Objective: to compare the efficacy of 80% (w/v) alcohol, rubbed for 30 and 60 seconds, in the manual processing of stainless-steel wash bowls, after cleaning with running water and neutral detergent. Method: experimental study conducted in a hospital in the state of São Paulo, Brazil, on 50 bowls randomly divided into two groups of 25 bowls each for interventions of 30 and 60 seconds of rubbing with 80% (w/v) alcohol. Results: based on the microbiological analyses collected, before and after the interventions for both groups, partial efficacy of the disinfectant was observed even when extending rubbing time. In both groups, there was a higher prevalence of survival of Pseudomonas aeruginosa, with 14 strains that were resistant to carbapenems, being, specifically, 11 to imipenem and three to meropenem. Conclusion: stainless-steel bed wash bowls decontaminated for reuse by 80% (w/v) alcohol, after cleaning with running water and neutral detergent, showed to be reservoirs of hospital pathogens. The use of bed wash bowls for patients with intact skin would not have worrying consequences, but considering those with non-intact skin and the contamination of professionals’ hands, the results in this study justify the search for other decontamination methods or the adoption of disposable bed baths. Descriptors: Objetivo: comparar a eficácia do álcool 80% (p/v), friccionado por 30 e 60 segundos, no processamento manual de bacias de banho em aço inoxidável, após limpeza com água corrente e detergente neutro. Método: estudo experimental realizado em hospital do estado de São Paulo, Brasil, com 50 bacias randomicamente distribuídas em dois grupos de 25, para as intervenções de 30 e 60 segundos de fricção com álcool 80% (p/v). Resultados: das análises microbiológicas coletadas, antes e após as intervenções para os dois grupos, verificou-se eficácia parcial do desinfetante, mesmo ampliando o tempo de fricção. Em ambos os grupos, observou-se maior prevalência de sobrevida de Pseudomonas aeruginosa, 14 cepas resistentes a carbapenens, especificamente, 11 ao imipenen e três ao meropenen. Conclusão: bacias de banho no leito em aço inoxidável, descontaminadas para reuso com álcool 80% (p/v), após limpeza com água corrente e detergente neutro, apresentam-se como reservatórios de patógenos hospitalares. O uso das bacias de banho no leito para pacientes com pele íntegra não teria consequências preocupantes, mas para aqueles com pele não íntegra e pensando na contaminação das mãos dos profissionais, os resultados dessa pesquisa justificam a busca de outros métodos de descontaminação ou a adoção de banho de leito descartável. Objetivo: comparar la eficacia del alcohol al 80% (p/v), frotado durante 30 y 60 segundos, en el proceso de descontaminación manual de palanganas de baño de acero inoxidable, después de lavarlas con agua corriente y detergente neutro. Método: estudio experimental realizado en un hospital del estado de São Paulo, Brasil, con 50 palanganas divididas aleatoriamente en dos grupos de 25, para intervenciones de 30 y 60 segundos de frotamiento con alcohol al 80% (p/v). Resultados: los análisis microbiológicos recolectados, antes y después de las intervenciones para ambos grupos, demostraron efectividad parcial del desinfectante, incluso cuando se extendió el tiempo de fricción. En ambos grupos, se observó una mayor prevalencia de supervivencia de Pseudomonas aeruginosa, 14 cepas resistentes a carbapenemas, específicamente 11 a imipenem y tres a meropenem. Conclusión: las palanganas de baño de cama de acero inoxidable, descontaminadas para su reutilización con alcohol al 80% (p/v), después del lavado con agua corriente y detergente neutro, actúan como reservorios de patógenos hospitalarios. El uso de las palanganas de baño de cama no tendría consecuencias preocupantes para pacientes con la piel íntegra, pero para aquellos cuya piel no conserva su integridad y pensando en la contaminación de las manos de los profesionales, los resultados de esta investigación justifican la búsqueda de otros métodos de descontaminación o la adopción del baño de cama desechable.

