship with the body in women with endometriosis, despite the condition involving significant changes to appearance and impaired bodily functionality. The current study aimed to understand how women with endometriosis feel about their body.Participants completed an online survey with open-ended questions on how they feel about their body, physical appearance, and level of daily functioning.Responses from 315 women with endometriosis were analysed using reflexive thematic analysis, generating three themes: 1) ‘It makes me feel broken and inadequate’ (Sense of being defective); 2) ‘I feel like I’m in a war with it’ (Sense of conflict); and 3) ‘I feel like my body isn’t mine; it’s out of control’ (Sense of alienation).The findings provide support for the notion that the relationship between the body and sense of self is particularly problematic for women with endometriosis and warrants therapeutic intervention. A spatial R.sub.0 was calculated at the neighborhood level, here defined by a 50-m buffer surrounding an index case, for mortality data from the 1878 yellow fever epidemic of New Orleans. The highest number of secondary mortalities linked to a neighborhood index case was twelve, with a further eighty-seven extrapolated morbidity cases. Results also highlight the importance of multideath residences and cultural contacts in neighborhood-level disease spread. Abstract Background Geographic Information Systems (GIS) can provide valuable insight into patterns of human activity. Online spatial display applications, such as Google Earth, can democratise this information by disseminating it to the general public. Although this is a generally positive advance for society, there is a legitimate concern involving the disclosure of confidential information through spatial display. Although guidelines exist for aggregated data, little has been written concerning the display of point level information. The concern is that a map containing points representing cases of cancer or an infectious disease, could be re-engineered back to identify an actual residence. This risk is investigated using point mortality locations from Hurricane Katrina re-engineered from a map published in the Baton Rouge Advocate newspaper, and a field team validating these residences using search and rescue building markings. Results We show that the residence of an individual, visualized as a generalized point covering approximately one and half city blocks on a map, can be re-engineered back to identify the actual house location, or at least a close neighbour, even if the map contains little spatial reference information. The degree of re-engineering success is also shown to depend on the urban characteristic of the neighborhood. Conclusion The results in this paper suggest a need to re-evaluate current guidelines for the display of point (address level) data. Examples of other point maps displaying health data extracted from the academic literature are presented where a similar re-engineering approach might cause concern with respect to violating confidentiality. More research is also needed into the role urban structure plays in the accuracy of re-engineering. We suggest that health and spatial scientists should be proactive and suggest a series of point level spatial confidentiality guidelines before governmental decisions are made which may be reactionary toward the threat of revealing confidential information, thereby imposing draconian limits on research using a GIS.

In an attempt to rediscover what has been lost to past time, I explore the memory of the past as a process that occurs through time in video meditations on objects, photographs and landscapes. This research explores the concept of absence as a presence in itself while asking the question: can what is absent become present, and can this happen through moving image art practice? I reflect on Barthes’ meditations on photography as death and a Bergsonian understanding of time as duration, while considering the moving image as an art form embedded in time. This research paper accompanies my body of work which is comprised of three video pieces: Places That Sometimes Disappear, Something in the Room, and Out of Time. These video works are meditative reflections on memory, loss and death.

Abstract

This paper is a personal account of the author’s understanding of her countertransference in relationship to a variety of clients. It was written during the second semester of her third year of bioenergetic training. Trainees were required to keep a journal of their process throughout the semester, noting specifically their awareness of their countertransference issues as they read the required readings and participated in the experiential exercises throughout the training weekends. The author explores ways in which her process in training and her process in her own therapy impact her growth and understanding of her countertransference.

Abstract

In the aftermath of a disaster like Hurricane Katrina, remote-sensing methods are often employed in an effort to assess damage. However, their utility may be limited by the aerial perspective and image resolution. The Spatial Video Acquisition System (SVAS), in conjunction with a Geographic Information System (GIS), has the potential to be a complementary methodology for obtaining damage assessment information as well as capturing recovery related geographies associated with post-traumatic stress. An example is provided from the Lower 9th Ward of New Orleans with data that could be used to predict neighborhood post-traumatic stress. Results reveal six dimensions in which a SVAS can improve existing disaster-related data collection approaches: organization, archiving, transferability, evaluation, objectivity, and feasibility.

Abstract

Although the link between fat talk and body dissatisfaction is well established, the link between fat talk and other body image disturbance components remains underexplored. Our meta-analytic review explored the cross-sectional, experimental, and longitudinal relations between fat talk and body dissatisfaction, body surveillance, body shame, pressure to be thin, thin-ideal internalization, body checking, and appearance-based comparisons. We identified 35 relevant studies via electronic databases. Meta-analyses provided effect size estimates based on study design and whether fat talk was the predictor or outcome of body image disturbance. Results showed that fat talk is related to a broader range of body image constructs than just body dissatisfaction and that accumulated evidence from longitudinal and experimental studies—although limited in number—suggests it is more plausible that fat talk is a risk factor for these body image constructs, rather than a consequence of them. Nevertheless, the suggestion that fat talk may play a role in the causal sequence of body image issues highlights this as a potential area of intervention for researchers and clinicians. Given that fat talk is common and often well intentioned, awareness-raising exercises for parents and peers may be necessary to curb its incidence and impacts.
