Objectives: To investigate the alveolar bone morphology of the mandibular second and third molars in skeletal Class III patients from a buccolingual direction., Methods: Sixty skeletal Class III patients were recruited. The alveolar bone width, buccal cortical bone thickness and lingual cortical bone thickness were measured in five planes from mesial to distal and at five depths from gingival to root. The effects of the gender of the patients, the second molar lingual inclination and the third molar on alveolar bone width and cortical bone thickness were evaluated. To explore the effect of third molar extraction on alveolar bone morphology, the measurements before and after third molar extraction were compared., Results: The impacted third molar had significantly greater alveolar bone width and thicker buccal cortical bone at the cervical third of the molar, while the erupted third molar had greater alveolar bone width at the apical third. Three weeks after third molar extraction, these advantages would weaken owing to the reconstruction of the alveolar bone. Patients with lingually inclined molar were observed to own thicker lingual cortical bone. Males tended to have greater alveolar bone width, but no significant differences were shown in this study., Conclusions: The growth of the third molar and the second molar lingual inclination affect the alveolar bone morphology of the mandibular second and third molars significantly, but gender has trivial effects on the morphology. Purpose: To explore the risk factors for mesioangular and vertical impactions of the mandibular third molars and to construct a predictive model based on logistic regression analysis., Methods: Clinical data of 243 mandibular third molars collected from June 2021 to December 2023 at Tianjin Stomatology Hospital were classified into the eruption group and the impaction group, with the latter including mesioangular and vertical unilateral impactions. The clinical data were subjected to univariate analysis to screen for statistically significant factors, followed by multivariate analysis using logistic regression to further delineate risk factors for mandibular third molar impaction, with the construction of a nomogram for prediction.SPSS 27.0 software package was used for statistical analysis., Results: Totally 243 mandibular third molars were included, and 75 (30.86%) were in the eruption group and 168 (69.14%) in the impaction group. No significant difference was found between the groups regarding age, gender, number of tooth roots, Co-Go, Co-Cop, W2, W3, W4 and L (P＞0.05). Significant differences were observed between the eruption and impaction group concerning Nolla, L-6 missing, L-E missing, Co-Pog, Co-Go/Co-Pog, L6-MP, α and W1(P＜0.05). Multivariate regression analysis revealed that Nolla, absence of L-6, absence of L-E, Co-Pog, Co-Go/Co-Pog, L6-MP, α and W1 were independent risk factors for mesioangular and vertical impactions of the mandibular third molars (P＜0.05). The construction of nomogram demonstrated high predictive accuracy. Analysis of the receiver operating characteristic curve(ROC) indicated that the area under the curve(AUC) for the joint prediction of mesial and vertical impaction of the mandibular third molar by independent risk factors was 0.924, with a 95%CI of 0.887 to 0.960. The sensitivity was reported to be 86.9%, and the specificity was 86.7%., Conclusions: Nolla, absence of L-6, absence of L-E, Co-Pog, Co-Go/Co-Pog, L6-MP, α and W1 are major risk factors affecting the impaction of mandibular third molars. The use of logistic regression analysis and nomograms can effectively predict the risk of impaction, providing a scientific basis for clinical treatment.

Purpose: The aim of this study is to evaluate the relationship between the inclination/impaction depth and root proximity to the inferior alveolar canal/lingual plate on a millimeter scale using CBCT scans., Methods: 340 CBCT scans of the mandibular third molar (M3) of 219 patients were evaluated for this cross-sectional study. After adjustment of the 3D orientation of the individual's jaws, the study variables including the angulation between M3 and second molar (M2)/occlusal plane (OP), and the distance of mesial (M)/distal (D) roots of M3 from the inferior alveolar canal (IAC) and lingual plate (LP) were measured. Winter's classification was used for the specification of teeth angulation. The vertical position of each M3 was then evaluated using the 3D view window (based on Pell & Gregory classification). The Spearman correlation coefficient was used for reporting the correlation between quantitative variables. The One-way ANOVA test and the Welch analysis were used for inter-group comparisons. Finally, a multivariant analysis of variances was performed., Results: Most third molars had a mesioangular inclination (52.1%), and vertical inclination (25.9%) was in second place. There was no significant correlation between angulation and age or the D/LP. A strong positive correlation between M3:M2 and M3:OP was observed (0.983). Furthermore, there was a weak negative correlation between M/IAC or M/LP and angulation (- 0.16 and - 0.13, respectively). Concerning Winter's classification, the relationship between D/IAC and angulation was statistically significant (P = 0.003). Furthermore, teeth in position A had lesser inclination compared to those with B or C positions. Multivariate analysis revealed that there was no statistically significant relationship between particular combinations of inclination-impaction depth and proximity of the M3 roots to the IAC or LP (P = 0.211)., Conclusions: The findings of the current study revealed that there is a strong correlation between M3:M2 and M3:OP. The D/IAC was the only variable that had a significant relationship with the angulation of these teeth. The endodontic treatment of immature permanent teeth with necrotic pulp is a significant clinical challenge. The success of regenerative endodontic procedure is highly dependent on disinfection of the root canal and an accurate anatomical knowledge of the root canal. The aim of this study was to use micro-computed tomography (micro-CT) analysis to investigate the configuration of root canals in the upper permanent third maxillary molars with incomplete root development in their coronal, apical and middle third portions. Thirty immature third permanent maxillary molars were scanned using a micro-CT system. Then, we measured the diameters and areas of the root canal in the coronal, middle and apical third of the roots. The ratio between the long and short diameter of each root canal was then calculated and the canals were divided into several groups: round, oval, long oval, flat and irregular. The round configuration was not observed in the distobuccal and mesiobuccal roots in any of their anatomical regions. Oval and long oval canals predominated in the distobuccal root. The greatest variations were observed in the mesiobuccal root, with the ribbon-shaped canal predominating in the middle region and an irregular shape in the apical region. In the coronal region of the palatal canal, the round configuration predominated; in the middle third, we observed an almost equivalent distribution between round and oval configurations; apically, the oval shape predominated. In conclusion, we observed significant complexity and variation in the morphology and configuration of root canals in immature permanent molars, thus generating additional obstacles for the success of regenerative endodontics., Competing Interests: The authors declare no conflict of interest., (©2024 The Author(s). Objective: This study aimed to analyze the crown and root morphology of maxillary and mandibular third molars, and assess their position in relation to adjacent anatomical structures using clinical examination and radiographic images., Materials and Methods: A total of 176 extracted impacted or partially impacted third molars were included in this study. Orthopantomogram images (OPG) were used to evaluate the number of roots, position, depth of impaction relative to the occlusal plane of the second molar, and angulation of maxillary and mandibular third molars. The extracted teeth were examined to determine the actual number of roots. The root morphology was classified using the Alavi classification system for maxillary third molars and the Machado classification for mandibular third molars. Additionally, the number of cusps, crown dimensions, and morphology were analyzed using digital and dental calipers., Results: The extracted samples exhibited a significantly higher number of roots compared to the number assessed on OPG images (P<0.001). The majority of mandibular third molars had two fused roots (37.39%) and a five-cuspid crown (48.70%), while maxillary third molars had three fused roots (26.23%) and four cusps (42.62%). Both maxillary (60.66%) and mandibular third molars (61.74%) were predominantly vertically positioned. Regarding the depth of impaction, maxillary third molars were primarily classified as Class C (65.57%), while mandibular third molars were classified as Class B (47.83%). Differences in crown shape were observed, with maxillary crowns being mostly triangular (36.07%) and mandibular crowns being oval (38.26%). Maxillary third molars had shorter crowns compared to mandibular third molars (P<0.05)., Conclusion: The root morphology of maxillary and mandibular third molars in the population of Bosnia and Herzegovina displays variability. Orthopantomogram imaging was found to be inadequate for accurate determination of the number of roots in third molars. The number of cusps on third molars cannot be relied upon as a predictor of the number of roots. Restorative and prosthetic considerations usually necessitates endodontic treatment of third molars in order to retain them as a functional component of the dental arch. However, the anatomy of third molars has been described as unpredictable. To date, there has been little published work on root and canal morphology of third molars, with an adequate sample size. The aim of this study was to investigate root and canal morphology of third molars. Maxillary and mandibular third molars were gathered from dental surgeries within north Jordan. Following access cavity preparation, pulp tissue was removed and root canals stained. Teeth were subject to examination after rendering them clear by immersion in methyl salicylate, and the following features evaluated: (1) number of roots; (2) number and type of root canals; (3) number and position of lateral canals; and (4) presence of inter-canal communications. Out of the examined 592 maxillary third molars, 69.9% had three roots, 10.81% had one, 9.79% had two, 9.12% had four, and 0.34% had five roots. Most had three (52.36%) and four canals (28.2%) with less frequency of two (11.48%), one (5.91%) and five canals (2.03%). Of the 639 mandibular third molars, 89.76% had two roots, 7.35% had three, 2.5% had one, and 0.47% had four roots. Most had three (55.71%) and two canals (29.42%) with less frequency of four (13.61%), one (1.09%) and five canals (0.15%). The majority of maxillary third molars had 3 roots, while the majority of mandibular third molars had two. Overall, nearly half of the maxillary and mandibular third molars had three canals. New canal configurations, not previously described in the literature nor included in Vertucci's classifications, were identified in both maxillary and mandibular third molars., (© 2023. Background: Retromolar canal (RMC) arises from the mandibular canal (MC) behind the second or third molar and travels anterosuperiorly to a retromolar foramen (RMF). RMCs and RMFs have generally been ignored in anatomical textbooks and have rarely been reviewed or studied in the anatomical and dental literature until the last decades., Objective: This study aimed to characterize RMF in a Chinese population concerning its incidence, origin, and classification via anatomical study and periapical radiography., Methods: 123 dry adult Chinese mandibles were collected to observe the incidence of RMFs. RMFs were determined using a steel wire 0.5 mm in diameter. The passways or origins of the retromolar canal (RMC) were determined and classified via periapical radiography. For each RMF, two dentists independently measured the diameter and its distances to the lingual cortex, the buccal cortex, and the distal edge of the last tooth (or the alveolar fossa) using a vernier caliper., Results: The incidence of RMFs was 31.71%. The average RMF diameter was 0.78 ± 0.27 mm. From RMF, the distance was 4.27 ± 1.87 mm to the lingual cortex, 8.61 ± 2.23 mm to the buccal cortex, and 7.84 ± 3.87 mm to the distal edge of the last tooth (or the alveolar fossa). RMCs were classified into MC type originating from the mandibular canal and AF type originating from the alveolar fossa. The diameters of MC ones were more significant than those of AF ones. There was no apparent correlation between the existence of the third molar and the presence of an RMF., Conclusion: The incidence of RMFs in Chinese may be about one-third, which is a potential factor in the onset of surgery accidents. RMCs can be classified into two types by their origins. One of them is MC, which originates from the mandibular canal, and the other is AF, which originates from the alveolar fossa.