Objective: To identify the use of negative pressure therapy and its applicability in Fournier's Syndrome. Method: This is an integrative literature review, with a descriptive character and a qualitative approach, carried out on the bases PubMed Central and Scientific Electronic Library Online, from August to September 2020. Results: After the establishment of the eligibility criteria, 10 were selected studies, which were analyzed according to the content theme and categorized into: “Negative pressure therapy: use and benefits” and “Use of negative pressure therapy in Fournier's Syndrome”. Conclusion: Of all the studies selected for the on-screen review, 100% recommended the use of negative pressure therapy, either alone, with instillation or length of stay, to treat wounds resulting from Fournier's Syndrome, allowing patients less pain, less time hospitalization time, shorter total wound closure time. Objetivo: Identificar el uso de la terapia de presión negativa y su aplicabilidad en el Síndrome de Fournier. Método: Se trata de una revisión bibliográfica integradora, de carácter descriptivo y enfoque cualitativo, realizada sobre las bases PubMed Central y Scientific Electronic Library Online, de agosto a septiembre de 2020. Resultados: Tras el establecimiento de los criterios de elegibilidad, se seleccionaron 10 estudios, que fueron analizados según la temática de contenido y categorizados en: “Terapia de presión negativa: uso y beneficios” y “Uso de la terapia de presión negativa en el Síndrome de Fournier”. Conclusión: De todos los estudios seleccionados para la revisión en pantalla, el 100% recomendó el uso de la terapia de presión negativa, ya sea sola, con instilación o duración de la estancia, para tratar las heridas resultantes del síndrome de Fournier, lo que permite a los pacientes menos dolor, menos tiempo. tiempo de hospitalización, menor tiempo total de cierre de la herida. Objetivo: Identificar o uso da terapia por pressão negativa e sua aplicabilidade na Síndrome de Fournier. Método: Trata-se de uma revisão integrativa da literatura, de caráter descritivo e abordagem qualitativa, realizada nas bases PubMed Central e Scientific Electronic Library Online, de agosto a setembro de 2020. Resultados: Após o estabelecimento dos critérios de elegibilidade, foram selecionados 10 estudos, os quais foram analisados de acordo com a temática de conteúdo e categorizados em: “Terapia por pressão negativa: uso e benefícios” e “Uso da terapia por pressão negativa na Síndrome de Fournier”. Conclusão: De todos os estudos selecionados para a revisão em tela, 100% recomendou o uso de terapia por pressão negativa, seja sozinha, com instilação ou tempo de permanência, para tratamento de feridas resultantes da Síndrome de Fournier, possibilitando ao pacientes menor dor, menor tempo de hospitalização, menor tempo de fechamento total da ferida.

Introduction: As the Peripherally Inserted Central Catheter (PICC) is the first choice of prolonged vascular access in preterm infants, to recognize the risk factors for infection related to its use contributes to the establishment of procedures that qualify care. Objective: To evaluate the production of knowledge available in the literature about the work of nurses in the prevention of infection related to PICC. Method: An integrative literature review according to Ganong’s assumptions, was performed in the SciELO, LILACS, BDENF, MEDLINE and PubMed databases. Data collection took place in October 2017, including original articles made available in full from 2001. Results were organized using Bardin’s content analysis. Results: Only eleven articles met the inclusion criteria and four thematic categories emerged: 1) Theoretical and practical knowledge assessment of the nurse to prevent infection in the insertion and maintenance of PICC, 2) Permanent education of the nursing team, 3) Implementation and use of protocols, and 4) Constant monitoring of quality indicators. Conclusions: The present work, through the analysis of the collected studies, signals the need to create institutional protocols, training and permanent and continuing education, to use de indicators in the prevention of infection, aiming at patient care and safety and, consequently, resulting in lower incidence of bloodstream infections through the use of PICC. Introdução: Como o cateter central de inserção periférica (PICC) é a primeira escolha de acesso vascular prolongado em neonatos, reconhecer os fatores de risco associados a infecções relacionadas ao seu uso contribui para estabelecer critérios de manuseio e manutenção do dispositivo que qualifiquem a assistência do enfermeiro e de sua equipe. Objetivo: Avaliar a produção do conhecimento científico na literatura acerca da atuação do enfermeiro na prevenção de infecção de corrente sanguínea pelo uso do PICC. Método: Revisão integrativa da literatura seguindo os pressupostos de Ganong, nas bases de dados SciELO, LILACS, BDENF, MEDLINE e PubMed. A coleta de dados ocorreu em outubro de 2017, incluindo artigos disponibilizados na íntegra a partir de agosto de 2001 a outubro de 2017. Para a organização dos resultados, foi utilizada a análise de conteúdo de Bardin. Resultados: Onze artigos preencheram os critérios de inclusão, emergindo quatro categorias temáticas: 1) Conhecimento teórico-prático do enfermeiro para prevenção de infecção na inserção e manutenção do PICC, 2) Educação permanente da equipe de enfermagem, 3) Implantação e utilização de protocolos e 4) Vigilância constante de indicadores de qualidade. Conclusões: O presente trabalho observou o despreparo do profissional da enfermagem quanto ao dispositivo PICC e demonstrou a necessidade de elaboração de protocolos institucionais, treinamento e educação continuada permanente e o uso de indicadores, direcionados às medidas preventivas contra a infecção do PICC. Essas medidas visam melhorar a qualidade da assistência e segurança do paciente e consequentemente, resultar em menor incidência de infecções de corrente sanguínea pelo uso do PICC.

