4. The Current State of Antiracism Curricula in Undergraduate and Graduate Medical Education: A Qualitative Study of US Academic Health Centers

Abstract

PurposeWe undertook a study to evaluate the current state of pedagogy on antiracism, including barriers to implementation and strengths of existing curricula, in undergraduate medical education (UME) and graduate medical education (GME) programs in US academic health centers.MethodsWe conducted a cross-sectional study with an exploratory qualitative approach using semistructured interviews. Participants were leaders of UME and GME programs at 5 institutions participating in the Academic Units for Primary Care Training and Enhancement program and 6 affiliated sites from November 2021 to April 2022.ResultsA total of 29 program leaders from the 11 academic health centers participated in this study. Three participants from 2 institutions reported the implementation of robust, intentional, and longitudinal antiracism curricula. Nine participants from 7 institutions described race and antiracism-related topics integrated into health equity curricula. Only 9 participants reported having "adequately trained" faculty. Participants mentioned individual, systemic, and structural barriers to implementing antiracism-related training in medical education such as institutional inertia and insufficient resources. Fear related to introducing an antiracism curriculum and undervaluing of this curriculum relative to other content were identified. Through learners and faculty feedback, antiracism content was evaluated and included in UME and GME curricula. Most participants identified learners as a stronger voice for transformation than faculty; antiracism content was mainly included in health equity curricula.ConclusionsInclusion of antiracism in medical education requires intentional training, focused institutional policies, enhanced foundational awareness of the impact of racism on patients and communities, and changes at the level of institutions and accreditation bodies.

Abstract

Searching for neutrinoless double beta decays ($\beta\beta0\nu$) is the only practical way to establish if the neutrinos are their own antiparticles. Due to the smallness of neutrino masses, the lifetime of $\beta\beta0\nu$ is expected to be at least ten orders of magnitude smaller than the noise associated with the natural radioactive chains. A positive identification of $\beta\beta0\nu$ decays requires, ultimately, finding a signal that cannot be mimicked by radioactive backgrounds. This signal could be the observation of the daughter atom in the decay, since no known background processes induce a Z+2 transformation. In particular, the $\beta\beta0\nu$ decay of Xe-136 could be established by detecting the doubly ionised daughter atom, Ba$^{2+}$. Such a detection could be achieved via a sensor made of a monolayer of molecular indicators. The Ba$^{2+}$ would be captured by one of the molecules in the sensor, and the presence of the single chelated indicator would be subsequently revealed by a strong fluorescent response from repeated interrogation with a laser system. Here we describe a fluorescent bicolor indicator that binds strongly to Ba$^{2+}$ and shines very brightly, shifting its emission colour from green to blue when chelated in dry medium, thus allowing the unambiguous identification of single barium atoms in the sensor, and permitting a positive identification of the $\beta\beta0\nu$ decay of Xe-136 in a gas chamber, that could led to a background-free experiment.

18. Multifunctional gallium doped bioactive glasses: a targeted delivery for antineoplastic agents and tissue repair against osteosarcoma

Abstract

Osteosarcoma (OS) is the mostly commonly occurring primary bone cancer. Despite comprehensive treatment programs including neoadjuvant chemotherapy and tumour resection, survival rates have not improved significantly since the 1970s. Survival rates are dramatically reduced for patients who suffer a local recurrence. Furthermore, primary bone cancer patients are at increased risk of bone fractures. Consequently, there is an urgent need for alternative treatment options. In this paper we report the development of novel gallium doped bioactive glass that selectively kill bone cancer cells whilst simultaneously stimulating new bone growth. Here we show, using a combination of 3-(4.5-dimethylthiazol-2-yl)-2.5-diphenyltetrazolium bromide, LIVE/DEAD assays and image analysis, that bioactive glasses containing gallium oxide are highly toxic and reduce both the proliferation and migration of bone cancer cells (Saos-2) in a dose dependant manner. Glasses containing 5 mol% gallium oxide reduced the viability of OS cells by 99% without being cytotoxic to the non-cancerous normal human osteoblasts (NHOst) control cells. Furthermore, Fourier transform infrared and energy-dispersive x-ray spectroscopy results confirmed the formation of an amorphous calcium phosphate/hydroxyapatite like layer on the surface of the bioactive glass particulates, after 7 d incubating in simulated body fluid, indicating the early stages of bone formation. These materials show significant potential for use in bone cancer applications as part of a multimodal treatment.

Abstract

In this paper, we examine the early predictability of the market's directional movement using intraday high‐frequency data (695,764 observations) from an stock index (Ibex 35 Index) to provide, either private or institutional investors, an early warning system based on an “early indicator” of the financial market fluctuations with an optimal combination of the two more relevant variables for this strategy, accuracy, and earliness. A novel supervised machine learning early classification technique (Artificial Intelligence) has been applied, for the first time, to the high‐frequency time series of both price and certain technical indicators.The results obtained allow us to assert that the intraday movement of the Ibex 35 can be predicted with acceptable levels of accuracy 24 min after the start of the session and to establish certain informative intraday hourly patterns. Abstract

This paper presents a review of concepts related to wildfire risk assessment, including the determination of fire ignition and propagation (fire danger), the extent to which fire may spatially overlap with valued assets (exposure), and the potential losses and resilience to those losses (vulnerability). This is followed by a brief discussion of how these concepts can be integrated and connected to mitigation and adaptation efforts. We then review operational fire risk systems in place in various parts of the world. Finally, we propose an integrated fire risk system being developed under the FirEUrisk European project, as an example of how the different risk components (including danger, exposure and vulnerability) can be generated and combined into synthetic risk indices to provide a more comprehensive wildfire risk assessment, but also to consider where and on what variables reduction efforts should be stressed and to envisage policies to be better adapted to future fire regimes. Climate and socio-economic changes entail that wildfires are becoming even more a critical environmental hazard; extreme fires are observed in many areas of the world that regularly experience fire, yet fire activity is also increasing in areas where wildfires were previously rare. To mitigate the negative impacts of fire, those responsible for managing risk must leverage the information available through the risk assessment process, along with an improved understanding on how the various components of risk can be targeted to improve and optimize the many strategies for mitigation and adaptation to an increasing fire risk.
