Abstract Background Hypertension is a major health problem globally and in India. Around 60% of people with hypertension in India are not aware that they have the condition. Less than 30% of individuals with hypertension are on treatment. Existing studies have indicated that community health workers (CHWs) can play a useful role in expanding the care for hypertension. Evaluations are needed to study the impact when an intervention is implemented by the government in its existing large-scale CHW programme to improve the identification, regular follow-up and medication adherence for hypertension. Methods Chhattisgarh state implemented a pilot intervention to improve screening and follow-up for hypertension by equipping Mitanin-CHWs to measure blood pressure (BP). The study design involved an intervention-group and a comparison-group of urban slum population. The survey covered 5974 individuals (30–79 years age) in intervention-group and 5131 in comparison-group. Multivariate analysis was conducted to find out the effect of intervention on the desired outcomes. Results In intervention-group, 80.2% of the individuals (30–79 years age) had been screened for hypertension whereas the proportion was 37.9% in comparison-group. For 47.0% of individuals in intervention-group, Mitanin CHW was the provider who measured BP for the first time. Around 16.3% of individuals in intervention-group and 9.5% in comparison-group had been diagnosed with hypertension. Around 85.9% of hypertension cases in intervention-group and 77.0% in comparison-group were on treatment. BP had been measured in preceding 30 days for 81.8% of hypertension-cases in intervention-group and 64.3% in comparison-group. Around 70.3% of hypertension-cases in intervention-group and 55.1% in comparison-group had taken their complete medication for last seven days. Multivariate analysis showed that CHW intervention was associated significantly with improvements in all the desired outcomes. Conclusion Equipping the CHWs to measure BP was effective in increasing the screening and identification of hypertension, regular measurement of BP of individuals with hypertension and the adherence to medication. This shows the potential if the one-million strong work-force of Accredited-Social-Health-Activists (ASHA) CHWs in India gets equipped for this role. Governments need to provide a stronger policy push to get this materialised.

Abstract Lateritic soil is a kind of residual soil widespread in tropical countries. This soil usually possesses acceptable engineering properties to be laid under the construction projects. However, it needs treatment for transportation infrastructure such as railway and road subgrade and embankment, particularly when it is in fine-grained form. Thus, cement, one of the very common stabiliser agents in soil stabilisation, was selected to study its influence on lateritic soil at macro- and micro-levels. In order to achieve this goal, UCS, durability, FESEM and EDX tests were conducted. The results obtained indicate that the UCS increase occurs with an increase in cement content and curing time. It was also found that the shear modulus increases with cement content and curing time. The durability test results disclosed that 3% cement is not enough for soil stabilisation when used for projects in the areas subjected to cyclic wetting-drying cycles. The durability test results revealed that the UCS decreased for specimens treated with 6% cement, while on the other hand, the UCS increased for samples treated with 9% and 12% cement. The FESEM results revealed that the soil micro-structure changed with the addition of cement and curing time. The EDX results presented the chemical elements change upon adding cement and increasing curing time. Overall, it was found that cement-stabilised residual soil can be used for road construction. However, the cement percentage needed to stabilise residual soil differs depending on the standards.

