Objectives: To compare the outcomes of bipolar Transurethral Enucleation Resection of the Prostate (TUERP) and simple retropubic prostatectomy in patients with prostate volumes larger than 80 cc. Patients and methods: A prospective randomized study included all patients amenable to surgeries for benign prostate hyperplasia (BPH) with prostate size over 80 cc at a tertiary care hospital between January 2020 to February 2022. Bipolar TUERP and Retropubic open prostatectomy techniques were compared regarding patients' demographics, intraoperative parameters, outcomes, and peri-operative complications. Results: Ninety patients were included in our study and randomly assigned to bipolar TUERP (Group 1 = 45 patients) and retropubic open prostatectomy (Group 2 = 45 patients). The TUERP group demonstrated significantly lower operative time (77 ± 11 minutes vs. 99 ± 14 minutes, p < 0.001), hemoglobin drop (median = 1.1 vs. 2.5, p < 0.001), and resected tissue weight (71 ± 6.6 cc vs. 84.5 ± 10.6 cc, p < 0.001). Postoperatively, the TUERP group demonstrated significantly lower catheter time (median = 2 vs. 7 days, p < 0.001) and less hospital stay. IPSS, Qmax, and patient satisfaction were better in the TUERP group within six months of surgery. We reported 90-day complications after TUERP in 13.3% of patients compared to 17.8% after retropubic prostatectomy, with a statistically insignificant difference. Urethral stricture predominated after TUERP, while blood transfusion dominated in retropubic prostatectomy. Conclusions: The present study found that TUERP had equivalent efficacy and safety to open retropubic prostatectomy for patients with BPH and prostate volumes > 80 ml.

The response of rice to salt stress (200 mmol/L NaCl) was investigated at the transcription level in Egyptian varieties Giza 177 (salt sensitive variety) and Giza 178 (salt tolerant variety). We applied a genome-wide RNA-Seq transcriptome study at 21-day-old seedlings of both varieties, exposed or not to salt stress for 24 h. Most differentially expressed genes (DEGs) between the two varieties in response to salt stress were related to the expression of genes active at the cell wall (CW) level, including wall modification, hemicellulose/cellulose synthesis and transcripts of the peroxidase family activated in response to oxidative stress/oxidation reduction, which were significantly more represented in Giza 178. Consistently, Gene Ontology (GO) analysis showed differentially expressed transcripts, involved in response to oxidative stress and chemical stimulus, directly implicated in salt stress response and up-regulated in Giza 178, as well as oxidoreductase, peroxidase and antioxidant activities. When the two varieties were directly compared in exposed or not to salt stress conditions, Giza 177 showed a higher number of differentially expressed and unique loci than Giza 178, including transposable elements (TE). However, Giza 178 showed a higher number of transcription factors (TF) expressed, mostly involving myeloblastosis (MYB) family members and bZIP elements, with annotated elements including zinc finger domain, kinase, expansin, cellulose, sucrose synthase, peroxidase precursor, dehalogenase-like hydrolase, and sodium/ calcium exchanger protein.

Objective The present study was conducted to investigate the potential effects of dietary supplemented propolis in two growing rabbit breeds on growth performance, immune response, blood parameters, carcass characteristics, and cecal microflora composition. Methods A total of 90 growing rabbits aged 6 weeks from two breeds (V-line and Jabali) were randomly allocated to 3 dietary propolis experimental treatments. The experimental treatments consisted of a 2×3 factorial arrangement with two rabbit breeds and three levels of dietary propolis supplementation (0, 250 mg/kg, and 500 mg/kg). Each sub-treatment has 15 rabbits. The experimental period lasted six weeks. User adoption and usage of end-to-end encryption tools is an ongoing research topic. A subset of such tools allows users to encrypt confidential emails, as well as manage their access control using features such as the expiration time, disabling forwarding, persistent protection, and watermarking. Previous studies have suggested that protective attitudes and behaviors could improve the adoption of new security technologies. Therefore, we conducted a user study on 19 participants to understand their perceptions of an email security tool and how they use it to manage access control to confidential information such as medical, tax, and employee information if sent via email. Our results showed that the participants’ first impression upon receiving an end-to-end encrypted email was that it looked suspicious, especially when received from an unknown person. After the participants were informed about the importance of the investigated tool, they were comfortable sharing medical, tax, and employee information via this tool. Regarding access control management of the three types of confidential information, the expiration time and disabling forwarding were most useful for the participants in preventing unauthorized and continued access. While the participants did not understand how the persistent protection feature worked, many still chose to use it, assuming it provided some extra layer of protection to confidential information and prevented unauthorized access. Watermarking was the least useful feature for the participants, as many were unsure of its usage. Our participants were concerned about data leaks from recipients’ devices if they set a longer expiration date, such as a year. We provide the practical implications of our findings.

