Abstract

Patients with tuberous sclerosis complex (TSC) develop multi-organ disease manifestations, with kidney angiomyolipomas (AML) and cysts being one of the most common and deadly. Early and regular AML/cyst detection and monitoring are vital to lower TSC patient morbidity and mortality. However, the current standard of care involves imaging-based methods that are not designed for rapid screening, posing challenges for early detection. To identify potential diagnostic screening biomarkers of AML/cysts, we performed global untargeted metabolomics in blood samples from 283 kidney AML/cyst-positive or -negative TSC patients using mass spectrometry. We identified 7 highly sensitive chemical features, including octanoic acid, that predict kidney AML/cysts in TSC patients. Patients with elevated octanoic acid have lower levels of very long-chain fatty acids (VLCFAs), suggesting that dysregulated peroxisome activity leads to overproduction of octanoic acid via VLCFA oxidation. These data highlight AML/cysts blood biomarkers for TSC patients and offers valuable metabolic insights into the disease.

Abstract

We have studied multi-Dirac/Weyl systems with arbitrary topological charge n in the presence of a lattice of local magnetic moments. To do so, we propose a multi-Dirac/Weyl Kondo lattice model which is analyzed through a mean-field approach appropriate to the paramagnetic phase. We study both the broken time-reversal and the broken inversion-symmetry Weyl cases. The multi- Dirac and broken-time reversal multi-Weyl cases have similar behavior, which is in contrast to the broken-parity case. For the former, low-energy particle-hole symmetry leads to the emergence of a critical coupling constant below which there is no Kondo quenching, reminiscent of the pseudogap Kondo impurity problem. Away from particle-hole symmetry, there is always Kondo quenching. For the broken inversion symmetry, there is no critical coupling. Depending on the conduction electron filling, Kondo insulator, heavy fermion metal or semimetal phases can be realized. In the last two cases, quasiparticle renormalizations can differ widely between opposite chirality sectors, with characteristic dependences on microscopic parameters that could in principle be detected experimentally., Comment: 15 pages, 12 figures

3. Signatures of infinite randomness in transport properties of disordered spin chains

Abstract

We study the spin transport properties of some disordered spin chains with a special focus on the distribution of the frequency-dependent spin conductivity. In the cases of interest here, the systems are governed by an effectively infinite disorder at low energies. A hallmark of this behavior is the wide discrepancy between the average and the typical values of some physical quantities, which are described by extremely broad distributions. We show that such is also the case of the spin conductivity, whose average value is metallic but whose typical one, the physically relevant quantity, is insulating. This solves the apparent contradiction between the prediction of a spin metallic phase of the spin-1/2 disordered XX chain and its known localized behavior (after a mapping to free fermions). Our results are based on analytical and numerical implementations of a strong-disorder renormalization group as well as exact diagonalization studies. We present our analyses in very general terms, valid for systems of any spin $S$ value, but the cases of $S=1/2$ and 1 are studied in greater detail., Comment: 20 pages including Appendices, 9 figures

4. The BNS invariants of the braid groups and pure braid groups of some surfaces

Abstract

We compute and explicitly describe the Bieri-Neumann-Strebel invariants $\Sigma^1$ for the full and pure braid groups of the sphere $\mathbb{S}^2$, the real projective plane $\mathbb{R}P^2$ and specially the torus $\mathbb{T}$ and the Klein bottle $\mathbb{K}$. In order to do this for $M=\mathbb T$ or $M=\mathbb K$, and $n \geq 2$, we use the $n^{th}$-configuration space of $M$ to show that the action by homeomorphisms of the group $Out(P_n(M))$ on the character sphere $S(P_n(M))$ contains certain permutation of coordinates, under which $\Sigma^1(P_n(\mathbb T))^c$ and $\Sigma^1(P_n(\mathbb K))^c$ are invariant. Furthermore, $\Sigma^1(P_n(\mathbb T))^c$ and $\Sigma^1(P_n(\mathbb{S}^2))^c$ (the latter with $n \geq 5$) are finite unions of pairwise disjoint circles, and $\Sigma^1(P_n(\mathbb K))^c$ is finite. This last fact implies that there is a normal finite index subgroup $H \leq Aut(P_n(\mathbb K))$ such that the Reidemeister number $R(\varphi)$ is infinite for every $\varphi \in H$.

