Objective: This retrospective single‐centre study aimed to compare the efficacy of maxillary second molar intrusion with two different approaches, miniscrew‐assisted molar intrusion and traditional segmental archwire intrusion, and to compare orthodontically induced external apical root resorption (OIERR) during intrusion between two groups via cone beam computed tomography (CBCT). Materials and Methods: A total of 40 adult patients (33.6 ± 10.3 years old) with supraerupted maxillary second molars due to the loss of antagonistic teeth were recruited, with 20 patients in each group. A segmental archwire with adjacent teeth as an anchorage was used in the control group, and 60–100 g of intrusive force was applied by using miniscrews in the experimental group to intrude the overerupted molars. Full‐volume CBCT was performed before and after intrusion, and the amount of intrusion and extent of OIERR of the overerupted molars were compared between the two groups. Results: Supraerupted maxillary second molars could be successfully intruded in an average of 5 months. There was more intrusive movement of the buccal and palatal cusps in the mininscrew group than that in the segmental archwire group (P <.05). The intrusive amount of palatal cusp was 3.67 ± 1.13 mm in the miniscrew group and 2.38 ± 0.74 mm in the segmental archwire group. More palatal OIERR was observed in the miniscrew group (30.3 ± 11.6 mm3) than in the segmental archwire group (21.0 ± 8.66 mm3) (P =.0063). There was no significant difference in OIERR between the two groups for mesial and distal buccal roots (P >.05). Conclusion: Miniscrews help effectively with supraerupted maxillary second molar intrusion, especially for palatal cusps. Objective: To determine the optimum miniscrew head design in orthodontic treatments for primary stability and compare stress distribution on a representative bone structure. Methods: Miniscrews with cross heads, mushroom-shaped heads, button heads, bracket heads, and through-hole heads were compared using finite element analysis. Miniscrews, whose three-dimensional drawings were completed using the SolidWorks computer-aided software package, were inserted in the bone block. Orthodontic force was applied to the head, and stress distributions, strains, and total deformations were investigated. Results: The lowest von Mises stress of 5.67 MPa was obtained using the bracket head. On the other hand, the highest von Mises stress of 22.4 MPa was found with the button head. Through mesh convergence analysis, the most appropriate mesh size was determined to be 0.5 mm; approximately 230,000 elements were formed for each model. Conclusion: Because the need for low stress is substantial for the primary stability of the miniscrew, this study demonstrated that the bracket head miniscrew is the optimal head design. In addition, it is posited that the success rate of orthodontic anchorage treatments will increase when bracket head miniscrews are used.

