Mahmoud Mohamed Genaidy, Hosny Ahmed Zein, Ahmed Mostafa Eid Department of Ophthalmology, Faculty of Medicine, Minia University, Minia, Egypt Purpose of the study: To evaluate efficacy, safety, and success rates of canaloplasty combined with phacoemulsification and intraocular lens implantation in patients with open-angle glaucoma (OAG) and visually significant cataract.Patients and methods: A prospective interventional noncomparative case series carried out in Minia University Hospital from April 2015 to October 2016 on 20 eyes of 18 patients who had visually significant cataract and primary OAG. All the cases had combined phacocanaloplasty. Preoperative best-corrected visual acuity, intraocular pressure (IOP), and number of antiglaucoma medications were collected and compared to postoperative levels, and complications rates were recorded.Results: 20 eyes of 18 patients (5 males and 15 females), with a mean age 57.6 years (range 48–69years), underwent phacocanaloplasty. Preoperative mean IOP was 25.20 ±1.009mmHg. Postoperative IOP decreased to a mean of 14.20±0.9, 14.85±0.8, and 15.85±0.7 mmHG at 3, 6, and 12months, respectively, with 37% reduction from preoperative IOP level at one year follow-up visit (P=0.0005). The number of antiglaucoma medications dropped from mean of 1.55 preoperatively to 0.35 postoperatively. LogMAR of best-corrected visual acuity improved from 0.6950±0.07 preoperatively to 0.3670±0.056, 0.3460±0.056, and 0.03370±0.052 at 3, 6, and 12months postoperatively (P=0.0005). Complications were limited to mild hyphemia (one case), mild corneal edema (one case), and mild inflammatory membrane (one case) that resolved in the first week after surgery.Conclusion: Canaloplasty combined with clear corneal phacoemulsification and intraocular lens implantation may be a safe and effective procedure to lower IOP in adult patients with OAG and visually significant cataract. Increasing resistance of microbes to available antifungals has necessitated a search for alternative natural products with antifungal properties. Combinations of essential oils (EOs) with known antifungal activities may increase antimicrobial activity, reduce side effects, and overcome resistance. This study determined the antifungal activity of basil, marjoram, clove, cumin and caraway EOs, both individually and in combination. The EOs and the major constituents of clove, cumin and caraway were screened against Candida albicans (C. albicans), and Aspergillus niger (A. niger) using a broth dilution method. Fluconazole was used as a positive control. The MICs of the mixtures of EOs were better than those of the single oils (except in the case of clove oil) especially against C. albicans (1 mg/ml) suggesting a synergistic effect against both C. albicans and A. niger (2 mg/ml) when increasing the concentration of one oil over the other two. Single active components (eugenol, carvone and cuminaldehyde) have better activity than single total oils against both organisms and showed synergistic effects when combined. In summary, mixtures of EOs or their active components had similar/better antifungal activities compared to the single oils or ingredients suggesting a synergistic effect against both C. albicans and A. niger. Clove oil was the best against both organisms while eugenol was the best against A. niger. In conclusion, the synergistic effect of the combinations of the EOs and their active components against C. albicans and A. niger could lead to decreased side effects and consequently they can be used to treat various fungal infections caused by these organisms.

Purpose. To evaluate the anterior segment, the anatomical position of the implantable collamer lenses (ICL), and its relationship to adjacent ocular structures using Ultrasound Biomicroscopy (UBM).Methods. In a prospective study, 142 myopic eyes of 93 patients implanted with Visian ICL were subjected to UBM examination between March 2010 and January 2015. The relative position of ICL to the adjacent structure and the overall iris configuration were evaluated. The machine calibers were used to measure the minimum central distance between the ICL and anterior lens capsule (vault) and the vertical central distance between the corneal endothelium and the ICL (E-ICL).Results. The mean ICL vault was376±105 μm. The mean E-ICL was2826±331 μm. Contact between ICL and the posterior epithelium of the iris was present in all eyes. The overall iris configuration was flat in 89 eyes. Central anterior convexity was present in 41 eyes and mild peripheral iris bombe in 12 eyes. The haptics could be imaged in the ciliary sulcus in 112 eyes and at least one haptic resting on the lens periphery and zonules in 30 eyes.Conclusion. UBM can provide valuable anatomical information that allows detailed postoperative in vivo assessment of ICL.

Purpose The aim of this study was to compare direct brow lift and transblepharoplasty browpexy for the management of brow ptosis in terms of cosmetic results, complications, and patient satisfaction. Patients and methods It is a surgical intervention prospective comparative study, 40 brows of 24 patients with brow ptosis were divided into two groups. Group A included 20 brows that were subjected to direct brow lift and group B included 20 brows that were subjected to transblepharoplasty browpexy. Preoperative evaluation included assessment of general and ophthalmological history, brow evaluation, any associated dermatochalasis, ocular examination, cranial nerve examination, visual field assessment, and photographic documentation. Results Bilateral brow ptosis was present in 16 patients, whereas eight patients had unilateral brow ptosis. There were 16 female patients and eight male patients. Their age ranged between 53 and 75 years. The cosmetic complaint of an undesirable appearance was the presenting symptom in 18 patients, whereas functional issues with heaviness and temporal visual field defects were the presenting symptoms in four patients. In group A (direct brow lift), 16 brows were corrected with the brow level at or slightly above the superior orbital margin, whereas four brows were undercorrected. In group B (transblepharoplasty browpexy), 12 brows were fully corrected, whereas eight brows were undercorrected. Conclusion Both direct brow lift and transblepharoplasty browpexy are effective and safe techniques for the correction of brow ptosis. Each procedure has its own advantages.
