1. Integrated Care Intervention Supported by a Mobile Health Tool for Patients Using Noninvasive Ventilation at Home: Randomized Controlled Trial

Abstract

BackgroundHome-based noninvasive ventilation has proven cost-effective. But, adherence to therapy still constitutes a common clinical problem. We hypothesized that a behavioral intervention supported by a mobile health (mHealth) app could enhance patient self-efficacy. It is widely accepted that mHealth-supported services can enhance productive interactions among the stakeholders involved in home-based respiratory therapies. ObjectiveThis study aimed to measure changes in self-efficacy in patients with chronic respiratory failure due to diverse etiologies during a 3-month follow-up period after the intervention. Ancillary objectives were assessment of usability and acceptability of the mobile app as well as its potential contribution to collaborative work among stakeholders. MethodsA single-blind, single-center, randomized controlled trial was conducted between February 2019 and June 2019 with 67 adult patients with chronic respiratory failure undergoing home-based noninvasive ventilation. In the intervention group, a psychologist delivered a face-to-face motivational intervention. Follow-up was supported by a mobile app that allowed patients to report the number of hours of daily noninvasive ventilation use and problems with the therapy. Advice was automatically delivered by the mobile app in case of a reported problem. The control group received usual care. The primary outcome was the change in the Self Efficacy in Sleep Apnea questionnaire score. Secondary outcomes included app usability, app acceptability, continuity of care, person-centered care, and ventilatory parameters. ResultsSelf-efficacy was not significantly different in the intervention group after the intervention (before: mean 3.4, SD 0.6; after: mean 3.4, SD 0.5, P=.51). No changes were observed in adherence to therapy nor quality of life. Overall, the mHealth tool had a good usability score (mean 78 points) and high acceptance rate (mean score of 7.5/10 on a Likert scale). It was considered user-friendly (mean score of 8.2/10 on a Likert scale) and easy to use without assistance (mean score of 8.5/10 on a Likert scale). Patients also scored the perception of continuity of care and person-centered care as high. ConclusionsThe integrated care intervention supported by the mobile app did not improve patient self-management. However, the high acceptance of the mobile app might indicate potential for enhanced communication among stakeholders. The study identified key elements required for mHealth tools to provide effective support to collaborative work and personalized care. Abstract

BackgroundTelemedicine seems to offer reliable solutions to health care challenges, but significant contradictory results were recently found. Therefore, it is crucial to carefully select outcomes and target patients who may take advantage of this technology. Continuous positive airway pressure (CPAP) therapy compliance is essential to treat patients with obstructive sleep apnea (OSA). We believe that OSA patients could benefit greatly from a telemedicine approach for CPAP therapy management. ObjectiveThe objective of our study was to evaluate the application of a telemedicine-based approach in the CPAP therapy management, focusing on patients’ CPAP follow-up and training. MethodsWe performed two studies. First, (study 1) we enrolled 50 consecutive OSA patients who came to our sleep center for the CPAP follow-up visit. Patients performed a teleconsultation with a physician, and once finalized, they were asked to answer anonymously to a questionnaire regarding their opinion about the teleconsultation. In a second randomized controlled trial (RCT) (study 2), we included 40 OSA patients scheduled for CPAP training. There were 20 that received the usual face-to-face training and 20 that received the training via videoconference. After the session, they were blindly evaluated on what they learned about OSA and mask placement. ResultsMore than 95% (49/50) of the interviewed patients were satisfied with the teleconsultation, and 66% (33/50) of them answered that the teleconsultation could replace 50%-100% of their CPAP follow-up visits. Regarding the RCT, patients who received the CPAP training via videoconference demonstrated the same knowledge about OSA and CPAP therapy as the face-to-face group (mean 93.6% of correct answers vs mean 92.1%; P=.935). Performance on practical skills (mask and headgear placement, leaks avoidance) was also similar between the two groups. ConclusionsOSA patients gave a positive feedback about the use of teleconsultation for CPAP follow-up, and the CPAP training based on a telemedicine approach proved to be as effective as face-to-face training. These results support the use of this telemedicine-based approach as a valuable strategy for patients’ CPAP training and clinical follow-up.

Abstract

BackgroundAn analysis of cardiac injury markers in patients with OSA who sustain an episode of acute coronary syndrome (ACS) may contribute to a better understanding of the interactions and impact of OSA in subjects with ACS. We compared peak cardiac troponin I (cTnI) levels in patients with OSA and patients without OSA who were admitted for ACS.MethodsBlood samples were collected every 6 hours from the time of admission until two consecutive assays showed a downward trend in the cTnI assay. The highest value obtained defined the peak cTnI value, which provides an estimate of infarct size.ResultsWe included 89 patients with OSA and 38 patients without OSA with an apnea-hypopnea index of a median of 32 (interquartile range [IQR], 20.8-46.6/h and 4.8 [IQR, 1.6-9.6]/h, respectively. The peak cTnI value was significantly higher in patients without OSA than in patients with OSA (median, 10.7 ng/mL [IQR, 1.78-40.1 ng/mL] vs 3.79 ng/mL [IQR, 0.37-24.3 ng/mL]; P = .04). The multivariable linear regression analysis of the relationship between peak cTnI value and patient group, age, sex, and type of ACS showed that the presence or absence of OSA significantly contributed to the peak cTnI level, which was 54% lower in patients with OSA than in those without OSA.ConclusionsThe results of this study suggest that OSA has a protective effect in the context of myocardial infarction and that patients with OSA may experience less severe myocardial injury. The possible role of OSA in cardioprotection should be explored in future studies.