Key Clinical Message: Successful treatment by miniscrew‐assisted rapid expansion and clinical aligner therapy for an adult diagnosed with maxillary transverse deficiency (MTD) by clinical examinations and Yonsei transverse analysis. Adult orthodontic diagnosis and treatment with maxillary transverse deficiency (MTD) is challenging. Miniscrew‐assisted rapid palatal expansion (MARPE) is a fast and low‐risk method to expand the width of maxillary basal bone. This case report describes a 23‐year‐old female with mandible deviation and bilateral posterior teeth crossbite. She was diagnosed as MTD by the clinical examinations and Yonsei transverse analysis, and treated by the MARPE and clinical aligner therapy. After 26 months' treatment, cone‐beam computed tomography (CBCT) images showed that the width of maxillary basal bone increased by 3.8 mm, that of zygomatic arch and nasal bone increased by 1.0 and 1.9 mm, respectively, and the bilateral posterior teeth crossbite was corrected. Furthermore, the dental midline of lower arch was consistent with that of upper arch and face, the molars arrived Angle Class II and canines was Class I relationships, the profile was maintained, the facial asymmetry was improved. The results of seventeen‐month follow‐up showed that the effect of orthodontic treatment is very stable. This case report demonstrates that MARPE is the effective approach for patients with MTD and facial asymmetry, which provide the alternative for the treatment of similar cases. Introduction: Skeletal anchorage devices for the efficient orthodontic treatment mechanics can be placed at various interradicular and extraradicular sites intra orally. However, their placement in the infrazygomatic crest area can broaden the horizon of orthodontic treatment. The bone thickness in the infrazygomatic crest (IZC) area can vary at different heights and insertion angles during the placement of IZC screws between maxillary second premolar and second molar region. Aims and objectives: The purpose of our study was to evaluate the variation in bone thickness in the IZC area at different insertion angles along the mesiobuccal root of maxillary first molar and at different heights from the crest of the alveolar bone in the apical direction between maxillary second premolar and second molar region using cone beam computed tomography, so as to determine the best possible site where the bone thickness will be maximum that will suit to a particular size of IZC screw. Materials and Methods: CBCT images of 50 subjects were analyzed by a single observer, the bone thickness in the IZC area was measured and interpreted at different heights from the alveolar crest in the apical direction, that is, 5 mm, 7 mm, 9 mm, 11 mm at six regions between maxillary second premolar and second molar. Bone thickness was also measured at different insertion angles ranging from 55º to 75º to occlusal plane of permanent maxillary first molar along its mesiobuccal root. Results: The bone thickness in the IZC area was 4.5 mm-3.5 mm, when it was measured at different angles ranging from 55º to 75º to the occlusal plane of maxillary first molar along its mesiobuccal root with maximum bone thickness at 75° with a mean ± SD of 4.56 ± 1.4. Statistically significant differences in bone thickness were found between maxillary first and second molar on both right and left sides at different heights with maximum bone thickness at 11 mm apical from the alveolar crest with a mean ± SD of 2.03 ± 0.76 and 1.91 ± 0.88, respectively, and along the mesial root of maxillary second molar on right and left sides at 11 mm apical from the alveolar crest with a mean ± SD of 2.12 ± 0.80 and 1.95 ± 0.75, respectively. Conclusions: The best site and safe zone for miniscrew insertion in IZC area is at 7 mm to 9 mm distance from the alveolar crest apically along the mesial root of maxillary second molar and at an angle of 55°-75º to the occlusal plane of maxillary first molar, so as to ensure the adequate stability of the miniscrew and without causing any damage to the adjacent anatomical structures. Abstract Digital dental technology covers oral cone-beam computed tomography (CBCT) image processing and low-dose CBCT dental applications. A low-dose CBCT image enhancement method based on image fusion is proposed to address the need for subzygomatic small screw insertion. Specifically, firstly, a sharpening correction module is proposed, where the CBCT image is sharpened to compensate for the loss of details in the underexposed/over-exposed region. Secondly, a visibility restoration module based on type II fuzzy sets is designed, and a contrast enhancement module using curve transformation is designed. In addition to this, we propose a perceptual fusion module that fuses visibility and contrast of oral CBCT images. As a result, the problems of overexposure/underexposure, low visibility, and low contrast that occur in oral CBCT images can be effectively addressed with consistent interpretability. The proposed algorithm was analyzed in comparison experiments with a variety of algorithms, as well as ablation experiments. After analysis, compared with advanced enhancement algorithms, this algorithm achieved excellent results in low-dose CBCT enhancement and effective observation of subzygomatic small screw implantation. An efficient treatment plan using a temporary anchorage device should be built following the principle of reducing the number of tads to obtain a multiple biomechanical advantage. The following case report concerns the Biga system, a strategy that supports orthodontists during class II corrections and vertical control through treatment. A 12-year-old girl with a high angle of skeletal class II was selected. A novel biomechanical strategy was effectively applied using two tads on the upper arch to obtain sequential distalization of the upper teeth and to correct the lower arch spee curve using third-class elastics. Eventually, on the same tads, a double cantilever was applied to control the overbite and intrusion during incisors' retraction. BACKGROUND: The application of miniscrew in adolescents is increasing day by day, but at present, there are few studies on bone mass in the external oblique line of the mandible in adolescents at home and abroad, and there is no systematic study on bone mass in the external oblique line of the mandible in adolescents in different growth and development periods. OBJECTIVE: To measure the bone mass in the external oblique line of the mandible in adolescents with different cervical vertebral bone ages using a conebeam CT and to investigate the difference of bone mass in the external oblique line of the mandible in adolescents with different cervical vertebral bone ages and the correlation between bone mass in this area and the cervical vertebral bone age. METHODS: The cone-beam CT data of 105 adolescent patients before orthodontic treatment were collected and divided into CS3 group (n=24), CS4 group (n=26), CS5 group (n=29) and CS6 group (n=26) using the cervical vertebral maturation method. The adolescent mandibular buccal shelf was reconstructed by Mimics Medical 21.0 software. The width of buccal bone at 6 and 11 mm under the cemento-enamel junction and the bone height at 4 and 5 mm buccal to the cemento-enamel junction of right mandibular first and second molars were measured. The measured data were statistically analyzed. The measurement was made on four planes: plane 1 is the plane where the proximal mesial root of the mandibular right first molar is located; plane 2 is the plane where the distal mesial root of the mandibular right first molar is located; plane 3 is the plane where the proximal mesial root of the mandibular right second molar is located; and plane 4 is the plane where the distal mesial root of the mandibular right second molar is located. RESULTS AND CONCLUSION: In each group, the bone width on the buccal side of the external oblique line increased gradually from the first molar proximally to the second molar distally in adolescents, and the width of buccal bone at 6 and 11 mm under the cemento-enamel junction showed significant difference among different layers (P < 0.05). The bone width of buccal bone at 11 mm under the cemento-enamel junction was greater than that at 6 mm. The bone height on the buccal side of the external oblique line increased gradually from the first molar proximally to the second molar distally in all four groups, and the bone height at 4 and 5 mm buccal to the cemento-enamel junction showed significant differences at different layers (P < 0.05). The bone height at 4 mm buccal to the cemento-enamel junction was greater than that at 5 mm. On the fourth plane, the bone width at 11 mm buccal to the cemento-enamel junction was smaller in the CS3, CS4, and CS5 groups than in the CS6 group (P < 0.05). On the third plane, the bone heights at 4 mm and 5 mm buccal to the cementoenamel junction were smaller in the CS3 and CS4 groups than in the CS6 group (P < 0.05). On the fourth plane, the bone height at 5 mm buccal to the cementoenamel junction was smaller in the CS3 and CS4 groups than in the CS6 group (P < 0.05). On the fourth plane, the bone height at 4 mm buccal to the cementoenamel junction was smaller in the CS3 group than in the CS6 group (P < 0.05). Spearman correlation analysis showed that there was no correlation between bone mass and the cervical vertebral bone age, except that there was a weak correlation between bone mass at some measurement sites and cervical vertebral bone age. To conclude, the bone mass in the external oblique area of the mandible in adolescents does not change significantly with the increase of cervical vertebral bone age. The buccal side of the mesial root and distal root of the mandibular second molar in the external oblique area of CS3-CS6 adolescents meets the requirement of bone mass for miniscrew implantation, which is a site available for miniscrew implantation. Objectives: This finite element study aimed to simulate maxillary canine movement during anterior teeth retraction. Materials and methods: Three methods of maxillary canine movement including miniscrew sliding with high hooks (MSH), miniscrew sliding with low hooks (MSL), and the traditional sliding method (TS) without using miniscrews were simulated using three-dimensional finite element analysis. The initial displacement of the maxillary canine, the maximum principal stress of the periodontal ligament and the Von Mises stress were calculated. Results: The distolingual tipping movements of the canine were shown in three movement modes. MSH showed a small tendency to lingual tipping movement and a extrusion movement while MSL had the largest lingual inclination. TS demonstrated a tendency toward distolingual torsion displacement. Compressive stress values were mainly concentrated in the range − 0.003 to -0.006 MPa. For tensile stress, the distribution of MSH and MSL was concentrated in the range 0.005 to 0.009 MPa, TS was mainly distributed about 0.003 MPa. Von Mises equivalent stress distribution showed no significant difference. Conclusions: The loss of tooth torque was inevitable, irrespective of which method was used to close the extraction space. However, miniscrew application and higher hooks reduced the loss of torque and avoided lingual rotation. Clinical relevance: This study shows that miniscrew implants with different hooks can better control the movement of the maxillary canines. The non-invasive nature of the finite element analysis and its good simulation of dental stress and instantaneous motion trend have a clinical advantage in the analysis of tooth movement. The article focuses on utilizing skeletal anchorage to improve inadequate maxillary incisor display caused by vertical maxillary deficiency. Topics include diagnosis and etiology of inadequate incisor display, treatment alternatives such as orthognathic surgery or nonextraction orthodontic treatment with skeletal anchorage, and treatment progress involving the use of miniscrews and extrusion assemblies to achieve successful outcomes in improving maxillary incisor display and facial esthetics.

