Abstract The detection of Highly Pathogenic Avian Influenza (HPAI) A(H5N1) in dairy cattle in the United States has raised concerns about human exposure. This study evaluated the efficacy of various doses of an inactivated H5 AI vaccine in cattle and assessed antibody transfer in milk against a recent bovine isolate of HPAI A(H5N1, clade 2.3.4.4b). Calves were inoculated with different vaccine doses, while lactating cows received the vaccine four weeks later. The humoral immune response was measured using the Hemagglutination Inhibition (HI) test and ELISA. Results showed a dose-dependent immune response, with higher doses producing stronger and more sustained antibody levels. Group 1 maintained a stable HI titer of 6 log2, while Groups 2, 3, and 4 peaked at 8, 9, and 9 log2, respectively, by the fourth week post-vaccination. Milk antibody transfer was observed, with strong positive responses in milk samples by the second week post-vaccination. The ID Screen ELISA demonstrated higher sensitivity for detecting antibodies in milk compared to serum. The immune response to the AI vaccine differed from responses to other vaccines used in cattle such as Foot and Mouth Disease Virus (FMDV) and Lumpy Skin Disease Virus (LSDV), indicating the need for optimizing vaccine dosage and formulation, including adjuvant and antigen content. Future research should extend the monitoring period, increase sample sizes, and explore different vaccine formulations to develop effective vaccination strategies for cattle. Abstract Foot and mouth disease (FMD) is a highly contagious viral infection affecting cloven-hoofed ruminants, leading to significant economic losses. In 2022, Egypt faced a severe outbreak of Foot and Mouth Disease (FMD) caused by the A/Africa/G-IV variant. This study assessed the efficacy of local and imported FMDV vaccines (A Iran-05 lineage) against this new variant using in vitro and in vivo methods. Sera from vaccinated calves showed inadequate cross-protection, with mean r1-values of 0.235 and 0.243 for local and imported vaccines, respectively. Challenge tests indicated low protection levels (20% and 40%) against A/Africa/G-IV compared with A/Iran/05. Current vaccines were deemed ineffective, prompting a formulation update incorporating the variant. Neuroblastoma is an aggressive malignancy commonly found in children, with adult patients being quite rare. Pancreatic neuroblastoma, even among adult cases, is especially rare. We present a case of a 26-year-old woman presented with mild abdominal pain and a palpable mass diagnosed as neuroblastoma affecting the pancreas. After initial diagnostic challenges, a biopsy confirmed the diagnosis of neuroblastoma. The patient thereafter underwent a course of neoadjuvant chemotherapy, followed by a complex surgical resection. This case highlights the importance of a comprehensive diagnostic approach, including repeated biopsies and the involvement of a multidisciplinary team in the management of rare presentations of adult neuroblastoma. more...