6. Clear aligner therapy practices among orthodontists practicing in Canada

Abstract

Abstract Background The acceptability and preference for clear aligner therapy (CAT) has been increasing among orthodontists, but there is still a lack of consensus regarding CAT best practices. Consequently, this study aimed to investigate CAT practices among orthodontists practicing in Canada. Methods The survey was conducted among orthodontists practicing in Canada using a modified previously published survey. Sixty orthodontists participated (6.1% response rate). It consisted of 11 sections with open and closed questions related to demographic information and particularities about using or not using CAT. The survey responses were exported from REDCap to a Microsoft Excel (Microsoft, Redmond, Wash) spreadsheet, then statistically analyzed using SPSS software (SPSS for Windows, version 21.0; IBM Inc., Armonk, NY, USA). The comments were categorized under themes and subthemes. Data were organized in descriptive statistics, expressing frequencies and percentages. Results Almost 30% of the orthodontist’s annual caseload was treated with CAT, most frequently prescribed to adult patients. Case complexity and patient cooperation were the factors that most influenced the decision to prescribe CAT. Almost half of orthodontists reported sometimes combining CAT with adjunctive fixed appliances. Conclusions Most orthodontists prescribe CAT, and its use is based on the malocclusion’s complexity. Orthodontists who do not prescribe CAT believe that fixed appliance therapy has superior treatment outcomes.

Abstract

BackgroundWorking memory involves the temporary storage and manipulation of information and is frequently an area of challenge for individuals with Down syndrome (DS). Despite the potential benefits of intervention, laboratory assessments of working memory that could capture intervention effects have not undergone rigorous evaluation for use with young children with DS. It is critical to evaluate assessments of working memory in young children with DS to ensure the reliable and accurate measurement of performance.AimThis study evaluated an adapted laboratory measure of working memory for young children with DS 2-8 years old.MethodA self-ordered pointing task, the Garage Game, was administered to 78 children with DS (mean = 5.17 years; SD = 1.49). Adaptations were made to the task to minimize potential DS phenotype-related language and motor confounds.ResultsResults indicate that the measure is feasible, scalable, and developmentally sensitive, with minimal floor and practice effects for this population within this chronological and developmental age range.ConclusionThese findings demonstrate that the Garage Game is promising for use in studies of early working memory and treatment trials that aim to support the development of this critical dimension of executive functioning for children with DS.

14. The Borsuk-Ulam theorem for n-valued maps between surfaces

Abstract

In this work we analysed the validity of a type of Borsuk-Ulam theorem for multimaps between surfaces. We developed an algebraic technique involving braid groups to study this problem for $n$-valued maps. As a first application we described when the Borsuk-Ulam theorem holds for splits and non-splits multimaps $\phi \colon X \multimap Y$ in the following two cases: $(i)$ $X$ is the $2$-sphere eqquiped with the antipodal involution and $Y$ is either a closed surface or the Euclidean plane; $(ii)$ $X$ is a closed surface different of the $2$-sphere eqquiped with a free involution $\tau$ and $Y$ is the Euclidean plane. The results are exhaustive and in the case $(ii)$ are described in terms of an algebraic condition involving the first integral homology group of the orbit space $X / \tau$.