This case report of an 11-year-old subject shows the digital workflow for the management of an impacted mandibular canine using Computer-Aided Design/Computer-Aided Manufacturing (CAD/CAM) technology along with Temporary Anchorage Devices (TADs). The miniscrew insertion site was planned using software, and a surgical guide was digitally designed and 3D printed. Orthodontic traction was performed using a 3D-designed and -printed device. In a single sitting, the miniscrew was inserted and the disimpaction device was also delivered. The primary objective of recovery and the repositioning of the impacted mandibular canine in the axis with its eruptive path was achieved. The space available and the subject's early stage of mixed dentition was considered favourable to a spontaneous eruption. This case report shows how CAD/CAM digital technology, combined with 3D printing, enables the creation of a surgical guide to position the miniscrew and the customized devices used for mandibular canine disimpaction. CAD/CAM surgical guides can help clinicians to position TADs with more accuracy and predictability, ensuring high quality bone support offering primary stability. Although orthodontic traction is the most complex therapeutic choice to implement, with the aid of CAD/CAM technology it is possible to proceed with accurate and minimally invasive orthodontic traction in order to recover a mandibular canine. (1) Objective: This scoping review evaluates the effects of miniscrew-assisted rapid palatal expansion (MARPE) on different regions of the upper airway in adult patients and investigates various methods of measurement. (2) Methods: The search encompassed Pubmed, Cochrane Library, Scopus and Web of Science. This review was conducted following the PRISMA_ScR guidelines, and the inclusion criteria for examined studies were chosen in accordance with the PICOS framework. (3) Results: Seven studies were included in this review, comprising four retrospective studies, one prospective and two case reports. All studies involved the use of Cone Beam Computed Tomography (CBCT) for measurements of the areas of interest. The percentage of increase in the volume of the nasal cavity varied between 31% and 9.9%, depending on the study. Volumetric variations in the nasopharynx were reported as increases between T0 (before expansion) and T1 (immediately after expansion) of 6.4%, 20.7% and 14.1%. All studies considered T0 before expansion and T1 immediately after expansion. Only one study evaluated remote follow-up to assess if the results were maintained after one year. (4) Conclusions: MARPE appears to lead to a statistically significant increase in the upper airway, especially in the nasal cavity and nasopharynx immediately after expansion. Abstract Aim There is limited research on the clinical performance of double-thread orthodontic miniscrews. This study aimed to compare the stability of double-thread and single-thread orthodontic miniscrews and identify the potential associations between patient-related and location-related factors with miniscrew stability. Methods This retrospective cohort study involved 90 orthodontic miniscrews (45 single-thread, 45 double-thread) with identical dimensions (8 mm length, 1.6 mm diameter). The screws were inserted in various locations within the upper jaw of 83 patients (54 females, 29 males; mean age = 15.1 ± 2.4 years). Failure was defined as excessive mobility or loss of miniscrew after placement. The data recorded were patient age, gender, insertion site, side of insertion (buccal or lingual), duration of force application, and failure occurrence. Results The overall success rate within the sample was 92.2%. Double-thread miniscrews exhibited a significantly higher success rate than single-thread miniscrews (P = 0.049), with 97.8% and 86.7% success rates, respectively. Gender, age, insertion location, and side of insertion did not show significant associations with failure (P > 0.05). Log-rank analysis revealed a significant difference between the two groups (P = 0.046), indicating a higher probability of survival for the double-thread design. Conclusions The overall success rate of orthodontic miniscrews was high in the present sample. Double-thread miniscrews placed in various locations within the maxillary arch demonstrated superior stability and survival rates compared to their single-thread counterparts. Therefore, double-thread miniscrews may be preferred when bone quality is inadequate, such as in young patients.

Abstract Objectives This study aimed to investigate vertical changes in the maxillary central incisor and the maxillary first molar, along with alterations in the mandibular plane angle during space closure using miniscrew sliding mechanics. Methods Twenty adult patients treated at Peking University Hospital of Stomatology between 2008 and 2013 were included. Digital dental models and craniofacial cone-beam computed tomography (CBCT) scans were obtained at the start of treatment (T0) and immediately after space closure (T1). Stable miniscrews were used for superimposing maxillary digital dental models (T0 and T1), and vertical changes in the maxillary first molar and the maxillary central incisor were measured. Three-dimensional changes in the mandibular plane were assessed through CBCT superimposition. Results The maxillary central incisor exhibited an average extrusion of 2.56 ± 0.18 mm, while the maxillary first molar showed an average intrusion of 1.25 ± 1.11 mm with a distal movement of 0.97 ± 0.99 mm. Additionally, the mandibular plane angle decreased by an average of 0.83 ± 1.65°. All three indices exhibited statistically significant differences. Conclusion During space closure using the miniscrew sliding technique, significant changes occurred in both the sagittal and vertical dimensions of the upper dentition. This included extrusion of the maxillary central incisors, intrusion of the maxillary first molars, and a slight counterclockwise rotation of the mandibular plane.

Aim: The objective of this systematic review was to offer quantitative evidence regarding the influence of surface properties on the mechanical stability of miniscrews. Materials and Methods: The comprehensive search strategy involved querying databases, namely PubMed, Web of Science, and Scopus. PRISMA guidelines were followed to determine relevant studies according to specific eligibility criteria. The final search was conducted on 30 August 2023. In this systematic review, in vivo studies published in the English language were included. Results: A total of 364 articles were viewed, and 17 of them were considered for evaluation. Two of the articles are about human studies, while the rest are about animal studies. The number of miniscrew samples ranged between 18 and 144 (totaling 1097 with a mean of 64.52). Among the surface modifications in the articles, the sandblasting with large grit and acid etching (SLA) method was most frequently applied, followed by acid etching in second place. The control groups' (machined surface) maximum removal torque (MRT) values varied from 2.05 to 50.50 Ncm, while maximum insertion torque (MIT) values varied from 7.23 to 19.25 Ncm. Conclusions: The development of novel applications to improve the surface properties and survival rates of miniscrews is ongoing. In future studies, emerging surface modifications should be evaluated clinically, taking into account their cost and associated harm to the environment. [ABSTRACT FROM AUTHOR]