Abstract This paper examines the association between audit committee characteristics (ACC) and risk disclosure (RD) of firms, focusing on the moderating effect of audit quality (AQ). The importance of RD to investors and stakeholders is that such a practice offers a better evaluation of the overall risk profile of companies. Based on agency theory complemented by the resource dependence theory, this paper therefore suggests that effective audit committees have an effect of reducing information asymmetry while increasing audit quality monitoring capacities and consequently enhance risk disclosure practices. In this respect, the literature sources were reviewed, and a hypothetical framework was developed to test the hypotheses. The sample selection comprises 54 companies with non-financial companies listed in the Egyptian Exchange Market, EGX100, for the period 2018 to 2021, which amounts to 216 observations. According to the results, ACC, such as size and financial expertise, are significant for RD, while ACC relating to meetings and independence are less important for RD. The study further established that AQ moderates the relationship between ACC and RD, implying that the significant influence of ACC is stronger when AQ is high. These inferences are of essence to policy makers and companies in understanding the implication of ACC on RD and the relation to high-quality audits in improving the effectiveness of audit committees. Abstract Introduction Direct oral anticoagulant (DOAC) dose adjustment is based on age, renal function, and body weight. There is a paucity of data describing the factors associated with the prescription of inappropriate dosage and their impact on clinical outcomes among patients receiving transcatheter aortic valve implantation (TAVI). Methods In a single-center study, 432 patients who were on long-term DOAC therapy and underwent TAVI between 2015 and 2022 were included. We analyzed the predictors and outcomes of inappropriate dosing of DOACs; namely apixaban, dabigatran, edoxaban, and rivaroxaban. A composite endpoint, including all-cause mortality, life-threatening/major bleeding, stroke, peripheral thromboembolic complications, or myocardial infarction, was assessed after 1 year. Results In this TAVI cohort, inappropriate DOAC dosing was observed in 20.6% of patients. Purpose – This study examines the correlation between ownership structure (OS) and financial reporting integrity (FRI), with emphasis on the impact of earnings quality (EQ) in the Egyptian context. Design/methodology/approach – The study uses data from 472 firm-year observations of Egyptian publicly listed companies between 2014 and 2021 and carried out descriptive statistics, correlation tests, multiple regression analysis and two-stage least squares (2SLS) to test the hypotheses. Findings – The results revealed that blockholders and institutional ownership significantly enhance reporting integrity through effective oversight and monitoring. The findings underscore the vital role of concentrated OS in overseeing reporting practices and mitigating managerial opportunism, thereby improving the transparency and reliability of financial disclosures in Egypt. Practical implications – The findings enrich the literature on corporate governance and financial reporting quality and have important implications for policymakers, regulators and corporate stakeholders. Abstract Purpose The main objective of the research is to examine the influence of audit committee attributes on the integrated reporting quality (IRQ) and to investigate whether this association is moderated by board gender diversity in the manufacturing and non-manufacturing sectors. The study specifically investigates the association between audit committee independence, meetings frequency, financial expertise, audit committee size, and integrated reporting quality. Design/methodology/approach The data collection was from the financial statements and corporate governance reports of 54 Egyptian firms listed on the stock market [EGX100] for the period of 2018–2022. The study used pooled effect modelling and performed data analysis using the STATA software. Findings For the manufacturing sector, the results showed a significant impact of audit committee independence and meetings frequency on IRQ, while audit committee size and financial expertise showed insignificant association with IRQ. Board gender diversity moderates the association between audit committee meetings only and IRQ. In contrast, audit committee financial expertise and meetings frequency significantly impact IRQ, while audit committee independence and size showed an insignificant impact on IRQ in the non-manufacturing sector. Also, board gender diversity moderates the association between audit committee and IRQ. The findings highlight the significance of AC attributes and provide guidance to firms to strengthen their audit committees in order to provide high quality of integrated reports to increase investor trust in the Egyptian business environment as it is now mandatory by the government. Originality/value This research offers unique perspectives on the association between variables in Egypt. This study is one of the first investigations on the impact of audit committee attributes and board diversity on integrated reporting quality within the context of Egyptian listed companies. Abstract Background Imaging is a crucial diagnostic tool in focal liver lesions (FLLs) diagnosis. Without the need for an intravenous contrast agent, two such MRI methods that can distinguish between benign and malignant FLLs are diffusion-weighted imaging (DWI) and magnetic resonance elastography (MRE). The purpose of this study was to assess the utility of diffusion-weighted magnetic resonance imaging and magnetic resonance elastography in the identification and differentiation of benign and malignant hepatic focal lesions. Methods This cross-sectional study was carried out on ninety patients (with mean age 52 years) with hepatic focal lesions (29 benign and 61 malignant). Both MRE and DWI were performed on the patients. A modified gradient-echo sequence was used for MRE, and respiratory-triggered fat-suppressed single-shot echoplanar DW imaging (b = 0.800) was used for DWI. Maps of the apparent diffusion coefficient (ADC) and stiffness were produced. Regions of interest were placed over the FLLs on stiffness and ADC maps to get FLL ADC values and mean stiffness. Receiver operating curve (ROC) analysis was used to compare the roles of MRE and DWI in the differentiation of benign and malignant FLL. Results The ADC of FLLs and MRE stiffness exhibited strong negative correlation [(r: −0.559; p 4.23 and ≤ 1.43, respectively; the area under the curve (AUC) values were 0.991 and 0.894, and the sensitivity and specificity results were 96.7%, 93.1%, and 85.2%, 89.7%, respectively. Abstract This study examines the impact of environmental, social, and governance (ESG) disclosure on integrated reporting disclosure quality (IRDQ) and the moderating role of board of directors (BOD) characteristics. Prior research on ESG disclosure and IRDQ has been limited, especially in emerging economies like Egypt. Furthermore, the influence of BOD attributes has been underexplored. This study aims to address these gaps. A sample of 34 Egyptian non-financial companies under ESG disclosure mandates from 2015 to 2021 is analyzed using regression analysis. The results show a significant positive association between ESG disclosure and IRDQ. Furthermore, BOD gender diversity and size are found to positively moderate the ESG–IRDQ relationship. These findings highlight that appointing more women on boards and increasing board size can improve IRDQ when firms engage in ESG disclosure. However, the small sample size of ESG-mandated companies and inherent limitations in constructing disclosure indices constrain the generalizability of results. Overall, this study provides timely empirical evidence on ESG adoption within the unique Egyptian context and its influence on integrated reporting (IR). It contributes to literature by identifying specific BOD characteristics that enhance IRDQ. Abstract This paper investigates the effects of corporate risk disclosure (CRD) and dividend policy (DP) on firm value (FV) for non-financial companies listed on the Egyptian Stock Exchange. Using a sample of 45 non-financial firms from 2016 to 2022, which yielded 315 firm-year observations, we find a significant positive relationship between CRD and FV, supporting signaling theory. DP also exhibits a positive association with FV. Additionally, DP positively moderates the CRD-FV relationship, indicating a complementary effect where dividends enhance the CRD's positive signal. The results are robust across fixed effects, random effects, and pooled OLS models. This study makes key empirical and theoretical contributions by validating the hypothesized relationships in the Egyptian context. It also provides managerial insights into value drivers for public firms in developing economies. Further research can corroborate the findings in other emerging markets. Abstract Research purpose This paper aims to examine the impact of institutional design, glass ceiling syndromes, traditional stereotypes on gender equality in public organizations in Egypt, and finally the effect of age as a moderating factor of the relation between the dependent and independent variables. Research methodology Both qualitative and quantitative research approaches were utilized, employing self-administered questionnaires and in-depth interviews to test the research hypotheses. Research findings The findings indicate that institutional design factors significantly affect gender equality, while traditional stereotypes also have a significant impact. However, the "Glass Ceiling Syndrome" was found to have no significant effect. Moreover, the study reveals that women's age plays a positive moderating role in the relationship between institutional design, glass ceiling syndromes, traditional stereotypes, and gender equality. Originality/value The originality and value of this study lie in its examination of age as a moderator in the relationship between dependent and independent variables, specifically institutional design, glass ceiling syndromes, traditional stereotypes, and gender equality. Purpose – This paper aims to examine the impact of environmental, social and governance (ESG) disclosure and firm value (FV), as well as, pinpoints the role of the audit quality (AQ) as a moderating variable on such impact; where the authors hypothesize that AQ modulates the relationship between ESG disclosure and the FV. Design/methodology/approach – Data of a sample of firms listed on the Egyptian Stock Exchange Market (EGX) were collected over the period of 2017–2021 and analyzed using the regression and 2SLS models. Findings – The results suggested that: (1) the ESG has a significant positive impact on the FV in the EGX, and (2) AQ has a significant impact, as a moderating variable, on the relationship between ESG disclosure and FV. Research limitations/implications – The findings would help the Egyptian market authorities in realizing the importance of integrating ESG information within the financial reports of the listed firms. The findings could also help in developing effective disclosure procedures to provide shareholders with useful information. Abstract Purpose This study aims to investigate the impact of corporate reputation on investors and whether it influences company investment returns. While previous research has focused on specific components of reputation, neglected risks associated with the analyzed firms, and relied on accounting performance metrics, this study takes into account both return and risk factors to examine how they affect corporate reputation and investment decision-making. Design/methodology/approach Data were collected from 300 active investors in the Egyptian exchange stock market, and the seven-paradigm RepTrak model was used to assess reputation structures and expected return and risk. Structural equation modeling was employed to confirm the theories. Findings The findings highlight that investors demonstrate a primary focus on the financial performance of organizations when making stock market decisions, while also considering non-financial factors such as products, innovation, citizenship, governance, and emotional appeal. Their prioritization of investment returns and risks takes precedence over evaluating workplace practices. Originality/value This study's originality lies in its incorporation of return and risk measurements as crucial investment choice variables, contributing to the financial markets’ research on corporate reputation. Egyptian Red Sea fisheries face the same challenges as most of the world’s fisheries, including overexploitation, habitat loss, IUU fishing, pollution, and climate change. These fisheries are highly diverse with multiple species targeted by multiple fleets, using different fishing gears. Much work has been performed in recent years to assess Red Sea fish stocks. However, not all fish stocks in the Egyptian Red Sea are assessed, and those that are assessed only cover 30% of landings. The assessments are unbalanced by area, with the Gulf of Suez being much better covered than the southern Red Sea and Gulf of Aqaba. The results show that most of the analyzed stocks are overexploited. There is an urgent need to take action to protect, conserve, and restore the different fish stocks in different fishing grounds. These actions will ensure the sustainability of the fisheries, making them ecologically friendly and economically and socially efficient. more...