15. Dietary intake, obesity, and metabolic risk factors among children and adolescents in the SEACO-CH20 cross-sectional study

Abstract

Abstract We investigated the association between dietary intake and metabolic risk factors in children and adolescents within a semi-rural Malaysian community. Using an interviewer-led questionnaire, we surveyed 623 participants aged 7–18 from the South East Asia Community Observatory (SEACO). Anthropometric and blood pressure data were collected from all participants, while a subset (n = 162) provided blood samples for biomarker analysis, including fasting blood glucose (FBG), total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C). Metabolic syndrome was determined using the International Diabetes Federation’s Definition of Metabolic Syndrome in Children and Adolescents. Most participants were Malay (66.8%), with a median household income of MYR1,500 and a balanced sex distribution. Cereals, processed foods, beverages, fruits, and vegetables were commonly consumed. Obesity and abdominal obesity were prevalent, affecting more than a third of participants. Adherence to dietary recommendations was generally poor (ranging from 19.9 to 58.1%) and varied across age, sex, and ethnicity. Notably, some food groups displayed unexpected associations with health markers; for instance, fruit consumption was linked to abdominal obesity in children (abdominal obesity vs. normal: 2.4 servings/day vs. 1.6 servings/day). These findings emphasise the necessity of longitudinal studies to explore the complex relationship between diet and long-term health outcomes, including cardiometabolic diseases, while acknowledging the unique challenges posed by the COVID-19 pandemic on data collection and analysis.

19. A Theory and Evidence-Informed e-Cycling Intervention for Individuals Diagnosed With Cancer: Development Study

Abstract

BackgroundPhysical activity engagement following a cancer diagnosis is positively associated with survival, reduced risk of disease recurrence, and reduced cancer-specific and all-cause mortality. However, rates of physical activity engagement are low among individuals diagnosed with and being treated for breast cancer or prostate cancer. ObjectiveThe purpose of this study was to describe the systematic process of developing an e-cycling intervention aimed at increasing physical activity among individuals living with prostate cancer or breast cancer and outline the key components to be implemented. MethodsThe Medical Research Council guidance for developing complex interventions and the Behaviour Change Wheel were used to guide intervention development. Information was gathered from the literature and through discussions with end users to understand factors influencing e-cycling. These factors were mapped onto the Theoretical Domains Framework to identify potential mechanisms of action. Behavior change techniques were selected from theory and evidence to develop intervention content. Interested parties, including cycling instructors, end users, and behavior change experts, reviewed and refined the intervention. ResultsAnticipated barriers and facilitators to e-cycling engagement were mapped onto 11 of the 14 domains of the Theoretical Domains Framework. A total of 23 behavior change techniques were selected to target these domains over 4 one-to-one e-cycling sessions delivered by trained cycling instructors in the community. Cycling instructors were provided a 3-hour classroom training session on delivering the intervention and a 3-hour practical session with feedback. The outcome of this work is a theory and evidence-informed intervention aimed at promoting e-cycling behavior among individuals being treated for breast cancer or prostate cancer, which is currently being implemented and evaluated. ConclusionsTransparent intervention development and reporting of content is important for comprehensively examining intervention implementation. The implementation of this intervention package is currently being evaluated in a pilot randomized controlled trial. If the intervention is found to be effective and the content and delivery are acceptable, this intervention will form a basis for the development of e-cycling interventions in other survivors of cancer. Abstract

Summary: Background: Mucositis is a common and highly impactful side effect of conventional and emerging cancer therapy and thus the subject of intense investigation. Although common practice, mucositis assessment is heterogeneously adopted and poorly guided, impacting evidence synthesis and translation. The Multinational Association of Supportive Care in Cancer (MASCC) Mucositis Study Group (MSG) therefore aimed to establish expert recommendations for how existing mucositis assessment tools should be used, in clinical care and trials contexts, to improve the consistency of mucositis assessment. Methods: This study was conducted over two stages (January 2022–July 2023). The first phase involved a survey to MASCC-MSG members (January 2022–May 2022), capturing current practices, challenges and preferences. These then informed the second phase, in which a set of initial recommendations were prepared and refined using the Delphi method (February 2023–May 2023). Consensus was defined as agreement on a parameter by >80% of respondents. Findings: Seventy-two MASCC-MSG members completed the first phase of the study (37 females, 34 males, mainly oral care specialists). High variability was noted in the use of mucositis assessment tools, with a high reliance on clinician assessment compared to patient reported outcome measures (PROMs, 47% vs 3%, 37% used a combination). The World Health Organization (WHO) and Common Terminology Criteria for Adverse Events (CTCAE) scales were most commonly used to assess mucositis across multiple settings. Initial recommendations were reviewed by experienced MSG members and following two rounds of Delphi survey consensus was achieved in 91 of 100 recommendations. For example, in patients receiving chemotherapy, the recommended tool for clinician assessment in clinical practice is WHO for oral mucositis (89.5% consensus), and WHO or CTCAE for gastrointestinal mucositis (85.7% consensus). The recommended PROM in clinical trials is OMD/WQ for oral mucositis (93.3% consensus), and PRO-CTCAE for gastrointestinal mucositis (83.3% consensus). Interpretation: These new recommendations provide much needed guidance on mucositis assessment and may be applied in both clinical practice and research to streamline comparison and synthesis of global data sets, thus accelerating translation of new knowledge into clinical practice. Abstract

