Abstract

Myositis-specific autoantibodies (MSAs) are highly specific biomarkers for idiopathic inflammatory myopathies (IIMs). We investigated whether self-reported race and ethnicity were associated with the presence of specific MSAs. Charts of patients with IIM seen at 3 large healthcare systems in the same US city were reviewed. Demographic data and MSA test results were abstracted. Associations between race and ethnicity and presence of MSAs were analyzed using bivariate analysis and further characterized using separate unadjusted and adjusted logistic regression models. One hundred twenty-one subjects were included (19% Asian, 10% Black or African American, 27% Latinx or Hispanic, 36% non-Hispanic White, and 7% Other). In a bivariate analysis, anti-Jo-1 and anti-MDA5 autoantibodies were associated with race and ethnicity (p = 0.03 and 0.02, respectively). Black or African American subjects had increased odds of a positive anti-Jo-1 result compared to non-Hispanic White subjects on unadjusted logistic regression analysis (OR 8.61, 95% CI 1.61-46.07), although after adjustment for age and gender this finding was not significant. Subjects categorized as Other had increased odds of a positive anti-MDA5 result compared to non-Hispanic White subjects on both unadjusted (OR 55.0, 95% CI 2.02-1493) and adjusted analyses (OR 44.8, 95% CI 1.55-1298). Anti-Jo-1 and anti-MDA5 autoantibodies were significantly associated with race and ethnicity on bivariate analysis. Black or African American subjects had increased odds of positive anti-Jo-1 autoantibody on unadjusted, but not adjusted, logistic regression analysis. Subjects characterized as Other had increased odds of positive anti-MDA5 autoantibody, although confidence intervals were wide. Abstract

BACKGROUND: Due to geographic isolation and border controls Aotearoa New Zealand (AoNZ) attained high levels of population coronavirus disease-19 (COVID-19) vaccination before widespread transmission of COVID-19. We describe outcomes of SARS-CoV-2 infection (Omicron variant) in people with inflammatory rheumatic diseases in this unique setting. METHODS: This observational study included people with inflammatory rheumatic disease and SARS-CoV-2 infection in AoNZ between 1 February and 30 April 2022. Data were collected via the Global Rheumatology Alliance Registry including demographic and rheumatic disease characteristics, and COVID-19 vaccination status and outcomes. Multivariable logistic regression was used to explore associations of demographic and clinical factors with COVID-19 hospitalisation and death. FINDINGS: Of the 1599 cases included, 96% were from three hospitals that systematically identified people with inflammatory rheumatic disease and COVID-19. At time of COVID-19, 1513 cases (94.6%) had received at least two COVID-19 vaccinations. Hospitalisation occurred for 104 (6.5%) cases and 10 (0.6%) patients died. Lower frequency of hospitalisation was seen in cases who had received at least two vaccinations (5.9%), compared to the unvaccinated (20.6%) or those with a single vaccine dose (10.7%). In multivariable adjusted models, people with gout or connective tissue diseases (CTD) had increased risk of the combined outcome of hospitalisation/death, compared to people with inflammatory arthritis. Glucocorticoid and rituximab use were associated with increased rates of hospitalisation/death. All patients who died had three or more co-morbidities or were over 60 years old. INTERPRETATION: In this cohort with inflammatory rheumatic diseases and high vaccination rates, severe outcomes from SARS-CoV-2 Omicron variant were relatively infrequent. The outcome of Omicron variant infection among vaccinated but SARS-CoV-2 infection-naive people with inflammatory rheumatic disease without other known risk factors were favourable. Abstract

