Abstract Aims To evaluate echocardiographic and biomarker changes during chemotherapy, assess their ability to early detect and predict cardiotoxicity and to define the best time for their evaluation. Methods and results Seventy‐two women with breast cancer (52 ± 9.8 years) treated with anthracyclines (26 also with trastuzumab), were evaluated for 14 months (6 echocardiograms/12 laboratory tests). We analysed: high‐sensitivity cardiac troponin T, NT‐proBNP, global longitudinal strain (GLS), left ventricle end‐systolic volume (LVESV), left ventricle end‐diastolic volume (LVEDV), and left ventricular ejection fraction (LVEF). Cardiotoxicity was defined as a reduction in LVEF>10% compared with baseline with LVEF

Aims: There is a lack of therapies able to prevent anthracycline cardiotoxicity (AC). Remote ischaemic conditioning (RIC) has shown beneficial effects in preclinical models of AC. Methods: REmote iSchemic condItioning in Lymphoma PatIents REceiving ANthraCyclinEs (RESILIENCE) is a multinational, prospective, phase II, double‐blind, sham‐controlled, randomized clinical trial that evaluates the efficacy and safety of RIC in lymphoma patients receiving anthracyclines. Patients scheduled to undergo ≥5 chemotherapy cycles including anthracyclines and with ≥1 AC‐associated risk factors will be randomized to weekly RIC or sham throughout the chemotherapy period. Patients will undergo three multiparametric cardiac magnetic resonance (CMR) studies, at baseline, after the third cycle (intermediate CMR), and 2 months after the end of chemotherapy. Thereafter, patients will be followed up for clinical events over an anticipated median of ≥24 months. The primary endpoint is the absolute change from baseline in CMR‐based left ventricular ejection fraction (LVEF). The main secondary outcome is the incidence of AC events, defined as (1) a drop in CMR‐based LVEF of ≥10 absolute points, or (2) a drop in CMR‐based LVEF of ≥5 and <10 absolute points to a value <50%. Intermediate CMR will test the ability of T2 mapping to predict AC versus classical markers (left ventricular strain and cardiac injury biomarkers). A novel CMR sequence allowing ultrafast cine acquisition will be validated in this vulnerable population. Conclusions: The RESILIENCE trial will test RIC (a novel non‐invasive intervention to prevent AC) in a cohort of high‐risk patients. The trial will also test candidate markers for their capacity to predict AC and will validate a novel CMR sequence reducing acquisition time in a vulnerable population. [ABSTRACT FROM AUTHOR]

- Abstract
OBJECTIVE: Experimental evidence suggests that metabolic syndrome (MetS) is associated with changes in cardiac metabolism. Whether this association occurs in humans is unknown. RESEARCH DESIGN AND METHODS: 821 asymptomatic individuals from the Progression of Early Subclinical Atherosclerosis (PESA) study (50.6 [46.9–53.6] years, 83.7% male) underwent two whole-body
18 F-fluorodeoxyglucose positron emission tomography-magnetic resonance (18 F-FDG PET-MR) 4.8 ± 0.6 years apart. Presence of myocardial18 F-FDG uptake was evaluated qualitatively and quantitatively. No myocardial uptake was grade 0, while positive uptake was classified in grades 1–3 according to target-to-background ratio tertiles. RESULTS: One hundred fifty-six participants (19.0%) showed no myocardial18 F-FDG uptake, and this was significantly associated with higher prevalence of MetS (29.0% vs. 13.9%, P < 0.001), hypertension (29.0% vs. 18.0%, P = 0.002), and diabetes (11.0% vs. 3.2%, P < 0.001), and with higher insulin resistance index (HOMA-IR, 1.64% vs. 1.23%, P < 0.001). Absence of myocardial uptake was associated with higher prevalence of early atherosclerosis (i.e., arterial18 F-FDG uptake, P = 0.004). On follow-up, the associations between myocardial18 F-FDG uptake and risk factors were replicated, and MetS was more frequent in the group without myocardial uptake. The increase in HOMA-IR was associated with a progressive decrease in myocardial uptake (P < 0.001). In 82% of subjects, the categorization according to presence/absence of myocardial18 F-FDG uptake did not change between baseline and follow-up. MetS regression on follow-up was associated with a significant (P < 0.001) increase in myocardial uptake. CONCLUSIONS: Apparently healthy individuals without cardiac18 F-FDG uptake have higher HOMA-IR and higher prevalence of MetS traits, cardiovascular risk factors, and early atherosclerosis. An improvement in cardiometabolic profile is associated with the recovery of myocardial18 F-FDG uptake at follow-up. [ABSTRACT FROM AUTHOR]- Published