Pell and Gregory, and Winter's classifications are frequently implemented to classify the mandibular third molars and are crucial for safe tooth extraction. This study aimed to evaluate the classification accuracy of convolutional neural network (CNN) deep learning models using cropped panoramic radiographs based on these classifications. We compared the diagnostic accuracy of single-task and multi-task learning after labeling 1330 images of mandibular third molars from digital radiographs taken at the Department of Oral and Maxillofacial Surgery at a general hospital (2014-2021). The mandibular third molar classifications were analyzed using a VGG 16 model of a CNN. We statistically evaluated performance metrics [accuracy, precision, recall, F1 score, and area under the curve (AUC)] for each prediction. We found that single-task learning was superior to multi-task learning (all p < 0.05) for all metrics, with large effect sizes and low p-values. Recall and F1 scores for position classification showed medium effect sizes in single and multi-task learning. To our knowledge, this is the first deep learning study to examine single-task and multi-task learning for the classification of mandibular third molars. The aim of this cross-sectional study was to test the accuracy of the third molar maturity index ( I 3M ) cut-off value (0.08) to distinguish between individuals above and below the adult age of legal responsibility (18 years) in a Russian population. A sample of 571 digital panoramic radiographs of healthy Russian minors and young adults (363 females and 208 males), aged between 14 and 24 years, was evaluated. The lower left third molars were analyzed by applying the cut-off value of 0.08 determined by Cameriere et al. (2008). Lin's concordance correlation coefficient ( ρ c ) and Cohen's kappa coefficient ( κ ) showed that repeatability and reproducibility are high for both intra- and interobserver errors. The I 3M value decreased with age in both sexes. Age distribution gradually decreases as I 3M increases in both girls and boys. In the male group, molar maturity stages 0-0.04, 0.04-0.08, 0.08-0.3, 0.3-0.5, and 0.9-1.4 were reached slightly earlier than in the female group. The results demonstrated that sensitivity is 0.96 in boys and 0.93 in girls; associated specificity values were both 0.98. The cut-off value of I 3M is statistically robust and thus valid for forensic application in a Russian population to determine whether or not a subject has reached 18 years of age. Finally, we compared our results with those of other studies in which the same I 3M cut-off value was tested on different populations. Third permanent molars (M3s) are the last tooth to form but have not been used to estimate age at dental maturation in early fossil hominins because direct histological evidence for the timing of their growth has been lacking. We investigated an isolated maxillary M3 (SK 835) from the 1.5 to 1.8-million-year-old (Mya) site of Swartkrans, South Africa, attributed to Paranthropus robustus. Tissue proportions of this specimen were assessed using 3D X-ray micro-tomography. Thin ground sections were used to image daily growth increments in enamel and dentine. Transmitted light microscopy and synchrotron X-ray fluorescence imaging revealed fluctuations in Ca concentration that coincide with daily growth increments. We used regional daily secretion rates and Sr marker-lines to reconstruct tooth growth along the enamel/dentine and then cementum/dentine boundaries. Cumulative growth curves for increasing enamel thickness and tooth height and age-of-attainment estimates for fractional stages of tooth formation differed from those in modern humans. These now provide additional means for assessing late maturation in early hominins. M3 formation took ≥ 7 years in SK 835 and completion of the roots would have occurred between 11 and 14 years of age. Estimated age at dental maturation in this fossil hominin compares well with what is known for living great apes.