Abstract Third molar development is used for dental age estimation when all the other teeth are fully mature. In most medicolegal facilities, dental age estimation is an operator-dependent procedure. During the examination of unaccompanied and undocumented minors, this procedure may lead to binary decisions around age thresholds of legal interest, namely the ages of 14, 16 and 18 years. This study aimed to test the performance of artificial intelligence to classify individuals below and above the legal age thresholds of 14, 16 and 18 years using third molar development. The sample consisted of 11,640 panoramic radiographs (9680 used for training and 1960 used for validation) of males (n = 5400) and females (n = 6240) between 6 and 22.9 years. Computer-based image annotation was performed with V7 software (V7labs, London, UK). The region of interest was the mandibular left third molar (T38) outlined with a semi-automated contour. DenseNet121 was the Convolutional Neural Network (CNN) of choice and was used with Transfer Learning. After Receiver-operating characteristic curves, the area under the curve (AUC) was 0.87 and 0.86 to classify males and females below and above the age of 14, respectively. For the age threshold of 16, the AUC values were 0.88 (males) and 0.83 (females), while for the age of 18, AUC were 0.94 (males) and 0.83 (females). Specificity rates were always between 0.80 and 0.92. Artificial intelligence was able to classify male and females below and above the legal age thresholds of 14, 16 and 18 years with high accuracy.

It has been verified that the originally proposed action violates the unitarity consistency condition in the gauge field sector by exhibiting negative norm states in the spectrum. Summary: Continuous assessment of the impact of SARS-CoV-2 on the host at the cell-type level is crucial for understanding key mechanisms involved in host defense responses to viral infection. We investigated host response to ancestral-strain and Alpha-variant SARS-CoV-2 infections within air-liquid-interface human nasal epithelial cells from younger adults (26–32 Y) and older children (12–14 Y) using single-cell RNA-sequencing. Ciliated and secretory-ciliated cells formed the majority of highly infected cell-types, with the latter derived from ciliated lineages. Strong innate immune responses were observed across lowly infected and uninfected bystander cells and heightened in Alpha-infection. Alpha highly infected cells showed increased expression of protein-refolding genes compared with ancestral-strain-infected cells in children. Furthermore, oxidative phosphorylation-related genes were down-regulated in bystander cells versus infected and mock-control cells, underscoring the importance of these biological functions for viral replication. Overall, this study highlights the complexity of cell-type-, age- and viral strain-dependent host epithelial responses to SARS-CoV-2.

30. Mott domain walls: a (strongly) non-Fermi liquid state of matter

Abstract

Most Mott systems display a low-temperature phase coexistence region around the metal-insulator transition. The domain walls separating the respective phases have very recently been observed both in simulations and in experiments, displaying unusual properties. First, they often cover a significant volume fraction, thus cannot be neglected. Second, they neither resemble a typical metal nor a standard insulator, displaying unfamiliar temperature dependence of (local) transport properties. Here we take a closer look at such domain wall matter by examining an appropriate unstable solution of the Hubbard model. We show that transport in this regime is dominated by the emergence of "resilient quasiparticles" displaying strong non-Fermi liquid features, reflecting the quantum-critical fluctuations in the vicinity of the Mott point.