ObjectiveTo describe people with gout who were diagnosed with coronavirus disease 2019 (COVID-19) and hospitalized and to characterize their outcomes.MethodsData on patients with gout hospitalized for COVID-19 between March 12, 2020, and October 25, 2021, were extracted from the COVID-19 Global Rheumatology Alliance registry. Descriptive statistics were used to describe the demographics, comorbidities, medication exposures, and COVID-19 outcomes including oxygenation or ventilation support and death.ResultsOne hundred sixty-three patients with gout who developed COVID-19 and were hospitalized were included. The mean age was 63 years, and 85% were male. The majority of the group lived in the Western Pacific Region (35%) and North America (18%). Nearly half (46%) had two or more comorbidities, with hypertension (56%), cardiovascular disease (28%), diabetes mellitus (26%), chronic kidney disease (25%), and obesity (23%) being the most common. Glucocorticoids and colchicine were used pre-COVID-19 in 11% and 12% of the cohort, respectively. Over two thirds (68%) of the cohort required supplemental oxygen or ventilatory support during hospitalization. COVID-19-related death was reported in 16% of the overall cohort, with 73% of deaths documented in people with two or more comorbidities.ConclusionThis cohort of people with gout and COVID-19 who were hospitalized had high frequencies of ventilatory support and death. This suggests that patients with gout who were hospitalized for COVID-19 may be at risk of poor outcomes, perhaps related to known risk factors for poor outcomes, such as age and presence of comorbidity.

5. Factors Associated With Hospitalization and Death After COVID‐19 Diagnosis Among Patients With Rheumatic Disease: An Analysis of Veterans Affairs Data

Abstract

ObjectiveIndividuals with autoimmune rheumatic disease (RD) are considered to be at increased risk for infection. However, few US population-based studies have assessed whether these patients are at increased risk of hospitalization or death due to COVID-19 compared with those without RD.MethodsWe performed a retrospective cohort study using national Veterans Affairs Health Care System data for individuals who tested positive for SARS-CoV-2. Outcomes of interest were hospitalization or death due to any cause within 30 days of COVID-19 diagnosis. Outcomes were compared among veterans with RD and those without RD by using propensity score matching (PSM) and mixed-effects multivariate logistic regression.ResultsOf 26,116 veterans with COVID-19, 501 (1.9%) had an underlying RD. Prior to matching, patients with RD were more likely to have poor outcomes compared with controls (37.7% vs. 28.5% hospitalized; 6.4% vs. 4.5% died). In the PSM analysis, RD was not a significant predictor for poor outcomes; however, patients with prescriptions for glucocorticoids had increased odds of poor outcomes in a dose-dependent manner (odds ratio [95% confidence interval] for hospitalization or death: 1.33 [1.20-1.48] for doses >0 and ≤10 mg/day; 1.29 [1.09-1.52] for doses >10 mg/day).ConclusionAmong US veterans with COVID-19, we did not find a significant association between RD and hospitalization or death. Poor outcomes appear to be mostly driven by age and other comorbidities, similar to the general veteran population. However, we observed an increased risk for poor outcomes among patients who received glucocorticoids, even at daily doses less than or equal to 10 mg.

10. Porphyromonas gingivalis peptidylarginine deiminase in the aetiology of rheumatoid arthritis

Abstract

Anti-citrullinated protein antibodies (ACPA) are the main autoantibodies in rheumatoid arthritis (RA). Citrullination is the conversion of arginine to citrulline by peptidylarginine deiminase (PAD) enzymes. Periodontitis (PD) is a risk factor for RA, and there is evidence this may be due to citrullination by the unique prokaryotic PAD enzyme expressed by PD pathogen Porphyromonas gingivalis (PPAD). The aim of this work was to characterise PPAD, and investigate its potential role in the aetiology of RA. The 3D crystal structure of PPAD was solved, and functional residues identified. The catalytic domain comprises five ββαβ motif repeats, which create a canonical binding groove into the active site, and the immunoglobulin-like domain is an anti-parallel β-sandwich. PPAD lacks significant homology to human PADs, and forms distinct citrullinated peptides from those formed by human PAD isoforms PAD2 and PAD4 due to unique C-terminal substrate specificity. In vivo, immunisation with P. gingivalis proteins: PPAD, arginine gingipain (Rgp), and enolase (PgEnolase), induced ACPA responses in mice. In human cohorts, antibodies to PPAD, Rgp, and PgEnolase were increased in RA patients compared to osteoarthritis or healthy controls, and in RA patients with PD compared to those without. Six months of PD treatment saw a significant reduction in antibodies to P. gingivalis in RA patients with PD, but no change in ACPA. These results demonstrate P. gingivalis induces ACPA responses comparable to those observed in RA, potentially through formation of novel citrullinated peptides by PPAD. Structural and mechanistic information obtained here can be used in future studies targeting PPAD as a potential therapeutic strategy in RA.