Introduction and Aim: The aim of our study was to investigate the influence of the occlusal support, and mandibular third molars on mandibular angle and condylar fractures., Method and Results: Patients with unilateral and isolated angle or condylar fractures were included in this retrospective, cross-sectional study. Data was collected from patient records and panoramic x-rays. The predictor variables included the type of occlusal support and the presence or absence of third molars. The outcome variable was angle or condylar fracture, while other predictor variables included demographic factors. Bivariate (χ2 test) and logistic regression analyses were conducted to investigate the associations between variables and the outcome. 43 angle (mean age: 29.9 ± 12.8 years; 98.4% male) and 37 condylar (mean age: 46.8 ± 20.2 years; 62.2% male) fracture cases were included in this study. Bilateral occlusal support was present in 81.4% of angle fracture group and in 51.3% of condylar fracture group (p<0.001). In the case of bilateral occlusal support, an odds ratio (OR) of 4.2 was found for angle fractures (p<0.006). A third molar was present in 86% of the angle fracture group and in 43.2% of the condylar fracture group (p<0.001). The presence of a third molar exhibited an odds ratio of 8.1 for the angle fractures (p<0.001). When bilateral occlusal support and third molar were present simultaneously, the risk was 15.9 times higher for an angle fracture (p<0.001)., Conclusion: The presence of occlusal support and/or third molars was significantly associated with angle fractures, however, the absence of occlusal support and/or third molars significantly correlated with condylar fractures. The study was designed to assess, by direct anatomical observations and cone-beam computed tomography (CT), the prevalence of a retromolar canal and foramen in relation to the mandibular third molar in dry human mandibles. Dry mandibles from European skeletons (n=89) were observed directly and after cone-beam CT scanning (Newtom VGI evo). The following variables were assessed: the presence of a retromolar foramen and canal; the presence of a third molar, and orientation of the third molar. From the total of 89 mandibles selected, 73 showed a retromolar foramen (49 of which were bilateral). A retromolar canal was identified in 64 mandibles based on cone-beam CT assessment, with a total of 101 canals, including 74 that were bilateral. A total of 112 hemimandibles contained a third molar. Orientation of the third molar did not seem to indicate the presence of retromolar foramina or canals. A similar prevalence of retromolar canals was found for both vertically (41/79) and mesially (17/33) orientated third molars. A retromolar foramen and canal were present in most mandibles, with more than half being bilateral. Introduction: This study aimed to identify significant factors affecting the spontaneous angular changes of impacted mandibular third molars as a result of second molar protraction. Temporary skeletal anchorage devices in the missing mandibular first molar (ML-6) or missing deciduous mandibular second molar (ML-E) with missing succedaneous premolar spaces provided traction., Methods: Forty-one mandibular third molars of 34 patients (10 male and 24 female; mean age 18.3 ± 3.7 years) that erupted after second molar protraction were included in this study. They were classified into upright (U) and tilted (T) groups. Linear and angular measurements were performed at the time of treatment initiation (T1) and of ML-6 or ML-E space closure (T2). Regression analyses were used to identify significant factors related to third molar uprighting., Results: Nolla stage (odds ratio [OR] 4.1), sex (OR 0.003 for male), third molar angulation at T1 (OR 1.1), missing tooth space (OR 0.006), rate of third molar eruption (OR 23.3), and rate of second molar protraction (OR 0.2) significantly affected third molar uprighting. Age, third molar angulation at T1, rate of third molar eruption, and rate of second molar protraction were significant factors for predicting third molar angulation at T2., Conclusions: Available space for third molar eruption before and after second molar protraction is not associated with uprighting of erupting third molars. Older patients whose third molars are in greater Nolla stage, are in a more upright position at T1, and have a greater eruption rate have a greater chance for third molar uprighting. Objective: To analyse the accuracy of a meta-analysis-based dental age assessment (DAA) method in Finnish paediatric patients and to compare the dental development between two generations of Finnish children., Methods: Panoramic radiographs of Finnish Caucasian healthy children from two generations (early: born 1981-1984; subsequent: born 1996-2008) were analysed. All developing teeth on the left maxilla and mandible as well as the third permanent molars were analysed following Demirjian's classification. For each patient, dental age was calculated and compared with chronological age. Dental maturation patterns between the two groups were compared., Results: The study included 200 Finnish Caucasian healthy children from two generations (early: aged 7-13 years; subsequent: aged 6-15 years). In the early generation, DAA underestimated the chronological age by a mean of 3.15 years. The underestimation was only 0.11 years in patients < 10 years, but 3.86 years in patients ≥ 10 years. In the subsequent generation, the dental age was overestimated by a mean of 0.34 years; by 0.40 years in patients < 10 years and by 0.08 years in patients ≥ 10 years., Conclusions: The present DAA method is applicable to current Finnish children. Differences in dental development between two generations of Finnish children were detected.

Third molar development and eruption are two related areas of major interest in dental research into the etiology of "wisdom tooth" impaction. Third molars are not only an excellent model for studying dental development but also of fundamental clinical importance because they are very frequently impacted. Because the third molar is located in the distal-most region of the oral cavity, clinical access is relatively challenging. With the increasingly widespread use of cone-beam computed tomography (CBCT) in dentistry, studies and measurements of the third molar and its eruption area have become considerably easier to do. Here we present a novel CBCT-based measurement methodology we developed for our recent investigations that we hope will also be useful for the broader dental research community.

Objectives: To investigate the anatomical variations of the root and root canal configuration of the human third molars., Designs: A total of 130 maxillary and 130 mandibular third molars were collected from a native Chinese population. All teeth were scanned by micro-computed tomography. After 3D reconstruction, the root and canal morphology of each tooth was examined both qualitatively and quantitatively., Results: For maxillary molars, a single fused root (67 cases, 51.5%) and a single root canal system (64 cases, 49.2%) was most common root/canal form; the typical three-rooted molars were detected only in 33 cases (25.4%), and the secondary MB canals were detected only in 9 molars (6.9%). For mandibular molars, 62 teeth were single-rooted (47.7%) and 42 had a single root canal system (32.3%); 20 singled-rooted and 60 double-rooted molars exhibited independent mesial and distal root canal systems (61.5%), and the type 1-1 canal was the most common configuration for mesial (57 cases) and distal (81 cases) root canal systems. C-shaped canals were detected in 11 maxillary and 36 mandibular single-rooted molars. The mean root surface area, root and crown volume of mandibular third molars were significantly higher than the maxillary third molars (P <  0.01)., Conclusion: The root canal system of the third molars may exhibit several anatomic variations. Introduction: In this study, we aimed to determine the effect of maxillary and mandibular posterior space discrepancies and third molar angulations on the overbite., Methods: Pretreatment lateral cephalograms of 131 subjects were analyzed. The sample included 83 open-bite and 48 deepbite subjects. A multiple regression analysis was used to evaluate the influence of maxillary and mandibular posterior space discrepancies and third molar angulations (predictor variables) on overbite. Correlations between posterior space discrepancy and third molar angulation, and correlations between predictor variables and dental angulation and height of posterior teeth and incisors were evaluated with the Pearson correlation coefficient. Subgroups with accentuated negative overbite and deepbite (27 open-bite, 37 deepbite) were compared with t tests., Results: The multiple linear regression analysis showed a positive association of the mesial angulation of the mandibular third molar with overbite. Posterior space discrepancy was negatively associated with posterior teeth mesial angulation and dentoalveolar height. However, these associations were weak, with no clinically significant implications. The deepbite subgroup showed significantly greater mesial angulation of the mandibular third molars than did the open-bite subgroup., Conclusions: There was no clinically significant effect of posterior space discrepancies and third molar angulations on overbite and dental angulation and height of posterior teeth and incisors., (Copyright © 2018 American Association of Orthodontists. Autotransplantation exhibits a number of advantages compared with other treatment options (ie, dental implants or fixed partial prostheses), such as greater resistance to occlusal loading, maintenance of the periodontal ligament and surrounding bone, and the potential for better esthetics. The aim of this study was to determine clinical outcomes for autotransplanted teeth with complete root formation using 3-dimensional-printed guiding templates and tooth replicas. Twenty-seven third molars with completely formed roots were autotransplanted. Each donor tooth and recipient site were examined clinically and radiographically (periapical radiographs). A selective cone-beam computed tomographic scan was taken of each donor tooth and recipient site. The images of the selected donor teeth were segmented and saved as stereolithography files. Similar to virtual planning of dental implants, correct angulation, rotation, and accurate positioning of the donor teeth were predefined using the stereolithography files. According to the virtually defined positions and dimensions of the donor teeth, 3-dimensional guiding templates and donor tooth replicas were printed. All autotransplantations were performed according to 1 treatment protocol and surgical technique. In 22 of the 24 transplanted teeth, no inflammation occurred during the healing period. At 2 years, no pathologic radiolucency or tooth resorption was observed in the 22 donor teeth. The autotransplanted teeth fulfilled the success criteria in 22 cases for a 91.7% success rate. Objectives: The aim of the study was to evaluate the proximity of the mandibular third molar (M3) and the inferior alveolar canal (IAC) in a panoramic radiograph of 20-year-old subjects. The specific aim was to assess differences in this proximity over time., Materials and Methods: Two similar samples of panoramic radiographs taken in a routine oral health examination with 20-year time interval were examined retrospectively and images with both mandibular M3s were included. The material consisted of 300 subjects (25% men, mean age 20.5 ± 0.6 years). The radiographic relationship between the mandibular M3 root and the IAC was assessed as follows: the M3 root was either apart from, tangential to, superimposed with, or inferior to the IAC. Differences between frequencies were tested using the chi-squared test., Results: In the combined samples, only 16% of the M3s located apart from the IAC, 15% located tangential to, 61% superimposed with, and 8% inferior to the IAC. The proportion of the intimate locations had increased during the 20-year time interval from 79 to 88% (P < 0.01) and especially in females (P < 0.05)., Conclusion: The vast majority of the mandibular M3s situated very close to the mandibular canal., Clinical Relevance: Our results suggest that in the cohort of 20-year-old non-extraction subjects, most of the M3s are possibly at risk for inferior alveolar nerve injury at removal, as judged from the panoramic radiograph, and also the number of such teeth has increased over the 20-year period.