Abstract Introduction Patients at high bleeding risk (HBR patients) represent an important subset of patients undergoing percutaneous coronary intervention (PCI). It remains unclear whether a shortened duration of dual antiplatelet therapy (DAPT) confers benefits compared with prolonged duration of DAPT in this patient population. The aim of this study was to investigate and compare bleeding and ischemic outcomes among HBR patients receiving short- versus long-term DAPT after PCI. Methods A meta-analysis of studies comparing short-term (1–3 months) and long-term (6–12 months) DAPT after PCI with second-generation drug-eluting stents in HBR patients was performed. Results Six studies [1 randomized controlled trial (RCT), 2 RCT subanalyses, and 3 prospective propensity-matched studies] involving 15,908 patients were included in the meta-analysis. During a follow-up of 12 months, short-term DAPT was associated with a reduction in major bleeding events [odds ratio (OR) 0.63, 95% confidence interval (CI) 0.42–0.95; p = 0.03, I 2 = 71] and comparable definite/probable stent thrombosis, all-cause mortality, cardiovascular mortality, myocardial infarction (MI), and ischemic stroke, compared with long-DAPT. Single antiplatelet therapy (SAPT) with aspirin was comparable to SAPT with P2Y12 inhibitor, with no treatment-by-subgroup interaction for major bleeding events (p-interaction = 0.27). In studies including patients presenting with MI, a trend of more frequent MI was noted in the short-DAPT arm (OR 1.25, 95% CI 0.98–1.59; p = 0.07; I 2 = 0). In a sensitivity analysis comparing 3- and 12-month DAPT, the 3-month DAPT strategy was associated with a higher risk of ischemic stroke (OR 2.37, 95% CI 1.15–4.87; p = 0.02, I 2 = 0%). Conclusion Short-term DAPT after PCI in HBR patients was associated a reduction in major bleeding events and similar ischemic outcomes. However, a higher risk of ischemic stroke and MI at 1 year of follow-up was seen in some subsets. more...

Abstract The lodging industry is an important source of national income being a main ingredient for tourism, also, as it contributes to solving the problem of unemployment. Predominantly, the lodging sector supports the balance of payments. At this place, a reading to “Alpha”; “Beta”; “Gamma”; “Delta”; and “Omicron”—Corona Variants reflects significant implications. On the one hand, there exists a challenge to Egypt’s hotel industry following the pandemic attack; on the other hand, and by considering the case of relaxing the international restrictions, the international tourism is expected to recoup. Egypt has relatively benefited in the first half of 2021 when many Europeans escaped the lower temperature in winter in their homelands opting warm holiday destinations. The primary readings also expect the continuity in this increase. The macro-trend right here considers the shift in favor of a thoroughly managed risk with enhanced pliability, adopting a view of creating value, and increasing trust, while the micro-management considerations signify enhancing the sustainability of the sector performance by means of raising lodging hygienic attributes and approving innovative approaches. Luxury hotel, led by their executives, can significantly contribute and steer this trend mainly for cost tolerance estimates and for the potentiality of cost recovery. This study addresses the ways with which “Alpha”; “Beta”; “Gamma”; “Delta”; and “Omicron” Corona variants necessitate more sustained hotel management practices and helps to establish knowledge and techniques that assist the lodging sector in Egypt. A further goal of this study is to endorse the hotel managers’ impact on counteracting the upshots of Variants’ progressions. The study is multifaceted in “scale” and “scope,” since it is based on an investigation of the effects of large-scale public health emergencies in the lodging sector. A qualitative methodology is set via conducting online and offline surveys to examine how the pandemic affect lodging sector operations, and how far Egypt’s luxury hotel managers tolerate both burdens of managing the emergency and creating sustained plans for recovery. Henceforward, the study constructs a frame for sustained lodging sector recovery. The paper’s scientific value is that it functionalizes a broad inductive method of qualitative research in which a merge of the interpretation of management enabling factors and health enabling trends in the lodging sector and luxury hotel managers’ responses obtained through exploratory qualitative research is utilized to cluster the managers’ responses and thus generalize the information in the setting of a Framework for Health Crisis Management in the lodging sector. The Framework for Health Crisis Management also adds value as it amalgamates a combination of input factors of management and health enablers; a transformation via the change actions; an output of containing the effect of the pandemic through the enhanced managers’ capabilities and hotels’ health hygiene readiness; and an outcome implication of medium-term stabilizing effects that leads to an impact effect of sustained lodging sector recovery. This configures an input, transform (actions), output, outcome, and impact result chain. Egypt produces more rice straw than any other nation, and this residue is currently discarded in untreated landfills and open fields, causing environmental damage in the country. Furthermore, eggshells, a waste product used in bakeries, restaurants, and poultry farms, were typically disposed of in landfills, posing health hazards. So, grinding eggshells into nanoparticles, known as nano eggshell powder (NEP), increases its activity. In addition, to its high calcium content, NEP may be used with pozzolanic substances like rice straw ash (RSA), which have a lower calcium concentration. This study examined the characteristics of ultra-high-performance concrete (UHPC) produced using RSA and NEP eco-friendly components within partial cement substitution. The ratios of RSA to cement were 0 %, 10 %, 20 %, and 30 %, while the NEP quantities were 2 %, 4 %, and 6 % of the total binder. The results indicate that combining NEP and RSA as pozzolanic materials can improve concrete characteristics while decreasing cement content and reducing environmental contamination. Using RSA and NEP as substitutes for cement has also improved the material's mechanical characteristics, durability, and dry shrinkage. more...

It is known that sperm and seminal plasma (SP) affect uterine immunity. In cattle, artificial insemination enables breeding by depositing frozen and largely diluted sperm with a negligible amount of SP into the uterus. Thus, the present study focused on the impact of frozen-thawed sperm on bovine uterine immunity. We have previously shown that in the bovine uterus, sperm swim smoothly over the luminal epithelium and some sperm interact with uterine glands to induce a weak inflammatory response mainly via the endometrial Toll-like receptor 2 (TLR2) signaling. However, the process by which sperm is encountered in the uterine glands is not completely clear. The present study intended to evaluate the role of sperm-TLR2 in sperm-uterine mucus penetration for reaching the glandular epithelium to induce the uterine immune response. To activate and block sperm-TLR2, they were treated with TLR2 agonist and antagonist, respectively. TLR2 activation enhanced sperm hyperactivation and improved its capacity to penetrate the artificial viscoelastic fluid and estrous-uterine-mucus. In contrast, TLR2-blocked sperm showed completely opposite effects. It is noteworthy, that the TLR2-activated sperm that penetrated the uterine mucus exhibited increased motile activity with hyperactivation. In the sperm-endometrial ex-vivo model, a greater amount of TLR2-activated sperm entered the uterine glands with an immune response, which was seen as the upregulation of mRNA expression for TNFA, IL1B, IL8, PGES, and TLR2 similar to those in control sperm. On the other hand, a lesser amount of TLR2-blocked sperm entered the uterine glands and weakened the sperm-induced increase only in PGES, suggesting that penetration of a certain number of sperm in the uterine gland is necessary enough to trigger the inflammatory response. Altogether, the present findings indicate that activation of sperm-TLR2 promotes their hyperactivation and mucus penetration with greater motility, allowing them to enter into the uterine glands more. This further suggests that the hyperactivated sperm contributes to triggering the pro-inflammatory cascade partly via TLR2 in the uterus. more...

