Abstract

The optimal management of locally advanced rectal cancer is rapidly evolving. The National Cancer Institute Rectal-Anal Task Force convened an expert panel to develop consensus on the design of future clinical trials of patients with rectal cancer. A series of 82 questions and subquestions, which addressed radiation and neoadjuvant therapy, patient perceptions, rectal cancer populations of special interest, and unique design elements, were subject to iterative review using a Delphi analytical approach to define areas of consensus and those in which consensus is not established. The task force achieved consensus on several areas, including the following: 1) the use of total neoadjuvant therapy with long-course radiation therapy either before or after chemotherapy, as well as short-course radiation therapy followed by chemotherapy, as the control arm of clinical trials; 2) the need for greater emphasis on patient involvement in treatment choices within the context of trial design; 3) efforts to identify those patients likely, or unlikely, to benefit from nonoperative management or minimally invasive surgery; 4) investigation of the utility of circulating tumor DNA measurements for tailoring treatment and surveillance; and 5) the need for identification of appropriate end points and recognition of challenges of data management for patients who enter nonoperative management trial arms. Substantial agreement was reached on priorities affecting the design of future clinical trials in patients with locally advanced rectal cancer.

2. The mid-infrared molecular inventory towards Orion IRc2

Abstract

We present the first high spectral resolution mid-infrared survey in the Orion BN/KL region, covering 7.2 to 28.3 micron. With SOFIA/EXES we target the enigmatic source Orion IRc2. While this is in the most prolifically studied massive star-forming region, longer wavelengths and molecular emission lines dominated previous spectral surveys. The mid-infrared observations in this work access different components and molecular species in unprecedented detail. We unambiguously identify two new kinematic components, both chemically rich with multiple molecular absorption lines. The "blue clump" has vLSR = -7.1 \pm 0.7 km/s and the "red clump" 1.4 \pm 0.5 km/s. While the blue and red clumps have similar temperatures and line widths, molecular species in the blue clump have higher column densities. They are both likely linked to pure rotational H2 emission also covered by this survey. This work provides evidence for the scenario that the blue and red clumps are distinct components unrelated to the classic components in the Orion BN/KL region. Comparison to spectroscopic surveys towards other infrared targets in the region show that the blue clump is clearly extended. We analyze, compare, and present in depth findings on the physical conditions of C2H2, 13CCH2, CH4, CS, H2O, HCN, H13CN, HNC, NH3, and SO2 absorption lines and an H2 emission line associated with the blue and red clumps. 5. Effective Opacity of the Intergalactic Medium from Galaxy Spectra Analysis

Abstract

We measure the effective opacity ($\tau_{eff}$) of the Intergalactic Medium (IGM) from the composite spectra of 281 Lyman-Break Galaxies (LBGs) in the redshift range $2 \lesssim z \lesssim 3$. Our spectra are taken from the COSMOS Lyman-Alpha Mapping And Tomographic Observations (CLAMATO) survey derived from the Low Resolution Imaging Spectrometer (LRIS) on the W.M. Keck I telescope. We generate composite spectra in two redshift intervals and fit them with spectral energy distribution (SED) models composed of simple stellar populations. Extrapolating these SED models into the Ly$\alpha$ forest, we measure the effective Ly$\alpha$ opacity ($\tau_{eff}$) in the $2.02 \leq z \leq 2.44$ range. At $z = 2.22$, we estimate $\tau_{eff} = 0.159 \pm 0.001$ from a power-law fit to the data. These measurements are consistent with estimates from quasar analyses at $z<2.5$ indicating that the systematic errors associated with normalizing quasar continua are not substantial. We provide a Gaussian Processes model of our results and previous $\tau_{eff}$ measurements that describes the steep redshift evolution in $\tau_{eff}$ from $z = 1.5 - 4$., Comment: accepted to ApJ on 05/18/2020

