L’objectiu general d’aquest informe és avaluar l’ús de la mesura de TcPO2 en el tractament del peu diabètic i comparar la seva seguretat, validesa diagnòstica, utilitat pronòstica en l’evolució del peu diabètic i utilitat predictiva en el tractament vascular del peu diabètic en comparació amb altres proves utilitzades per avaluar l’estat vascular del pacient, principalment l’ITB. The overall aim of this report is to evaluate the use of TcPO2 measurement in diabetic foot management and to compare its safety, diagnostic validity, prognostic utility in diabetic foot outcomes, and predictive utility in diabetic foot vascular management with other tests used to assess the patient’s vascular status, primarily ABI.

L’objectiu general de l’informe va ser avaluar la seguretat, l’eficàcia i els aspectes econòmics, organitzatius, socials, legals i ètics de la implementació de l’espirometria telemàtica autoadministrada per part dels pacients per al seguiment de les MRC en comparació de l’espirometria convencional feta per personal d’infermeria o tècnics especialitzats en una consulta de pneumologia hospitalària o al centre d’atenció primària, segons el protocol de seguiment del centre en qüestió, centrant-se en pacients de qualsevol edat amb asma o FQ i en adults amb MPOC. El presente informe se planteó con la finalidad de obtener información relevante para determinar si la espirometría telemática autoadministrada por parte de los pacientes con asma, EPOC o FQ, debería ofrecerse dentro de la cartera de servicios comunes del Sistema Nacional de Salud (SNS). El objetivo general del informe fue evaluar la seguridad, la eficacia y los aspectos económicos, organizativos, sociales, legales y éticos de la implementación de la espirometría telemática autoadministrada por parte de los pacientes para el seguimiento de las ERC en comparación con la espirometría convencional realizada por personal de enfermería o técnicos especializados en una consulta de neumología hospitalaria o en el centro de atención primaria, según el protocolo de seguimiento del centro en cuestión, centrándose en pacientes de cualquier edad con asma o FQ y adultos con EPOC. The present report was carried out to obtain relevant information with a view to determining whether asthma, COPD or CF patient self-performed telematic spirometry should be offered among the common services of the Spanish National Health System (Sistema Nacional de Salud [SNS]). The general objective of the report was to assess the safety, efficacy and economic, organizational, social, legal and ethical aspects of the adoption of patient self-performed telematic spirometry for the monitoring of CRDs versus conventional spirometry performed by nursing staff or specialized technicians in the hospital pneumology or primary care setting, based on the monitoring protocol of the centre involved, and focused on patients of any age with asthma or CF, and on adults with COPD.

Atenció primària; Teleconsulta; Avaluació Atención primaria; Teleconsulta; Evaluación Primary care; Teleconsultation; Evaluation L'atenció primària de salut actualment s’enfronta a una limitació en recursos, sobretot econòmics i de força de treball. A més, la pandèmia pel coronavirus SARS-CoV-2 (COVID-19) ha posat de manifest més que mai la importància de poder atendre pacients de manera remota (a distància). La teleconsulta (entesa com la comunicació a distància entre professional de la salut i pacient, bé de forma síncrona a través de videotrucada, trucada telefònica o missatgeria instantània, o bé de forma asíncrona a través de correu electrònic en plataformes segures, eConsulta o similar) es planteja com una possible eina que podria ajudar a estalviar recursos econòmics i de disponibilitat de temps a tots els actors que formen part de la cadena de valor, mantenir o millorar la qualitat de l’atenció (resultats en salut) i l’experiència de l’usuari, i garantir la futura viabilitat dels serveis d’atenció primària. La atención primaria de salud se enfrenta actualmente a una limitación en recursos, sobre todo económicos y de fuerza de trabajo. Además, la pandemia por el coronavirus SARS-CoV-2 (COVID19) ha puesto de manifiesto más que nunca la importancia de poder atender a pacientes de forma remota (a distancia). La teleconsulta (entendida como la comunicación a distancia entre profesional de la salud y paciente, bien de forma síncrona a través de videollamada, llamada telefónica o mensajería instantánea, bien de forma asíncrona a través de correo electrónico en plataformas seguras, eConsulta o similar) se plantea como una posible herramienta que podría ayudar a ahorrar recursos económicos y de disponibilidad de tiempo a todos los actores que forman parte de la cadena de valor, manteniendo o mejorando la calidad de la atención (resultados en salud) y la experiencia del usuario, y garantizando la viabilidad futura de los servicios de atención primaria. Primary healthcare is currently facing a restriction of available resources, especially in terms of finances and workforce. In addition, the SARS-CoV-2 coronavirus pandemic (COVID-19) has highlighted more than ever the importance of being able to care for patients remotely. Teleconsultation (understood as remote communication between health professional and patient, either synchronously via video call, telephone call or instant messaging, or asynchronously via email on secure platforms, eConsultation or similar) is seen as a possible tool that could help save financial resources and time availability for all the actors involved in the value chain, maintaining or improving the quality of care (health outcomes) and the user experience, and guaranteeing the future viability of primary care services.