Abstract

We present a case study on the design and implementation of a value-based bundled package of care for patients with early-stage breast cancer treated in the private health sector in Australia. Value-based healthcare is an essential change to how we deliver healthcare, shifting the focus from paying for individual services provided to a focus on the health outcomes gained over a full cycle of care. The Australian health system has unintentionally created barriers to value-based cancer care through fragmented care pathways and complex funding arrangements where patients can unexpectedly encounter high out-of-pocket costs. A team of clinicians, service providers, health systems and funding experts, private health insurers and consumers have collaborated to design and pilot a complete bundled package of care for breast cancer patients which aims to address these challenges. With 40 patients recruited to date, early evaluation results show positive patient experience of 'joined-up' care and financial transparency. This case study provides a high-level overview of the approach taken to design and implement the Breast Cancer Bundle and the lessons learned for its expansion in both public and private settings. 33. The Impact of a Therapeutic HIV-1 Vaccine on HIV-1 Proviral DNA and RNA Transcription in a Phase I Clinical Trial

Abstract

Human immunodeficiency virus (HIV) is an ongoing public health issue. Currently, antiretroviral therapy (ART) is used to treat people living with HIV (PLWH), but the virus persists in latent memory T cells despite viral suppression on ART. Recent therapeutic advances are aimed at finding functional cures of HIV. Alongside therapeutic research, there is a need for the development of quantitative assays to accurately measure the viral reservoir within individuals and evaluate the impact of interventions on reducing the size of the reservoir. In this thesis, I implement an automated nucleic acid extraction method followed by quantitative molecular assays to quantify longitudinal changes in Cell-Associated HIV RNA and DNA (CARD) using samples from a Phase I clinical trial. The results will help determine if study participants responded to vaccination with autologous dendritic cells (DC) that were loaded with or without HIV peptides, directly or through natural processing, compared to unloaded DC. As this is the first usage of the automated CARD system on clinical trial samples, we are also further evaluating its performance characteristics compared to the existing manual CARD assay.

Abstract

Objective In addition to the long-established link with smoking, periodontitis (PD) is also a risk factor for rheumatoid arthritis (RA). To elucidate the mechanism by which PD could induce antibodies to citrullinated peptides (ACPA), we examine the antibody response to a novel citrullinated peptide from cytokeratin type I 13 identified in gingival crevicular fluid (GCF), and compare the response to 4 other citrullinated peptides in patients with RA, well-characterized for PD and smoking. Methods The citrullinomes of GCF and periodontal tissue from people with PD were mapped by mass spectrometry. Antibodies to citrullinated peptides from cytokeratin type I 13 (cCK13), tenascin-C (cTNC5), vimentin (cVIM), enolase (CEP-1) and fibrinogen β (cFIBβ) were examined by ELISA in patients with RA (n=287) and osteoarthritis (OA) (n=330), and cross-reactivity assessed by inhibition assays. Results A novel citrullinated peptide cCK13-1 (444TSNASGR-cit-TSDV-cit-RP458) identified in GCF, exhibited elevated antibody responses in RA patients (24%). Anti-cCK13-1 antibodies correlated with anti-cTNC5 antibodies, and absorption experiments confirmed this was not due to cross-reactivity. Only anti-cCK13-1 and anti-cTNC5 were associated with antibodies to the periodontal pathogen Prevotella intermedia (p=0.05 and p =0.001 respectively), but not with antibodies to Porphyromonas gingivalis arginine gingipains. Antibodies to CEP-1, cFIBβ and cVIM correlated with each other, and with smoking and shared epitope risk factors in RA. Conclusion This study identifies two groups of ACPA fine specificities associated with different RA risk factors; one predominantly linked to smoking and shared epitope, the other linking anti- cTNC5 and cCK13-1 to infection with the periodontal pathogen P. intermedia.