Molar size in Mammals shows considerable disparity and exhibits variation similar to that predicted by the Inhibitory Cascade model. The importance of such developmental systems in favoring evolutionary trajectories is also underlined by the fact that this model can predict macroevolutionary patterns. Using backcross mice, we mapped QTL for molar sizes controlling for their sequential development. Genetic controls for upper and lower molars appear somewhat similar, and regions containing genes implied in dental defects drive this variation. We mapped three relationship QTLs (rQTL) modifying the control of the mesial molars on the focal third molar. These regions overlap Shh, Sostdc1, and Fst genes, which have pervasive roles in development and should be buffered against new variation. It has theoretically been shown that rQTL produces new variation channeled in the direction of adaptive changes. Our results provide evidence that evolutionary/disease patterns of tooth size variation could result from such a non-random generating process.

Purpose: The retromolar canal (RMC) and foramen (RMF) are anatomic variants in the retromolar area of the mandible. The purpose of this study was to clarify the relationship between the RMC and RMF and related complications, and to reveal how the RMC could impact the mandibular anatomy using cone-beam computed tomography (CBCT) and panoramic images (PAN)., Materials and Methods: CBCT and PAN images of 50 patients were retrospectively analyzed to investigate the morphology of the RMC and RMF, and their impact on impacted third molar surgery and orthognathic surgery., Results: In PAN images, neither the RMC nor RMF was detected. In CBCT images, the RMCs were detected in 26% (13/50) of the patients. A double RMC was detected on one side of one patient. The diameter of the RMC ranged from 0.8 to 2.9 mm (mean; 1.5 ± 0.6 mm), and the RMF ranged from 0.6 to 2.3 mm (mean; 1.1 ± 0.5 mm). No patients experienced unexpected bleeding. Unilateral postoperative hypoesthesia of the buccal gingiva in the molar region was reported in 6.7% of patients with the RMC., Conclusions: Hypoesthesia of the buccal gingiva in the lower molar region may be the main complication when the RMC is damaged.

The removal of displaced dental elements from deep anatomical spaces is a condition that requires the knowledge of the region and skills to perform the procedure. The lateral pharyngeal space contains important structures such as the internal carotid artery and close proximity with the cranium basis. The aim of this paper is to report a clinical case of a lower third molar displaced to the lateral pharyngeal space after a mandibular angle fracture and its treatment by surgical intervention. The tooth was removed under general anesthesia by direct approach and the fracture was reduced and fixed with a plate and screws. This case report illustrates the importance of an immediate procedure to avoiding severe complications and further damage to important anatomical structures.

Objectives: The root anatomy and position of mandibular third molars are important factors in assessing the level of difficulty of surgical removal procedures. In this light, this study aims to identify the most frequent position and the root anatomy of mandibular third molars., Methods: The study sample comprised 1205 mandibular third molars from 710 panoramic radiographs evaluated. Regarding the position, all of them were based on Pell and Gregory's and Winter's classifications. The root anatomy was classified according to the type and number of roots, as well as the presence of laceration, fusion, or divergence. Gender and the quadrant were also considered. Following an interexaminer analysis, a statistical analysis was performed using the Kappa test., Results: The results showed that the IA (40.3%) and mesioangular (53.8%) position was predominant in mandibular third molars. Regarding the anatomy, the most common were two roots (98.3%), of the conical simple type (88.9%), with absence of laceration (89.2%), divergence (84%), and fusion (80%)., Conclusion: The present study showed that the most prevalent mandibular third molar type in Brazilian patients was the IA and mesioangular position, with simple root anatomy. The result of this study can assist oral surgeons to better plan and assess the level of difficulty of surgical removal procedures.

Purpose: The positional relationship between the mandibular canal and corresponding third molars is a key anatomic factor of inferior alveolar nerve (IAN) injury. The aim of the present study is to classify the anatomic three-dimensional relationship between the mandibular third molar and the mandibular canal on cone-beam computed tomography (CBCT) images., Methods: This study used CBCT images to classify the positional relationship between the mandibular canal and corresponding third molars. CBCT images of 749 patients (1296 mandibular third molars) were analyzed to draw up a classification., Results: On a total of 1296 third molars, the mandibular canal relative to the roots of the mandibular third molar was on the apical side (88.1%), followed by the buccal side (7.9%), the lingual side (3.5%), and then between the roots (0.5%). Ninety-five (7.1%) third molars had a close relation with the mandibular canal, while 1201 (92.7%) third molars had no direct contact. The percentage of the mandibular canal contacts with the mandibular third molar was higher when the mandibular canal was lingually positioned., Conclusions: The anatomic structures of the mandibular third molar and the mandibular canal may be helpful to make adequate surgical planning to avoid or reduce nerve involvement.

