Abstract

Urinary tract infections (UTIs) are among the most frequent bacterial diseases in infants and children. Physician adherence to recommendations is notoriously often poor, but no data are available on UTIs management in the emergency setting. In this multicenter national study, we investigated the policies regarding UTIs management in children aged 2 months to 3 years in Italian emergency units. Between April and June 2021, directors of the emergency units were invited to answer an online survey on the following items: diagnostic approach to children with fever without an apparent source, therapeutic approach to UTIs, the use of kidney and urinary tract ultrasound, and the criteria for hospitalization. A total of 121 (89%) out of 139 of invited units participated in the study. Overall, units manage children with a suspected or confirmed UTI according to available recommendations for most of the items. However, in almost 80% (n = 94) of units, a sterile perineal bag is used to collect urine for culture. When urine is collected by cathether, heterogeneity exists on the threshold of bacterial load considered for UTI diagnosis. Conclusions: Available recommendations on UTIs in children are followed by Italian emergency units for most of the items. However, the methods to collect urine specimens for culture, one of the crucial steps of the diagnostic work-up, often do not align with current recommendations and CFU thresholds considered for diagnosis largely vary among centers. Abstract

There are growing evidence of clinical manifestations other than acute respiratory syndrome in severe acute respiratory syndrome associated with coronavirus 2-infected children. In our multicenter retrospective analysis, we observed among 127 severe acute respiratory syndrome associated with coronavirus 2 positive children that the presence of gastrointestinal symptoms was more frequently associated with severe and critical phenotype (P = 0.029). Moreover, having gastrointestinal symptoms was more frequently reported in patients who developed cardiac impairment.

Abstract

We report a case of a 10-year-old child, who presented with headache, nausea and fever in the past 72 hours. Past medical history was insignificant; parents did not signal any helpful information. Laboratory tests and radiology imaging were normal except for high inflammatory indices and modest spleen enlargement. Rotavirus infection was isolated from the blood cultures. We want to describe this unusual case of cat-scratch disease presenting with soft mono-lateral optic nerve involvements without the typical scratch related regional lymphadenopathy in order to highlight the heterogeneous presentation of Bartonella Henselae infection, especially in children. Therefore, we should consider cat-scratch disease in any patients with FUO and unspecific systemic symptoms even if there is not a recent cat relate contact and a typical clinical presentation.

Abstract

We report a case of pneumococcal neonatal meningitis in an Italian girl, occurred 3 days after birth. CSF microscopic examination revealed the presence of Gram Positive diplococci, identified as S. pneumoniae after detection of pneumococcal capsular antigen. Tests for Group B Streptococcus on mother and child were both negative. The case was peculiar because in industrialized countries the most common agents responsible of neonatal meningitis are GBS, E. coli and Listeria monocytogenes, where pneumococcal meningitis in the neonatal period is quite uncommon: It is then necessary to consider that, in any case of early onset neonatal meningitis, S. pneumoniae is a possible yet uncommon causative agent, and that the microbiological profile of early onset neonatal meningitis in industrialized countries might change substantially because of migratory streams and the subsequent diffusion of emerging microorganisms, which are unusual at the present time.

Abstract

Fluorescent materials are widely used in biological and material applications as probes for imaging or sensing; however, their customization is usually complicated without the support of an organic chemistry laboratory. Here, we present a straightforward method for the customization of BODIPY cores, which are among the most commonly used fluorescent probes. The method is based on the formation of a new C−C bond through Friedel–Crafts electrophilic aromatic substitution carried out at room temperature. The method presented can be used to obtain completely customized fluorescent materials in one or two steps from commercially available compounds. Examples of the preparation of fluorescent materials for cell staining and functionalization of silica colloids are also presented.

Abstract

Urinary tract infections (UTIs) are among the most frequent bacterial diseases in infants and children. Physician adherence to recommendations is notoriously often poor, but no data are available on UTIs management in the emergency setting. In this multicenter national study, we investigated the policies regarding UTIs management in children aged 2 months to 3 years in Italian emergency units. Between April and June 2021, directors of the emergency units were invited to answer an online survey on the following items: diagnostic approach to children with fever without an apparent source, therapeutic approach to UTIs, the use of kidney and urinary tract ultrasound, and the criteria for hospitalization. A total of 121 (89%) out of 139 of invited units participated in the study. Overall, units manage children with a suspected or confirmed UTI according to available recommendations for most of the items. However, in almost 80% (n = 94) of units, a sterile perineal bag is used to collect urine for culture. When urine is collected by cathether, heterogeneity exists on the threshold of bacterial load considered for UTI diagnosis. Conclusions: Available recommendations on UTIs in children are followed by Italian emergency units for most of the items. However, the methods to collect urine specimens for culture, one of the crucial steps of the diagnostic work-up, often do not align with current recommendations and CFU thresholds considered for diagnosis largely vary among centers. Efforts should be addressed to validate and implement new child and family friendly urine collection techniques. What is Known:• Several guidelines are published on the management of children with suspected or confirmed urinary tract infection.• No data are available on the management of pediatric urinary tract infections in the emergency setting. What is New:• Almost 80% of the Italian emergency units employ a sterile perineal bag to collect urine for culture.• Diagnostic CFU thresholds largely vary among centers.

