Abstract

The continual emergence of novel influenza A strains from non-human hosts requires constant vigilance and the need for ongoing research to identify strains that may pose a human public health risk. Since 1999, canine H3 influenza A viruses (CIVs) have caused many thousands or millions of respiratory infections in dogs in the United States. While no human infections with CIVs have been reported to date, these viruses could pose a zoonotic risk. In these studies, the National Institutes of Allergy and Infectious Diseases (NIAID) Centers of Excellence for Influenza Research and Surveillance (CEIRS) network collaboratively demonstrated that CIVs replicated in some primary human cells and transmitted effectively in mammalian models. While people born after 1970 had little or no pre-existing humoral immunity against CIVs, the viruses were sensitive to existing antivirals and we identified a panel of H3 cross-reactive human monoclonal antibodies (hmAbs) that could have prophylactic and/or therapeutic value. Our data predict these CIVs posed a low risk to humans. Importantly, we showed that the CEIRS network could work together to provide basic research information important for characterizing emerging influenza viruses, although there were valuable lessons learned.

Abstract

We propose and analyse numerical schemes for a system of quasilinear, degenerate evolution equations modelling biofilm growth as well as other processes such as flow through porous media and the spreading of wildfires. The first equation in the system is parabolic and exhibits degenerate and singular diffusion, while the second is either uniformly parabolic or an ordinary differential equation. First, we introduce a semi-implicit time discretisation that has the benefit of decoupling the equations. We prove the positivity, boundedness, and convergence of the time-discrete solutions to the time-continuous solution. Then, we introduce an iterative linearisation scheme to solve the resulting nonlinear time-discrete problems. Under weak assumptions on the time-step size, we prove that the scheme converges irrespective of the space discretisation and mesh. Moreover, if the problem is non-degenerate, the convergence becomes faster as the time-step size decreases. Finally, employing the finite element method for the spatial discretisation, we study the behaviour of the scheme, and compare its performance to other commonly used schemes. 14. A posteriori error estimates for the Richards equation

Abstract

The Richards equation is commonly used to model the flow of water and air through soil, and it serves as a gateway equation for multiphase flows through porous media. It is a nonlinear advection-reaction-diffusion equation that exhibits both parabolic-hyperbolic and parabolic-elliptic kinds of degeneracies. In this study, we provide reliable, fully computable, and locally space-time efficient a posteriori error bounds for numerical approximations of the fully degenerate Richards equation. For showing global reliability, a nonlocal-in-time error estimate is derived individually for the time-integrated $H^1(H^{-1})$, $L^2(L^2)$, and the $L^2(H^1)$ errors. A maximum principle and a degeneracy estimator are employed for the last one. Global and local space-time efficiency error bounds are then obtained in a standard $H^1(H^{-1})\cap L^2(H^1)$ norm. The reliability and efficiency norms employed coincide when there is no nonlinearity. Moreover, error contributors such as flux nonconformity, time discretization, quadrature, linearization, and data oscillation are identified and separated. The estimates are also valid in a setting where iterative linearization with inexact solvers is considered. Numerical tests are conducted for nondegenerate and degenerate cases having exact solutions, as well as for a realistic case. Abstract

We study an imbibition problem for porous media. When a wetted layer is above a dry medium, gravity leads to the propagation of the water downwards into the medium. In experiments, the occurrence of fingers was observed, a phenomenon that can be described with models that include hysteresis. In the present paper, we describe a single finger in a moving frame and set up a free boundary problem to describe the shape and the motion of one finger that propagates with a constant speed. 17. Capillary Hysteresis and Gravity Segregation in Two Phase Flow Through Porous Media

Abstract

We study the gravity-driven flow of two fluid phases in a one-dimensional homogeneous porous column when history dependence of the pressure difference between the phases (capillary pressure) is taken into account. In the hyperbolic limit, solutions of such systems satisfy the Buckley-Leverett equation with a non-monotone flux function. However, solutions for the hysteretic case do not converge to the classical solutions in the hyperbolic limit in a wide range of situations. In particular, with Riemann data as initial condition, stationary shocks become possible in addition to classical components such as shocks, rarefaction waves, and constant states. We derive an admissibility criterion for the stationary shocks and outline all admissible shocks. Depending on the capillary pressure functions, flux function, and the Riemann data, two cases are identified a priori for which the solution consists of a stationary shock. In the first case, the shock remains at the point where the initial condition is discontinuous. In the second case, the solution is frozen in time in at least one semi-infinite half. 18. Existence and properties of solutions of extended play-type hysteresis model

Abstract

This paper analyses the well-posedness and properties of the extended play-type model which was proposed in [van Duijn & Mitra (2018)] to incorporate hysteresis in unsaturated flow through porous media. The model, when regularised, reduces to a nonlinear degenerate parabolic equation coupled with an ordinary differential equation. This has an interesting mathematical structure which, to our knowledge, still remains unexplored. The existence of solutions for the non-degenerate version of the model is shown using the Rothe's method. An equivalent to maximum principle is proven for the solutions. Existence of solutions for the degenerate case is then shown assuming that the initial condition is bounded away from the degenerate points. Abstract