Introduction: The control of metabolic acidosis in dialysis patients focuses on the supply of bicarbonate during the dialysis session, and it is not standard in all hemodialysis to assess serum bicarbonate concentrations. Bicarbonate expressed in blood gas analysis is the most sensitive standard of analysis and it is measured indirectly, using the Henderson-Hasselbalch equation. There are no studies in this population evaluating the concordance between the calculated bicarbonate with the direct method of biochemical analysis. The aim of this study was to analyze the concordance between the measured and calculated serum bicarbonate levels using blood gas analysis. Methods: We analyzed blood samples from chronic kidney patients undergoing hemodialysis, using the same sample of bicarbonate analysis by biochemistry and gasometry. The concordance was assessed using the Bland-Altman method. Results: 51 samples were analyzed. The analysis revealed a high correlation (r = 0.73) and a mean difference (bias) of 1.15 ± 3 mmol/L. The median time between collection and examination was 241 minutes. Discussion: We can conclude that the biochemical bicarbonate analysis compared to that calculated from blood gas analysis in chronic renal patients was consistent. For greater concordance between the data, it is important that the time between the collection of the samples and the referral to the laboratory for carrying out the dosages does not exceed four hours. The serum bicarbonate dosage can result in cost savings when compared to that of bicarbonate in blood gas analysis. Resumo Introdução: O controle da acidose metabólica em pacientes dialíticos está voltado, principalmente, para o suprimento de bicarbonato durante a sessão de diálise, não sendo padrão em todas as hemodiálises avaliar as concentrações séricas do bicarbonato. O bicarbonato expresso na gasometria é considerado o padrão mais sensível de análise e é medido indiretamente por meio da equação de Henderson-Hasselbalch. Não há estudos nessa população avaliando a concordância do bicarbonato calculado com o método direto de análise bioquímica. O objetivo deste estudo é analisar a concordância entre o bicarbonato sérico medido e o calculado por meio da gasometria. Métodos: Foram analisadas amostras de sangue de pacientes renais crônicos em hemodiálise sendo feito na mesma amostra de análise do bicarbonato pela bioquímica e análise pela gasometria. A concordância foi avaliada pelo método de Bland-Altman. Resultados: Foram analisados um total de 51 amostras. A análise de correlação revelou alta correlação (r = 0.73) e a diferença média (bias) de 1.15 ± 3 mmol/L. O tempo mediano entre a realização da coleta e do exame foi de 241 minutos. Discussão: Podemos concluir que a realização da dosagem bioquímica do bicarbonato comparada com a calculada a partir da gasometria em pacientes renais crônicos foi concordante. Para maior concordância entre os dados, é importante que o tempo entre a coleta das amostras e o encaminhamento ao laboratório para a realização das dosagens não exceda quatro horas. A dosagem do bicarbonato sérico pode resultar numa economia de custos comparada à do bicarbonato da gasometria.

Methods: This was a parallel, randomized clinical trial with an intervention group (bag bath) and a control group (conventional bed bath), conducted in a public hospital in Sao Paulo, Brazil, from November 2014 to December 2015. The participants were adult and older inpatients, bedridden and depending on the procedure. The product assessed was Bag Bath (R). Results: The microbial load decreased in the intervention group (20 patients), while it increased significantly (p < 0.001) in the control group (20 patients). The estimated efficacy of the product for bag bath was 90%, compared with 20% for the conventional bed bath. Conclusion: The product assessed was 4.5 times more effective to decrease the inpatient skin microbial load when compared with the conventional bed bath, suggesting the need for nursing teams to re-evaluate this procedure. It is a publication based on "Recommendations for Clinical Preventive Services" published by the United States Preventive Services Task Force (USPSTF). These recommendations are provided only as an aid to physicians in making clinical decisions about the care of their patients. They reflect the clinic references existing at the time of publication. But its only should be used with the clear understanding that continued research may result in new knowledge and consequently there is a need for updates. Some recommendations of the USPSTF are important in clinical practice with the elderly.

