2. ATP-induced fibrogenic pathway in circulating cells obtained by idiopathic pulmonary fibrotic (IPF) patients is not blocked by nintedanib and pirfenidone

Idiopathic pulmonary fibrosis (IPF) is a severe disability due to progressive lung dysfunction. IPF has long been viewed as a non-immune form of pulmonary fibrosis, but nowadays it is accepted that a chronic inflammatory response can exacerbate fibrotic patterns. IL-1-like cytokines and ATP are highly detected in the lung and broncho-alveolar lavage fluid of IPF patients. Because ATP binds the purinergic receptor P2RX7 involved in the release of IL-1-like cytokines, we aimed to understand the role of P2RX7 in IPF. PBMCs from IPF patients were treated with nintedanib or pirfenidone in the presence of ATP. Under these conditions, PBMCs still released IL-1-like cytokines and the pro-fibrotic TGFβ. Bulk and scRNAseq demonstrated that lung tissues of IPF patients had higher levels of P2RX7, especially on macrophages, which were correlated to T cell activity and inflammatory response with a TGFBI and IL-10 signature. A subcluster of macrophages in IPF lung tissues had 2055 genes that were not in common with the other subclusters, and that were involved in metabolic and PDGF, FGF and VEGF associated pathways. These data confirmed what observed on circulating cells that, although treated with anti-fibrotic agents, nintedanib or pirfenidone, they were still able to release IL-1 cytokines and the fibrogenic TGFβ. In conclusion, these data imply that because nintedanib and pirfenidone do not block ATP-induced IL-1-like cytokines and TGFβ induced during P2RX7 activation, it is plausible to consider P2RX7 on circulating cells and/or tissue biopsies as potential pharmacological tool for IPF patients.

Severe plastics pollution derived from the wide use and the past incorrect dispersion of petroleum-based plastics. The development of bioplastics/biopolymers with the same chemical/physical characteristics of conventional plastics can be a solution to the problems of their degradability. Polybutylene succinate was among the most investigated biopolymers for these characteristics that can be produced from bio-based succinic acid and butan-1,4-diol. Succinic acid can be produced through fermentation process using microorganisms as Actinobacillus succinogenes that were able to utilise glucose from several sources as lignocellulosic biomasses. Lignocellulosic biomass can be ideal candidates for glucose supply but the processes to release fermentable sugars can produce inhibitors (acids and furans) of the biological processes for succinic acid production. The aim of this paper was to evaluate the growth of A. succinogenes for the production of succinic acid under acetic acid and furfural at different concentration on inoculum with preadaptation and without adaptation. The results highlighted that pre-inoculum adaptation was essential for the growth of the strain and succinic acid production that decreased strongly under the synergistic effect of acetic acid and furfural in the broth (63% inhibition rate of growth) respect to inoculum with pre-inoculum adaptation (29% inhibition rate of growth). Succinic acid was a building block chemical of many industrial products, among these, one of the most interesting applications concerns the bioplastics market. This compound can be produced through the biotechnological routes, but several challenges still need to be addressed. One of the greatest challenges has been the downstream process to recover and purify succinic acid from the fermentative broth. Succinic acid must be taken from the fermentation broth, that was composed of various compounds, such as cellular residues, proteins, and other acids, to reach a recovery and a purity >90%, in order to be marketed. Generally, the downstream process can be divided into three phases: pretreatment, separation and purification. Several studies have been carried out on different technologies with the aim to improve recovery and purity. To date, all these methods were applied only on laboratory-scale, due to various critical issues that emerged, such as high costs, which prevented their application on an industrial scale. Succinic acid is one of the most used building blocks in different production chains. Conventionally, this carboxylic acid is produced through petrochemical routes, but can be also produced via biological routes. Many microorganisms, such as Actinobacillus succinogenes, synthesized succinic acid as an intermediate of many biochemical pathways, as the Krebs cycle. Nowadays, the industrial production of bio-succinic acid has been limited due to different critical issues, like the low productivity and yield and downstreaming processes. To improve the bio-based production, the operative conditions of the fermentation process needed to be optimized especially when agro-industrial residues were used as sugars feedstock. To this aim, in this study fermentation batch tests were performed by using different concentrations of A. succinogenes at a given glucose concentration. The results suggested that glucose consumption and succinic acid production were not directly influenced by the initial bacterial concentration respect to glucose concentration. But the best results were obtained at the ratio of bacteria:glucose equal to 1:1 and further tests will be necessary to implement succinic acid yield and productivity. [ABSTRACT FROM AUTHOR]

High-flow nasal cannula (HFNC) has recently emerged as a crucial therapeutic strategy for hypoxemic patients both in acute and chronic settings. Indeed, HFNC therapy is able to deliver higher fractions of inspired oxygen (FiO
2 ) with a heated and humidified gas flow ranging from 20 up to 60 L per minute, in a more comfortable way for the patient in comparison with Conventional Oxygen Therapy (COT). In fact, the flow keeps the epithelium of the airways adequately moisturized, thus positively affecting the mucus clearance. Finally, the flow is able to wash out the carbon dioxide in the dead space of the airways; this is also enhanced by a modest positive end-expiratory pressure (PEEP) effect. Recent evidence has shown applications of HFNC in exercise training and chronic settings with promising results. In this narrative review, we explored how HFNC might contribute to enhancing outcomes of exercise training and pulmonary rehabilitation among patients dealing with chronic obstructive pulmonary disease, interstitial lung diseases, and lung cancer.