Concretamente, el presente informe aborda seis subgrupos o poblaciones de pacientes: aquellos con cánceres primarios localizados (estadio I o II) en pulmón o bronquios (a partir de aquí: cáncer de pulmón), próstata o hígado, así como aquellos con tumores con oligometástasis (estadio IV) que incluyen al menos una en pulmón o bronquios, hígado, o con oligometástasis ganglionares secundarias a cáncer próstata. The aim of this report is to describe the safety, efficacy/effectiveness and efficiency of SBRT in comparison with conventional non-pharmacological treatments, as well as the organizational, ethical, social and legal aspects linked to its use, in adult patients with lung, prostate and/or liver cancer. Specifically, this report addresses six subgroups or patient populations: those with localized primary cancers (stage I or II) in the lung or bronchus (hereafter: lung cancer), prostate or liver, as well as those with tumors with oligometastases (stage IV) that include at least one in the lung or bronchus, liver, or with lymph node oligometastases secondary to prostate cancer.

- Abstract
Purpose: To evaluate whether consumption of organic food and reduced intake of meat products in pregnancy are associated with lower prevalence of gestational diabetes (GD). Methods: Women participating in the KOALA Birth Cohort Study with valid informed consent, a singleton pregnancy and information on their food intake were considered in this cross-sectional analysis. Participants with and without GD were compared with each other in terms of dietary characteristics (n = 37 and n = 2766, respectively). Multivariable logistic regression (LR) was used to adjust for relevant covariates. Results: Organic food consumption tended to be lower, although not significantly, in women with GD compared to women without GD, whereas consumption of meat was positively associated with GD prevalence. LR modelling showed that GD was significantly associated with higher consumption of meat and, in addition, also of cheese, after adjustment for other relevant covariates. GD was associated with some indicators of animal product intake, namely dietary animal to plant protein ratio and maternal plasma arachidonic acid (for the latter, data available for n = 16 and n = 1304, respectively). Food patterns of participants with GD were characterised by more meat products and less vegetarian products. Conclusions: Due to the low number of participants with GD, results have to be interpreted cautiously. Consumption of organic food during pregnancy does not seem to be markedly associated with a lower GD prevalence; lower intake of meat and cheese, irrespective of its origin (organic or conventional), does. The latter supports previous studies suggesting a causal association between consumption of animal products and GD. Recerca realitzada durant el periode Gener 2006-Abril 2007, a la Facultat de Farmàcia de la Universitat de Barcelona sota la direcció de la Dra. M. Carmen López Sabater. La nutrició durant els primers anys de vida té una enorme importància, amb repercussions en el creixement i desenvolupament del nen i en les possibles malalties futures. Tenint en compte que la llet materna és l’aliment ideal per al nadó durant els primers sis mesos de vida, és important conéixer la composició nutricional de la mateixa i la seva evolució al llarg de la lactància (calostre, llet de transició i madura). La llet conté nutrients majoritaris (proteïnes, carbohidrats i lípids) i nutrients minoritaris, com vitamines i minerals. S'ha desenvolupat i validat un mètode per a la determinació d'àcids grassos (AG) i Àcid Linoleic Conjugat (CLA) en llet materna per Cromatografia de Gasos "fast" (fast-GC), permetent reduir el temps d'anàlisi considerablement en comparació amb els mètodes utilitzats fins el moment. La repetibilitat i reproducibilitat trobades han estat bones, amb coeficients de variació inferiors al 10% en tots els casos. L'aplicació a 6 mostres de llet materna ha demostrat que es tracta d'un mètode senzill, ràpid, eficient i pràctic per a l'anàlisi rutinari d'un elevat nombre de mostres. També s'ha començat a desenvolupar un mètode per a l'anàlisi dels anàlegs de