Abstract Purpose The theme of the St. Gallen International Breast Cancer Conference 2021 held virtually for the first time, due to the COVID-19 pandemic, was on tailoring therapies for patients with early breast cancer. A monkey survey that included an Egyptian Panel voted on most of the questions of the original St. Gallen consensus, and some added new questions most relevant to oncology practice in the country, to be able to compare voting results that reflect differences in breast cancer management and decision making. Methods The panel included 74 Egyptian scientists from different oncology specialties. Management issues including controversial diagnostic and therapeutic interventions were prepared by a small committee and then projected using the online monkey survey website: https://www.surveymonkey.com . The survey included 130 questions. Results were then analyzed, tabulated, and compared to the voting results of the original St. Gallen consensus. Results and conclusions Voting questions and resulting percentages of answers from the Egyptian panel were summarized. There was no consensus between the Egyptian and the original St. Gallen panels on 28/130 statements. They mostly included genetic and pathologic aspects, specifically the routine use of gene signature assays and a few queries involving surgical, radiotherapeutic, and systemic interventions. Probably, available resources and healthcare system differences in Egypt compared to European and the USA were the cause of these differences. This would also be applicable to other low- and low-middle-income healthcare scenarios present in many countries, especially with the present constraints of the COVID-19 pandemic. more...

Abstract The purpose of this research is to build and evaluate a theoretical model capable of forecasting public participation in mega events. This study predicts event tourism participation using a mixed behaviour model based on a trust model and a deliberate actions model. Using data from 261 local and international visitors, structural equation modelling was engaged to examine the study hypotheses. According to the study's results, observed benefits were positively connected with people’s perspective, but observed obstacles were negatively associated, and observed benefits had a positive influence on interactive mechanism and observed obstacles had a negative influence on interactive mechanism. Additionally, the study's results suggested that individuals' intention to participate in mega events was positively influenced by their observed benefits, perspective, interactive mechanism, and normative beliefs and negatively influenced by observed obstacles. Finally, we perceived that observed risk significantly moderated the associations between observed benefits, observed obstacles and perspective, observed benefits, observed obstacles and interactive, lastly, observed benefits, observed obstacles, perspective, interactive mechanism, normative beliefs and intention to participate in events. Recently, we reported that sperm induce cluster of differentiation 44 (CD44) expression and Toll-like receptor 2 (TLR2)-mediated inflammatory response in bovine uterus. In the present study, we hypothesized that the interaction between CD44 of bovine endometrial epithelial cells (BEECs) and hyaluronan (HA) affects sperm attachment and thereby enhancing TLR2-mediated inflammation. To test our hypothesis, at first, in-silico approaches were employed to define the binding affinity of HA for CD44 and TLR2. Further, an in-vitro experiment using the sperm-BEECs co-culture model was applied to investigate the effect of HA on sperm attachment and inflammatory response. Here, low molecular weight (LMW) HA at different concentrations (0, 0.1, 1, or 10 µg/mL) was incubated with BEECs for 2 h followed by the co-culture without- or with non-capacitated washed sperm (106/ml) for additional 3 h was performed. The present in-silico model clarified that CD44 is a high-affinity receptor for HA. Moreover, TLR2 interactions with HA oligomer (4- and 8-mers) target a different subdomain (h-bonds) compared to TLR2-agonist (PAM3) which targets a central hydrophobic pocket. However, the interaction of LMW HA (32-mers) with TLR2 revealed no stability of HA at any pocket of TLR2. Notably, the immunofluorescence analysis revealed the HA localization in both endometrial stroma and epithelia of ex-vivo endometrial explant. Moreover, ELISA showed significant levels of HA in BEECs culture media. Importantly, BEECs pretreatment with HA prior to sperm exposure increased the number of attached sperm to BEECs, and upregulated the transcriptional levels of pro-inflammatory genes (TNFA, IL-1B, IL-8, and PGES) in BEECs in response to sperm. However, BEECs treated with HA only (no sperm exposure) did not show any significant effect on the transcript abundance of pro-inflammatory genes when compared to the non-treated BEECs. Toll-like receptor 2 (TLR2) signaling pathway is involved in the sperm-triggered uterine inflammatory response at insemination, but its precise mechanism at molecular-level remains unknown. According to the ligand specificity, TLR2 forms a heterodimer with TLR1 or TLR6 as an initial step to mediate intracellular signaling, leading to a specific type of immune response. Hence, the present study aimed to identify the active TLR2 heterodimer (TLR2/1 or TLR2/6) that is involved in sperm-uterine immune crosstalk in bovine using various models. First, in-vitro (bovine endometrial epithelial cells, BEECs) and ex-vivo (bovine uterine explant) models were employed to test different TLR2 dimerization pathways in endometrial epithelia after exposure to sperm or TLR2 agonists; PAM3 (TLR2/1 agonist), and PAM2 (TLR2/6 agonist). Additionally, in-silico approaches were performed to confirm the dimer stability using de novo protein structure prediction model for bovine TLRs. The in-vitro approach revealed that sperm triggered the mRNA and protein expression of TLR1 and TLR2 but not TLR6 in BEECs. Moreover, this model disclosed that activation of TLR2/6 heterodimer, triggers a much stronger inflammatory response than TLR2/1 and sperm in bovine uterine epithelia. In the ex-vivo model that mimics the intact uterine tissue at insemination, sperm also induced the protein expression of both TLR1 and TLR2, but not TLR6, in bovine endometrium, particularly in uterine glands. Importantly, PAM3 and sperm induced similar and low mRNA expression of pro-inflammatory cytokines and TNFA protein to a lesser extent than PAM2 in endometrial epithelia. This implied that sperm might trigger a weak inflammatory response via TLR2/TLR1 activation which is similar to that of PAM3. Additionally, the in-silico analyses showed that the existence of bridging ligands is essential for heterodimer stability in bovine TLR2 with either TLR1 or TLR6. Altogether, the present findings revealed that sperm utilize TLR2/1, but not TLR2/6, heterodimerization to trigger a weak physiological inflammatory response in the bovine uterus. Background. Ocular surface disease (OSD) is a multifactorial and highly frequent problem. Inadequate or unstable tear film is the main cause, which leads to visual impairments. One of the primary causes of OSD is meibomian gland dysfunction (MGD), with a prevalence of 3.5 to 70%. The aim of this work was to compare the efficacy of azithromycin topical eye drops versus oral doxycycline in MGD individuals. Methods. This prospective comparative cohort research was carried out on 56 patients of both sexes of any age with symptomatic MGD. Randomly, patients were classified into two equal groups: Group 1 was treated twice daily for 4 weeks with topical azithromycin 1% eye drops, while group 2 received oral doxycycline 100 mg capsules twice daily for 4 weeks. Results. In the 1st follow-up, there was a significant difference between the studied groups in pain and discomfort degree (P value = 0.024) as group 1 showed a higher number of patients with a mild pain degree (P value = 0.013) while group 2 showed a higher number of patients with a severe pain degree (P value = 0.022). There was an insignificant difference between the studied groups in moderate pain degree and lid margin telangiectasia. Conjunctivitis, frothy discharge, and meniscus floaters were significantly higher in group 2 than in group 1 (P value = 0.013, 0.028, and 0.031, respectively). In group 1, the break-up time test was significantly higher than in group 2 (P value = 0.023). In the 2nd follow up, in group 2 only meniscus floaters were significantly higher than in group 1 (P value = 0.044), while in group 1 break-up time test was significantly higher than in group 2 (P value = 0.029). Otherwise, there is no significant difference between both the groups. Conclusions. Meibomian gland dysfunction (MGD) could be treated effectively with oral doxycycline and topical azithromycin by improving symptoms, clinical signs, and stabilization of tear film. Moreover, the topical azithromycin group seemed to be superior over the oral doxycycline group in improving the quality of tear film in the short term, having fewer side effects, more compliance, and better tolerability. more...