The aims of this study were to find the incidence of bifurcation of the inferior dental nerve (IDN) canal, to describe the characteristics of this variant, and to examine the sensitivity and specificity of dental panoramic tomography to identify it. We classified bifurcations by size and position relative to the main canal and the lower third molar using cone-beam computed tomography (CT) and dental panoramic tomography. In our study of 281 patients, 106 (38%) had bifurcations, and in one quarter, these were classified as large accessory canals. Bifurcations were most commonly found posterior to the lower third molar (n=64, 57%) or within 2mm of the roots of the third molar (n=40, 38%). The sensitivity and specificity of dental panoramic tomography to identify all bifurcations was 11% (95% CI: 5.67 to 17.97) and 91% (95% CI: 85.58 to 94.68), respectively; this was 33% (95% CI: 15.63 to 55.32) and 94% (95% CI: 90.34 to 96.50), respectively, for large bifurcations. Our use of cone-beam CT suggested an incidence of bifid canals of 38%, with a variation in size and distribution in relation to the lower third molar. Objectives: The aim of this study was to determine the effect of first premolar extraction with different anchorages as well as non-extraction on mandibular third molar angulation., Materials and Methods: The pretreatment (T1) and posttreatment (T2) panoramic radiographs of one hundred patients from Hamadan (Iran), who underwent fixed orthodontic treatment were investigated and the angle between long axis of right mandibular third molars and a constructed horizontal plan (HRP) was measured. The sample was equally divided into four groups including first premolar extraction with maximum anchorage, that with moderate anchorage, that with minimum anchorage and non-extraction group., Results: In the first premolar extraction groups with moderate and minimum anchorages, third molar angulation increased (8.12 and 7.48°, respectively) significantly from T1 to T2 (P=0.001 and 0.003, respectively), but in the first premolar extraction group with maximum anchorage and non-extraction group, increase in third molar angulation (2.84 and 0.8°, respectively) was not statistically significant (P=0.082 and 0.943, respectively). Third molar uprighting was significantly different among four groups using one-way ANOVA (P=0.004). Tukey (HSD: honestly significant difference) post hoc test indicated that difference in third molar angulation was statistically significant between first premolar extraction group with moderate anchorage and non-extraction group (P=0.007) and also between the first premolar extraction group with minimum anchorage and non-extraction group (P=0.024)., Conclusion: The first premolar extraction groups with moderate and minimum anchorage may lead to more chance of third molar eruption and this can be helpful in borderline cases to consider third molar angulation as one of determining indicators to decide whether to extract or not to extract first premolars., (Copyright © 2018 CEO. Introduction: The aim of this study was to analyze the root canal morphology of maxillary third molars (MTMs) using micro-computed tomography (micro-CT)., Materials and Methods: Seventy-eight consecutively-extracted human MTMs were scanned using micro-CT (spatial resolution=13.68μm per pixel). Dedicated software (SkyScan ® ) was used to create virtual reconstructions and perform 3D-analysis. A range of anatomical features were assessed; externally (root number, length, fusion, curvature, apex), within the pulp chamber (distance between canal orifices, floor thickness) and within the root canal system (root canal number, classification, ramifications, isthmuses, apical constriction)., Results: The donor age ranged from 19 to 73 years (mean±SD 32.3±16.5years). MTMs possessed one or three roots, which principally curved buccally/palatally (75.9%), had 1-4 root canals and typically no apical constriction (84.4%). The average external root length was 11.89±1.53mm, while root canal length was 10.18±0.35mm. The root canal diameter 1mm from the apex was 0.37±0.23mm and negatively correlated with donor's age (r=-0.76; p=0.01), while pulp chamber thickness positively correlated with age (r=0.58; p=0.035). Significantly, furcation canals, canal loops and root canal calcifications were sporadic findings., Conclusions: In some cases the anatomy of MTMs may not be as complicated as previously documented, being similar to the reported anatomy of other maxillary molars. During root canal treatment of MTMs, the frequent deviation of the apical foramen from the radiographic apex should be considered, as should the absence of an apical constriction in the majority of cases. Background: Through a systematic literature review, the authors assess the effect of premolar extractions on third-molar (M3) eruption considering eruption rate, retromolar space, and molar angulation., Types of Studies Reviewed: The authors performed a systematic search using MEDLINE and Web of Science databases up through April 2017 to identify quality studies available comparing M3 eruption between a group with premolar extraction and a group without premolar extraction., Results: Twelve comparative retrospective cohort studies met all the inclusion criteria. The authors found in 5 studies comparing the rate of M3 eruption that there were significantly higher results in the group with extractions. They found in 5 studies comparing the evolution of the retromolar space significantly higher results in the group with extractions. Lastly, concerning the uprighting of the M3 during treatment, the authors found only 2 studies showing significant differences between the 2 groups, each time in favor of the group with extractions., Conclusions and Practical Implications: The dental literature on premolar extraction related to the eruption of the M3 is composed of average-quality retrospective studies. Premolar extraction significantly improves the chances of M3 eruption, but the level of evidence of comparative retrospective cohort studies is low. Clinicians must continue to rely on their judgment regarding premolar extraction on a case-by-case basis until the evidence is stronger. Background: The Evaluation of the degree of lower third molar (L3M) extraction difficulty is extremely important for both clinicians and patients. This study aimed to evaluate the validity of a new index (Kharma scale) as a preoperative predictor index of the difficulty of surgical removal of impacted L3M., Material and Methods: Extraction difficulty of a series of 49-impacted L3M was predicted preoperatively by Kharma scale, and postoperative difficulty was assessed with a modified Parant scale., Results: The sensitivity of Kharma scale, as a predictor of difficulty, was 18.2% and the specificity was 68.4%. Likelihood ratios for the Kharma categories also indicated that the scale is of little value for predicting a difficult extraction. There was no significant association between the Kharma score and duration of operation, but high-modified Parant scores were significantly associated with longer operations., Conclusions: The proposed Kharma scale was unreliable as preoperative predictor of the L3M extraction difficulty, and both radiological and clinical information must be taken into account.

