Abstract

Purpose: To analyze the effects of projection-view (PV) distribution on the contrast and spatial blurring of microcalcifications on the tomosynthesized slices (X-Y plane) and along the depth (Z) direction for the same radiation dose in digital breast tomosynthesis (DBT). Methods: A GE GEN2 prototype DBT system was used for acquisition of DBT scans. The system acquires PV images from 21 angles in 3 deg. increments over a {+-}30 deg. range. From these acquired PV images, the authors selected six subsets of PV images to simulate DBT of different angular ranges and angular increments. The number of PV images in each subset was fixed at 11 to simulate a constant total dose. These different PV distributions were subjectively divided into three categories: uniform group, nonuniform central group, and nonuniform extreme group with different angular ranges and angular increments. The simultaneous algebraic reconstruction technique (SART) was applied to each subset to reconstruct the DBT slices. A selective diffusion regularization method was employed to suppress noise. The image quality of microcalcifications in the reconstructed DBTs with different PV distributions was compared using the DBT scans of an American College of Radiology phantom and three human subjects. The contrast-to-noise ratio (CNR) and the fullmore » width at half maximum (FWHM) of the line profiles of microcalcifications within their in-focus DBT slices (parallel to detector plane) and the FWHMs of the interplane artifact spread function (ASF) in the Z-direction (perpendicular to detector plane) were used as image quality measures. Results: The results indicate that DBT acquired with a large angular range or, for an equal angular range,with a large fraction of PVs at large angles yielded superior ASF with smaller FWHM in the Z-direction. PV distributions with a narrow angular range or a large fraction of PVs at small angles had stronger interplane artifacts. In the X-Y focal planes, the effect of PV distributions on spatial blurring depended on the directions. In the X-direction (perpendicular to the chestwall), the normalized line profiles of the calcifications reconstructed with the different PV distributions were similar in terms of FWHM; the differences in the FWHMs between the different PV distributions were less than half a pixel. In the Y-direction (x-ray source motion), the normalized line profiles of the calcifications reconstructed with PVs acquired with a narrow angular range or a large fraction of PVs at small angles had smaller FWHMs and thus less blurring of the line profiles. In addition, PV distributions with a narrow angular range or a large fraction of PVs at small angles yielded slightly higher CNR than those with a wide angular range for small, subtle microcalcifications; however, PV distributions had no obvious effect on CNR for relatively large microcalcifications. Conclusions: PV distributions affect the image quality of DBT. The relative importance of the impact depends on the characteristics of the signal and the direction (perpendicular or parallel) relative to the direction of x-ray source motion. For a given number of PVs, the angular range and the distribution of the PVs affect the degree of in-plane and interplane blurring in opposite ways. The design of the scan parameters of tomosynthesis systems would require proper consideration of the characteristics of the signals of interest and the potential trade-off of the image quality of different types of signals.« less

Abstract

The National Lung Screening Trial (NLST) is a randomized multicenter study comparing low-dose helical computed tomography (CT) with chest radiography in the screening of older current and former heavy smokers for early detection of lung cancer, which is the leading cause of cancer-related death in the United States. Five-year survival rates approach 70% with surgical resection of stage IA disease; however, more than 75% of individuals have incurable locally advanced or metastatic disease, the latter having a 5-year survival of less than 5%. It is plausible that treatment should be more effective and the likelihood of death decreased if asymptomatic lung cancer is detected through screening early enough in its preclinical phase. For these reasons, there is intense interest and intuitive appeal in lung cancer screening with low-dose CT. The use of survival as the determinant of screening effectiveness is, however, confounded by the well-described biases of lead time, length, and overdiagnosis. Despite previous attempts, no test has been shown to reduce lung cancer mortality, an endpoint that circumvents screening biases and provides a definitive measure of benefit when assessed in a randomized controlled trial that enables comparison of mortality rates between screened individuals and a control group that does not undergo the screening intervention of interest. The NLST is such a trial. The rationale for and design of the NLST are presented.

Abstract

International audience; Exotic earthworm impacts on temperate forest soils are influenced by earthworm community composition and are likely constrained by the degree of organic matter redistribution following earthworm introductions across different soil types; however, the relative importance of these factors remains unknown. We examined how exotic earthworm communities affected leaf litter carbon (C) and nitrogen (N) mineralization and transport in two Spodosols with contrasting textures and organic matter contents. In reconstructed soil mesocosms, we measured organic C pools, quantified C-13 and N-15 transport from isotopically labeled red maple (Acer rubrum) leaf litter, and linked leaf litter redistribution to sub-surface burrow system structures following 150-day incubations. Transport of C and N from surface litter into soil was greatest with multi-species earthworm communities, and A-horizon and burrow pools functioned as dominant sinks for this material. Litter-derived C:N recovery ratios of soil pools revealed higher retention of litter-derived N over litter-derived C; recovery of litter N (mg N-15 m(-2)) transported from surface litter was greater in the sandy loam (98.2 +/- A 2.73 %) than in the sandy soil (66.2 +/- A 4.92 %) following earthworm community additions. Earthworm biomass was as a minor sink for litter C (mg C-13 m(-2)) and N transported from surface litter (0.56 +/- A 0.13 and 2.26 +/- A 0.31 %, respectively). Recovery of litter-derived C and N in earthworm biomass increased with the degree of direct leaf litter consumption (A. trapezoides < E. fetida < L. terrestris). Surface-dwelling epigeic and mineral-soil dwelling endogeic species produced burrow systems with the highest volume, surface connectivity, and density in the A-horizon; these properties were associated with greater CO2 losses and with greater litter C and N transport into A-horizons and burrows. Burrow systems with high continuity and large burrows produced by vertical-burrowing anecic species were associated with greater litter C and N transport into B-horizons, and greater dissolved organic C leaching losses. This study shows that the degree of organic matter redistribution in temperate forest soils following earthworm introductions is directly related to earthworm community composition, while the preferential retention of N over C and the potential stabilization of this material is determined by soil type.