25. "Click" Chemistry: Application of Copper Metal in Cu-Catalyzed Azomethine Imine-Alkyne Cycloadditions.

Abstract

A series of 16 copper-catalyzed azomethine imine-alkyne cycloaddition (CuAIAC) reactions between four pyrazolidinone-1-azomethine imines and four terminal ynones gave the corresponding fluorescent cycloadducts as bimane analogues in very high yields. The applicability of CuAIAC was demonstrated by the fluorescent labeling of functionalized polystyrene and by using Cu-C and Cu-Fe as catalysts. Experimental evidence, kinetic measurements, and correlation between a clean catalyst surface and the reaction rate are in agreement with a homotopic catalytic system with catalytic Cu(I)-acetylide formed from Cu(0) by "in situ" oxidation. The availability of azomethine imines, mild reaction conditions, simple workup, and scalability make CuAIAC a viable supplement to the Cu-catalyzed azide-alkyne cycloaddition reaction in "click" chemistry.

Abstract

A Turkish female infant of 96 days was admitted to the pediatric emergency room because of inconsolable crying, persistent cough, and difficulty in feeding during the previous day. She was conscious and did not show any signs or symptoms of multiorgan failure. A few minutes afterwards, the child experienced cardiac arrest with an initial cardiac rhythm of asystole and died 75 minutes later following cardiopulmonary resuscitation maneuvers. As the pathological cause of death, autopsy findings revealed a rare type of idiopathic infantile arterial calcification resulting from a mutation in the gene encoding for the ENPP1 enzyme.

27. In situ laser-imprinted surface realignment of a nematic liquid crystal.

Abstract

We present a new method for the in-plane realignment of nematic liquid crystals in already fully assembled cells with uni-directionally rubbed polyimide as an aligning layer. We use nematic liquid crystals (NLCs) with a relatively high nematic-isotropic transition temperature and we focus the IR laser beam of the laser tweezers selectively onto one or the other of the inner interfaces. The heat generated by the IR absorption locally melts the liquid crystal and creates an isotropic island with well-defined molecular anchoring at the nematic-isotropic interface. By scanning the laser beam along a pre-defined line, the moving isotropic-nematic interface leaves behind a well oriented LC domain, with LC molecules aligned at 45° to the rubbing direction. If we in addition move the sample with respect to this scanning line, we would be able to selectively realign micro-domains of the liquid crystal with respect to the original alignment induced by the PI rubbing. The realignment can be performed independently on each LC-glass interface, thereby producing predefined domains with customized and controllable alignment within an otherwise uniformly aligned cell.

28. Stabilisation of 2D colloidal assemblies by polymerisation of liquid crystalline matrices for photonic applications.

Abstract

Colloidal crystals in anisotropic matrices are extremely stable and versatile, but disassemble as soon as the anisotropy of the matrix disappears. We present an approach to first custom-assemble colloidal structures and subsequently stabilize them through photo-polymerisation of the liquid crystalline matrix. The resulting 2D colloidal assemblies are stable at high temperatures and can even be obtained as free-standing films without a decrease in the degree of organization. This approach could be used to stabilize and extract recently proposed soft-matter photonic microcircuits based on liquid crystal optical microresonators, microlasers and microfibers, and opens up routes towards real soft matter photonic devices that are stable over extended time and temperatures.

Abstract

A chiral ferrocene-based boronic acid interacts with (R)- and (S)-Binol to form two complexes that exhibit significantly different ferrocene-based electrode potentials. This difference in redox behavior can be exploited to demonstrate in principle how high levels of enantiomeric excess in a mixture of enantiomers can be quantified and read-out using an electrochemical method.

Isolipoic acid, a symmetrical and achiral isomer of the commonly used alpha-lipoic acid, has previously been overlooked as a tether group for the formation of self-assembled monolayers (SAMs). Here its ready synthesis through a new route and its functionalization with ferrocenyl groups for redox-active SAM formation on gold electrodes are described.