Abstract. Concepts surrounding the mechanisms of thrombocytopenia in ITP have shifted from the traditional view of autoantibody mediated platelet destruction to more complex mechanisms in which impaired platelet production, T-cell-mediated effects, and disturbed cytokine profiles play a role. Interleukin 27 (IL-27) plays pleiotropic roles in immunomodulation and autoimmune diseases. We aimed to determine the level of IL-27 in patients with ITP and its relationship to patient and disease characteristics as well as disease chronicity and response to treatment. Sixty childrens with primary immune thrombocytopenia were consequetively enrolled in this study as well as 20 age and sex matched healthy controls. ITP patients had significantly higher levels of IL-27 than controls (770.6 and 373.8 pg/ml, respectively). Patients with acute ITP had the highest levels of IL-27 among patient groups, while patients in remission had the lowest IL-27 levels (860.1and 622.9 pg/ml, respectively). Patients who received IVIG and combined steroids plus IVIG had significantly higher IL-27 levels than others. Patients who received Eltrombopag had significantly lower IL-27 levels than others. IL-27 seems to play a role in pathogenesis of childhood ITP. IL-27 can be used as a predictor for disease occurrence as well as responsiveness to treatment.

The study quantified the effect of gibberellic acid (GA3) as a pre-flowering treatment for male parent Giza 178 R and the influence of male to female ratio (2R:10A, 2R:12A, 2R:14A and 2R:16A) between male (R) to female (A) for two Cytoplasmic Male Sterility (CMS) lines (‘IR69625’ and ‘G46’) on hybrid rice seed production. The main plots were occupied by CMS lines while; GA3 application for male parent Giza 178R were arranged in the sub plots and male to female ratio was arranged in the sub-sub plots. The results indicated that, the, duration of floret opening, angle of floret opening, filaments exsertion, filaments length, anther length, plant height and number of tiller hill-1 of male parent Giza 178R were significantly at 300 g GA3 ha-1 concentration. Plant height, panicle exsertion, panicle length, flag leaf angle and 1000-grain mass of CMS were not significantly affected by the GA3 application for male parent and male to female ratio, while, number of fertile panicles hill-1, panicle mass, seed set, seed yield and harvest index of CMS lines were highly significantly affected. The highest seed yield (2.880 and 2.950 t ha-1) was obtained by CMS line IR69625A using 300 g GA3 ha-1 of male parent Giza 178R with male to female ratio of 2R:14A during both seasons.

Privacy laws in South Asian countries are still at a nascent stage. Therefore, South Asian websites are susceptible to user privacy violation. This paper presents an assessment of website privacy policies from 10 sectors in the three largest South Asian economies, namely, India, Pakistan, and Bangladesh. Using a manual qualitative analysis on a dataset of 284 popular websites, we assessed the policies based on accessibility, readability, and compliance with 11 privacy principles. Our findings show that overall, the privacy statement accessibility, and privacy compliance of websites from the three countries is low especially in the education, healthcare, and government sectors. Readability is quite low for websites in all 10 sectors of the three countries. Privacy compliance in each country is the highest for the principles of data processing and third-party transfer, whereas it is the lowest for protection of children's data, data retention and portability. Indian websites performed comparatively better amongst the three countries on all three metrics, followed by Pakistan, and Bangladesh. Based on our results, we provide recommendations involving all stakeholders (i.e., website owners, privacy regulators, and users) to help improve privacy protection of user data in South Asia.