Numerous elements, such as the composition and characteristics of carbon nanomaterials, the composition and characteristics of the matrix material, moisture levels, temperature, and loading circumstances, influence the piezoresistive behavior of self-sensing cementitious composites. While some past research has explored the impact of some of these factors on the performance of self-sensing cementitious composites, additional investigations need to be conducted to delve into how loading conditions affect the sensitivity of self-sensing cement-stabilized composites. Therefore, this study explores the influences of various loading conditions (i.e., location of loading regarding the location of recording electrodes, and loading level) on the electromechanical performance of self-sensing cement-stabilized sand. To this end, firstly, the evaluation of the percolation threshold based on 10% cement-stabilized sand specimens containing various multiwall carbon nanotubes (MWCNTs) and graphene nanoplatelets (GNPs) was performed. Then, 10% cement-stabilized sand containing 4% MWCNTs/GNPs was tested under various cyclic compressive stresses. The results suggested that the distance between the loading area and the electrode location used for recording the electrical resistance significantly impacted the sensitivity of cement-stabilized sand. Optimal sensitivity was achieved when the electrodes were positioned directly beneath the loading area. Moreover, the study showed that the stress sensitivity of self-sensing cement-stabilized sand increased proportionally with the stress level. An examination through scanning electron microscopy (SEM) demonstrated that the loading condition influences the bridging characteristics of carbon nanomaterials in cement-stabilized sand, leading to diverse electromechanical behaviors emerging based on the loading condition. This study underscores the importance of considering specific parameters when designing self-sensing cement-stabilized sand for application in practical field use.

In deep-seated coal seams (greater than 250 m), a few Indian coal mines have been developed using the bord and pillar method up to 525 m depth. This paper presents a three-dimensional numerical simulation of a complete panel deliberating the distribution of mining-induced stress, factor of safety of pillar, stook and rib, plastic zone formation and roof deformation at higher depth. The induced stress on the rib was found to be 4.65 MPa, which is high enough to crush the rib easily. The pillar's safety factor (FoS) was estimated to be 2.43, sufficient enough to take the overlying load, whereas, for ribs, the FoS of 0.9 signified complete collapse within few days. The plastic zone extended up to 6.1 m in the immediate roof, demanding heavy support to control the roof.

Urothelial carcinoma and nephrolithiasis are a common cause of obstructive uropathy which can be relieved by percutaneous nephrostomy catheter placement. A rare, but known complication of this procedure is iatrogenic seeding of tumor cells along the nephrostomy tract. We describe a case of 68-year-old-female with cutaneous metastasis of high-grade urothelial carcinoma with seeding of tumor cells along the percutaneous nephrostomy catheter tract 8 months after the removal of the catheter. Given its severity, interventional radiologists should be mindful of the number of percutaneous access attempts, exchanges, and catheter manipulations in patients with urothelial carcinoma due to the risk of metastatic seeding along the percutaneous tract or to nearby tissues.

In this paper, we study the compact derivations on W* algebras. Let M be W*-algebra, let LS(M) be algebra of all measurable operators with M, it is show that the results in the maximum set of orthogonal predictions. We have found that W* algebra A contains the Center of a W* algebra ß and is either a commutative operation or properly infinite. We have considered derivations from W* algebra two-sided ideals.

The subgrade layer’s stability considerably influences the long-term performance of pavement systems. This study investigates the influence of lime as a traditional stabiliser and activated carbon with coir fibre (ACF) as waste materials and an environmentally friendly binder to stabilise lateritic subgrade soil. Experiments, including the one-dimensional consolidation and unconfined compressive strength (UCS) tests, have been conducted to investigate the geotechnical properties of stabilised soil in various percentages of additives 3%, 6%, 9%, and 12% lime and 1%, 2%, and 3% ACF. The results demonstrate that 3% ACF and 12% lime can significantly improve the strength parameters and decrease the void ratio and permeability in the stabilised soil. Furthermore, microstructural analysis was performed before and after stabilisation for optimum content. The microstructural analysis proves that AC and lime particles fill soil voids, and gel formation binds the soil particles in the stabilised soil matrix. The results show that 3% ACF stabilised soil is comparable with 12% lime in UCS value and decreasing void ratio. Furthermore, both are suitable for subgrade of low-volume road stability according to Malaysian standards.

We studied the clinical and epidemiologic features of an outbreak of Crimean-Congo hemorrhagic fever in Herat Province, Afghanistan. The study comprised 63 patients hospitalized in 2017. The overall case-fatality rate was 22.2%; fatal outcome was significantly associated with a negative IgM test result, longer prothrombin time, and nausea.