Objectives: We aimed to determine safe areas to apply miniscrews in the interradicular region of the maxilla and mandible in individuals with various sagittal skeletal malocclusions. Materials and methods: Cone beam–computed tomography images of 159 individuals were used. Individuals were divided into three groups: Class I, Class II, and Class III. In the sagittal plane, 3–6–9-mm apical sections were determined from the alveolar crest apex. The buccal cortical bone thickness, interradicular distance, and buccolingual bone distances were measured. Results: In the buccal cortical bone thickness, we observed statistically significant differences between the classes except for the 1–1 region in the maxilla and all regions and sections in the mandible (p < 0.05). The differences in the buccolingual bone distance between classes were statistically significant, except for the 3-mm and 6-mm sections in the 3–4 and 4–5 regions of the maxilla, the 9-mm sections in the 1–2 and 2–3 regions, the 6-mm and 9-mm sections in the 3–4 region, and the 6-mm section in the 4–5 regions of the mandible (p < 0.05). The differences in the interradicular bone distance were statistically significant between the classes in all regions and sections of the mandible except the 6-mm sections in the 1–2 region and in all sections of the maxilla except the 6-mm sections in the 3–4 region (p < 0.05). Conclusions: We observed significant differences in the buccal cortical bone thickness, interradicular bone distance, and buccolingual bone distance among individuals. The eligibility criteria were the following: studies on patients with transverse maxillary deficiency treated with MARPE in adults and adolescents over 13.5 years of age. Results Ultimately, twelve articles were included in the analysis, one prospective and eleven retrospective observational studies. Five studies showed a moderate risk of bias, while the remaining seven studies were at a serious risk of bias. The GRADE quality of evidence was very low. MARPE is an effective treatment modality for transverse maxillary deficiency (mean success rate: 93.87%). Patients showed increased mean in the skeletal and dental transverse expansion. The basal bone composition, mean alveolar bone and mean dental expansion accounted for 48.85, 7.52, and 43.63% of the total expansion, respectively. There was a certain degree of skeletal and dental relapse over time. MARPE could also cause dental, alveolar, and periodontal side effects, and have an impact on other craniofacial bones, upper airway, and facial soft tissue. Conclusions MARPE is an effective treatment for transverse maxillary deficiency, with a high success rate and a certain degree of skeletal and dental relapse over time.

Orthodontic miniscrews (MSs) are used for enhancing orthodontic anchorage either by supporting the teeth of the reactive unit or by obviating the need for the reactive unit altogether. Despite MSs’ popularity, their clinical application is not lacking in complications. The limited space of the insertion site (inter-radicular space), temporary use (limiting osseointegration) and the necessity to minimize the biological cost of insertion (bone incision) required the size of this auxiliary to be reduced, making it susceptible to mechanical failure. This review aimed to investigate factors influencing MS plastic deformation and fracture. The search applied five engines: PubMed, PMC, Web of Science, Scopus, Embase, and Ebsco. Quality assessment was performed according to the QUIN tool. After a thorough search process, 22 articles were included in this review. The most important factor influencing miniscrews’ plastic deformation and fracture was the screw diameter. The MS length and metal alloy did not influence its plastic deformation or fracture. The cylindrical design of the screw is preferable. If the cortical bone thickness in the insertion site exceeds 3 mm, pre-drilling upon insertion is recommended. Orthodontic MSs should not be reused. There is a need for high-quality clinical studies on the subject of MS deformation and fracture. The PROSPERO number is CRD42024509895.

Abstract Introduction Maxillary expansion in patients at the end of their growth relies on the possibility to use miniscrew supported expanders to apply expansion forces directly to the midpalatal suture. Although miniscrews provide a stable anchorage unit, several studies have reported that they do not remain in exactly the same position during treatment. The aim of the present study was to analyze miniscrew position changes after the expansion using bone-borne appliances in late adolescent patients. Methods Nineteen patients (13 females, 6 males), with a mean age of 17.81 (SD = 4.66), were treated with a Bone-Borne Expander Device. The appliance was designed with 4 miniscrews: 2 in the anterior palatal area, at the third rugae level; 2 in the posterior area. A CBCT and an intraoral scan were obtained before treatment (T0), and then, a second CBCT was obtained after the expansion (T1). Data on peri-suture bone thickness were collected at T0, then the CBCTs were superimposed, and changes between mini-screws position on T0 and T1 were evaluated, both by linear and angular displacements. Results Significant longitudinal differences were found in the distance of the head and the tip of miniscrews measured at the occlusal plane, as well as angular changes. Correlations between displacement measurements and peri-suture bone thickness and height measurements were found as well. Conclusions While acting as bone anchor units, miniscrews do not remain in the same position during bone-borne expansion. The amount of displacement was related to peri-sutural total bone height and cortical thickness, especially in the anterior area of the naso-frontal maxillary complex.

Objective: To investigate the difference and distribution of bone depth at different three-dimensional simulated paths to help optimize the insertion path for miniscrew placement in the infrazygomatic crest. Methods: Cone beam computed tomography scans of 80 adults (38 males and 42 females; mean age, 27.0 years) were assessed. For each subject, bone depth of 81 simulated insertion paths at different insertion points and three-dimensional angulations was measured in 160 infrazygomatic crests; the differences were evaluated using the adjusted Friedman test. This case report describes an 18-year-old female patient with a skeletal class I pattern, mandibular asymmetry, a molar class III on the right side, a molar class II on the left side, and an anterior open bite (AOB). Treatment of AOB is often difficult for orthodontists and a multidisciplinary approach is sometimes required to rehabilitate the correct function and posture of the tongue. In this case the AOB was treated using the Nuvola® OP System: a treatment system that combines orthodontic aligners and a myofunctional elastodontic device called Freedom. An upper molar intrusion with miniscrews was performed during the first phase of the treatment programme, and then the aligner treatment began. The Nuvola® OP System 's aligners are characterized by lingual pins that guide the tongue to the palatal spot position and take advantage of tongue function. The patient was instructed to wear the Freedom device for 30 min a day and to clench every 3 s with closed lips while wearing the aligners. These exercises improve the fitting of the aligners, improve the tongue's posture, and take advantage of the masticatory forces to resolve the malocclusion. The patient was successfully treated, thus demonstrating that the Nuvola® OP System is a valid alternative for the treatment of AOB. [ABSTRACT FROM AUTHOR]