A single coronary artery is one of the most rarely seen coronary artery anomalies. In addition, the specific subtype (Lipton RII-A) that our patient presented is one of the least common, and its clinical presentation as myocardial infarction and cardiac arrest has not been described in the literature. The case shows that although it is essential to exclude a malignant interarterial course of the vessel, cardiac arrest is a possible clinical presentation produced by myocardial ischemia in the context of acute myocardial infarction and should be managed according to clinical practice guidelines. Resumo: Uma artéria coronária única é uma das anomalias mais raras das artérias coronárias. Além disso, o subtipo específico (Lipton R-IIA) presente no nosso doente é um dos menos frequentes e a sua apresentação clínica como enfarte do miocárdio e como paragem cardíaca não foi ainda descrita na literatura. O caso mostra que, embora o descarte de um percurso interarterial maligno do vaso seja essencial, a paragem cardíaca é uma apresentação clínica possível causada por isquemia do miocárdio no cenário de um enfarte agudo do miocárdio, que deverá ser tratada de acordo com as recomendações práticas clínicas. In this work, we developed the first study in Colombia to obtain and evaluate syngas compositions derived from asphaltenes gasification. These asphaltenes came from the implementation of a Deasphalting process to San Fernando crude oil, with the purpose of looking for technological options for their utilization. We performed the design, installation and commissioning of facilities for the gasification of asphaltenes at laboratory scale, it following an experimental methodology, performing nine tests and considering temperature and agent gasification quantity (oxygen) as independent variables. The syngas derived from gasification was analyzed by two chromatographic techniques, which reported the presence of refinery gases and sulfur. We evidenced a growth tendency of CO, H2 and sulfur composition and a decrease in CH4 and CO2 composition with temperature. The composition of the syngas was evaluated with different quantities of gasification agent (33%, 40% and 47% the amount of oxygen theoretically required for complete combustion) at each temperature levels operated. It was established that when using a 40% of gasification agent, you get greater average content of CO and H2, which are the interest gases in the gasification process.En este trabajo se desarrolló el primer estudio en el país de la obtención y evaluación de las composiciones del syngas a partir de la gasificación de asfaltenos provenientes de la aplicación del proceso de Desasfaltado al crudo San Fernando, con el objeto de evaluar opciones tecnológicas para su utilización. Para ello se realizó el diseño y puesta en marcha de un sistema de gasificación de asfaltenos a escala de laboratorio, donde se empleó una metodología experimental, realizando nueve pruebas y tomando como variables independientes la temperatura y el agente gasificante (oxígeno). El syngas obtenido de la gasificación fue analizado por dos técnicas de cromatografía, las cuales reportaron la presencia de gases de refinería y azufrados. De estos resultados experimentales se evidencia una tendencia de crecimiento en la composición del CO, H2 y azufrados, y una disminución en la composición del CH4 y CO2 con la temperatura. Se evaluó la composición del syngas con la variación del agente gasificante (33%, 40% y 47% del oxígeno teórico requerido en una combustión total) en cada uno de los niveles de temperatura operados y se estableció que al utilizar un 40% de agente gasificante, se obtienen en promedio los mejores resultados en las composiciones del CO y H4, siendo estos los gases de interés en el proceso de gasificación.Neste trabalho desenvolveuse o primeiro estudo no país da obtenção e avaliação das composições do syngas a partir da gaseificação de asfaltenos provenientes da aplicação do processo de Desasfaltado ao cru São Fernando, com o objeto de avaliar opções tecnológicas para a sua utilização. Destes resultados experimentais evidenciase uma tendência de crescimento na composição do CO, H2 e enxofrados, e uma diminuição na composição do CH4 e CO2 com a temperatura. Avaliouse a composição do syngas com a variação do agente gaseificante (33%, 40% e 47% do oxigênio teórico requerido em uma combustão total) em cada um dos níveis de temperatura operados e estabeleceuse que ao utilizar um 40% de agente gaseificante, obtém-se em média os melhores resultados nas composições do CO e H4, sendo estes os gases de interesse no processo de gaseificação.