48. Histone deacetylase expression following cisplatin-induced acute kidney injury in male and female mice.

Abstract

The chemotherapeutic agent, cisplatin, accumulates in the kidneys leading to acute kidney injury (AKI). Pre-clinical and clinical studies have demonstrated sex-dependent outcomes of cisplatin-AKI. Deranged histone deacetylase (HDAC) activity is hypothesized to promote the pathogenesis of male murine cisplatin-AKI; however, it is unknown if there are sex differences in the kidney HDACs. We hypothesized that there would be sex specific Hdacexpression, localization, or enzymatic activity and this may explain sexual-dimorphic responses to cisplatin-AKI. In normal human kidney RNA samples, HDAC10was significantly greater in the kidney of women compared to men, while HDAC1, HDAC6, HDAC10, and HDAC11were differentially expressed between the kidney cortex and medulla, regardless of sex. In a murine model of cisplatin-AKI (3 days post a 15 mg/kg injection) we found few sex- or cisplatin- Hdackidney transcript differences among the mice. Although Hdac9was significantly greater in female mice compared to males, HDAC9 protein localization did not differ. Hdac7transcripts were greater in the inner medulla of cisplatin-AKI mice, regardless of sex, and this agreed with a greater HDAC7 abundance. HDAC activity within the cortex, outer, and inner medullae was significantly lower in cisplatin-AKI mice but did not differ between the sexes. In agreement with these findings a class I HDAC inhibitor didn't improve kidney injury or function. In conclusion, even though cisplatin-AKI was evident and there were transcript level differences among the different kidney regions in this model, there were few sex or cisplatin-dependent effects on kidney HDAC localization or activity.

49. Combination Acupuncture and Cupping for Treating Adult Idiopathic Scoliosis

Abstract

Background:There is limited previous English-language literature on acupuncture's ability to treat idiopathic scoliosis. This report shows the potential effectiveness of a combination of acupuncture and cupping therapies to treat curvature progression and thoracic back pain in adult idiopathic scoliosis.Intervention:A 34-year-old male veteran of the U.S. Armed Forces with thoracic back pain and muscle spasms originally presented to the chiropractic clinic at the Fargo Veterans Affairs Healthcare System, Fargo, ND. Per radiographs taken prior to the consultation, the chiropractor determined that the patient had an idiopathic right-convex scoliosis curve of ∼21°. Due to immobility of the thoracic spinal vertebra upon adjustment, he was referred to acupuncture care. After a clinical review, the acupuncturist hypothesized that a combination of utilizing acupuncture and cupping techniques would help relax the muscles along the thoracic spine, allowing correction of the patient's abnormal spinal curvature. Combination Traditional Chinese Medicine (TCM), using acupuncture with cupping therapy was given ∼2 times per week for several weeks, resulting in a total of 15 treatments.Results:After 15 treatments, this patient's subjective pain decreased by 85%. Anecdotally, he reported “feeling a bit looser.” He continued to receive acupuncture and cupping treatments when his schedule allowed.Conclusions:The combination therapy was effective for treating musculoskeletal pain but was inconclusive with respect to its ability to treat adult idiopathic scoliosis. More research is needed on the efficacy of TCM for treating adult idiopathic scoliosis.