Introduction: This retrospective study aimed to evaluate the skeletal and dental effects of the miniscrew‐anchored facemask in skeletal Class III growing patients and compare them with those of conventional tooth‐anchored facemasks. Methods: Retrospectively a total of 50 patients with skeletal Class III (mean ANB: −1.12°) were investigated and divided into two groups according to the treatment modality. Twenty‐five patients were treated using the conventional tooth‐anchored facemask (T group: mean age 9.3 ± 1.1 years, mean ANB: −0.93°) whereas the other 25 were treated using a miniscrew‐anchored facemask (M group: mean age 9.7 ± 1.3 years. mean ANB: −1.61°). Two miniscrews were placed on the palate for bone anchorage. In both T and M groups, facemasks applied a force of 20–30° down on the occlusal plane, and the force increased from 200 g to 300–350 g per side throughout the treatments. The patients were instructed to wear facemasks for at least 14 h per day. A total of 16 angular and 11 linear cephalometric measurements were analysed to determine the skeletal and dental changes before and after facemask treatment. A paired t‐test was used to verify the effects before and after treatment in each group. Results: All miniscrews were well maintained during treatment. The values of SNA, SN‐ANS, ANB and A to N‐Perp, which indicate anterior protraction of the maxilla, were significantly higher in the M group compared with the T group (P <.05). Proclination of the maxillary incisors, extrusion and mesialization of the maxillary molars were significantly greater in the T group (P <.05). Conclusions: Miniscrew‐anchored facemask treatment increased the amount of maxillary protraction and reduced the dental side effects compared with conventional tooth‐anchored facemask treatment in growing patients with skeletal Class III malocclusion. Objective To assess the bone thicknesses of the infrazygomatic crest in the adult patients with different vertical skeletal patterns of skeletal classⅠmalocclusion based on cone-beam computed tomography (CBCT), and to provide the basis for the choice of clinical treatment methods for the orthodontists. Methods A total of 90 adult patients with skeletal class I malocclusion were selected and divided into low angle group, average angle group, and high angle group, and there were 30 patients in each group. The buccal bone thickness of the patients at different vertical levels and proximal, distal and mesial levels were detected on the CBCT images and the effective bone volume was measured at buccal alveolar bone thickness ≥ 3 mm with the 50°, 60°and 70° implantation angles. Results In different vertical sections, except for the proximal mesiobuccal root level of the maxillary first molar (U6mb), the buccal alveolar bone thickness was increased gradually from the top of the alveolar ridge to the root side (P<0.05) . At different proximal, distal and mesial levels, the buccal bone thickness of the patients was gradually increased from the proximal to the distal mesial direction in all levels except U6mb (P<0.05) . The maximum buccal bone thickness of the patients was 11 mm from the top of the alveolar ridge at the proximal mesiobuccal root of the maxillary second molar (U7mb) ； the minimum buccal bone thickness was 7 mm from the top of the alveolar ridge at U6mb.There was significant difference in the buccal bone thickness of the patients with different vertical bone facial types (P<0.05), the buccal bone thickness of the patients in low angle group was higher than those in average angle group and high angle group (P<0.05), and the buccal bone thickness of the patients in average angle group was higher than that in high angle group (P<0.05) .Among the patients in low angle group, there were significant differences in bone thicknesses between the distal mesiobuccal root of the maxillary first molar and the proximal mesiobuccal root of the maxillary second molar (U6db-U7mb) at from the top of the alveolar crest was 7 mm, at U7mb when the distance from the top of the alveolar crest was 7 mm and 9 mm, at U6mb-U6db when the distance from the top of the alveolar crest was 11 mm at three angles (P<0.05) .The bone thickness was ≥6 mm at U6db-U7mb, when the distance from the top of the alveolar crest was 7 mm at three angles, and when the distance from the top of the alveolar crest was 9 mm at 70° at U7mb.Among the patients in average angle group, there were significant differences in the bone thicknesses between U6db-U7mb at 9 mm from the top of the alveolar crest, U7mb at 9 mm and 11 mm from the top of the alveolar crest at three implantation angles, and the bone thickness ≥6 mm at U7mb when the distance from the top of the alveolar crest at three angles was 7 mm and at U7mb when the distance from the top of the alveolar crest was 9 mm at 70° (P<0.05). Among the patients in high angle group, there were significant differences in bone thickness between U6db-U7mb 11 mm from the top of the alveolar ridge and U7mb 11 mm from the top of the alveolar ridge at three implantation angles (P<0.05) . The thicknesses of bone implantation at different angles could be ranked as follows, 70°>50°>60° in all dimensions. Conclusion There are differences in the buccal bone thickness in the zygomatic crest area among the adult patients with different vertical facial bone patterns. The patients in low angle group have more options for the implantation sites, while the patients in high angle group are more prone to maxillary sinus perforation.Therefore, when placing dental implants and designing orthodontic treatment plans in this area, the vertical facial bone pattern should be taken into consideration. To identify and analyze the 50 most cited articles on temporary anchorage devices (TADs) and investigate the achievement and development of scientific research about the topic through a bibliometric analysis. On August 22, 2022, a computerized database search was performed to detect papers published in the scientific literature about TADs from 2012 to 2022. Metrics data were identified using the Incites Journal Citation Reports (Clarivate Analytics) data set. The Scopus database was used to obtain information on the authors' affiliations, country of origin, and h-index. Key words were automatically harvested from the selected articles to implement the visualized analysis. From a total of 1858 papers screened by searching the database, a list of the top 50 most cited articles was created. The total number of citations collected by the 50 most cited articles in TADs was 2380. Among the 50 most cited articles on TADs, 38 were original research papers (76.0%) and 12 were reviews (24.0%). As shown by the key word–network analysis, Orthodontic anchorage procedure was identified as the larger node. Findings of this bibliometric study showed an increasing number of citations for papers on TADs, accompanied by a simultaneous rise in scientific interest in this topic in the past decade. There is no unequivocal scientific consensus for the temporary anchorage device (TAD) positioning in the infrazygomatic crest area (IZC). The two principal aims of this systematic review were to assess bone availability in the IZC and to establish both the target site and the need for cone-beam computed tomography (CBCT) prior to miniscrew placement. The study was performed following PRISMA guidelines (PROSPERO: CRD42023411650). The inclusion criteria were: at least 10 patients, three-dimensional radiological examination, and IZC assessment for the TAD placement. ROBINS-I tool and Newcastle-Ottawa Scale were used for quality evaluation. No funding was obtained. The study was based on the information coming from: PubMed, Google Scholar, Web of Science Core Collection, MDPI, Wiley, and Cochrane Libraries. The last search was carried out on 1 August 2023. Fourteen studies were identified for analysis. A narrative synthesis was performed to synthesize the findings of the different studies. Unfortunately, it is not possible to establish the generally recommended target site for IZC TAD placement. The reasons for this are the following: heterogeneity of available studies, inconsistent results, and significant risk of bias. The high variability of bone measurements and the lack of reliable predictors of bone availability justify the use of CBCT for TAD trajectory planning. An efficient treatment plan using a temporary anchorage device should be built following the principle of reducing the number of tads to obtain a multiple biomechanical advantage. The following case report concerns the Biga system, a strategy that supports orthodontists during class II corrections and vertical control through treatment. A 12-year-old girl with a high angle of skeletal class II was selected. A novel biomechanical strategy was effectively applied using two tads on the upper arch to obtain sequential distalization of the upper teeth and to correct the lower arch spee curve using third-class elastics. Eventually, on the same tads, a double cantilever was applied to control the overbite and intrusion during incisors’ retraction. The Biga system is an easy biomechanical strategy that ensures the three-dimensional control of treatment mechanics in class II patients.