Abstract

There is limited information on how indoor tanning promotes melanoma development. We investigated indoor tanning use in patients with melanomas in sun-exposed skin and studied the clinicopathological and molecular characteristics in relation to indoor tanning exposure. Patients from a multidisciplinary clinic for cutaneous cancers completed standardized questionnaires on risk factors for melanoma as a component of medical history at their initial consultations. For this study, we included patients from December 2013 to May 2015. The 114 patients who reported indoor tanning exposure were younger at diagnosis than the 222 patients who did not (51.5 vs 64.0 years, two-sided P < .001). BRAF V600E genotype was more prevalent in ever-users than in nonusers (42.9% vs 28.3%, two-sided P = .04) and higher in ever-users who initiated indoor tanning prior to age 25 years compared with age 25 years or older (62.2% vs 31.1%, two-sided P = .003). There were more melanomas in intermittently sun-exposed skin in ever-users than nonusers (65.7% vs 51.9%, respectively, two-sided P = .02). Our data suggest indoor tanning may promote melanomas that arise in skin with low-chronic sun-induced damage through BRAF V600E-mediated melanomagenesis.

22. Imaging response to immune checkpoint inhibitors in patients with advanced melanoma: a retrospective observational cohort study.

Abstract

Background: The association between objective imaging response and first line immune checkpoint inhibitor (ICI) therapy regimes in advanced melanoma remains uncharacterized in routine practice. Methods: We conducted a multi-center retrospective cohort analysis of advanced melanoma patients receiving first line ICI therapy from August 2013-May 2020 in Alberta, Canada. The primary outcome was likelihood of RECIST v1.1 assessed objective imaging response between patients receiving anti-programmed cell death protein 1 (anti-PD1) monotherapy and those receiving combination ipilimumab-nivolumab. Secondary outcomes were identification of baseline characteristics associated with non-response and the association of imaging response with overall survival (OS) and time to next treatment (TTNT). Results: 198 patients were included, 41/198 (20.7%) had complete response, 86/198 (43.4%) had partial response, 23/198 (11.6%) had stable disease, and 48/198 (24.2%) had progressive disease. Median OS was not reached (NR) (95% CI 49.0-NR) months for complete responders, NR (95%CI 52.9-NR) months for partial responders, 33.7 (95%CI 15.8-NR) months for stable disease, and 6.4 (95%CI 5.2–10.1) months for progressive disease (log-rank p<0.001). Likelihood of objective imaging response remained similar between anti-PD1 monotherapy and ipilimumab-nivolumab groups (OR 1.95 95%CI 0.85–4.63, p=0.121). Elevated LDH level (OR 0.46; 95%CI 0.21–0.98, p=0.043), mucosal primary site (OR 0.14; 95%CI 0.03–0.48, p=0.003), and BRAF V600E mutation status (OR 0.31; 95%CI 0.13–0.72, p=0.007) were associated with decreased likelihood of response. Conclusion: No significant difference in likelihood of imaging response between anti-PD1 monotherapy and combination ipilimumab-nivolumab was observed. Elevated LDH level, mucosal primary site, and BRAF V600E mutation status were associated with decreased likelihood of response. Given that pivotal clinical trials of ipilimumab-nivolumab did not formally compare ipilimumab-nivolumab with nivolumab monotherapy, this work adds context to differences in outcomes when these agents are used. These results may inform treatment selection, and aid in counseling of patients treated with first-line ICI therapy in routine clinical practice settings. [ABSTRACT FROM AUTHOR]

Abstract

Background: Hemorrhage is the leading cause of preventable death after trauma. In high‐income countries first responders are trained in hemorrhage control techniques but this is not the case for developing countries like Guatemala. We present a low‐cost training model for tourniquet application using a combination of virtual and physical components. Methods: The training program includes a mobile application with didactic materials, videos and a gamified virtual reality environment for learning. Additionally, a physical training model of a bleeding lower extremity is developed allowing learners to practice tourniquet application using inexpensive and accessible materials. Validation of the simulator occurred through content and construct validation. Content validation involved subjective assessments by novices and experts, construct validation compared pre‐training novices with experts. Training validation compared pre and post training novices for improvement. Results: Our findings indicate that users found the simulator useful, realistic, and satisfactory. We found significant differences in tourniquet application skills between pre‐training novices and experts. When comparing pre‐ and post‐training novices, we found a significantly lower bleeding control time between the groups. Conclusion: This study suggests that this training approach can enhance access to life‐saving skills for prehospital personnel. The inclusion of self‐assessment components enables self‐regulated learning and reduces the need for continuous instructor presence. Abstract