Semen cryopreservation is very important in animal agriculture to maximize the number of daughters of genetically superior males and to distribute the cryopreserved semen of good males all over the world. However, the freezing process generates some damage to sperm that reduce their fertilizing ability after thawing. Moreover, egg yolk, which is the most common animal-origin cryoprotectant used in semen dilution, is considered a source of biosecurity risk. In the current study, we aimed to compare the replacement of egg yolk in the extender by gum arabic (5%) along with supplementation with antioxidant cysteine or ascorbic acid on semen quality and freezability in Noemi rams in vitro. Semen from six rams were collected with an artificial vagina two times per week. Semen evaluation parameters such as color, volume, pH, general motility, percentage motility, concentration and cell viability ratio were assessed. Spermatozoa motility and concentration were estimated with the computer-assisted semen analysis system. The semen samples were frozen using a Tris extender containing either 15% egg yolk or 5% gum arabic. For antioxidant-supplemented extenders, cysteine or ascorbic acid was dissolved at concentrations of 0.10, 0.50 or 1.0 mM in egg yolk or gum arabic extender. The semen from each ejaculate of each ram were resuspended with a specific extender with glycerol (5%); the final volume after dilution was 1 mL semen to 4 mL extender. The samples were then cooled to 4 °C for 120 min, loaded into 0.5 mL straws and frozen in liquid nitrogen for 7 days. Supplementation of gum arabic or egg yolk extenders for ram semen with antioxidants such as cysteine or ascorbic acid has beneficial effects on semen quality after cold storage or cryopreservation. However, supplementation of a 5% gum arabic extender with cysteine at 0.5 or 1 mM concentration or ascorbic acid at 0.5 mM concentration improved the quality of spermatozoa postcryopreservation. It could be concluded that gum arabic is a good alternative for egg yolk in Noemi ram semen extenders. Antioxidants are necessary to support the addition of gum arabic to the extender to help the ram spermatozoa to survive freezing–thawing and oxidative stresses.

Abstract Background Acute hydrocephalus can cause neurological deterioration after aneurysmal subarachnoid hemorrhage (aSAH). Predicting which patient would require shunting is challenging. Methods This prospective study was conducted upon twenty patients who suffered acute hydrocephalus due to subarachnoid hemorrhage of ruptured aneurysms. Surgical or non-surgical management of hydrocephalus was conducted. Glasgow Coma scale (GCS) was assessed, and hydrocephalus was graded by bicaudate index. Fisher grade was determined from CT scan. Aneurysm site was determined by conventional or CT angiography. Either surgical clipping or endovascular coiling of aneurysms was performed. Results Initially, 3 (15%) patients had emergency CSF diversion on admission due to poor GCS on arrival. Initially, the remaining 17 patients were managed conservatively. Five patients did not require any intervention. Twelve patients had external ventricular drainage placement, 4 were weaned, and 8 failed weaning. High bicaudate index (> 0.2) correlated with shunting. Aneurysm site correlated well with shunting (ACoA or PCoA). Conclusions Patients with fair GCS can be managed conservatively. Any deterioration warrants shifting to CSF diversion. Higher bicaudate index will usually need CSF diversion. The value of Fisher carries no significant value. Aneurysm location (ACoA or PCoA) correlates with an increased incidence of ventriculoperitoneal shunt placement.

Abstract This paper reports on a study that examines the learning behaviors and characteristics of students in a mobile applications computer programming class that adopted a “flipped” learning style. By harvesting learning analytics data from a learning management system, we created visualizations of work intensity to explore temporal patterns of students’ behavior and then correlate them with the students’ performance. Findings indicate that low, medium, and high performing students tend to access learning materials late with work intensity spiking on the lecture day, specifically during the lecture session. While high and low performing students show no difference in temporal access to material, medium performing students demonstrate the greatest degree of vibrancy regarding course content material access. Further a discussion of implications and insights on procrastination in the context of flipped classrooms are included.

Thiamine (vitamin B1), a water-soluble vitamin, is an essential factor in cellular metabolism and fundamental cofactor in important biochemical cycles. Thiamine deficiency is a wellknown cause of neurological and cardiologic disorders, especially in patients with alcohol dependence. Recently, several researchers have studied the role of thiamine deficiency in critically ill patients and the link between thiamine supplementation and changes in lactate levels in septic shock patients. The role of thiamine in this group of patients is still unclear; however, thiamine supplementation does not cause toxic side effects or increase morbidity or mortality. In this review, we discuss the most common conditions associated with thiamine deficiency and the limited literature available on thiamine supplementation in critically ill patients.

Privacy compliance of the Middle East’s financial sector has been relatively unexplored. This paper evaluates the privacy compliance and readability of privacy statements for top banks and mobile money services in the Middle East. Our analysis shows that, overall, Middle Eastern banks have better privacy policy availability and language distribution, and are more privacy compliant compared to mobile money services. However, both the banks and mobile money services need to improve (1) compliance with the principles of children/adolescent’s data protection, accountability and enforcement, and data minimization/retention, and (2) privacy statement texts to be comprehensible for a reader with ~8 years of education or less.