If $M$ is different from the $2$-sphere $\mathbb{S}^2$, we prove that $B_n(M)/\Gamma_2(P_n(M))$ is isomorphic rho $P_n(M)/\Gamma_2(P_n(M)) \rtimes_{\varphi} S_n$, and that $B_n(M)/\Gamma_2(P_n(M))$ is a crystallographic group if and only if $M$ is orientable. If $M$ is orientable, we prove a number of results regarding the structure of $B_n(M)/\Gamma_2(P_n(M))$. We characterise the finite-order elements of this group, and we determine the conjugacy classes of these elements. We also show that there is a single conjugacy class of finite subgroups of $B_n(M)/\Gamma_2(P_n(M))$ isomorphic either to $S_n$ or to certain Frobenius groups. We prove that crystallographic groups whose image by the projection $B_n(M)/\Gamma_2(P_n(M))\to S_n$ is a Frobenius group are not Bieberbach groups. Finally, we construct a family of Bieberbach subgroups $\tilde{G}_{n,g}$ of $B_n(M)/\Gamma_2(P_n(M))$ of dimension $2ng$ and whose holonomy group is the finite cyclic group of order $n$, and if $\mathcal{X}_{n,g}$ is a flat manifold whose fundamental group is $\tilde{G}_{n,g}$, we prove that it is an orientable K\"ahler manifold that admits Anosov diffeomorphisms.

Abstract

BackgroundQuantifying movement behaviours over 24-hours enables the combined effects of and inter-relations between sleep, sedentary time and physical activity (PA) to be understood. This is the first study describing 24-hour movement behaviours in school-aged children and adolescents in South-East Asia. Further aims were to investigate between-participant differences in movement behaviours by demographic characteristics and timing of data collection during Ramadan and COVID-19 restrictions.MethodsData came from the South-East Asia Community Observatory health surveillance cohort, 2021-2022. Children aged 7-18 years within selected households in Segamat, Malaysia wore an Axivity AX6 accelerometer on their wrist for 24 hours/day over 7 days, completed the PAQ-C questionnaire, and demographic information was obtained. Accelerometer data was processed using GGIR to determine time spent asleep, inactive, in light-intensity PA (LPA) and moderate-to-vigorous PA (MVPA). Differences in accelerometer-measured PA by demographic characteristics (sex, age, ethnicity, socioeconomic group) were explored using univariate linear regression. Differences between data collected during vs outside Ramadan or during vs after COVID-19 restrictions, were investigated through univariate and multiple linear regressions, adjusted for age, sex and ethnicity.ResultsThe 491 participants providing accelerometer data spent 8.2 (95% confidence interval (CI) = 7.9-8.4) hours/day asleep, 12.4 (95% CI = 12.2-12.7) hours/day inactive, 2.8 (95% CI = 2.7-2.9) hours/day in LPA, and 33.0 (95% CI = 31.0-35.1) minutes/day in MVPA. Greater PA and less time inactive were observed in boys vs girls, children vs adolescents, Indian and Chinese vs Malay children and higher income vs lower income households. Data collection during Ramadan or during COVID-19 restrictions were not associated with MVPA engagement after adjustment for demographic characteristics.ConclusionsDemographic characteristics remained the strongest correlates of accelerometer-measured 24-hour movement behaviours in Malaysian children and adolescents. Future studies should seek to understand why predominantly girls, adolescents and children from Malay ethnicities have particularly low movement behaviours within Malaysia.