tocoferol de la llet materna, degut a l'interès de la vitamina E com a antioxidant natural. És un mètode per Cromatografia Líquida d'Ultra Resolució (UPLC) amb detecció per Fluorescència (FD) i per Photodiode Array (PDA) que permet treballar amb quantitats de mostra petites mantenint o inclús millorant la sensibilitat i reduint el temps d’anàlisi. Research developed during January 2006-April 2007 at the Faculty of Pharmacy, University of Barcelona, under the direction of Dra. M. Carmen López Sabater. Early nutrition is extremely important, not only because it directly affects children's health but also because it is associated with some diseases in adulthood. Taking into account that human milk is the ideal form for nourishing infants during the first 6 months of life, it should be interesting to know human milk's composition as well as its evolution during the different stages of lactation (colostrum, transitional and mature milk). Milk contains major and minor nutrients (proteins, carbohydrates and lipids, and vitamins and minerals, respectively). We have developed and validated a method for the determination of Conjugated Linoleic Acid (CLA) in human milk by fast-Gas Chromatography (fast-GC). It permits to save time in comparison with the current methods for the analysis of CLA. Repetibility and reproducibility have shown good results, with relative standard deviations (R.S.D.) &10% in all cases. The application of the method to 6 human milk samples have demostrated that this is a simple, fast, efficient and practical method for the rutine analysis of a large number of samples. In addition, we have developed a new method for the determination of tocopherols in human milk by Ultra Performance Liquid Chromatography (UPLC) with Fluorescence Detector (FD) and Photodiode Array Detector (PDA), which permits to save time and use smaller amounts of sample in comparison with HPLC methods.

Objective: Single nucleotide polymorphisms (SNPs) in genes involved in fatty acid metabolism (FADS1 FADS2 gene cluster) are associated with plasma lipid levels. We aimed to investigate whether these associations are already present early in life and compare the relative contribution of FADS SNPs vs traditional (non-genetic) factors as determinants of plasma lipid levels. Methods: Information on infants’ plasma total cholesterol levels, genotypes of five FADS SNPs (rs174545, rs174546, rs174556, rs174561, and rs3834458), anthropometric data, maternal characteristics, and breastfeeding history was available for 521 2-year-old children from the KOALA Birth Cohort Study. For 295 of these 521 children, plasma HDLc and non-HDLc levels were also known. Multivariable linear regression analysis was used to study the associations of genetic and non-genetic determinants with cholesterol levels. Results: All FADS SNPs were significantly associated with total cholesterol levels. Heterozygous and homozygous for the minor allele children had about 4% and 8% lower total cholesterol levels than major allele homozygotes. In addition, homozygous for the minor allele children had about 7% lower HDLc levels. This difference reached significance for the SNPs rs174546 and rs3834458. The associations went in the same direction for non-HDLc, but statistical significance was not reached. The percentage of total variance of total cholesterol levels explained by FADS SNPs was relatively low (lower than 3%) but of the same order as that explained by gender and the non-genetic determinants together. Conclusions: FADS SNPs are associated with plasma total cholesterol and HDLc levels in preschool children. This brings a new piece of evidence to explain how blood lipid levels may track from childhood to adulthood. Moreover, the finding that these SNPs explain a similar amount of variance in total cholesterol levels as the non-genetic determinants studied reveals the potential importance of investigating the effects of genetic variations in early life. Background: It has been suggested that prenatal exposure to n-3 long-chain fatty acids protects against asthma and other allergy-related diseases later in childhood. The extent to which fish intake in pregnancy protects