The influence of subcutaneous injections of silver nanoparticles (AgNPs) on rabbit performance, hematological and biochemical parameters of blood, antioxidant status, and the residues of silver in meat and blood in two breeds (New Zealand White (NZW) and Jabali) of rabbits growing under high ambient temperature was evaluated. A total of 90 six-week-old rabbits (45 NZW and 45 Jabali) were randomly distributed into three equal treatment groups (control, 0.5 mg, and 1.0 mg AgNPs/kg body weight). The treated rabbits were injected twice a week for four consecutive weeks. The results revealed that AgNPs administration had no significant effect on average daily gain (ADG), feed intake, and feed conversion ratio (FCR). The NZW breed surpassed the Jabali breed in growth performance traits, carcass weight, dressing percentage, and cuts of mid parts and hind cuts. Administration of AgNPs had a significant effect on hematocrit (HCT) and platelet (PLT) values. Rabbits injected with AgNPs at a dose of 0.5 mg showed a lower plasma concentration of total cholesterol and triglycerides than that of control rabbits. The NZW breed had significantly low platelet, total cholesterol, and triglyceride values. Rabbits injected with 0.5 mg/kg BW had the lowest total antioxidant capacity and highest malondialdehyde (MDA) and glutathione peroxidase. The Ag residues were higher in blood than those in meat in treated rabbits. The local breed (Jabali) had significantly lower residues than the imported one (NZW) either in meat or in blood. However, the amount of accumulated silver in blood plasma and meat increased with increasing dose.

AbstractPrecision medicine allows a dramatic expansion of biological data, while there is still an urgent need to understand and insight the exact meaning of those data to human health and disease. This has led to an increasing wealth of data unanalyzed. The concept of precision medicine is about the customization of healthcare, with decisions and practices tailored to an individual patient based on their intrinsic biology in addition to clinical “signs and symptoms”. Construction of a standardized model for the integration of data from various platforms is the central mission of the ‘New Disease Management Model’. The model is helpful for the development of new taxonomy of diseases and subtypes, to personalize therapy based on patient genetic profiles. A rapid progression of precision therapy has been made recently. Clinical trials have shown the therapeutic efficacy of discovered and developed therapeutic agents has improved. However, next‐generation drugs would be designed for disease subtypes with more specificity, efficacy and lower toxicity.

Background: Elevated vitamin B12 (B12) levels are linked to an increased risk of cancers, including hematological malignancies. This study focuses on the relationship between elevated B12 and myeloproliferative neoplasms (MPNs): Polycythemia Vera (PV), Primary Myelofibrosis (MF), Essential Thrombocytosis (ET), and Chronic Myeloid Leukemia (CML). Elevated B12 in MPNs is believed to arise from increased transcobalamin I (TCI) secretion by proliferating leukocytes, leading to higher serum levels. B12 may serve as a diagnostic and prognostic biomarker for these conditions. However, its sensitivity, specificity, and cutoff levels are unclear. Aim: To assess the prevalence of high B12 levels in MPN patients, determine the median levels, identify a diagnostic cutoff, and evaluate the sensitivity and specificity of B12 as a marker. Methods: Data were retrieved from the National Center for Cancer Care and Research in Doha, Qatar, for MPN patients from January 2016 to December 2022. Results: A total of 467 patients were included: 232 with CML, 98 with PV, 88 with ET, and 50 with MF. The majority were male (66%) and of Asian origin (56%), with a median age of 48.7 years. CBC results showed median hemoglobin of 9.2 g/dL, WBC count of 73 x 10^3/uL, and platelet count of 531 x 10^3/uL. Elevated B12 levels were found in 95 patients (20%): 71% CML, 14% PV, 10% MF, and 5% ET. Extreme elevations were seen in 59 patients. The mean B12 level decreased from 747.3 ± 686.5 pg/mL before treatment to 397.9 ± 343.7 pg/mL after one year (p=0.01). Median levels were 458 pg/mL (718) before treatment and 301 pg/mL (229) after. In the extreme high B12 group, the mean was 1722 pg/mL before and 677 pg/mL after treatment. Conclusion: Elevated B12 levels are associated with disease activity in CML. However, their role as a reliable marker for disease monitoring remains uncertain, and further studies are needed to confirm their utility for CML progression. [ABSTRACT FROM AUTHOR]