Our main goal is to study the effects on the carbohydrate metabolism. Thus, we designed various experimental surgical models on healthy non-obese Wistar rats to reproduce several conditions. In this sense, we report a new experimental model. It is well known that bariatric surgery has important effects on the control of Type 2 Diabetes Mellitus. The underlying reasons are yet unknown, although the different theories focused in the release of different hormones after the pass of the nutrients through the tract. These released hormones have opposite effects that come together in a balanced glycemic metabolism.After bariatric surgical techniques, the modified anatomy resulted in an imbalance of the secreted hormones. Wistar rats were randomized in two groups Sham and surgical group. Our model consisted on the transposition of the terminal ileum right after the pylorus. Weight gain, food intake, and basal glycemia were measured weekly.We did not obtain significant differences between both groups for these functional variables.This technique involved an early pass of the bolus through the ileum. The change on the luminal pH, along with the lack of enzymes to absorb the content, or the changes in the release of several hormones must be variables to the study. The mortality rate was assumable considering it was an experimental model on animals.Crear un nuevo modelo quirúrgico experimental en ratas Wistar sanas no obesas para estudiar los efectos del metabolismo glucídico. Es bien sabido que las técnicas de cirugía bariátrica tienen un efecto importante sobre la resolución de la diabetes mellitus tipo 2. Se han invocado diferentes hipótesis, algunas centradas en el papel que tienen distintas hormonas secretadas por el propio tubo digestivo tras el paso de los nutrientes a su través, pero las razones últimas subyacentes permanecen desconocidas. El efecto contrapuesto de dichas hormonas consigue un efecto de control glucémico. El desequilibrio hormonal tras las alteraciones anatómicas de las cirugías bariátricas podría estar en la base de dicha mejora del metabolismo glucídico final.Las ratas fueron operadas en dos grupos (control quirúrgico y experimental) y se procedió a disponerles el íleon anastomosado al antro pilórico, previo al esfínter pilórico. In this sense, we report a new experimental model. It is well known that bariatric surgery has important effects on the control of Type 2 Diabetes Mellitus. The underlying reasons are yet unknown, although the different theories focused in the release of different hormones after the pass of the nutrients through the tract. These released hormones have opposite effects that come together in a balanced glycemic metabolism. Materials and methods: After bariatric surgical techniques, the modified anatomy resulted in an imbalance of the secreted hormones. Wistar rats were randomized in two groups Sham and surgical group. Our model consisted on the transposition of the terminal ileum right after the pylorus. Weight gain, food intake, and basal glycemia were measured weekly. Results: We did not obtain significant differences between both groups for these functional variables. Conclusions: This technique involved an early pass of the bolus through the ileum. The change on the luminal pH, along with the lack of enzymes to absorb the content, or the changes in the release of several hormones must be variables to the study. We report the case of a patient who presented an out-of-hospital cardiorespiratory arrest while being transported by the Helicopter Emergency Service, refractory to standard, electrical and medical treatment. The patient recovered spontaneous circulation after 58 minutes with heart arrest and 31 delivered shocks. The reanimation manoeuvres were monitored with capnography (titrated over 20 mmHg EtCO2). Upon arrival at the hospital, the patient went directly to the haemodynamic laboratory where a percutaneous coronary intervention was performed, with a stent in the right coronary artery. The patient was discharged after 8 days without any neurological handicap (cerebral performance category 1).

Background Numerous hypotheses are called to explain the beneficial effect on glucose metabolism after bariatric surgery. Some authors advocate for the secretion and release of various substances with endocrine functions for the explanation on this event. One of the substances most marked as effector, with contrasting effects but controversial data, is GLP-1. Methods Our study was performed in healthy male Wistar rats, to avoid the absence of confounding factors such as T2DM and obesity. In order to know the adaptation of GLP-1 secretion after surgery 5 groups were designated: two control groups (fasting and surgical stress), and three surgical groups (gastric sleeve, 50% resection of the midgut and the Roux en Y gastric bypass). After three months the GLP-1 synthesis in the different portions of the small intestine and the expression of the membrane receptors in pancreatic islet cells were studied by immunohistochemical techniques. Results There was a significant increase in the number of secretory cells in ileum, duodenum and jejunum in mixed (RYGB) and malabsorptive (RI50) surgical groups. An elevation of pancreatic receptors signal was also observed in the same techniques versus controls. Conclusions Our data indicate that intestinal secretion of GLP-1 and its sensitivity to the pancreatic changes were increased like a response of an adaptive effect to the mechanical aggression of the digestive tube and as alteration of nutrient flow after surgery.