Within cloud-based internet of things (IoT) applications, typically cloud providers employ Service Level Agreements (SLAs) to ensure the quality of their provisioned services. Similar to any other contractual method, an SLA is not immune to breaches. Ideally, an SLA stipulates consequences (e.g. penalties) imposed on cloud providers when they fail to conform to SLA terms. The current practice assumes trust in service providers to acknowledge SLA breach incidents and executing associated consequences. Recently, the Blockchain paradigm has introduced compelling capabilities that may enable us to address SLA enforcement more elegantly. This paper proposes and implements a blockchain-based approach for assessing SLA compliance and enforcing consequences. It employs a diagnostic accuracy method for validating the dependability of the proposed solution. The paper also benchmarks Hyperledger Fabric to investigate its feasibility as an underlying blockchain infrastructure concerning latency and transaction success/fail rates.

Abstract

Postoperative acute kidney injury (PO-AKI) is a common complication of major surgery that is strongly associated with short-term surgical complications and long-term adverse outcomes, including increased risk of chronic kidney disease, cardiovascular events and death. Risk factors for PO-AKI include older age and comorbid diseases such as chronic kidney disease and diabetes mellitus. PO-AKI is best defined as AKI occurring within 7 days of an operative intervention using the Kidney Disease Improving Global Outcomes (KDIGO) definition of AKI; however, additional prognostic information may be gained from detailed clinical assessment and other diagnostic investigations in the form of a focused kidney health assessment (KHA). Prevention of PO-AKI is largely based on identification of high baseline risk, monitoring and reduction of nephrotoxic insults, whereas treatment involves the application of a bundle of interventions to avoid secondary kidney injury and mitigate the severity of AKI. As PO-AKI is strongly associated with long-term adverse outcomes, some form of follow-up KHA is essential; however, the form and location of this will be dictated by the nature and severity of the AKI. In this Consensus Statement, we provide graded recommendations for AKI after non-cardiac surgery and highlight priorities for future research.

Abstract

Kidney involvement in patients with coronavirus disease 2019 (COVID-19) is common, and can range from the presence of proteinuria and haematuria to acute kidney injury (AKI) requiring renal replacement therapy (RRT; also known as kidney replacement therapy). COVID-19-associated AKI (COVID-19 AKI) is associated with high mortality and serves as an independent risk factor for all-cause in-hospital death in patients with COVID-19. The pathophysiology and mechanisms of AKI in patients with COVID-19 have not been fully elucidated and seem to be multifactorial, in keeping with the pathophysiology of AKI in other patients who are critically ill. Little is known about the prevention and management of COVID-19 AKI. The emergence of regional 'surges' in COVID-19 cases can limit hospital resources, including dialysis availability and supplies; thus, careful daily assessment of available resources is needed. In this Consensus Statement, the Acute Disease Quality Initiative provides recommendations for the diagnosis, prevention and management of COVID-19 AKI based on current literature. We also make recommendations for areas of future research, which are aimed at improving understanding of the underlying processes and improving outcomes for patients with COVID-19 AKI.

Abstract

In 2012, Kidney Disease: Improving Global Outcomes (KDIGO) published a guideline on the classification and management of acute kidney injury (AKI). The guideline was derived from evidence available through February 2011. Since then, new evidence has emerged that has important implications for clinical practice in diagnosing and managing AKI. In April of 2019, KDIGO held a controversies conference entitled Acute Kidney Injury with the following goals: determine best practices and areas of uncertainty in treating AKI; review key relevant literature published since the 2012 KDIGO AKI guideline; address ongoing controversial issues; identify new topics or issues to be revisited for the next iteration of the KDIGO AKI guideline; and outline research needed to improve AKI management. Here, we present the findings of this conference and describe key areas that future guidelines may address.

Abstract

Cytomegalovirus (CMV) infection is a relatively late complication of AIDS. Like other viruses contributing to co-morbidity of HIV infection, cytomegalovirus has the propensity to cause multiorgan involvement. We report the case of a 34-year-old seropositive man who presented with bilateral lower limb weakness and symptomatic pallor. He was already on antiretroviral drugs for a month prior to presentation. Detailed clinical examination and laboratory investigations revealed cytomegalovirus polyradiculoneuropathy associated with bone marrow dysplasia. Dysplasia of haematopoeitic cell lines occurs in 30% to 70% of HIV infected patients, and is often indistinguishable from myelodysplastic syndrome. However, in our case, the bone marrow picture reverted back to normal with treatment of the CMV infection, pointing to a possible role of CMV as the causative agent of bone marrow dysplasia. Moreover, CMV has been incriminated as a pathogen producing the immune reconstitution inflammatory syndrome. The onset of the disease in our case one month after initiation of HAART strongly raises the possibility of this being a case of CMV related IRIS. This is the first reported case where IRIS has presented with CMV polyradiculoneuropathy and bone marrow dysplasia. We would like to highlight that in today's era of HIV care, clinicians should be aware of the possibility of multiorgan involvement by CMV, for appropriate management of this disease in the background of AIDS.