Objective: To identify the key contributors to postoperative mortality in patients undergoing orthotopic heart transplantation (OHT) for late Fontan failure. Methods: This retrospective review of failing Fontan patients who underwent OHT in our tertiary care center between 2007 and 2019 included adult patients with congenital heart disease and single ventricle physiology who were palliated with a Fontan circulation for >1 year. We excluded patients undergoing combined heart-liver transplantation. Results: The study cohort comprised 31 patients, including 18 males (58.1%), with a mean weight of 58.4 kg, median age at Fontan of 6.9 years (interquartile range [IQR], 2-38 years), and a median age at OHT of 27.1 years (IQR, 16.7-53.3). Almost all (93.5%) of the patients were in New York Heart Association class III-IV, and the majority (74.2%) were in Interagency Registry for Mechanically Assisted Circulatory Support class 3. Overall survival at 30 days, 1 year, and 5 years after OHT was 81%, 71%, and 67%, respectively. Major intraoperative bleeding was associated with increased mortality after OHT (odds ratio, 30; 95% confidence interval, 2.8-322; P = .002). Neither preoperative systemic ventricular function nor the development of primary graft dysfunction (PGD) was significantly associated with postoperative death. Nevertheless, PGD determined significant morbidity of this population. Conclusions: In our cohort, major intraoperative bleeding was the key factor associated with mortality after OHT for late Fontan failure. Novel strategies for the prevention and management of postoperative bleeding will improve outcomes in this group of patients. more...

High-Performance Translucent Concrete (HPTC) is an innovative energy-saving building material, as it is a high-quality translucent concrete with high workability, superior mechanical properties, and good light transmittance. This paper investigates the performance of HPTC incorporated with plastic optical fibers (POF), its fresh properties, mechanical properties, and light transmittance performance. The data presented here refer to specimens of 21 mixtures divided into six main groups, the latter group upgraded to be ultra-high performance translucent concrete (UHPTC). Besides fresh concrete tests, bulk density, all-ages compressive strength, splitting tensile strength, flexural strength, bond strength, modulus of elasticity strength, and light transmittance as well as relations between light transmittance and mechanical properties were investigated in the HPTC hardened phase. Although the HPTC mechanical properties increase with increasing POF volume ratios and diameters, the light transmittance significantly increases with increasing POF volume ratios. In this paper, HPTC performs up to 21.35% light transmittance for specimens with a volume ratio of 4% POF and 2 mm POF diameter as well as good mechanical properties such as compressive strength of 54.30 MPa at 91 days. In addition, UHPTC performs up to 154.2 MPa compressive strength, 13.8 MPa tensile strength, 23 MPa flexural strength, 25.7 MPa bond strength, and 49.71 GPa modulus of elasticity, as well as 10.31 % light transmittance. more...

Abstract Over the past decade, financial inclusion has been a trending topic and key priority in developing countries seeking to build a resilient financial sector and pursuing economic growth. Most of the recently launched financial inclusion initiatives in Egypt, especially those aligned with the 2030 sustainability strategy, have targeted marginalized and excluded individuals. Only a few have addressed the financial inclusion of small- and medium-sized enterprises (SMEs). Accordingly, this paper aims to identify the main pillars of financial inclusion for SMEs. In keeping up with the mainstream literature, it introduces a number of financial inclusion determinants designed to attract SMEs. They include supply-side determinants such as access to financial services and marketing awareness campaigns, which act as tools to segment financial services and market their benefits to SMEs, and demand-side determinants, which involve the use of financial services. Finally, there is an assessment of the macroeconomic risks to investors and SMEs. The researchers’ methodology was based on first deriving a novel dataset from responses to a questionnaire addressing bankers who manage SME portfolios, second analyzing the dataset through descriptive and inferential statistics and third undertaking a twofold econometric estimation. The econometric estimations started with principal component analysis (PCA) and proceeded to a logistic regression to determine the significant variables pertinent to increasing the financial inclusion of SMEs. The PCA suggested three main pillars determining financial inclusion. They are integrated marketing tools, which increase SMEs’ awareness of and access to the most sophisticated banking services, usage of banking services, and assessment of the macroeconomic risks that would prevent SMEs from gaining access to financial services. Despite the global decline in marine species biodiversity, relatively few countries have enacted national endangered and threatened species legislation. Tailoring an adequate legislative framework with clear objectives and regulations consistent with the available scientific evidence is fundamental for the effective conservation of marine endangered and threatened species. This paper analyzes the legal framework and current institutional tools for the conservation of marine endangered and threatened species in the Russian Federation. In this regard, important legislative tools include federal laws, as well as internationally binding signed agreements, among others, such as the Convention on International Trade in Endangered Species of Wild Fauna and Flora (CITES) and the Convention on Wetlands of International Importance Especially as Waterfowl Habitat (Ramsar). A strategy and an action plan for the conservation of biological diversity were also developed. Besides, the most important tool for the conservation and protection of marine endangered and threatened species is the Red Book of the Russian Federation (RBRF) and other regional Red Books. Responsibility for causing harm to the species listed in the RBRF and their habitat is specified in the code of administrative offenses and the criminal code of the Russian Federation. Finally, conclusions and identified gaps were highlighted in the last section, including, among other things, that legislation is still limited in how it takes the impacts of climate change into account. Such type of study is highly recommended, considering the relatively few number of papers dedicated to the study of the impact and/or implications of the conservation tools and strategies mentioned in this paper on the status of the marine endangered and threatened species. more...

Introduction The aim of the study was to evaluate whether physical exercise (PE) in addition to extracorporeal shockwave therapy (ESWT) is more effective in improving erectile function as compared to ESWT and PE alone in diabetic patients with erectile dysfunction (ED). Material and methods Forty-five patients with type 2 diabetes mellitus (DM) and ED were divided into three equal groups: group 1 (ESWT group) received treatment with ESWT twice weekly for 6 weeks, comprising 3000 shockwaves at an energy density of 0.25 mJ/mm2 and an emission frequency of 6 Hz; group 2 (PE group) received treatment with physical exercise three times per week for 12 weeks; and group 3 (combined group) was treated using physical exercise in the form of the program followed by the PE group, plus ESWT in the form of the same parameter and protocol as that of the ESWT group. Treatment outcomes were measured by International Index of Erectile Function-5 (IIEF-5) score variations recorded at 4 and 12 weeks after the end of treatment with respect to the baseline. Results The mean IIEF-5 scores significantly improved in all groups at the 4-week follow-up without intergroup differences. At the 12-week follow-up, the mean IIEF-5 improvement and durability were significantly higher among patients in combined groups. Conclusions The combined approach of ESWT and PE provides significant advantages in erectile dysfunction improvement and durability as compared to ESWT or PE alone in diabetic patients with ED. more...