- Abstract
Research in medicine is the only tool the physician has, to improve and to change his activity. In this aspect, bariatric surgery is not different from the rest of the medical field. From the beginning to the present time, indications and addressings have been changing. Scientific societies with their organization and guidelines marking the general steps and direction of clinical performances, but also the research training. Over the last few years the amount and quality of papers have increased. This projection makes progress and innovation of new technologies possible, such as endoscopic techniques or the better indications for metabolic surgery. The research in bariatric surgery and its comorbidities is not only the outcome of the routine clinical practices. It is unavoidable their spreading to deeper and more complex fields as the knowledge of its mechanisms. The link between human and animal research is mandatory in order to ensure the progress in this surgical field., La investigación en medicina es la única herramienta útil utilizada por el médico para avanzar en su actividad. La cirugía bariátrica no es ajena a esta dinámica natural instaurada. Desde sus comienzos y hasta la época actual las indicaciones y directrices han ido variando. Las sociedades científicas, cada vez más estructuradas y organizadas, han ido marcando las pautas generales en las actuaciones clínicas además de orientar la investigación, asistiendo en los últimos años a un importante crecimiento tanto del número como de la calidad de los estudios. Ésta proyección hace posible el progreso y la innovación de nuevas técnicas que van ocupando su lugar dentro de este campo, como son el caso de las técnicas endoscópicas o las indicaciones para la cirugía metabólica. La investigación en cirugía bariátrica y sus comorbilidades no son solo el resultado de las actuaciones clínicas. Es inevitable su necesaria ampliación a campos más complejos como el conocimiento de los mecanismos causantes. Para poder asegurar el progreso en esta rama quirúrgica de la medicina son aún forzosos los lazos entre la investigación humana y animal.

37. Optimal Application Conditions of Solvent Injection Into Oil Sands To Minimize the Effect of Asphaltene Deposition: An Experimental Investigation

Summary Solvent injection into heavy-oil reservoirs is quite complex because of the asphaltene destabilization that occurs because of the changes in temperature, pressure, and solvent type dissolved in oil. As a result of this destabilization, the asphaltene flocculates, agglomerates, and eventually plugs the pores in the reservoir because of the formation of asphaltene clusters. In solvent applications, light-molecular-weight hydrocarbon solvents are preferred because of their high diffusion coefficient; however, as the carbon number of n-alkane solvents decreases, asphaltene precipitation increases. Therefore, the selection of the solvent and application condition is highly critical in cold and thermally aided solvent applications. In this research, low-carbon-number n-alkane (propane, n-hexane, and n-decane) and a distillate-hydrocarbon (obtained from a heavy-oil-upgrading facility) injection into glass-bead-pack systems saturated with heavy oil (87,651 and 20,918 cp at 25°C) were evaluated at different pressure conditions that are applicable to typical Canadian oil-sand reservoirs (698–2068 kPa) and temperatures (25–120°C). First, the asphaltene behavior of different solvents at different pressures and temperatures was studied through deasphalting work in a pressure/volume/temperature (PVT) cell in previous work [Moreno and Babadagli (2013)]. By use of quantitative (amount of asphaltene precipitated) and qualitative (microscopic images of asphaltene clusters) observations, asphaltenes were classified in terms of their shape, size, and quantity for different oil/solvent types, pressure, and temperature. Continually, the same n-alkane, distillate-hydrocarbon solvents, and heavy oil were used in gravity-stable-displacement glass-bead-pack experiments. 3-D (cylindrical) glass-bead-pack experiments were carried out at the same temperature and pressure conditions used for the PVT experiments. The operational conditions, oil composition, and solvent type showed significant effects on oil-recovery factor. Asphaltene deposition and residual oil saturation (ROS) in the glass-bead pack and the amount of asphaltene in the produced oil were measured, and the standard saturate, aromatic, resin, and asphaltene (SARA) analysis was applied to determine the optimal operating conditions yielding the highest recoveries with minimal pore plugging. Moreover, the pore-plugging process was analyzed through a visual scanning electron microscope (SEM) and optical microscope to find the different organic deposition formation and agglomeration. Oil production was evaluated by use of microscope visualization, viscosity reduction, and refractive-index values. Eventually, optimal application conditions for solvent and thermally aided solvent injection were listed for a wide range of heavy-oil and solvent types.

