BACKGROUND: "Slow flow" is one very important concept in modern fundamental and clinical biomedicine. Slow coronary flow is indicative of delayed filling of the terminal coronary artery vessels, occurring in the absence of significant coronary stenosis. This group patient of patients exhibits a high incidence of disability and represents a significant financial and material burden for the state and the healthcare system in general. OBJECTIVE: The primary objective of our study was to examine patients with slow coronary flow. METHODS: We studied the standard parameters recommended by the international health care system (electrocardiography (by Medica QRS-12, Germany), through the electrical activity of a patient's heart by the electrical impulses (beating) of the heart; HC1(Germany); coagulogramma by Coatron M1 (Germany), troponin by AQT 90 (Germany); general blood test we used automatic human counting device HC1(Germany). Also, we investigate the original parameters (non-standard parameters, which we use in this pilot study) that we were first studied for this diagnosis and non-standard parameters. RESULTS: A general blood test showed that patients with slow flow had a higher blood leukocyte count than the control group, but the amount of hemoglobin was normal, the hematocrit was much higher than in the control group, and the platelet count was close to the lower limit of clinical standards. We obtained details of blood flow by coagulation situation, such as prothrombin time, prothrombin index, international normalized ratio, activated partial thromboplastin time, thrombin time, fibrinogen, and rheological properties such as index of erythrocyte aggregability, index of erythrocyte deformability, plasma viscosity, in silico blood rheological index. CONCLUSSION: Blood flow can be considered as a superposition of vortices with similar frequencies and wave vectors that change after bifurcations or other obstacles in the vascular network. These factors together determine the conditions for structuring the flow of moving blood. Disruption or alteration of these factors results in slow flow. It has been found that the speed of blood flow in the coronary arteries depends on changes in the number and function of red blood cells. Slow flow is directly influenced by the aggregation and deformation of red blood cells, their number, and plasma viscosity. BACKGROUND: Limb-threatening ischemia is one of more important problem of fundamental and practice modern angiology. Despite the development of modern diagnostic and therapeutic approaches good results, Limb-Threatening Ischemia deserves special attention and occupies a special place among medical, biomedical and socio-economic problems. A comprehensive study will make it possible to develop new approaches, the result of which should be a decrease in the total number of diseases, as well as a minimization of complications leading to large-scale tissue losses and amputations. OBJECTIVE: We set out to develop a new non-invasive method for studying microcirculation in patients with limb-threatening ischemia. METHODS: We investigated control group and adults of both sexes (age: 50–75 years) with a confirmed diagnosis of the claudication. Men and women in ratio 1 : 1 in control and target groups. RESULTS: The research results showed that the original microcirculation coefficient calculated according to our algorithm in patients with intermittent claudication was 70% better than in the control group and 120% of patients were worse than in patients with Limb-Threatening Ischemia. CONCLUSION: Our research team created an original so called coefficient of microcirculation. In order to determine it, we studied the blood flow in the pulsations on the left and right side of the posterior tibial arteries of the legs at rest (names of lines SIN F, DEX F) and during post-ischemic hyperemia in the same lines. The study of microcirculation based on the microcirculation coefficient is very informative. The importance of this methodology for studying the total peripheral blood flow and arteriolar response is due to fundamental and clinical developments, as well as financial costs. Considering the technique described in the article can be easily mastered by medical personnel, it is easy to use in the inpatient and outpatient sectors, its use does not require large areas of the medical facility, the equipment is not expensive and the test itself does not cause inconvenience to the patient, then we can recommend including the described method is included in the list of functional diagnostic tests as a direct and quantitative method for determining microcirculation (without special costs on the part of medical institutions, insurance companies and the patient). The aim of the present study was the comparative analysis of the role of specific hemorheological derangements in the pathogenesis of ischemic brain infarcts with and without diabetes mellitus. Blood plasma viscosity, as well as red blood cells (RBC) aggregability were quantitatively investigated in all patients during the study. Both of the above mentioned indices of hemorheological disorders were significantly higher in the patients with brain infarcts and diabetes mellitus as compared to the control and the group of ischemic brain infarcts without diabetes. During the ischemic stroke, blood plasma viscosity was increased by a mean of 9.2 per cent in the first and by a mean of 17.6 per cent in the second group of patients. As to the erythrocyte aggregability index, it was changed considerably more – by a mean of 113.8 per cent during stroke without diabetes, while in the diabetic patients the RBC aggregability index was found to be increased by a mean of 147.3 per cent. Ischemic brain infarcts in patients with diabetes mellitus were associated with a significant increase of the patients' blood rheological disorders in all the investigated cases. The aim of the present study was to evaluate blood rheological disorders, in particular RBC enhanced aggregation, and compare changes in these parameters in patients with type 1 and 2 diabetes mellitus. For evaluation of RBC aggregability in the present study we applied the “Georgian technique”, which was developed and applied for several years in our laboratory. Its advantage is that it is a direct and quantitative method. In all the investigated patients, diabetes mellitus was complicated with the foot gangrenes. The patients were divided into two groups: (a) with type 1 and (b) with type 2 diabetes mellitus. We matched the RBC aggregability indices in both groups of diabetic patients and compared the obtained results with those in the healthy control group. We found that rheological disorders were considerably pronounced. The RBC aggregability index increased by 62 percent (p<0.001) in type 1 and by 57 percent (p<0.001) in type 2 diabetic patients as compared to the control group. However, there were insignificant differences of the RBC aggregability changes between the two groups of patients. Therefore we conclude that blood rheological disorders are similar in both types of diabetes mellitus. The disturbed blood fluidity related to the increased RBC aggregability in the microcirculation promotes, in particular, the development of legs gangrene in both types of diabetes mellitus.