Objectives: To analyse the effect of first and second premolar extractions on eruption space for upper and lower third molars and on third molar position and angulation during orthodontic treatment., Methods: The sample consisted of 296 patients of which 218 patients were orthodontically treated without extraction and 78 patients with extraction of first or second premolars. The eruption space for third molars was measured on pre- and posttreatment lateral cephalograms, whereas the angulation, vertical position, the relation with the mandibular canal and the mineralization status of third molars were evaluated using pre- and posttreatment panoramic radiographs. All data were statistically analyzed., Results: The increase in eruption space and the change in vertical position of upper and lower third molars significantly differed between patients treated with and without premolar extractions, whereas the change in angulation, relationship with the mandibular canal and mineralization status of the third molars did not significantly differ between patients treated with and without premolar extractions., Conclusions: The retromolar space and the position of third molars significantly change during orthodontic treatment in growing patients. Premolar extractions have a positive influence on the eruption space and vertical position of third molars, whereas they do not influence the angular changes of third molars. Due to the retrospective character of the study, these conclusions should be carefully considered. Purpose: The purpose of this study was to compare the prevalence of third molars from the US National Health and Nutrition Examination Survey (NHANES) and the Swedish survey., Materials and Methods: This cross-sectional study involved the comparison of the only published data on third molar prevalence. The number of visible third molars in the NHANES of 2011 through 2012 were assessed in nonclinical settings by trained, calibrated dental hygienists and reported by age decade (approximately 5,000 patients). Similar data were reported for the Swedish population with data collected in clinical settings (approximately 700 patients). The primary outcome variable was the number of third molars (0 to 4); the predictor variables were age cohorts (20 to 29 through 70 to 79 yr). Outcome data were reported with descriptive statistics., Results: In the youngest cohort (20 to 29 yr), having no visible third molars was more likely in the US population than in the Swedish population (47 vs 2%, respectively). By 50 to 59 years, outcomes for no third molars were similar in the United States and Sweden (53 and 57%, respectively)., Conclusion: The presence or absence of third molars reported from the US and Swedish populations presented contrasting patterns, particularly in the younger cohorts. The risk of postoperative neurosensory impairment after extraction of a third molar will be greater if the inferior alveolar nerve is in close proximity to the apices of the root. Precise preoperative evaluation of the relative positions of the nerve and the mandibular apices is important to protect the nerve from mechanical irritation. The aim of this study was to assess the accuracy of cone-beam computed tomography (CT) in the identification of the positional relations of the mandibular third molar and the inferior alveolar canal using Rood's criteria. Panoramic images of 10 dry hemimandibles were obtained, and 20 teeth each with one of Rood's criteria indicating close relations to the mandibular canal were selected. Cone-beam CT images of the selected points were obtained. The closest distance between the apex of the root and the inferior alveolar canal was measured on cross-sectional cone-beam CT images. The same measurements were made on the corresponding osteometric sections with digital callipers. The intra-class correlation coefficient (ICC) was used to measure the consistency between the two measurements as the reference method, and showed that there was a high level of inter-rater agreement (r>0.90). The mean (SD) deviation of cone-beam CT measurements from the gold standard osteometric measurements was small at 0.30 (0.24). There were almost perfect matches between cone-beam CT and gold standard measurements. Background: Established dental age estimation methods in sub-adults study the development of third molar root apices on radiographs. In living individuals, however, avoiding ionising radiation is expedient. Studying dental development with magnetic resonance imaging complies with this requirement, adding the advantage of imaging in three dimensions., Aim: To elaborate the development of an MRI protocol to visualise all third molars for forensic age estimation, with particular attention to the development of the root apex., Subjects and Methods: Ex vivo scans of porcine jaws and in vivo scans of 10 volunteers aged 17-25 years were performed to select adequate sequences. Studied parameters were T1 vs T2 weighting, ultrashort echo time (UTE), fat suppression, in plane resolution, slice thickness, 3D imaging, signal-to-noise ratio, and acquisition time. A bilateral four-channel flexible surface coil was used. Two observers evaluated the suitability of the images., Results: T2-weighted images were preferred to T1-weighted images. To clearly distinguish root apices in (almost) fully developed third molars an in plane resolution of 0.33 × 0.33 mm 2 was deemed necessary. Taking acquisition time limits into account, only a T2 FSE sequence with slice thickness of 2 mm generated images with sufficient resolution and contrast. UTE, thinner slice T2 FSE and T2 3D FSE sequences could not generate the desired resolution within 6.5 minutes., Conclusion: Three Tesla MRI of the third molars is a feasible technique for forensic age estimation, in which a T2 FSE sequence can provide the desired in plane resolution within a clinically acceptable acquisition time.

Background: Established methods to stage development of third molars for forensic age estimation are based on the evaluation of radiographs, which show a 2D projection. It has not been investigated whether these methods require any adjustments in order to apply them to stage third molars on magnetic resonance imaging (MRI), which shows 3D information., Aim: To prospectively study root stage assessment of third molars in age estimation using 3 Tesla MRI and to compare this with panoramic radiographs, in order to provide considerations for converting 2D staging into 3D staging and to determine the decisive root., Subjects and Methods: All third molars were evaluated in 52 healthy participants aged 14-26 years using MRI in three planes. Three staging methods were investigated by two observers. In sixteen of the participants, MRI findings were compared with findings on panoramic radiographs., Results: Decisive roots were palatal in upper third molars and distal in lower third molars. Fifty-seven per cent of upper third molars were not assessable on the radiograph, while 96.9% were on MRI. Upper third molars were more difficult to evaluate on radiographs than on MRI (p < .001). Lower third molars were equally assessable on both imaging techniques (93.8% MRI, 98.4% radiograph), with no difference in level of difficulty (p = .375). Inter- and intra-observer agreement for evaluation was higher in MRI than in radiographs. In both imaging techniques lower third molars showed greater inter- and intra-observer agreement compared to upper third molars. MR images in the sagittal plane proved to be essential for staging., Conclusion: In age estimation, 3T MRI of third molars could be valuable. Some considerations are, however, necessary to transfer known staging methods to this 3D technique.

Our aim was to assess the incidence and anatomical site of branches of the inferior dental canal that supply mandibular third molars using dental cone-beam computed tomography (CT). We evaluated the incidence and diameter of branches of the inferior dental canal using 272 cone-beam CT mandibular scans from 172 patients referred for imaging before the extraction of impacted mandibular third molars. We found three typical branching patterns from the inferior dental canal in the third molar region: the retromolar canal (in the retromolar triangle), the dental canal (that courses directly beneath the socket of the third molar), and the accessory canal (that courses through the socket and leads from the inferior dental canal to a bony ridge). The incidences of retromolar, dental, and accessory canals were 75 (28%), 223 (82%), and 21 (8%), respectively, with mean diameters of 0.9 (0.4), 0.7 (0.5), and 1.1 (0.4) mm, respectively. Operative injury to the neurovascular contents within the branches of the inferior dental canal can lead to excessive bleeding and postoperative paraesthesia, so identification of its branches on preoperative cone-beam CT images may prove useful during extraction of impacted mandibular third molars or when harvesting bone blocks from the region of mandibular third molars. Background: Lower third molar (M3) eruption is unpredictable. The purpose of this study was to correlate radiographic position of M3 on a preexistent film with the current clinical, histopathological, and radiographic findings., Methods: A retrospective cohort study was performed. The sample was collected from a database of patients covered by Medical Fund of Brazilian Army. Radiographs were obtained a minimum of 5 years prior to the presurgical visit and after their clinical exam. The primary outcome variables were the teeth positions using Pell and Gregory/Winter classifications on panoramic X-rays. Those variables were analyzed at both the beginning (T0) and end of the study (T1). Clinical assessments and histopathological study of the thirds that were extracted were performed only at T1. Correlation between the teeth positions were related to the clinical, histopathological, and radiographic parameters using statistical analysis tests with significance set at p < 0.05., Results: Twenty-six patients with 49 M3 were assessed over 10 months. Mean age was 14.92 years at T0 and 21.87 years at T1. The average time between T0 and T1 was 6.77 years. A significant relationship (p = 0.024) was found between the presences of root resorption on the second molar if M3 presented in an IB horizontal position at T1. There was also a significant correlation (p = 0.039) between dental crowding of the anterior lower teeth with IIIB position at T0 and if the patient finished orthodontic treatment without lingual retainers., Conclusions: Lower M3 in position IIIB seen in a teenager and IB seen in an adult is more likely to have negative consequences and should be followed closely.

Background: The aim of this study was to determine the effect of the maxillary posterior dentoalveolar discrepancy (MPDD) on the angulation of maxillary molars in open bite subjects., Methods: Pre-treatment lateral cephalograms of 90 young adults with skeletal open bite were examined. The sample initially included six groups categorized according to MPDD condition (present or absent) and sagittal skeletal facial growth patterns (classes I, II, or III). Then, the sample was separated into two groups according to MPDD (present = 50, absent = 40). When the eruption of the maxillary third molar was apparently blocked by the presence of an erupted second molar, a MPDD was considered. Maxillary molar angulation was measured. Independent T test was performed to determine differences between the groups considering MPDD condition. Principal component analysis (PCA) and multivariate analysis (MANCOVA) test were also developed., Results: A decreased molar angulation was found in all groups with MPDD (overall p < 0.001, class I-p < 0.001, class II-p < 0.001, and class III-p < 0.05). The maxillary first and second molars angulations were lower between approximately 7° and 14° in cases with posterior discrepancy. The PCA was used to reduce the number of initial cephalometric variables; thereafter, a MANCOVA test was applied. Significance was only found for MPDD (p < 0.001), APDI (p = 0.001), and ratio (A'6'/A'P') (p = 0.026) for maxillary first molar angulation and APDI (p = 0.011) and MPDD (p < 0.001) for maxillary second molar angulation., Conclusions: The MPDD generates a major mesial displacement of the second and first molar roots with a concurrent simultaneous distal angulation of the associated crowns in individuals with skeletal open bite.