Phenolic compounds have gained international interest due to their carcinogenic, toxic, and bioaccumulative properties, causing adverse effects in both animals and humans. As a result, there is a growing interest in finding alternative and eco-friendly treatment routes for phenol by exploring new microbial cultures with potential adaptation and biodegradation capabilities. In this study, the phenol removal efficiency of Yarrowia lipolytica YQ22 under laboratory conditions was determined. The YQ22 strain was obtained from diesel samples from a fuel station in Trujillo, Peru, isolated through serial dilutions on Sabouraud agar, and identified through its morphological characteristics using microscopy and molecular analysis by polymerase chain reaction of the ribosomal DNA internal transcribed spacer (ITS) and 5.8S regions. In the treatment, the effect of pH (5, 6 and 7) and temperature (25°C, 30°C and 35°C) on phenol removal with 2% (v/v) inoculum of Yarrowia lipolytica from 48-hour growth was evaluated. Phenol concentration was measured by high-performance liquid chromatography. A maximum phenol removal percentage of 61.18 was obtained for YQ22 at 30°C, pH 5 and 120 rpm during 48 hours. These findings demonstrate the ability of Yarrowia lipolytica to remove phenol and suggest its potential use in the field of bioremediation of phenolic compounds and their derivatives. Abstract

Background Adjuvant therapies have been approved for resected melanoma based on improved recurrence-free survival. We present early findings from a real-world study on adjuvant treatments for melanoma. Methods A comprehensive chart review was conducted for patients receiving adjuvant systemic therapy for resected high-risk stages III and IV melanoma. Statistical analysis was performed to assess recurrence-free survival and subgroup differences. Results A total of 149 patients (median age = 58.0 years, 61.1% men, 49.7% with BRAF V600E/K genotypes) were included, with 94.6% having resected stage III melanoma. Anti-PD-1 immunotherapy was received by 86.5% of patients, while 13.4% received BRAF-targeted therapy. At a median follow-up of 22.4 months, the recurrence rate was 31.5%, with 1-year and 2-year recurrence-free survival rates of 79% and 62%, respectively. Similar recurrence rates were observed between anti-PD-1 immunotherapy and BRAF-targeted therapy. Long-term toxicity affected 27.4% of patients, with endocrinopathies and late-emergent immune-related adverse events being common. Conclusions Real-world adjuvant systemic therapy aligns with clinical trial practice. Abstract

Objective: The purpose of this guideline update is to reassess and update recommendations in the prior guideline from 2016 on the appropriate management of patients with uveal melanoma. Methods: In 2021, a multidisciplinary working group from the Provincial Cutaneous Tumour Team, Cancer Care Alberta, Alberta Health Services was convened to update the guideline. A comprehensive review of new research evidence in PubMed as well as new clinical practice guidelines from prominent oncology groups informed the update. An enhancement in methodology included adding levels of evidence and strength of recommendations. The updated guideline was circulated to all members of the Provincial Cutaneous Tumour Team for review and endorsement. Results: New and modified recommendations address provider training requirements, diagnostic imaging for the detection of metastases, neo-adjuvant pre-enucleation radiotherapy, intravitreal anti-vascular endothelial growth factor agents for radiation retinopathy, genetic prognostic testing, surveillance following definitive local therapy, and systemic therapy for patients with metastatic uveal melanoma. Abstract

The optimal management of locally advanced rectal cancer is rapidly evolving. The National Cancer Institute Rectal-Anal Task Force convened an expert panel to develop consensus on the design of future clinical trials of patients with rectal cancer. A series of 82 questions and subquestions, which addressed radiation and neoadjuvant therapy, patient perceptions, rectal cancer populations of special interest, and unique design elements, were subject to iterative review using a Delphi analytical approach to define areas of consensus and those in which consensus is not established. The task force achieved consensus on several areas, including the following: 1) the use of total neoadjuvant therapy with long-course radiation therapy either before or after chemotherapy, as well as short-course radiation therapy followed by chemotherapy, as the control arm of clinical trials; 2) the need for greater emphasis on patient involvement in treatment choices within the context of trial design; 3) efforts to identify those patients likely, or unlikely, to benefit from nonoperative management or minimally invasive surgery; 4) investigation of the utility of circulating tumor DNA measurements for tailoring treatment and surveillance; and 5) the need for identification of appropriate end points and recognition of challenges of data management for patients who enter nonoperative management trial arms.