(1) Objective: This scoping review evaluates the effects of miniscrew-assisted rapid palatal expansion (MARPE) on different regions of the upper airway in adult patients and investigates various methods of measurement. (2) Methods: The search encompassed Pubmed, Cochrane Library, Scopus and Web of Science. This review was conducted following the PRISMA_ScR guidelines, and the inclusion criteria for examined studies were chosen in accordance with the PICOS framework. (3) Results: Seven studies were included in this review, comprising four retrospective studies, one prospective and two case reports. All studies involved the use of Cone Beam Computed Tomography (CBCT) for measurements of the areas of interest. The percentage of increase in the volume of the nasal cavity varied between 31% and 9.9%, depending on the study. Volumetric variations in the nasopharynx were reported as increases between T0 (before expansion) and T1 (immediately after expansion) of 6.4%, 20.7% and 14.1%. All studies considered T0 before expansion and T1 immediately after expansion. Only one study evaluated remote follow-up to assess if the results were maintained after one year. (4) Conclusions: MARPE appears to lead to a statistically significant increase in the upper airway, especially in the nasal cavity and nasopharynx immediately after expansion. However, further prospective and retrospective trails with long-term controls are required to verify the effects of MARPE on the upper airway.

This case report of an 11-year-old subject shows the digital workflow for the management of an impacted mandibular canine using Computer-Aided Design/Computer-Aided Manufacturing (CAD/CAM) technology along with Temporary Anchorage Devices (TADs). The miniscrew insertion site was planned using software, and a surgical guide was digitally designed and 3D printed. Orthodontic traction was performed using a 3D-designed and -printed device. In a single sitting, the miniscrew was inserted and the disimpaction device was also delivered. The primary objective of recovery and the repositioning of the impacted mandibular canine in the axis with its eruptive path was achieved. The space available and the subject’s early stage of mixed dentition was considered favourable to a spontaneous eruption. This case report shows how CAD/CAM digital technology, combined with 3D printing, enables the creation of a surgical guide to position the miniscrew and the customized devices used for mandibular canine disimpaction. CAD/CAM surgical guides can help clinicians to position TADs with more accuracy and predictability, ensuring high quality bone support offering primary stability. Although orthodontic traction is the most complex therapeutic choice to implement, with the aid of CAD/CAM technology it is possible to proceed with accurate and minimally invasive orthodontic traction in order to recover a mandibular canine.

Abstract Objective To compare the three-dimensional (3-D) movement of maxillary teeth in response to three common miniscrew anchorage systems in extraction patients with maxillary dentoalveolar protrusion. Materials and methods The study employed a randomized controlled single-blinded design with three arms. Thirty extraction patients who required maximum anchorage to retract maxillary anterior teeth were included and randomly allocated into three treatment groups: space closure with direct miniscrew anchorage and low crimpable hooks (DL group), indirect miniscrew anchorage and low crimpable hooks (IL group), and direct miniscrew anchorage and high crimpable hooks (DH group). Cone beam computed tomography (CBCT) images of all included patients were obtained immediately before (T0) and after (T1) space closure. The outcomes were 3-D positional changes of maxillary central incisor, lateral incisor, canine, second premolar, and first molar. The repeated measures analysis of variance with post hoc LSD test was used to evaluate differences among groups. Results A significant intrusion (− 1.34 mm; 95% CI, − 1.60 mm, 1.08 mm) and buccal (− 6.92°; 95% CI, − 8.67°, − 5.13°) and distal (4.90°; 95% CI, 3.75°, 6.04°) inclination of the maxillary first molars were observed in the DL group, compared to the other two groups. The mesial movement (− 0.40 mm; 95% CI, − 0.83 mm, − 0.03 mm) of the maxillary first molars was found in the IL group, while the DL (0.44 mm; 95% CI, 0.15 mm, 0.73 mm) and IL (0.62 mm; 95% CI, 0.28 mm, 0.96 mm) groups exhibited distal movement. In the DH group, the lingual inclination changes of maxillary central incisor (5.04°; 95% CI, 2.82°, 7.26°) were significantly lower, which is indicative of good lingual root torque control of the maxillary anterior teeth. Conclusion Three miniscrew anchorage systems produced significantly different 3-D maxillary tooth movement. The maxillary first molars were significantly buccally and distally inclined and intruded in patients using direct miniscrew anchorages with low crimpable hooks. Direct miniscrew anchorages with high crimpable hooks could help to achieve better lingual root torque control of the maxillary incisors. Abstract Background Patients with unilateral cleft lip and palate were associated with different nasomaxillary complex from the normal population. Although the biomechanical effects of conventional rapid palatal expansion (Hyrax expansion) and bone-borne rapid palatal expansion (micro-implant-assisted expansion) in non-cleft patients have been identified by multiple studies, little is known in patients with unilateral cleft lip and palate. The purpose of this study was to investigate and compare the biomechanical effects of the conventional and bone-borne palatal expanders in a late adolescence with unilateral cleft lip and palate. Methods A cone beam CT scan of a late adolescence with unilateral cleft lip and palate was selected to construct the three-dimensional finite element models of teeth and craniofacial structures. The models of conventional and born-borne palatal expanders were established to simulate the clinical maxillary expansion. The geometric nonlinear theory was applied to evaluate the Von Mises stress distribution and displacements in craniofacial structures and teeth. Results Bone-borne palatal expander achieved more transverse movement than conventional palatal expander in the whole mount of craniofacial regions, and the maximum amount of expansion was occurred anteriorly along the alveolar ridge on cleft-side. The expanding force from born-borne palatal expander resulted in more advancement in nasomaxillary complex than it in conventional palatal expander, especially in the anterior area of the minor segment of maxilla. Stresses from the both expanders distributed in similar patterns, but larger magnitudes and ranges were generated using the bone-borne expander around the maxillary buttresses and pterygoid plates of sphenoid bone. The maximum expanding stresses from born-borne palatal expander were concentrated on palatal slope supporting minscrews, whereas those from conventional palatal expander were concentrated on the anchoring molars. In addition, the buccal tipping effect of teeth generated using the bone-borne expander was less than it using the conventional palatal expander. Conclusion Bone-borne expander generated enhanced skeletal expansion at the levels of alveolar and palate in transversal direction, where the miniscrews contributed increased expanding forces to maxillary buttresses and decreased forces to buccal alveolar. Bone-borne expanders presented a superiority in correcting the asymmetric maxilla without surgical assistant in late adolescence with unilateral cleft lip and palate.