Background: Bloodstream infections are responsible for thousands of deaths each year. The rapid identification of the microorganisms causing these infections permits correct therapeutic management that will improve the prognosis of the patient. In an attempt to reduce the time spent on this step, microorganism identification devices have been developed, including the VITEK® 2 system, which is currently used in routine clinical microbiology laboratories. Methods: This study evaluated the accuracy of the VITEK® 2 system in the identification of 400 microorganisms isolated from blood cultures and compared the results to those obtained with conventional phenotypic and genotypic methods. In parallel to the phenotypic identification methods, the DNA of these microorganisms was extracted directly from the blood culture bottles for genotypic identification by the polymerase chain reaction (PCR) and DNA sequencing. Results: The automated VITEK® 2 system correctly identified 94.7 % (379/400) of the isolates. The YST and GN cards resulted in 100 % correct identifications of yeasts (15/15) and Gram-negative bacilli (165/165), respectively. The GP card correctly identified 92.6 % (199/215) of Gram-positive cocci, while the ANC card was unable to correctly identify any Gram-positive bacilli (0/5). Conclusions: The performance of the VITEK® 2 system was considered acceptable and statistical analysis showed that the system is a suitable option for routine clinical microbiology laboratories to identify different microorganisms. Background: Species identification and antifungal susceptibility tests were carried out on 212 Candida isolates obtained from bloodstream infections, urinary tract infections and dialysis-associated peritonitis, from cases attended at a Brazilian public tertiary hospital from January 1998 to January 2005. Findings: Candida albicans represented 33% of the isolates, Candida parapsilosis 31.1%, Candida tropicalis 17.9%, Candida glabrata 11.8%, and others species 6.2%. In blood culture, C. parapsilosis was the most frequently encountered species (48%). The resistance levels to the antifungal azoles were relatively low for the several species, except for C. tropicalis and C. glabrata. Amphotericin B resistance was observed in 1 isolate of C. parapsilosis. Conclusions: The species distribution and antifungal susceptibility herein observed presented several epidemiological features common to other tertiary hospitals in Latin American countries. It also exhibited some peculiarity, such as a very high frequency of C. parapsilosis both in bloodstream infections and dialysis-associated peritonitis. C. albicans also occurred in an important number of case infections, in all evaluated clinical sources. C. glabrata presented a high proportion of resistant isolates. Objective: to compare the efficacy of 80% (w/v) alcohol, rubbed for 30 and 60 seconds, in the manual processing of stainless-steel wash bowls, after cleaning with running water and neutral detergent. Method: experimental study conducted in a hospital in the state of São Paulo, Brazil, on 50 bowls randomly divided into two groups of 25 bowls each for interventions of 30 and 60 seconds of rubbing with 80% (w/v) alcohol. Results: based on the microbiological analyses collected, before and after the interventions for both groups, partial efficacy of the disinfectant was observed even when extending rubbing time. In both groups, there was a higher prevalence of survival of Pseudomonas aeruginosa, with 14 strains that were resistant to carbapenems, being, specifically, 11 to imipenem and three to meropenem. Conclusion: stainless-steel bed wash bowls decontaminated for reuse by 80% (w/v) alcohol, after cleaning with running water and neutral detergent, showed to be reservoirs of hospital pathogens. The use of bed wash bowls for patients with intact skin would not have worrying consequences, but considering those with non-intact skin and the contamination of professionals' hands, the results in this study justify the search for other decontamination methods or the adoption of disposable bed baths. Desde 2002, foi instituído no país o Programa Nacional de Controle da Dengue (PNCD) com a função de reduzir sua incidência e letalidade, assim como a infestação do vetor. Aos Agentes Comunitários de Saúde (ACS) e Agentes Combate à Endemia (ACE) couberam as atividades de vistoria nos domicílios, prevenção, promoção e educação da comunidade, com objetivo de mobilizar a população no combate à doença, tendo em vista a expressiva epidemia de dengue na cidade de Bauru, estado de São Paulo (SP), com aumento de 50% nos casos de óbito e com aproximadamente 27 mil casos notificados em 2019. Objetivos: Analisar a efetividade das ações dos ACE na visita casa-a-casa e avaliar se as estratégias do PNCD são resolutivas. Método: Trata-se de um estudo retrospectivo baseado em dados vinculados ao Sistema de informação de Agravos de Notificação (SINAN ON-LINE) e ao banco de dados SISAWEB, da Secretaria de Estado da Saúde vinculado à Superintendência de Controle de Endemias (SUCEN) que dispõem das visitas casa-a-casa realizadas pelos agentes de combate a endemias de Bauru. Para a análise estatística foram realizadas associações, utilizando a estatística descritiva (frequências, média e desvio-padrão) e inferencial (testes Qui-quadrado, Exato de Fisher e o teste t de Student) adotando-se 5% de nível significância. Resultados: Das visitas realizadas pelos ACE em 2018, as mulheres, com idade média de 36,32 anos, com ensino médio completo e raça branca apresentaram maior positividade de dengue em 2019. Toda cidade de Bauru e, especificamente, as regiões Norte, Noroeste e Sul, a visita dos ACE representaram fatores de proteção para a dengue. Os locais onde foram realizadas atividades casa-a-casa rotina apresentaram menos casos de dengue. Para alcançar melhorias nos serviços de acompanhamento e atendimento pela Equipe Multidisciplinar é necessário cada vez mais o aprimoramento e eficácia nas condutas terapêuticas. Objetivos: Avaliar e comparar o atendimento multidisciplinar de 5 ambulatórios de Hepatite C do Estado de São Paulo; Avaliar o grau de satisfação dos pacientes quanto à qualidade do atendimento recebido por uma equipe multidisciplinar; Descrever o grau de dificuldade no acompanhamento do tratamento dos portadores de hepatite C; Relacionar e comparar a qualidade de atendimento entre os 5 ambulatórios, descrevendo as diferenças entre ambos. Método: estudo transversal, realizado por meio de aplicação de questionário semiestruturado a 425 pacientes dos Ambulatórios, UNESP/HCFMB; UNIFESP/EPM; USP/FMRP; FAMERP/S.J.RIO PRETO e SANTOS CRAIDES. Este trabalho foi aprovado pelo Comitê de Ética em Pesquisa número 1.850.408. Resultados: na avaliação do acompanhamento e atendimento dos serviços ambulatoriais nos diversos centros observaram-se diferenças estatisticamente significantes. A avaliação direta e indireta do atendimento médico e do atendimento de enfermagem foi classificada como boa e ótima, respectivamente. No entanto, os pacientes relataram não sentir diferenças no atendimento de cada profissional. Conclusão: Os resultados desta pesquisa apontam a necessidade de uma Padronização e Universalização dos Parâmetros Multidisciplinar nos diversos Ambulatórios. Também, sugerem que, apesar da grande importância do tema, na prática, ainda os usuários demonstraram o desconhecimento da existência da equipe multiprofissional durante o seu tratamento. Entretanto, classificam que a equipe médica e de enfermagem durante o acompanhamento e atendimento ambulatorial como boa e ótima. Assim, os resultados apontam a necessidade de que a gerência dos ambulatórios intensifique a informação aos pacientes sobre a existência e importância do papel da equipe multiprofissional. Introduction: According with World Health Organization (WHO), Brazil is classified as a country with intermediary endemicity for the hepatitis C virus. In order to improve monitoring and follow-up services by the Multidisciplinary Group it is necessary to improve efficacy on the therapeutic conduct. Objetivos: Evaluate and compare the Multidisciplinary care among ambulatories of Hepatitis C of the State of Sao Paulo, regarding satisfaction monitoring and follow-up services provided by the ambulatories. Methodology: cross-sectional study, conducted by applying a half structuralized questionnaire to 425 patients of the following ambulatories:, CRAIDES/SANTOS; UNIFESP; FAMERP; USPFMRP and HCFMB. Approved by the Ethics in Research Committee. Results: In the evaluation of the follow-up and care of the ambulatorial`s services in the various centers, significant statistical differences were observed. The direct and indirect evaluation of the medical care and nursing care was classified as good and excellent, respectively. However, patients reported no differences in care from one professional to another. Conclusion: The results of this research suggest that, despite the great importance of the theme, in practice, users still demonstrated the lack of knowledge of the multiprofessional team during their treatment. However, they rate the medical and nursing staff during follow-up and outpatient care as good and great. Thus, the results indicate the need for those responsible for outpatient clinics to intensify to patients the existence and importance of the role of the multidisciplinary team.