Abstract Background The benefit and safety of percutaneous coronary intervention (PCI) to chronic total occlusions (CTO) in patients with low and mid-range left ventricular ejection fraction (LVEF) continue to be evidence limited. The aim of our study was to investigate the impact of LVEF in patients undergoing CTO PCI and to evaluate the mid-term clinical outcome of those with low and mid-range LVEF. We assessed the periprocedural and mid-term outcomes of 75 patients undergoing CTO PCIs according to pre-procedural LVEF: group (N), ≥ 50% (normal, nom.= 25); group (M), 40-49% (mid-range, nom.= 25); and group (L), < 40% (low, nom.= 25); within 6 months of follow-up. Results The prevalence of DM and chronic kidney disease (CKD) was significantly higher in low LVEF group (60%, p = 0.04 and 48%, p = 0.01 respectively). Apart from significantly lower contrast volume in patients with low LVEF (p = 0.04), there was no significant difference between the three groups regarding the procedural time, SYNTAX score and J-CTO score. We noticed similar procedural success in the three groups (88% vs. 84% vs. 76%, p = 0.521). LVEF category failed to predict procedural success (OR = 0.652, p = 0.268). There was a highly significant improvement in angina 6 months following intervention in normal LVEF group (p value < 0.001). Grade of dyspnea significantly improved 6 months following intervention in mid-range LVEF and low LVEF groups (p value = 0.04 and 0.031 respectively). There was no significant difference between the three groups regarding the reported MACCEs (12% vs. 16% vs. 28%, p = 0.268). Conclusion CTO PCI represents an efficient and safe strategy in patients with low and mid-range LVEFs. Wild migratory birds have the capability to spread avian influenza virus (AIV) over long distances as well as transmit the virus to domestic birds. In this study, swab and tissue samples were obtained from 190 migratory birds during close surveillance in Egypt in response to the recent outbreaks of the highly pathogenic avian influenza (HPAI) H5N1 virus. The collected samples were tested for a variety of AIV subtypes (H5N1, H9N2, H5N8, and H6N2) as well as other pathogens such as NDV, IBV, ILT, IBDV, and WNV. Among all of the tested samples, the HPAI H5N1 virus was found in six samples; the other samples were found to be negative for all of the tested pathogens. The Egyptian HPAI H5N1 strains shared genetic traits with the HPAI H5N1 strains that are currently being reported in Europe, North America, Asia, and Africa in 2021–2022. Whole genome sequencing revealed markers associated with mammalian adaption and virulence traits among different gene segments, similar to those found in HPAI H5N1 strains detected in Europe and Africa. The detection of the HPAI H5N1 strain of clade 2.3.4.4b in wild birds in Egypt underlines the risk of the introduction of this strain into the local poultry population. Abstract Background Avian influenza viruses (AIVs) have been identified from more than 100 different species of wild birds around the globe. Wild migratory birds can act as potential spreaders for AIVs to domestic birds between different countries. Egypt is situated on important migratory flyways for wild birds between different continents. While much is known about circulation of zoonotic potential H5N1 and H9N2 AIVs in domestic poultry in Egypt, little is known about the pivotal role of migratory birds in the maintenance and transmission of the viruses in Egypt. Methods Targeted AIV surveillance has been conducted in 2017 in different wetlands areas in Northern and Eastern Egypt. Results AIV of subtype H5 was detected in two bird species. In addition, a novel reassortant strain of the H6N2 subtype was identified which reveals the continuous risk of new influenza virus(es) introduction into Egypt. This novel virus possesses a reassortant pattern originating from different AIV gene pools. Conclusions Intervention control strategies should be performed to minimize the possible contact of domestic birds with wild birds to lower the risk of virus transmission at this interface. In addition, constant monitoring of AIVs in migratory birds is essential in the early detection of influenza virus introduction into Egypt. more...

Abstract Background To evaluate corneal biomechanical properties by the Ocular Response Analyzer (ORA) in non keratoconic patients underwent penetrating keratoplasty (PK). Methods Corneal hysteresis (CH), corneal resistance factor (CRF), Goldmann- correlated intraocular pressure (IOPg), cornea-compensated IOP (IOPcc) using the ORA, and central graft thickness (CGT) were measured in 30 eyes at least two years after penetrating keratoplasty for non keratoconic indications. IOP using the Goldmann applanation tonometer (GAT) was also obtained after compensation for graft thickness and astigmatism. Results The mean age of patients was 33.1 ± 10.13 years; indications for PK were herpetic corneal scar (53.3%), corneal stromal dystrophy (23.3%), traumatic corneal opacity (10%), chemical corneal opacity (6.7%), and Fuchs endothelial dystrophy (6.7%). Mean CH and CRF were 8.52 ± 1.81 mmHg, and 8.56 ± 1.59 mmHg, respectively. Mean CGT was 532.43 ± 30 μm. Utilizing nanotechnology for the production of nano cement may be considered as an innovative potential approach that can be reached via increasing the specific surface area of cement. Van der Waals forces are the main reason behind the agglomeration of cement particles in concrete. The balance between repulsive and attractive forces of cement particles has a profound effect on concrete properties. The present study focuses on the effect of ultrasonic processing at relatively low frequency on the dispersion of cement particles and investigating the effectiveness of such approach in increasing the sub-nano metric cement content leading to increasing cement reactivity. A qualitative analysis was conducted with a view to investigate the sonication effect on the mechanical properties of cement. In addition micro-structural analyses were conducted to understand the surface morphology and microstructure of cement composites using particle size distribution, scanning electron microscope, Thermo-gravimetric analysis, X-ray diffraction and atomic force microscopy tests. The results of the investigational study showed that the mechanical, and microstructure of cement pastes can be remarkably improved by subjecting the cement slurry to an optimum time of sonication. The proposed method helped in achieving specific surface area of 138900 m2/kg with sub-nano metric cement particles of 80% and gain in compressive strength of about 27%. The microstructural analysis performed revealed that the proposed sonication method had influenced the micro structure of the cement matrix by increasing the reactivity of the cement particles as a result of their transfer to the nano level. Illegal, unreported and unregulated (IUU) fishing is recognised as one of the largest threats to the sustainability of the world’s fisheries. The fight against IUU fishing activities has recently become a high priority in the international fisheries management agenda. While a number of academic contributions have sought to improve the understanding of the problem, data are still limited, especially in developing countries. In the absence of any information in this regard in Egypt, this contribution is the first attempt to report the available information about cases of Egyptian fishing vessels detained for fishing illegally in neighbouring countries, including the Mediterranean and Red Seas. The highest number of cases was observed in 2015 (7 cases) followed by 2010, 2013, 2016 and 2020 (each with 5 cases). The highest number of detained vessels by far was observed in 2020 (44 vessels) followed by 2015 (19 vessels). Tunisia was the country with the highest number of cases (17 cases) followed by Libya (10 cases), while Somalia detained the highest number of vessels (42 vessels) followed by Libya (22 vessels). The results shed light on the problems that led to this situation, starting with depleted fisheries, a poor monitoring system and a difficult economic situation in the fishing sector. more...