16. El margalló (Chamaerops humilis L., Arecaceae) als litorals central i septentrional de Catalunya: autòcton, introduït o reaparegut?

Abstract

Chamaerops humilis L., el margalló, és l’única palmera nadiua de la conca Mediterrània occidental. A Catalunya (nord-est de la península Ibèrica), el seu límit nord de distribució natural se situa al massís del Garraf. Segons bibliografia antiga, però, l’espècie sembla que era present a la muntanya de Montjuïc (Barcelona), d’on s’hauria extingit. En aquest estudi presentem els resultats d’un extens treball de camp realitzat durant els últims cinc anys per tal de (1) comprovar si l’espècie encara persisteix a Montjuïc i (2) explorar si aquesta palmera també és present en altres zones de Catalunya (al nord del Garraf) no citades a la bibliografia i, en cas afirmatiu, intentar determinar si es tracta de poblacions naturals o introduïdes. Hem pogut detectar diverses poblacions a Montjuïc (una d’elles amb un miler d’individus aproximadament) compostes majoritàriament per individus joves i situades prop d’individus cultivats. Això fa pensar que aquests nuclis poblacionals tenen molt probablement el seu origen en diàspores provinents de jardins i, per tant, C. humilis hauria reaparegut en un indret que hauria format part de la seva àrea de distribució històrica. Caldria tractar l’espècie com a introduïda als indrets on l’hem observat situats al nord del riu Llobregat, perquè en la majoria dels casos les poblacions estan formades per individus isolats o agrupats en petits rodals., Chamaerops humilis L., the European fan palm, is the only native palm in the western Mediterranean basin. In Catalonia (north-western Iberian Peninsula), the Garraf Massif is generally regarded as its natural north distribution limit. According to old accounts, however, the species seems to have occurred in Montjuïc Mountain, Barcelona, from where it would have been extinct. Herein we are presenting the results of extensive fieldwork conducted during the last five years in order to (1) check whether the species is still persisting in Montjuïc, (2) explore whether this dwarf palm is also present in other areas of Catalonia further north of the Garraf not cited in the literature and, if so, to disentangle its biogeographic status (i.e., native or alien). We have been able to detect several populations in Montjuïc Mountain (with a population containing roughly a thousand individuals) close to cultivated plants. The youth of most individuals and their proximity to cultivated plants indicate that they are probably escapes. This might suggest that C. humilis is today present in an area that would be part of its historical range. This palm should be considered as introduced in the remaining locations where the species has been observed north to the Llobregat river. Notably, in most of these places C. humilis occurs as isolated individuals or as very small stands.

Chamaerops humilis L., el margalló, és l’única palmera nadiua de la conca Mediterrània occidental. A Catalunya (nord-est de la península Ibèrica), el seu límit nord de distribució natural se situa al massís del Garraf. Segons bibliografia antiga, però, l’espècie sembla que era present a la muntanya de Montjuïc (Barcelona), d’on s’hauria extingit. En aquest estudi presentem els resultats d’un extens treball de camp realitzat durant els últims cinc anys per tal de (1) comprovar si l’espècie encara persisteix a Montjuïc i (2) explorar si aquesta palmera també és present en altres zones de Catalunya (al nord del Garraf) no citades a la bibliografia i, en cas afirmatiu, intentar determinar si es tracta de poblacions naturals o introduïdes. Hem pogut detectar diverses poblacions a Montjuïc (una d’elles amb un miler d’individus aproximadament) compostes majoritàriament per individus joves i situades prop d’individus cultivats. Això fa pensar que aquests nuclis poblacionals tenen molt probablement el seu origen en diàspores provinents de jardins i, per tant, C. humilis hauria reaparegut en un indret que hauria format part de la seva àrea de distribució històrica. Caldria tractar l’espècie com a introduïda als indrets on l’hem observat situats al nord del riu Llobregat, perquè en la majoria dels casos les poblacions estan formades per individus isolats o agrupats en petits rodals., Chamaerops humilis L., the European fan palm, is the only native palm in the western Mediterranean basin. In Catalonia (north-western Iberian Peninsula), the Garraf Massif is generally regarded as its natural north distribution limit. According to old accounts, however, the species seems to have occurred in Montjuïc Mountain, Barcelona, from where it would have been extinct. Herein we are presenting the results of extensive fieldwork conducted during the last five years in order to (1) check whether the species is still persisting in Montjuïc, (2) explore whether this dwarf palm is also present in other areas of Catalonia further north of the Garraf not cited in the literature and, if so, to disentangle its biogeographic status (i.e., native or alien). We have been able to detect several populations in Montjuïc Mountain (with a population containing roughly a thousand individuals) close to cultivated plants. The youth of most individuals and their proximity to cultivated plants indicate that they are probably escapes. This might suggest that C. humilis is today present in an area that would be part of its historical range. This palm should be considered as introduced in the remaining locations where the species has been observed north to the Llobregat river. Notably, in most of these places C. humilis occurs as isolated individuals or as very small stands, Chamaerops humilis L., el margalló, és l’única palmera nadiua de la conca Mediterrània occidental. Herein, we are presenting the results of a five-year survey of the alien flora of Montjuïc, the largest urban hill in Barcelona (Spain). In just about 3.4 km2, we recorded up to 247 alien plant taxa, a figure much higher than those of many other Mediterranean cities and which clearly points to the role of Montjuïc as a hotspot for alien plants. The comparison with the alien flora of its surrounding region (coastal Catalonia) suggests that the alien flora of Montjuïc would have become enriched through many immigration episodes from close geographic areas. The hill, however, would have also acted as a source of the spread of alien plants, and indeed, some species have not been detected yet beyond the confines of Montjuïc.