Orientador: Carlos Emilio Levy Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: O uso de probióticos tem sido crescente para o tratamento e prevenção de diarreia causada por enteropatógenos, e os gêneros Lactobacillus e Bifidobacterium têm sido os mais utilizados. O objetivo deste estudo foi avaliar a atividade antimicrobiana de alguns probióticos contra enterobactérias patogênicas e seu potencial uso para descolonização de pacientes. Probióticos comercialmente disponíveis foram semeados e incubados para contagem de colônias (UFC) e identificação por espectrometria de massas (MALDI-TOF/MS). Para teste de sensibilidade a drogas, os probióticos foram submetidos ao E-test®. A atividade antimicrobiana dos probióticos contra Klebsiella pneumoniae produtora de carbapenemase (KPC), Escherichia coli, Salmonella enteritidis e Shigella flexneri foi avaliada pelo método de co-cultura, na concentração de 106 UFC/mL, tanto das enterobactérias como de cada microrganismo isolado dos produtos. Amostras foram semeadas nos tempos T0, 24hs, 48hs e 72hs para contagem de UFC remanescentes. Para Lactobacillus reuteri foi adicionado 1% de glicerol. Encontramos 40 produtos probióticos disponíveis no comércio, entre medicamentos e alimentos contendo probióticos, e um total de 23 espécies de diferentes microrganismos. Foram selecionadas e submetidas à identificação 6 medicamentos, 5 produtos alimentícios lácteos e uma cepa padrão ATCC® para controle. Para o teste de atividade antimicrobiana foram testados: Lactobacillus reuteri DSM 17938 Colidis®, Lactobacillus rhamnosus SD5217 AtillusMulti®, Lactobacillus acidophilus LA14 Prolive®, Lactobacillus casei Shirota Yakult®, Lactococcus lactis subsp. lactis ATCC 11454, Lactococcus lactis Activia® Shot, Bacillus clausii Enterogermina® e Saccharomyces boulardii 17 Floratil AT®. O MALDI-TOF/MS permitiu a identificação de todos os gêneros e a maioria das espécies dos probióticos analisados. A contagem de UFC dos produtos analisados se mostrou igual ou muito próxima as contagens que constam nas bulas e rótulos. As CIMs dos probióticos mostraram que todos os Lactobacillus foram resistentes à Vancomicina e com exceção do L. reuteri, resistentes ao Meropenem, sendo os demais resultados variáveis, mas sempre sendo sensíveis a dois ou mais antibióticos. Todos Lactobacillus e Lactococcus apresentaram atividade inibitória contra todas enterobactérias testadas, sendo >99% de redução em 48hs contra KPC, enquanto que B. clausii e S. boulardii não mostraram atividade inibitória. Em conclusão, todos os Lactobacillus spp e Lactococcus spp analisados mostraram potencial uso para inibição da colonização intestinal por enterobactérias patogênicas, incluindo KPC, em pacientes hospitalizados com risco de translocação bacteriana Abstract: Probiotics have been progressively used for treatment and prevention of diarrhea caused by enteropathogens with genera Lactobacillus and Bifidobacterium most commonly used. This study aimed to evaluate antimicrobial activity of some probiotics against pathogenic enterobacteria and its potential use for patient decolonization. Commercially available probiotics were incubated for colonies counting (CFU) and identification by mass spectrometry (MALDI-TOF/MS). Probiotics were submitted to E-test® for drug sensitivity. Co-culture, at 106 CFU/mL, was used to evaluate probiotic antimicrobial activity against carbapenemase-producing Klebsiella pneumoniae (KPC), Escherichia coli, Salmonella enteritidis and Shigella flexneri. Samples were seeded at times T0, 24, 48 and 72 hours for remaining CFU counting. For Lactobacillus reuteri 1% glycerol was added. We found 40 probiotic products commercially available among medicines and food, and a total of 23 species of different microorganisms. Selection and identification covered 6 medicines, 5 fermented dairy products and a standard strain ATCC® for control. For co-culture tests the following probiotics were checked: Lactobacillus reuteri DSM 17938 Colidis®, Lactobacillus rhamnosus SD5217 AtillusMulti®, Lactobacillus acidophilus LA14 Prolive®, Lactobacillus casei Shirota Yakult®, Lactococcus lactis subsp. lactis ATCC 11454, Lactococcus lactis Activia® Shot, Bacillus Clausii Enterogermina® e Saccharomyces boulardii 17 Floratil AT®. MALDI-TOF/MS allowed the identification of all genera and most probiotics species analyzed. All dairy products demonstrated CFU counting close to that declared in the lable. MICs showed that all Lactobacillus were resistant to Vancomycin and with the exception of L. reuteri, resistant to Meropenem. The other results were variable, but always sensitive to two or more antibiotics. All Lactobacillus and Lactococcus showed inhibitory activity against tested enterobacteria, with >99% reduction in 48 hours against KPC, while B. clausii and S. boulardii showed no inhibitory activity. As polimixinas estão sendo consideradas antibióticos de última linha para o tratamento de bactérias Gram-negativas