This retrospective study was performed to investigate the influence of occlusal support and the presence, state, and position of mandibular third molars on the incidence of mandibular angle and condylar fractures. The following variables were investigated: age, sex, cause of fracture, presence and state (impaction, angulation, and the number of roots) of the mandibular third molars, site of the mandibular fracture, presence of occlusal support, duration of intermaxillary fixation, and postoperative complications. Various risk factors for mandibular angle and condylar fractures were investigated by univariate analysis. The risk of mandibular angle fracture was significantly higher in patients with occlusal support and mandibular third molars. The risk of condylar fracture was significantly higher in patients without occlusal support or mandibular third molars. The position and angulation of the mandibular third molars were not significant risk factors in mandibular angle and condylar fractures. This study demonstrated the influence of occlusal support and the presence of mandibular third molars on the incidence of mandibular angle and condylar fractures. According to Recommendation N°196 of the Australian Law Reform Commission (ALRC), the age at which a child reaches adulthood for the purposes of criminal law should be 18 years in all Australian jurisdictions. With specific reference to age at majority, the only tooth with development spanning adolescence (and thus the legally relevant 18 years of age) is the third molar, which limits the number of methods that can be applied from those available in the published literature. The aim of the present study is to test the accuracy of the third molar index (I3M=0.08), based on the correlation between chronological age and normalized measures of the open apices and height of the third mandibular molar, in order to assess the legal adult age of 18 years. Digital orthopantomographs of 143 living Australian subjects (72 boys and 71 girls) are analyzed. The results demonstrate that the sensitivity is 0.90 in boys and 0.90 in girls; associated specificity values are 0.85 and 0.87 respectively. The third molar is a tooth of anatomical, surgical, prosthetic and forensic dental interest. However, there is currently a lack of updated data regarding its morphology. The aim of this study was to determine the morphometric features of third molars and their predictive capability as regards dental arch and side. Two calibrated operators (ƙ = 0.83) determined the cervicalocclusalvestibular (COV), cervicalocclusalpalatal (COP) and occlusalapical (OA) distances, mesiodistal (MD), and vestibularpalatal (VP) diameters, number of roots (R) and number of cusps (C) of 961 cadaveric third molars, both upper (n = 462) and lower (n = 499), using a CONCOR 050 thin mandible caliper (resolution 0.01 mm). Median and range for each variable were calculated and compared using Mann Whitney nonparametric test (p < 0.05). Multivariate cluster analysis was used to determine the predictive capability of each variable for dental arch and side. For upper molars (UM), 50.6% were from the right side (RS) and 49.4% from the left side (LS), while for lower molars (LM), 60.9% were from the RS and 39.1% from the LS. No significant difference was found in the study variables in LM according to side. For UM, MD diameter (10.90 mm), COP(7.42 mm) distance and number of R (3) were significantly higher (p < 0.05) forRS, and number of C (3) was higher (p < 0.0001) for LS. They were also significant predictive grouping factors for side. For dental arch, OA (17.84 mm) and COV (7.60 mm) distances, MD (11.26 mm) diameter and the number of C (5) were significantly higher (p < 0.0001) for LM, while VP (10.84 mm) and COP (7.34 mm) distances, and the number of R (3) were significantly higher (p < 0.0001) for UM. These variables were significant predictive factors for dental arch. Despite the morphometric heterogeneity of third molars, there are intrinsic parameters with predictive capability for dental arch and side, but it would be advisable to supplement this study with data from topographic occlusal variables in order to validate their predictive capability., (Sociedad Argentina de Investigación Odontológica.)

Mylagaulid fossorial rodents are a common component of North American Miocene fossil faunas. However outside of North America, only three species are known from Asia. Here we report two new mylagaulids, Irtyshogaulus minor gen. et sp. nov. and Irtyshogaulus major gen. et sp. nov., recovered from early Miocene sediments in the Junggar Basin in northwestern China. The two new taxa are small-sized, high-crowned promylagauline rodents. Their lower molars possess high metastylid crests, small mesostylids, broad and posterolingually expanded labial inflections, and transversely extending metalophid IIs. The mesoconid is absent in both species. The anterior and posterior fossettids are large and equally developed. Their upper M1-2s possess a square occlusal surface with five deep fossettes. The two new taxa are distinguished from each other mainly by their size, the morphology of fossettes and fossettids, development of mesial and distal lophs, posterior reduction of M3, and the orientation of m2 hypolophid. Our phylogenetic analysis indicates that Irtyshogaulus and Lamugaulus (another early Miocene Asian mylagaulid) are sister taxa. The two genera are nested among the North American promylagaulines, and share a common ancestor from North America, indicating early Miocene intercontinental dispersal within this clade of rodents.

Newer bulk-fill composites claim a greater depth of cure than conventional resin-based composites. To facilitate complete curing, the manufacturer of SonicFill (Kerr) recommends curing from the occlusal, as well as the buccal and lingual, surfaces of the tooth. The purpose of this study was to quantify the degree of curing light attenuation as it passes through natural tooth structure, and how this attenuation affects the depth of cure of different posterior resin composites. Ten noncarious extracted mandibular third molars were sectioned to produce 5-mm-thick pieces of buccal tooth structure. Sanding 0.5-mm increments from the flattened surface produced 4.5-, 4.0-, 3.5-, 3.0-, 2.5-, 2.0-, and finally 1.5-mm-thick sections. A Bluephase G2 (Ivoclar) curing light with an 8-mm-diameter light guide set on high for 20 seconds was used for measurement of irradiance as it passed through different thicknesses of tooth structure and air. The average irradiance (mW/cm(2)) was measured with a MARC-RC Resin Calibrator with a 4-mm-diameter sensor (BlueLight Analytics). To measure depth of cure of a conventional hybrid composite (Herculite Ultra; Kerr) vs a bulk-fill hybrid composite (SonicFill) through varying thicknesses of tooth structure, composites were cured in a 4-mm-diameter × 10.25-mm-long split mold according to International Organization for Standardization 4049. A mean and standard deviation was determined per group. Data were analyzed with a one-way analysis of variance (ANOVA)/Tukey test and two-way ANOVA/Tukey test (α=0.05). One-way ANOVA/Tukey found a significant decrease in irradiance based on thickness of tooth structure or distance through air (p<0.001). Two-way ANOVA/Tukey found a significant decrease in depth of cure based on thickness of tooth structure (p<0.001) and on composite type (p<0.001) with no significant interaction (p=0.623). SonicFill had a significantly greater depth of cure than Herculite Ultra.

Selecting either buccal or lingual approach for the mandibular third molar surgical extraction has been an intense debate for years. The aim of this observational retrospective study was to classify the molar based on the proximity to the external cortical bone, and analyze the position of inferior alveolar canal (IAC) of each type. Cone-beam CT (CBCT) data of 110 deeply impacted mandibular third molars from 91 consecutive patients were analyzed. A new classification based on the mean deduction value (MD) of buccal-lingual alveolar bone thickness was proposed: MD≥1 mm was classified as buccal position, 1 mm>MD>-1 mm was classified as central position, MD≤-1 mm was classified as lingual position. The study samples were distributed as: buccal position (1.8%) in 2 subjects, central position (10.9%) in 12 and lingual position (87.3%) in 96. Ninety-six molars (87.3%) contacted the IAC. The buccal and inferior IAC course were the most common types in impacted third molar, especially in lingually positioned ones. Our study suggested that amongst deeply impacted mandibular third molars, lingual position occupies the largest proportion, followed by the central, and then the buccal type.