The highly pathogenic avian influenza (HPAI) H5N8 virus was first detected in Egypt in late 2016. Since then, the virus has spread rapidly among different poultry sectors, becoming the dominant HPAI H5 subtype reported in Egypt. Different genotypes of the HPAI H5N8 virus were reported in Egypt; however, the geographic patterns and molecular evolution of the Egyptian HPAI H5N8 viruses are still unclear. Here, extensive epidemiological surveillance was conducted, including more than half a million samples collected from different poultry sectors (farms/backyards/live bird markets) from all governorates in Egypt during 2019–2021. In addition, genetic characterization and evolutionary analyses were performed using 47 selected positive H5N8 isolates obtained during the same period. The result of the conducted surveillance showed that HPAI H5N8 viruses of clade 2.3.4.4b continue to circulate in different locations in Egypt, with an obvious seasonal pattern, and no further detection of the HPAI H5N1 virus of clade 2.2.1.2 was observed in the poultry population during 2019–2021. In addition, phylogenetic and Bayesian analyses revealed that two major genotypes (G5 and G6) of HPAI H5N8 viruses were continually expanding among the poultry sectors in Egypt. Notably, molecular dating analysis suggested that the Egyptian HPAI H5N8 virus is the potential ancestral viruses of the European H5N8 viruses of 2020–2021. In summary, the data of this study highlight the current epidemiology, diversity, and evolution of HPAI H5N8 viruses in Egypt and call for continuous monitoring of the genetic features of the avian influenza viruses in Egypt. more...

In the presented study, we developed a nanogold lateral glow immunoassay-based technique (LFI-COVID-19 antigen test) for the detection of SARS-CoV-2 nucleocapsid proteins; the developed LFI-COVID-19 Ag test has been tested for limit of detection (LOD), cross-reactivity and interfering substances, and performance. It was found that the performance of the developed LFI-COVID-19 antigen test when it was evaluated by RT-qPCR indicated 95, 98, and 97% for sensitivity, specificity and accuracy, respectively. This complies with the WHO guidelines. It was concluded that the developed LFI-COVID-19 antigen test is a point of care and an alternative approach to current laboratory methods, especially RT-qPCR. It provides an easy, rapid (within 20 min), and on-site diagnostic tool for COVID-19 infection, and it is a cheap test if it is manufactured on a large scale for commercial use. more...

Background. Intrastromal corneal ring segments are widely adopted for keratoconus management. However, the complete ring (Myoring) was proposed to be superior in advanced cases. Myoring can be implanted either via femtoassisted or manual dissection techniques. A comparison between both techniques can delineate any differences in the outcomes. Methods. This was a prospective interventional case series study. Sixty-four eyes with progressive advanced keratoconus were enrolled: 36 and 28 had femtoassisted or manual Myoring, respectively. Uncorrected visual acuity (UCVA), corrected distance visual acuity (CDVA), maximal keratometry (Kmax), spherical equivalent (SE) and corneal thinnest location were measured in all eyes preoperatively and at one, six, and 12 months postoperatively. Epi-off corneal cross-linking (CXL) was performed eight weeks after Myoring implantation for all cases. Results. Femtoassisted Myoring dissection significantly improved UCVA and CDVA from 0.1 ± 0.06 and 0.18 ± 0.1 preoperatively to 0.29 ± 0.08 and 0.43 ± 0.1 at 12 months. Also, manual technique similarly enhanced UCVA and CDVA from 0.11 ± 0.05 and 0.2 ± 0.1 preoperatively to 0.27 ± 0.2 and 0.4 ± 0.2 at 12 months. In terms of safety, while no cases of ring extrusion were encountered with the femtoassisted technique, six (21.4%) cases of extrusion were encountered in the manual group. Conclusion. Femtoassisted or manual Myoring technique followed by CXL is an effective choice for advanced progressive keratoconus. Although it did not reach a statistical significance, the high extrusion rate with manual dissection is a red flag to be considered. more...

The utilization of cellulose nano fibers (NCell) as reinforcement to cement composites may account for enhancing particle packing and decreasing the crack growth at the nano level. Besides that, their high specific surface area can improve the bond between the cement hydration products. nano silica particles can also influence the performance of fiber reinforced cement composites through increasing the calcium-silicate-hydrate gel content, as well as enhancing the fiber to matrix interface cohesion. The aim of this investigation is to evaluate the effects of different contents of nano silica particles (0, 1, 1.5, and 2% by weight of cement) on the mechanical properties, and microstructure of cement composites reinforced with various contents of nano cellulose fibers (0, 0.35, 0.55, and 0.75% by weight of cement). The results out of studying such nano systems revealed that the addition of the NCell particles separately, helped in improving the tensile strength by about twice the value of the control mix when appropriate content of NCell was incorporated (0.35%). Moreover, the microstructural analyses demonstrated the effectiveness of nano silica particles to modify the transition zone between the nano cellulose fiber and the cementitious matrix and subsequently allowing greater reinforcement efficiency especially when external compression or tensile forces were transferred to the composite. more...

Two prostanglandins (luprostiol, LUP, and dinoprost, DIN) and two ovulation-inducing agents (human Chorionic Gonadotropin, hCG, and deslorelin, DES) were evaluated for luteolysis and estrus induction, and for ovulation induction, respectively, in embryo donor jennies. Twenty-six fertile Andalusian jennies were used. In Experiment 1, jennies (n = 112 cycles) were randomly treated with either LUP or DIN after embryo flushing. In Experiment 2, donors (n = 84 cycles) were randomly treated with either hCG or DES to induce ovulation. No differences were found between prostaglandins for all variables studied (prostaglandin–ovulation interval (POI), interovulatory interval (IOI), embryo recovery rate (ERR), positive flushing rate (PFR) and embryo grade (EG)). The ovulation rate was similar for hCG and DES (60.9% vs. 78.7%). However, the interval to ovulation (ITO) was affected (62.61 ± 7.20 vs. 48.79 ± 2.69 h). None of the other variables studied (ERR, PFR and EG) were affected (p > 0.05), except for embryo quality (p = 0.009). In short, both prostaglandins evaluated are adequate to induce luteolysis and estrus. Both ovulation-inducing agents hastened ovulation, but DES seems to be more effective than hCG. Duck Hepatitis virus (DHV) causes great economic losses in waterfowl industry worldwide. 3D gene, lies in the P3 segment of the picornavirus genome, is highly conserved and is a non-structural polyprotein gene, its encoding protein contains a conserved domain termed RNA-dependentRNA polymerase which participates in the synthesis of virus RNA during virus replication. The current work describes the surveillance of DHV in 20 commercial duck farms in Egypt with a history of high mortality in 3 to 15 day-old young ducklings from 2014 to 2016. Clinical samples were examined by Real Time-Polymerase Chain Reaction assays followed by partial sequence analysis of the 3D gene of the positive samples. Histopathological examination of the liver from selected samples was also conducted. The overall positive rate was 50% (n = 10/20). All duck breeds (Pekin, Muscovy and Mallard) used in the study were found to be susceptible to the disease. Histopathological findings of the liver samples showed pronounced lesions including hepatocyte degeneration and necrosis. Also, apoptosis were observed and bile duct hyperplasia, together with varying degrees of inflammatory cell response and haemorrhage. We isolated highly pathogenic avian influenza virus (H5N8) of clade 2.3.4.4 from the common coot (Fulica atra) in Egypt, documenting its introduction into Africa through migratory birds. This virus has a close genetic relationship with subtype H5N8 viruses circulating in Europe. Enhanced surveillance to detect newly emerging viruses is warranted. more...

