Abstract

ObjectiveTo evaluate longitudinal endothelial cell characteristics of children with posterior polymorphous corneal dystrophy (PPCD).MethodsIn this prospective case-control study, children with PPCD were followed with slit-lamp photography and non-contact specular microscopy. Patient's eyes were subdivided according to the clinical subtypes of PPCD (vesicular, band, diffuse, and unaffected) and the number of lesions present on the posterior corneal surface. Findings were then compared with age-matched controls.ResultsThirty eyes of 15 patients with PPCD with a mean age 10.5 ± 3.1 years were analysed. Mean follow-up was 3.0 ± 1.0 years. PPCD morphology was vesicular in 40%, diffuse in 37%, band type in 10% and 13% had no detectable lesions despite contralateral involvement. Fourteen eyes (47%) had ≥5 endothelial lesions. The accuracy of 16 IOL power calculation methods was evaluated: Barrett Universal II (BUII), Castrop, EVO 2.0, Haigis, Hill-RBF 3.0, Hoffer Q, Hoffer QST, Holladay 1, Kane, LSF AI, Naeser 2, Pearl-DGS, SRK/T, T2, VRF, and VRF-G. The non-optimized (ULIB/IOLcon) and optimized constants were used for IOL power calculation. The mean prediction error (PE), Performance Index (FPI), and all descriptive statistics were calculated. Results: Ninety-seven eyes of 97 pediatric patients aged 13.2 (IQR 11.2– 17.1) were included. No statistically significant difference (HS-test) was observed (p > 0.818) except for the Hoffer Q, and Naeser 2 (P = 0.014). With optimized lens constants, the best FPI indices were obtained by Hoffer Q (0.256) and VRF-G (0.251) formulas, followed by Hill-RBF 3.0 and BUII, with an index of 0.248. The highest FPI indices with non-optimized constants showed SRK/T and T2 formulas (0.246 and 0.245, respectively), followed by VRF-G and Holladay 1, with an index of 0.244. The best median absolute error values (MedAE) were achieved by Hoffer Q (0.50 D), VRF-G (0.53 D), and Hill-RBF 3.0 (0.54 D), all P ≥ 0.074. Conclusion: Our results place the Hoffer Q, VRF-G, Hill-RBF 3.0, and BUII formulas as more accurate predictors of postoperative refraction in pediatric cataract surgery. [ABSTRACT FROM AUTHOR]

33. Models to investigate the role of anti-transforming growth factor beta antibodies in lens and corneal wound healing

Abstract

The eye is a unique structure and needs to maintain transparency along its visual axis for good quality vision. The lens and cornea are vital transparent structures in the visual axis and are susceptible to scarring following the trauma of surgery. The aim of this thesis was to establish models to observe wound healing in the lens and cornea and investigate the role of transforming growth factor beta (TGF\beta) and its antibodies on the scarring process. In this thesis a system of high resolution imaging in the rabbit was established visualising down to the level of individual lens epithelial cells (LECs) in-vivo. Four phases of posterior capsular opacification (PCO) were characterised using the model. An initial proliferation of LECs in the first two weeks is followed by a week of regression. A plateau phase of two weeks represents no major changes on the posterior capsule but ongoing equatorial proliferation. Finally a wave of proliferating LECs marched in from the periphery to cover the posterior capsule. This process was consistent and could be quantified and analysed for use in translational research. The effect of anti TGF-\beta2 antibodies delivered at the time of surgery was to increase the proliferation of lens epithelial cells in the initial period with no toxic effects. Also in this thesis a model of corneal organ culture was established to allow quantification of epithelial and stromal wound healing. Scanning laser confocal microscopy was used to quantify stromal changes which correlated well with histology scores for scarring indicating the changes seen on confocal microscopy represent a real change in corneal stroma. Treatment with TGF\beta1 antibody did not promote or inhibit epithelial healing. However, it significantly reduced stromal scarring after epithelial scrape wound. The trend towards reduced stromal scarring after lamellar keratectomy did not reach significance after TGF\beta1 antibody treatment The models developed and validated in this thesis may be useful in evaluating new treatment modalities in translational research and aid in their progress to clinical applicability.