against child asthma and rhinitis symptoms remains unclear. We aimed to assess whether fish and seafood consumption in pregnancy is associated with childhood wheeze, asthma and allergic rhinitis. Methods: We pooled individual data from 60 774 mother-child pairs participating in 18 European and US birth cohort studies. Information on wheeze, asthma and allergic rhinitis prevalence was collected using validated questionnaires. The time periods of interest were: infancy (0-2 years), preschool age (3-4 years), and school age (5-8 years). We used multivariable generalized models to assess associations of fish and seafood (other than fish) consumption during pregnancy with child respiratory outcomes in cohort-specific analyses, with subsequent random-effects meta-analyses. Results: The median fish consumption during pregnancy ranged from 0.44 times/week in The Netherlands to 4.46 times/week in Spain. Maternal fish intake during pregnancy was not associated with offspring wheeze symptoms in any age group nor with the risk of child asthma [adjusted meta-analysis relative risk (RR) per 1-time/week = 1.01, 95% confidence interval 0.97-1.05)] and allergic rhinitis at school age (RR = 1.01, 0.99-1.03). These results were consistently found in further analyses by type of fish and seafood consumption and in sensitivity analyses. Conclusion: We found no evidence supporting a protective association of fish and seafood consumption during pregnancy with offspring symptoms of wheeze, asthma and allergic rhinitis from infancy to mid childhood. Background: Digital health technologies (dHTs) offer a unique opportunity to address some of the major challenges facing health care systems worldwide. However, the implementation of dHTs raises some concerns, such as the limited understanding of their real impact on health systems and people's well-being or the potential risks derived from their use. In this context, health technology assessment (HTA) is 1 of the main tools that health systems can use to appraise evidence and determine the value of a given dHT. Nevertheless, due to the nature of dHTs, experts highlight the need to reconsider the frameworks used in traditional HTA., Objective: This scoping review (ScR) aimed to identify the methodological frameworks used worldwide for digital health technology assessment (dHTA); determine what domains are being considered; and generate, through a thematic analysis, a proposal for a methodological framework based on the most frequently described domains in the literature., Methods: The ScR was performed in accordance with the guidelines established in the PRISMA-ScR guidelines. We searched 7 databases for peer reviews and gray literature published between January 2011 and December 2021. The retrieved studies were screened using Rayyan in a single-blind manner by 2 independent authors, and data were extracted using ATLAS.ti software. The same software was used for thematic analysis., Results: The systematic search retrieved 3061 studies (n=2238, 73.1%, unique), of which 26 (0.8%) studies were included. From these, we identified 102 methodological frameworks designed for dHTA. These frameworks revealed great heterogeneity between them due to their different structures, approaches, and items to be considered in dHTA. In addition, we identified different wording used to refer to similar concepts. Through thematic analysis, we reduced this heterogeneity. In the first phase of the analysis, 176 provisional codes related to different assessment items emerged. In the second phase, these codes were clustered into 86 descriptive themes, which, in turn, were grouped in the third phase into 61 analytical themes and organized through a vertical hierarchy of 3 levels: level 1 formed by 13 domains, level 2 formed by 38 dimensions, and level 3 formed by 11 subdimensions. From these 61 analytical themes, we developed a proposal for a methodological framework for dHTA., Conclusions: There is a need to adapt the existing frameworks used for dHTA or create new ones to more comprehensively assess different kinds of dHTs. Through this ScR, we identified 26 studies including 102 methodological frameworks and tools for dHTA.