By surgical interventions in maxillolingual groove area one should consider anatomical variations and topography of vessels, glands ducts and lingual nerve to prevent their injury. At the Department of Operative Surgery and Topographic Anatomy of the First Moscow State Medical University named after I.M. Sechenov we carried out anatomical study on cadavers (men and women, n=30).The study revealed topographical features of the lingual nerve and its relationship to other anatomical structures in the maxillolingual groove. It was found out that at the level of the second molar (96%) lingual nerve "crosses" duct of submandibular salivary gland, at the level of the third molar lingual nerve is located under the duct and lateral to it, closer to the inner surface of the body of the mandible. At the level of the first molar lingual nerve is located above and medial to Wharton duct and passes along sublingual-lingual muscles (m.hyoglossus).

Purpose: To determine the prevalence of third molars in the US population and to report the differences in prevalence of visible third molars in the most recent National Health and Nutrition Examination Survey (NHANES) population of 2011 through 2012 by participant, jaw, and demographics., Materials and Methods: The number of visible third molars in the NHANES data was assessed in nonclinical settings by trained, calibrated dental hygienists by decade of age beginning in the 20- to 29-year-old cohort through the 70- to 79-year-old cohort. Cross-sectional analyses of third molar data were conducted by the authors from the NHANES databases of 2001 through 2002, 2009 through 2010, and 2011 through 2012 to compare data for similarity of outcomes on third molar prevalence in the US population. Outcomes on third molar prevalence also were assessed from the NHANES of 2011 through 2012 by participant, jaw, and demographics: gender, race or ethnicity (Caucasian, African American, other), and education (less than high school, high school graduate, some college, college graduate)., Results: Data on the mean number of third molars from NHANES of 2001 through 2002, 2009 through 2010, and 2011 through 2012 were similar. The number of visible third molars in the NHANES of 2011 through 2012 decreased progressively from a mean of 1.48 in the 20- to 29-year-old cohort to 0.81 in the 60- to 69-year-old cohort, No visible third molars were observed in 47% of the 20- to 29-year-old cohort compared with 53% in the 50- to 59-year-old cohort. Participants who were male, non-Caucasian, and had less than a high school education were more likely to have a visible third molar in all age cohorts. No data were collected by NHANES examiners to determine why third molars were absent., Conclusion: Third molar prevalence did not appear to differ in the US population during the first decade of the 21st century. Numbers of visible third molars, prevalent in young adults, decreased progressively through each successive age cohort. In order to clarify the morphological characteristics and function of the protuberance and/or fossa on the lateral surface of the mandible, we examined 778 mandibles of 9 genera, 19 species of non-human primates. Both protuberance and fossa were found in Macaca, Chlorocebus, and Cercocebus at frequencies ranging from 0% to 60%. The protuberance was composed of compact bone and was similar to the mandibular torus in humans. A well-developed protuberance extended from the fourth premolar to third molar region, and was situated at the central part of the mandibular body, or continued on the oblique line. Sometimes the protuberance localized on the mandibular base. A deep and large fossa was found in all individuals of Papio, Theropithecus, and Mandrillus, and the bone width was thin in the center of the fossa. The fossa extended from the third premolar to the second molar region, and the deepest area was the first molar region. In Macaca, Chlorocebus, and Cercocebus, the curvature of the external table of the mandible created a fossa. In Colobus, and Hylobates, the external surface of the mandible looked concave because of the thickened mandibular base. In order to increase the validity of age estimation in adolescents and young adults when there is no legitimation for X-ray examinations, it seems desirable to be able to assess the mineralization of third molars using X-ray-free imaging procedures. In the present study, the mineralization stages of lower third molars were determined prospectively in 269 male and 248 female individuals aged 12 to 24 years using 3.0 T MRI. The classification system of Demirjian et al. was used to determine the stages. This study presents the minima and maxima, means and standard deviations, median values, and lower and upper quartiles separately for both sexes, for the mineralization stages B-H. Statistically significant sex differences were observed for the mineralization stages C, E, F, and G, and a faster developmental rate was observed for males. It was concluded that magnetic resonance imaging is an X-ray-free alternative to orthopantomography when assessing mineralization of third molars.

This study tests the hypothesis that the third molars of Callimico goeldii represent a reversal in evolutionary tooth loss within the Callitrichinae. Loss of third molars is part of a suite of unusual characters that has been used to unite marmosets and tamarins in a clade to the exclusion of Callimico. However, molecular phylogenetic studies provide consistent support for the hypothesis that marmosets are more closely related to Callimico than to tamarins, raising the possibility that some or all of the features shared by marmosets and tamarins are homoplastic. Here, I use the binary-state speciation and extinction (BiSSE) model and a sample of 249 extant primate species to demonstrate that, given the shape of the primate phylogenetic tree and the distribution of character states in extant taxa, models in which M3 loss is constrained to be irreversible are much less likely than models in which reversals are allowed to occur. This result provides support for the idea that the last common ancestor of Callimico and marmosets was characterized by the two-molared phenotype. The M3s of Callimico therefore appear to be secondarily derived rather than plesiomorphic. This conclusion may also apply to the other apparently plesiomorphic traits found in Callimico. Hypotheses regarding the re-evolution of M3 in the callitrichine clade and the origin and maintenance of the two-molared phenotype are discussed., (Copyright © 2015 Elsevier Ltd. Successful root canal treatment depends on having comprehensive information regarding the root(s)/canal(s) anatomy. Dentists may have some complication in treatment of third molars because the difficulty in their access, their aberrant occlusal anatomy and different patterns of eruption. The aim of this review was to review and address the number of roots and root canals in third molars, prevalence of confluent canals in third molars, C-shaped canals, dilaceration and fusion in third molars, autotransplantation of third molars and endodontic treatment strategies for third molars.

Statement of Problem: Mechanical and thermal fatigue may affect ceramic restorations in the oral environment., Purpose: The purpose of this study was to determine the influence of thermal and mechanical cycling on the fracture load and fracture patterns of 3 machinable ceramics., Material and Methods: Seventy-two human third molar teeth were prepared for bonding ceramic specimens of Sirona CEREC Blocs, IPS e.maxCAD, or inCoris ZI meso blocks. The 24 specimens of each ceramic were divided into 4 groups (n=6), which underwent no preloading (control), thermocycling (5°C-55°C, 2000 cycles), mechanical cycling (10(5) cycles, 100 N), and thermocycling (5°C-55°C, 2000 cycles) plus mechanical cycling (10(5) cycles, 100 N). The specimens were subsequently loaded to failure, and both stereomicroscopy and scanning electron microscopy were used to investigate the fracture patterns. The data were analyzed with 2-way ANOVA and the Fisher exact probability test (α=.05)., Results: Mechanical and thermal cycling had a significant influence on the critical load to failure of the 3 ceramics. No significant difference was found between mechanical cycling for 10(5) times and thermocycling for 2000 times within the same ceramic. The specimens of inCoris ZI experienced significantly higher fracture loads for all the groups. The fracture patterns of the 3 machinable ceramics showed that failure mainly occurred at the cement-dentin interface., Conclusions: The effects of combined thermal and mechanical cycling on the fracture load of ceramics were more significant than any individual mode of cyclic fatigue.