After January 25th 2011 Egypt witnessed political, economic and social instability leading to drastic consequences in the hospitality and tourism industry. Thus unstable situation reflected on the deteriorated occupancy percentages that led to declined profit margins, higher employee layoffs and degraded quality of product and services. The objectives of this research is to examine how the Egyptian hospitality properties manage this dilemma through their marketing practices, and to propose a new marketing mix model that adds new layers of depth to the traditional marketing mix model. A methodological framework was designed to help in the assessment process of management practices pertaining to marketing initiatives during times of crisis. Results indicated the presence of tactical elements that assembled the traditional marketing mix model in the investigated hotels. However, these elements are not effectively used and the interaction between them not appears very clear. Results also indicated that the new proposed model would help in providing a framework for the Egyptian hospitality industry to maintain their competitive position during crisis time and avoiding undesired situations for labour force and decline of companies׳ revenues. more...

Introduction: Cardiac surgery-associated acute kidney injury (CSA-AKI) remains a known complication, where postoperative (PO) Cystatin C (Cys C) has been shown to be an earlier marker than the relatively late appearing creatinine (Cr). We sought to evaluate the reliability of preoperative (pre-OP) Cys C as a predictor for CSA-AKI in patients with normal renal functions. Methodology: Our study included consecutive patients undergoing on-pump cardiac surgery from July 2011 to April 2012. Pre-OP and PO Cystatin C and renal profiles were compared in AKI (GP I) and non-AKI (GP II) patients. RIFLE and AKIN criteria were calculated at baseline and daily during the successive three PO days. Results: Out of 40 patients (16 males; mean age = 59 years), 20 developed AKI. Both Pre-and PO Cys C were significantly higher in GP I, and positively correlated with PO Cr. (r: 0.38 P; 0.01; r: 0.68, p 0.04 respectively). Using ROC curve, a cutoff value of 1.8 mg/l and 1.88 (sensitivity 50 and 80%; specificity 90 and 65%) for Pre-OP and PO Cys C respectively in predicting AKI. Multivariate analysis showed the Pre-OP Cys C and cardiopulmonary bypass time were independent predictors for AKI. Conclusion: In patients with apparently normal renal functions, preoperative Cys C may be a predictor of post cardiac surgery AKI. In those patients, especially diabetics, Cys C may uncover subtle nephropathy which makes them more prone to AKI posed by stresses of cardiac surgery. more...

Highly pathogenic avian influenza (HPAI) viruses continue to circulate worldwide, causing numerous outbreaks among bird species and severe public health concerns. H5N1 and H5N8 are the two most fundamental HPAI subtypes detected in birds in the last two decades. The two viruses may compete with each other while sharing the same host population and, thus, suppress the spread of one of the viruses. In this study, we performed a statistical analysis to investigate the temporal correlation of the HPAI H5N1 and HPAI H5N8 subtypes using globally reported data in 2015–2020. This was joined with an in-depth analysis using data generated via our national surveillance program in Egypt. A total of 6412 outbreaks were reported worldwide during this period, with 39% (2529) as H5N1 and 61% (3883) as H5N8. In Egypt, 65% of positive cases were found in backyards, while only 12% were found in farms and 23% in live bird markets. Overall, our findings depict a trade-off between the number of positive H5N1 and H5N8 samples around early 2017, which is suggestive of the potential replacement between the two subtypes. Further research is still required to elucidate the underpinning mechanisms of this competitive dynamic. This, in turn, will implicate the design of effective strategies for disease control. more...

Derzsy’s disease causes disastrous losses in domestic waterfowl farms. A genetically variant strain of Muscovy duck parvovirus (MDPV) and goose parvovirus (GPV) was named novel goose parvovirus (NGPV), which causes characteristic syndrome in young ducklings. The syndrome was clinically characterized by deformity in beaks and retarded growth, called short beaks and dwarfism syndrome (SBDS). Ten mule and pekin duck farms were investigated for parvovirus in three Egyptian provinces. Despite low recorded mortality rate (20%), morbidity rate was high (70%), but the economic losses were remarkable as a result of retarded growth and low performance. Isolation of NGPV was successful on primary cell culture of embryonated duck liver cells with a clear cytopathic effect. Partial gene sequence of the VP1 gene showed high amino acids identity among isolated strains and close identity with Chinese strains of NGPV, and low identity with classic GPV and MDPV strains. Objective: To evaluate the role of the detection of ischemia modified albumin (IMA) level in the differentiation between ischemic and hemorrhagic cerebrovascular stroke. Methods: Sixty elderly persons classified into three groups, 25 patients diagnosed with cerebral infarction, 15 patients diagnosed with cerebral hemorrhage, and 20 elderly healthy persons with matched age as control were enrolled in the study. IMA was measured using the available chemical method and computerized tomography (CT) was done for diagnosis of brain lesions. Results: IMA was significantly higher in the patient group than in the control group. There was a positive significant correlation between age, albumin with IMA, (P = 0.000 and 0.037 respectively). However there was no statistical significant difference between sex and diagnosis cross tabulation (0.51). It was found that, IMA was statistically higher in the infarction group than the hemorrhage group (P = 0.000) and IMD index was statistically higher in the infarction group than the hemorrhage group (P = 0.013). Conclusion: Our investigation in elderly patients suggests that IMA assay is a sensitive marker for early detection of ischemic and hemorrhagic stroke. more...

Introduction. The possible risk factors for chronic kidney disease in transplant recipients have not been thoroughly investigated after living-donor liver transplantation. Material and Methods. A retrospective cohort study of consecutive adults who underwent living-donor liver transplantation between May 2004 and October 2016, in a single center, was conducted. Kidney function was investigated successively for all the patients throughout the study period, with 12 months being the shortest follow-up. Postoperative renal dysfunction was defined in accordance with the Chronic Kidney Disease Epidemiology Collaboration criteria. The patients’ demographic data, preoperative and intraoperative parameters, and outcomes were recorded. A calcineurin inhibitor-based immunosuppressive regimen, either tacrolimus or cyclosporine, was used in all the patients. Results. Of the 413 patients included in the study, 33 (8%) who survived for ≥1 year experienced chronic kidney disease 1 year after living-donor liver transplantation. Twenty-seven variables were studied to compare between the patients with normal kidney functions and those who developed chronic kidney disease 1 year after living-donor liver transplantation. Univariate regression analysis for predicting the likelihood of chronic kidney disease at 1 year revealed that the following 4 variables were significant: operative time, P < 0.0005; intraoperative blood loss, P < 0.0005; preoperative renal impairment, P = 0.001; and graft-to-recipient weight ratio (as a negative predictor), P < 0.0005.