multirresistentes. Resistência em polimixinas foi relatada como resultado de mutações cromossomais durante décadas. Em 2015 uma importante publicação descreveu a resistência plasmidial às polimixinas mediada pelo gene mcr-1. Poucos meses após essa descoberta, o gene foi relatado em diversos países causando grande preocupação. O presente estudo teve como objetivo avaliar a ocorrência de enterobactérias resistentes à colistina e a presença do gene mcr em isolados obtidos a partir de amostras clínicas de pacientes atendidos no Complexo Hospitalar da UNICAMP. Foram avaliados 103 isolados provenientes de diversas amostras biológicas. Enterobactérias resistentes à colistina foram selecionadas a partir do resultado obtido pelo teste automatizado de sensibilidade aos antimicrobianos. A identificação dos microrganismos foi realizada por espectrometria de massas (MALDI-TOF/MS). A resistência à colistina foi confirmada por microdiluição em caldo. Foi realizado uma reação de PCR multiplex capaz de detectar os genes mcr-1, mcr-2, mcr-3, mcr-4 e mcr-5. Sete dos 103 isolados foram positivos para mcr-1. A presença do gene mcr-1 foi confirmada utilizando a técnica de qPCR e pelo sequenciamento do genoma total bacteriano, que também detectou outros genes de resistência e virulência. Todos os isolados apresentaram no mínimo resistência a duas classes de antibióticos, sendo três deles produtores de ESBL. Identificamos a presença de uma linhagem pandêmica de E. coli (O15:H1-D-ST393), descrita pela primeira vez na América do Sul. O gene mcr-1 estava presente nos plasmídeos do grupo IncX4 e HI2. Os isolados de E. coli pertenciam ao ST393, ST3024, ST354, ST224 e ST624. Dos 7 isolados, 6 foram recuperados de infecções do trato urinário e um de infecção da corrente sanguínea, sendo que cinco pacientes tinham imunidade comprometida. A pesquisa e a detecção de mecanismos de resistência emergentes são essenciais para o combater e controlar microrganismos multirresistentes. A presença do gene mcr-1 em nossa instituição destaca a importância de uma vigilância nesse contexto, principalmente em pacientes imunocomprometidos Abstract: Antimicrobial resistance has become a major public health problem, causing a global crisis. Polymyxin has been considered a last-resort antibiotic for the treatment of multidrug-resistant Gram-negative bacteria. Polymyxin resistance has been reported as a result of chromosomal mutations for decades. In 2015 a major publication described plasmidial resistance to polymyxins mediated by the mcr-1 gene. A few months after this discovery the gene was reported in several countries causing great concern. This study aimed to evaluate the occurrence of enterobacteria resistant to colistin and the presence of the mcr gene in isolates obtained from clinical samples of patients treated at UNICAMP Hospital Complex. We evaluated 103 isolates from several biological samples. Enterobacterales resistant to colistin was selected from the result obtained by the automated antimicrobial sensitivity test. The identification of microorganisms was performed by mass spectrometry (MALDI-TOF/MS). Resistance to colistin was confirmed by broth microdilution. A multiplex PCR capable of detecting the genes mcr-1, mcr-2, mcr-3, mcr-4, and mcr-5 was performed. Seven out of 103 isolates were positive for mcr-1. Whole genome sequencing analysis confirmed the presence of the mcr-1 gene and other resistance and virulence genes. All isolates showed at least resistance to two classes of antibiotics, three of which produced ESBL. We identified the presence of a pandemic strain of E. coli (O15:H1-D-ST393), described for the first time in South America. The mcr-1 gene was present in the plasmids of IncX4 and HI2. The E. coli isolates belonged to ST393, ST3024, ST354, ST224, ST624 and a novel ST. Of the 7 isolates, 6 were recovered from urinary tract infections and one from bloodstream infection. Five isolates were recovered from patients with compromised immunity. Surveillance and detection of emerging resistance mechanisms are essential to combat multi-resistant microorganisms. Falta ou deficiência de higiene bucal podem levar a quadros de infecção por microrganismos patogênicos, sendo a principal delas, a pneumônica associada à ventilação mecânica (PAVM). Procedimentos Operacionais Padrão - POP de higiene bucal nem sempre são seguidos à risca, além de não contarem, na maioria das vezes, com a participação de cirurgião dentista em suas elaborações. Diante desse quadro, o objetivo desse trabalho é a criação de novo protocolo de higiene bucal para pacientes internados e intubados no Serviço de Terapia Intensiva (SETI) do Hospital das Clínicas da Faculdade de Medicina de Botucatu – UNESP e avaliar seu impacto na prevenção de PAVM. Materiais e métodos: Foram estudados 72 pacientes divididos em dois grupos: estudo, internados na Ala 1 do SETI, e controle, internados na ala 2 do SETI. Critérios de inclusão: pacientes em ventilação mecânica