Abstract Background Eruption disturbances of permanent molars are uncommon; however, it is important to treat them as soon as they are diagnosed. The main objective was to analyze the effectiveness of the “miniscrew-supported pole technique,” a surgically assisted orthodontic procedure to force the eruption of impacted/retained second molars (M2s) when there are indicators of complex molar inclusion. An observational prospective study was carried out during a 2-year period. Sociodemographic, clinical and low-dose scanner variables were taken at baseline (T0). Follow-up variables (T1) were the time between surgery and tooth eruption, radiographic measurements, debonding of buttons, failure rate of miniscrews and success rate of eruption. Results A total of 21 patients (mean age of 13.9 years) with 24 retained/impacted M2s were recruited; 13 molars were maxillary (54.2%) and 11 (45.8%) were mandibular. Six (25%) were impacted molars and 18 (75%) primarily retained. At T0, molar angulation was mesial in six molars (25%), distal in five molars (20.8%) and 13 molars were vertically positioned (54.2%). Infraocclusion degree was moderate in four (16.7%) molars and severe in 20 (83.3%). Only three (12.5%) third molars were removed due to lack of space. All M2s managed to erupt, achieving a success rate of 100%; however, two molars of the same patient did not achieve occlusion. The period of eruption after surgery was 126.8 (117.3) days. Anatomical radicular alteration was the only variable independently related to a longer time of treatment (p = 0.027). Conclusions The pole technique, using one mesial miniscrew and simple orthodontic mechanics, applies forces that succeed in erupting complicated retained/impacted M2s in a short period of time and with a low failure rate.

This study aimed to analyze the treatment effects of miniscrew-assisted rapid palatal expansion using dental cone-beam computed tomography (CT) in adolescents. The study group comprised 20 patients who underwent cone-beam CT before and after miniscrew-assisted rapid palatal expansion (MARPE). Paired t-test and analysis of variance were used to analyze significant differences before and after treatment. Skeletal and dentoalveolar measurements indicated highly significant differences, with these expansions increasing progressively in the downward direction. The extent of anterior midpalatal suture expansion was significantly greater than that of the posterior part. Significant expansion was observed at the nasal width and inter-zygomaticomaxillary suture distance. MARPE can expand the craniofacial skeletal structure to the uppermost nasal bone and lateral to the zygomaticomaxillary sutures. The expansion was greater in the lower maxillary part, demonstrating an overall triangular maxillary expansion. The midpalatal suture opening was greater in the anterior than in the posterior side, whereas the posterior suture opening proportion was higher than that of the conventional rapid palatal expansion previously reported. Objective: To compare the transverse dental and skeletal changes in patients treated with bone‐anchored palatal expander (bone‐borne, BB) compared to patients treated with tooth and bone‐anchored palatal expanders (tooth‐bone‐borne, TBB) using cone‐beam computer tomography (CBCT) and 3D image analysis. Methods: The sample comprised 30 patients with transverse maxillary discrepancy treated with two different types of appliances: bone‐borne (Group BB) and tooth‐bone‐borne (Group TBB) expanders. CBCT scans were acquired before (T1) and after completion of maxillary expansion (T2); the interval was 5.4 ± 3.4 and 6.2 ± 2.1 months between the T1 and the T2 scans of Group TBB (tooth‐bone‐borne) and Group BB (bone‐borne), respectively. Transverse, anteroposterior and vertical linear and angular three‐dimensional dentoskeletal changes were assessed after cranial base superimposition. Results: Both groups displayed marked transverse skeletal expansion with a greater ratio of skeletal to dental changes. Greater changes at the nasal cavity, zygoma and orbital levels were found in Group BB. A relatively parallel sutural opening in an anterior–posterior direction was observed in Group TBB; however, the Group BB presented a somewhat triangular (V‐shaped) opening of the suture that was wider anteriorly. Small downward‐forward displacements were observed in both groups. Asymmetric expansion occurred in approximately 50% of the patients in both groups. Conclusion: Greater skeletal vs dental expansion ratio and expansion of the circummaxillary regions were found in Group BB, the group in which a bone‐borne expander was used. This case report presents the non-surgical orthodontic camouflage treatment of an 18-year-old male patient with skeletal Class III asymmetry and severe anterior open bite. The anterior open bite was corrected by extrusion of the maxillary and mandibular anterior teeth and clockwise and counterclockwise rotation of the maxillary and mandibular occlusal plane, respectively, using intermaxillary Class III elastics between the maxillary posterior buccal miniscrews and mandibular canines and anterior vertical elastics between the maxillary and mandibular canines. Class III dental relationships and dental asymmetry were corrected via unilateral distalization of the mandibular dentition on the left side using a closed coil spring between the buccal shelf screw and hook. The patient's smile esthetics and dental relationship were improved with a more favorable facial profile, and facial asymmetry was slightly alleviated after orthodontic camouflage treatment. The total treatment time was 15 months. A modified wraparound retainer with a scalloped labial bow, tongue crib, and tooth positioner was used simultaneously to prevent the potential relapse. Aim: The objective of this systematic review was to offer quantitative evidence regarding the influence of surface properties on the mechanical stability of miniscrews. Materials and Methods: The comprehensive search strategy involved querying databases, namely PubMed, Web of Science, and Scopus. PRISMA guidelines were followed to determine relevant studies according to specific eligibility criteria. The final search was conducted on 30 August 2023. In this systematic review, in vivo studies published in the English language were included. Results: A total of 364 articles were viewed, and 17 of them were considered for evaluation. Two of the articles are about human studies, while the rest are about animal studies. The number of miniscrew samples ranged between 18 and 144 (totaling 1097 with a mean of 64.52). Among the surface modifications in the articles, the sandblasting with large grit and acid etching (SLA) method was most frequently applied, followed by acid etching in second place. The control groups’ (machined surface) maximum removal torque (MRT) values varied from 2.05 to 50.50 Ncm, while maximum insertion torque (MIT) values varied from 7.23 to 19.25 Ncm. Conclusions: The development of novel applications to improve the surface properties and survival rates of miniscrews is ongoing. In future studies, emerging surface modifications should be evaluated clinically, taking into account their cost and associated harm to the environment.