Abstract

Abstract Background Many families and individuals do not meet criteria for a known hereditary cancer syndrome but display unusual clusters of cancers. These families may carry pathogenic variants in cancer predisposition genes and be at higher risk for developing cancer. Methods This multi-centre prospective study recruited 195 cancer-affected participants suspected to have a hereditary cancer syndrome for whom previous clinical targeted genetic testing was either not informative or not available. To identify pathogenic disease-causing variants explaining participant presentation, germline whole-genome sequencing (WGS) and a comprehensive cancer virtual gene panel analysis were undertaken. Results Pathogenic variants consistent with the presenting cancer(s) were identified in 5.1% (10/195) of participants and pathogenic variants considered secondary findings with potential risk management implications were identified in another 9.7% (19/195) of participants. Health economic analysis estimated the marginal cost per case with an actionable variant was significantly lower for upfront WGS with virtual panel ($8744AUD) compared to standard testing followed by WGS ($24,894AUD). Financial analysis suggests that national adoption of diagnostic WGS testing would require a ninefold increase in government annual expenditure compared to conventional testing. Conclusions These findings make a case for replacing conventional testing with WGS to deliver clinically important benefits for cancer patients and families. The uptake of such an approach will depend on the perspectives of different payers on affordability.

Abstract

Many Mott systems feature a first-order metal-insulator transition at finite temperatures, with an associated phase coexistence region displaying inhomogeneities and local phase separation. Here one typically finds "bubbles" or domains of the respective phases, which are separated by surprisingly thick, or fat, domain walls, as revealed both by imaging experiments and recent theoretical modeling. To gain insight into this unexpected behavior, we perform a systematic model study of the structure of such metal-insulator domain walls around the Mott point, within the dynamical mean-field theory framework. Our study reveals that a mechanism producing such "fat" domain walls can be traced to strong magnetic frustration, which is expected to be a robust feature of "spin-liquid" Mott systems.

Abstract

Quantum parity conservation is verified at all orders in perturbation theory for a massless parity-even $U(1)\times U(1)$ planar quantum electrodynamics (QED$_3$) model. The presence of two massless fermions requires the Lowenstein-Zimmermann (LZ) subtraction scheme, in the framework of the Bogoliubov-Parasiuk-Hepp-Zimmermann-Lowenstein (BPHZL) renormalization method, in order to subtract the infrared divergences induced by the ultraviolet subtractions at 1- and 2-loops, however thanks to the superrenormalizability of the model the ultraviolet divergences are bounded up to 2-loops. Finally, it is proved that the BPHZL renormalization method preserves parity for the model taken into consideration, contrary to what happens to the ordinary massless parity-even $U(1)$ QED$_3$., Comment: 10 pages, 2 figures

Abstract

Abstract Background In 2020, 1.4 and 2.3 million new cases of prostate cancer and breast cancer respectively were diagnosed globally. In the UK, prostate cancer is the most common male cancer, while breast cancer is the most common female cancer. Engaging in physical activity (PA) is a key component of treatment. However, rates of PA are low in these clinical populations. This paper describes the protocol of CRANK-P and CRANK-B, two pilot randomised controlled trials, involving an e-cycling intervention aimed at increasing PA in individuals with prostate cancer or breast cancer respectively. Methods These two trials are single-centre, stratified, parallel-group, two-arm randomised waitlist-controlled pilot trials in which forty individuals with prostate cancer (CRANK-P) and forty individuals with breast cancer (CRANK-B) will be randomly assigned, in a 1:1 allocation ratio, to an e-cycling intervention or waitlist control. The intervention consists of e-bike training with a certified cycle instructor, followed by the provision of an e-bike for 12 weeks. Following the intervention period, participants in the e-bike condition will be directed to community-based initiatives through which they can access an e-bike. Data will be collected at baseline (T0), immediately post intervention (T1) and at 3-month follow-up (T2). In addition, in the intervention group, data will be collected during the intervention and follow-up periods. Quantitative and qualitative methods will be used. The primary objectives are to determine effective recruitment strategies, establish recruitment and consent rates, adherence and retention in the study, and determine the feasibility and acceptability of the study procedures and intervention. The potential impact of the intervention on clinical, physiological and behavioural outcomes will be assessed to examine intervention promise. Data analyses will be descriptive. Discussion The findings from these trials will provide information on trial feasibility and highlight the potential of e-cycling as a strategy to positively impact the health and behaviour of individuals with prostate cancer and breast cancer.