a no máximo 24 horas e que permaneceram nessa condição por período mínimo de 96 horas, idade mínima de 18 anos. Critérios de exclusão: neoplasias sob quimioterapia, imunossuprimidos, extubação (voluntária ou não) ou óbito antes do prazo mínimo de 96 horas. No grupo de estudo, foi realizada higiene bucal com clorexidina 0,12% líquida, 2 X dia, de acordo com o procedimento operacional padrão desenvolvido; no grupo controle, a higiene bucal foi realizada de acordo com o procedimento operacional padrão existente no Hospital das Clínicas para pacientes internados no SETI. Nos dois grupos, foram colhidos material microbiológico dos pacientes, da porção mais posterior da língua em contato com o tubo oro traqueal, com swab estéril, no 1º e 4º dias de intubação. O diagnóstico de PAVM foi realizado e entregue para utilização do estudo pela Comissão de Controle de Infecção Relaciona à Assistência à Saúde (CCIRAS). Na análise estatística foram utilizados, para proporções, o teste estatístico Chi Quadrado e, para comparações médias, o t-Student; para significância foi utilizado p < 0,05. Resultados: A PAVM ocorreu em 12 pacientes, sendo 4 no grupo de estudo (33,33%) e 8 no grupo controle (66,67%). Apesar da forte tendência na diminuição da incidência de PAVM, não houve significância estatística (p = 0,2059). O total de óbitos foi de 32, sendo17 no grupo de estudo (47,22%) e 15 no grupo controle (41,67%), sem significância estatística (p = 0,6353). O tempo de internação em UTI, entre os grupos, foi de 15,06 +- 9,88 dias no grupo de estudo contra 15,25 +- 8,34 dias no grupo controle, sem significância estatística (p = 0,9284). O tempo de ventilação mecânica entre os grupos foi de 10,0 +- 4,72 dias no grupo de estudo e 10,5 +- 4,42 dias no grupo controle, sem significância estatística (p = 0,6441). Conclusão: Os resultados mostram forte tendência na diminuição de eventos de PAVM entre os grupos de estudo e controle, apenas lançando mão de escovação dental com solução de clorexidina a 0,12% e higienização oral padronizada. Introduction: One of the problems related to hospitalization in Intensive Care Units (ICU) is the patients' oral condition. Absence or deficiency of oral hygiene may lead to infection by pathogenic microorganisms, the main one being ventilator-associated pneumonia (VAP). Oral hygiene standard operating procedures are not always followed to the letter, and in most cases, dental surgeon do not participate in its elaborations. In view of this situation, the objective of this work is the creation of a new oral hygiene standard operational procedure for hospitalized and intubated patients at the Intensive Care Unit of the Hospital das Clínicas of Botucatu Medical School - UNESP and evaluation of its impact on the prevention of VAP. Materials and methods: We studied 72 patients divided into two groups: study, hospitalized in ICU Ward 1, and control, hospitalized in Ward 2 of the ICU. Inclusion criteria: patients on mechanical ventilation for, at most, 24 hours and who remained in this condition for a minimum period of 96 hours, minimum age of 18 years. Exclusion criteria: neoplasms under chemotherapy, immunosuppressed, extubated (voluntary or not) or death before the minimum period of 96 hours. In the study group, oral hygiene was performed with chlorhexidine 0.12% liquid, 2 times a day, according to the standard operational procedure developed; in the control group, oral hygiene was performed according to the standard operational procedure existing in Hospital das Clínicas for ICU patients. In both groups, the patients' microbiological material was collected from the posterior portion of the tongue in contact with the orotracheal tube, with sterile swab, on the first and fourth days of intubation. The diagnosis of VAP was performed and submitted for use of the study by the Infection Control Commission Related to Health Care (CCIRAS). In the statistical analysis, Chi square statistical test was used for proportion, t-Student for average comparisons and p < 0.05 for significance. Results: VAP occurred in 12 patients, 4 in the study group (33.33%) and 8 in the control group (66.67%). Despite the strong tendency to decrease the incidence of VAP, there was no statistical significance (p = 0.2059). The total number of deaths was 32, 17 in the study group (47.22%) and 15 in the control group (41.67%), with no statistical significance (p = 0.6353). The ICU stay between the groups was 15.06 + - 9.88 days in the study group versus 15.25 + - 8.34 days in the control group, with no statistical significance (p = 0.9284) . The mechanical ventilation time between the groups was 10.0 + - 4.72 days in the study group and 10.5 + - 4.42 days in the control group, with no statistical significance (p = 0.6441). Conclusion: The results show a strong trend in the reduction of VAP events between the study and control groups, only using dental brushing with 0.12% chlorhexidine solution and standard oral hygiene.