Abstract

Purpose: To analyze the effects of projection-view (PV) distribution on the contrast and spatial blurring of microcalcifications on the tomosynthesized slices (X-Y plane) and along the depth (Z) direction for the same radiation dose in digital breast tomosynthesis (DBT).Methods: A GE GEN2 prototype DBT system was used for acquisition of DBT scans. The system acquires PV images from 21 angles in 3° increments over a ±30° range. From these acquired PV images, the authors selected six subsets of PV images to simulate DBT of different angular ranges and angular increments. The number of PV images in each subset was fixed at 11 to simulate a constant total dose. These different PV distributions were subjectively divided into three categories: uniform group, nonuniform central group, and nonuniform extreme group with different angular ranges and angular increments. The simultaneous algebraic reconstruction technique (SART) was applied to each subset to reconstruct the DBT slices. A selective diffusion regularization method was employed to suppress noise. The image quality of microcalcifications in the reconstructed DBTs with different PV distributions was compared using the DBT scans of an American College of Radiology phantom and three human subjects. The contrast-to-noise ratio (CNR) and the full width at half maximum (FWHM) of the line profiles of microcalcifications within their in-focus DBT slices (parallel to detector plane) and the FWHMs of the interplane artifact spread function (ASF) in the Z-direction (perpendicular to detector plane) were used as image quality measures.Results: The results indicate that DBT acquired with a large angular range or, for an equal angular range,with a large fraction of PVs at large angles yielded superior ASF with smaller FWHM in the Z-direction. PV distributions with a narrow angular range or a large fraction of PVs at small angles had stronger interplane artifacts. In the X-Y focal planes, the effect of PV distributions on spatial blurring depended on the directions. In the X-direction (perpendicular to the chestwall), the normalized line profiles of the calcifications reconstructed with the different PV distributions were similar in terms of FWHM; the differences in the FWHMs between the different PV distributions were less than half a pixel. In the Y-direction (x-ray source motion), the normalized line profiles of the calcifications reconstructed with PVs acquired with a narrow angular range or a large fraction of PVs at small angles had smaller FWHMs and thus less blurring of the line profiles. In addition, PV distributions with a narrow angular range or a large fraction of PVs at small angles yielded slightly higher CNR than those with a wide angular range for small, subtle microcalcifications; however, PV distributions had no obvious effect on CNR for relatively large microcalcifications.Conclusions: PV distributions affect the image quality of DBT. The relative importance of the impact depends on the characteristics of the signal and the direction (perpendicular or parallel) relative to the direction of x-ray source motion. For a given number of PVs, the angular range and the distribution of the PVs affect the degree of in-plane and interplane blurring in opposite ways. The design of the scan parameters of tomosynthesis systems would require proper consideration of the characteristics of the signals of interest and the potential trade-off of the image quality of different types of signals.

We are investigating factors affecting the detection of microcalcifications in digital breast tomography (DBT). In this study, we analyzed the effects of projection-view (PV) distribution on spatial blurring of calcifications on the tomosynthesized slices (X-Y plane) and along the depth (Z) direction. DBT scans of a breast phantom with simulated microcalcifications were acquired with a GE prototype system at 21 angles in 3° increments over a ±30° range. Abstract

An ROC study was conducted to evaluate the usefulness of assessing breast lesion characteristics with stereomammography. Stereoscopic image pairs of 158 breast biopsy tissue specimens were acquired with a GE digital mammography system. Two stereo image pairs were taken at 1.8X magnification geometry and at approximately orthogonal orientations for each specimen. Display software was developed for a high resolution MegaScan CRT monitor driven by a DOME stereo display board. The specimens contained either a mass, microcalcifications, both, or normal tissue. About 40% of the specimens were found to contain malignancy by pathological analysis. Five MQSA radiologists participated in the observer performance experiment. The two views of each specimen were read independently and were separated by a large number of other specimen images to reduce any effects of memorization. Each observer read 316 specimen images in a systematically randomized order. The observer first read the monoscopic image and entered his/her assessment in terms of the confidence ratings on the presence of microcalcifications and/or masses, margin clearance, BI-RADS assessment, and the likelihood of malignancy. The corresponding stereoscopic images were then displayed on the same monitor and were viewed through stereoscopic LCD glasses. The observer was free to change the ratings in every category after stereoscopic reading. The ratings of the observers were analyzed by ROC methodology.