OBJECTIVES CALGB140503/JCOG0802 RCTs comparing lobectomy with sublobar resection in stage IA NSCLC have confirmed the non-inferiority of segmentectomy. Additional insight is needed to improve preoperative work-up and intraoperative strategies to increase safety and promote the dissemination of minimally invasive segmentectomy (MIS). A Delphi panel study assessed the level of consensus among surgeons for the planning and management of MIS. METHODS Twenty-one expert lung surgeons represented academic institutions, major teaching hospitals, and community hospitals from Europe, North America, and Asia. A 3-round Delphi methodology was used to analyse the answers of each panellist. Recognizing that questions with fewer response options have a higher consensus probability due to limited variability, weighted consensus thresholds were modified based on the number of response options. RESULTS The 21 panellists responded to all 3 rounds of questions. Based on the most robust consensus (94.4%), 3D chest CT reconstructions are recommended only when planning complex segmentectomies. Surgeons should perform 3D reconstructions chest CT scans (consensus = 83.3%). The most effective and safest technique is image-guided VATS in a hybrid operating room (consensus = 83.3%). Dyes with intravenous administration are the safest technique for identifying the intersegmental plane during MIS (consensus = 72.2%). Augmented/mixed reality will probably not immediately help reduce perioperative complications (consensus = 72.2%). CONCLUSIONS This Delphi consensus supports 3D reconstructions and preoperative pulmonary nodule localization before complex MIS. These recommendations should be considered when allocating resources to improve MIS's safety and oncologic efficacy for patients with small, early-stage lung cancers. [ABSTRACT FROM AUTHOR]

Introduction: Chronic myeloproliferative neoplasms (MPNs) such as polycythaemia vera (PV) and essential thrombocytosis are associated with atherosclerosis and cardiovascular events, especially if JAK2 mutation is there. In rare cases, also spontaneous coronary artery dissection (SCAD) can be seen. Case Presentation: This case series describes two case reports of MPNs in which SCAD was found in both of them in addition to standard atherosclerosis. First one was about a 42-year-old man who was admitted for acute myocardial infarction (MI) and was later found to have PV and SCAD. The second one was about a 52-year-old man who was also admitted for acute MI and was later found to have SCAD and essential thrombocythemia. JAK2V617F missense mutation was found in both cases. Conclusion: MPNs with JAK2 mutations are associated with SCAD in addition to usual atherosclerosis. This association needs further research. [ABSTRACT FROM AUTHOR]

The presence of BRAF mutation in hematological malignancies, excluding Hairy cell leukemia, and its significance as a driver mutation in myeloid neoplasms (MNs) remains largely understudied. This research aims to evaluate patient characteristics and outcomes of BRAF-mutated MNs. Among a cohort of 6667 patients, 48 (0.7%) had BRAF-mutated MNs. Notably, three patients exhibited sole BRAF mutation, providing evidence supporting the hypothesis of BRAF's role as a driver mutation in MNs. In acute myeloid leukemia, the majority of patients had secondary acute myeloid leukemia, accompanied by poor-risk cytogenic and RAS pathway mutations. Although the acquisition of BRAF mutation during disease progression did not correlate with unfavorable outcomes, its clearance through chemotherapy or stem cell transplant exhibited favorable outcomes (median overall survival of 34.8 months versus 10.4 months, p = 0.047). Furthermore, G469A was the most frequently observed BRAF mutation, differing from solid tumors and hairy cell leukemia, where V600E mutations were predominant. [ABSTRACT FROM AUTHOR]

This literature review explores the pivotal role of brain–computer interface (BCI) technology, coupled with electroencephalogram (EEG) technology, in advancing rehabilitation for individuals with damaged muscles and motor systems. This study provides a comprehensive overview of recent developments in BCI and motor control for rehabilitation, emphasizing the integration of user-friendly technological support and robotic prosthetics powered by brain activity. This review critically examines the latest strides in BCI technology and its application in motor skill recovery. Special attention is given to prevalent EEG devices adaptable for BCI-driven rehabilitation. The study surveys significant contributions in the realm of machine learning-based and deep learning-based rehabilitation evaluation. The integration of BCI with EEG technology demonstrates promising outcomes for enhancing motor skills in rehabilitation. The study identifies key EEG devices suitable for BCI applications, discusses advancements in machine learning approaches for rehabilitation assessment, and highlights the emergence of novel robotic prosthetics powered by brain activity. Furthermore, it showcases successful case studies illustrating the practical implementation of BCI-driven rehabilitation techniques and their positive impact on diverse patient populations. This review serves as a cornerstone for informed decision-making in the field of BCI technology for rehabilitation. The results highlight BCI's diverse advantages, enhancing motor control and robotic integration. The findings highlight the potential of BCI in reshaping rehabilitation practices and offer insights and recommendations for future research directions.