Abstract Background The introduction in the orthodontic field of the digital workflow for guided insertion of palatal TADs and the development of the 1-visit protocol led to the reduction of chair time and the possibility of complete customization of designs and materials. Conversely, the reduction of operative steps implicates a lower tolerance of deviations between the planned and the actual position of the miniscrews, particularly when the orthodontic device is fixed on 4 palatal TADs or has a rigid structure. This study aims to analyze the influence of each step of the digital workflow on the deviation of the miniscrews’ axis of insertion in a bicortical sample. The null hypothesis is that there are no significant differences in the deviations among the operative steps. Methods 33 subjects were selected for insertion of bicortical palatal miniscrews with a 1-visit protocol. Digital files were collected at the three stages of the workflow (i.e., digital planning, laboratory prototype, post-insertion impression). A 3D software analysis was performed on a total of 64 miniscrews. After automatic shape recognition of the guiding holes of the digital plan and the scanbodies of the laboratory prototype and post-insertion impression as geometric cylinders, their three-dimensional longitudinal axis was traced and the deviation among them was calculated. Abstract Background The goal of this systematic review was to assess the available evidence regarding the skeletal and dentoalveolar effects of bi-maxillary skeletal anchorage devices (BMSADs) used in treating growing class II malocclusion patients. Methods A comprehensive search was conducted on PubMed, Scopus, Science Direct, Web of Science, Cochrane, and LILACS up to November 2021, which was augmented by a manual search. The studies included were clinical trials (RCTs) and/or follow-up observational studies (retrospective and prospective). The outcomes of interest were the skeletal, dentoalveolar, and occlusal treatment-induced changes obtained from pre- and post-cephalometric measurements. The risks of bias of the included studies were assessed using an assessment tool from previous publications. Results Out of 742 screened articles, only 4 were eligible and thus included in the qualitative synthesis. They showed a moderate overall risk of bias. The results are presented as mean changes in both the study and control groups. All studies reported retrusion of the maxillary base and advancement of the mandible (meaning reduced ANB angle). Three of the included studies reported an increase in the vertical jaw relation, which was contrary to what the fourth study reported. Three studies reported an increase in the maxillary incisors’ inclination or position, while one study reported their retroclination. Proclination of the mandibular incisors happened in two studies, whereas the other two studies reported retroclination. The overjet was reduced in all included studies. Conclusion Apart from the protrusive effects on the mandible, retrusive effects on the maxilla, and the consequent reduction of the overjet, BMSADs results in inconsistent skeletal and dentoalveolar effects. However, the current evidence is limited due to the variability in the biomechanics of the intermaxillary components, type of anchorage, and comparable groups in the included studies. Further RCTs with more standardized methodologies are highly encouraged. Clinical relevance BMSADs (using miniscrews or miniplates on both jaws) induces more skeletal than dentoalveolar effects. However, this must be practiced with caution, based on the benefit to risk (surgical insertion) ratio, and the limited evidence available in hand so far. BackgroundMiniscrew-assisted rapid palatal expansion (MARPE) has been adopted in recent years to expand the maxilla in late adolescence and adult patients. Maxillary Skeletal Expander (MSE) is a device that exploits the principles of skeletal anchorage to transmit the expansion force directly to the maxillary bony structures and is characterized by the miniscrews' engagement of the palatal and nasal cortical bone layers. In the literature, it has been reported that the zygomatic buttress is a major constraint that hampers the lateral movement of maxilla, since maxilla is located medially to the zygomatic arches. The objective of the present study is to analyze the changes in the zygomatic bone, maxillary bone, and zygomatic arches and to localize the center of rotation for the zygomaticomaxillary complex in the horizontal plane after treatment with MSE, using high-resolution cone-beam computed tomography (CBCT) images.MethodsFifteen subjects with a mean age of 17.2 (± 4.2) years were treated with MSE. CBCT records were taken before and after miniscrew-assisted maxillary expansion; three linear and four angular parameters were identified in the axial zygomatic section (AZS) and were compared from pre-treatment to post-treatment using the Wilcoxon signed rank test.ResultsAnterior inter-maxillary distance increased by 2.8 mm, posterior inter-zygomatic distance by 2.4 mm, angle of the zygomatic process of the temporal bone by 1.7° and 2.1° (right and left side) (P  0.05).ConclusionsIn the horizontal plane, the maxillary and zygomatic bones and the whole zygomatic arch were significantly displaced in a lateral direction after treatment with MSE. The center of rotation for the zygomaticomaxillary complex was located near the proximal portion of the zygomatic process of the temporal bone, more posteriorly and more laterally than what has been reported in the literature for tooth-borne expanders. Bone bending takes place in the zygomatic process of the temporal bone during miniscrew-supported maxillary expansion.