Asphaltene destabilization during solvent-based heavy oil and bitumen recovery applications is a common problem due to continuous changes of temperature, pressure, and oil composition. The effects of these characteristics on the recovery performance should be investigated for a wide range of solvent and oil types. In this paper, two heavy oil samples from fields in Alberta, Canada were destabilized using three different types of paraffin: propane, n -hexane, and n -decane. The solvent-based process was conducted at different reservoir conditions with alterations made to the temperature, pressure, and oil composition to determine the effect on asphaltene flocculation in the produced fluid and its deposition on the rock surface. Initially, experiments were carried out using a pressure, volume, and temperature (PVT) cell at different reservoir pressures and under different temperature conditions. Next, a Focused Ion Beam (FIB) and Scanning Electron Microscope (SEM) were used to characterize the morphology of the organic deposition on the glass beads surface through core flooding experiments. The results obtained through these two sets of experiments showed that temperature, pressure and oil compositions have a critical influence on asphaltene solubility. The PVT cell experiments and organic deposition surface roughness calculations were fundamentally important to explain the plugging formation in the reservoir under different operational conditions and with different oil types.

Solvent-based recovery processes improves crude oil production and quality because underground in situ upgrading takes place after heavy oil is diluted with a light hydrocarbon solvent. Consequently, crude oil mobility increases due to significant reduction in viscosity caused by the asphaltene precipitation. However, solvent driven recovery processes are quite complex on account of the “asphaltene destabilization” that takes place due to changes in temperature, pressure, crude oil composition, and solvent dissolved in oil. As a result of this destabilization, the asphaltene particles start to flocculate, and eventually may plug the pores in the reservoir due to larger asphaltene particle agglomeration in the reservoir. In this paper, the deasphalting of a heavy oil sample [8.67°API] was carried out using an optical PVT cell. The experiments were undertaken at different temperature ranges [122 °F (50 °C)–230 °F (110 °C)] and pressure ranges (30–500 psig), which are the suggested ranges applicable to typical Canadian oilsands or similar shallow heavy-oil deposits around the world. Three light hydrocarbon solvents (propane, n-hexane, and n-decane) were used to break the stability of the asphaltene in the crude oil. An experimental methodology for “asphaltene precipitation fraction” was developed in order to determine the effect of temperature, pressure, and solvent type on the amount of asphaltene precipitation. The experimental methodology was complemented through visual observations of asphaltene characteristics on the PVT cell as well as using optical microscopy. In addition, the refractive index measurements at the onset of precipitation were used to evaluate the changes in the oil after interacting with the solvent at different temperatures and pressures. Overall, the results showed that precipitated asphaltene fraction increases when the concentration of the resins goes down after the deasphalting of crude oil with solvent. In addition, the aggregation of asphaltene particles increases with increasing temperature [122 °F (50 °C)–230 °F (110 °C)] and pressure (from 30 psig to 500 psig), and decreasing solvent carbon number (from C10 to C3). At the end, a comparative analysis of the quantitative and qualitative results from the experiments is provided. Based on these observations, the characteristics of asphaltene were classified in terms of their shape, size, and amount for different oil/solvent types, pressure, and temperature. This study will eventually lead to the identification of the effects of asphaltene characteristics on pore plugging during heavy-oil/bitumen recovery by gravity drainage from oilsands.

Presentamos el caso de un paciente que sufrió una parada cardiorrespiratoria extrahospitalaria en helicóptero sanitario, refractaria a tratamiento médico y eléctrico convencional. El paciente recuperó la circulación espontánea tras 58 minutos en paro cardiaco y 31 descargas eléctricas, estando monitorizado en todo momento de la reanimación cardiopulmonar con capnografía (valores > 20 mm/Hg). A su llegada al hospital, se le realizó una angioplastia primaria, colocándosele un stent en la arteria coronaria derecha. El paciente fue dado de alta hospitalaria, sin secuelas neurológicas (categoría de función cerebral 1). We report the case of a patient who presented an out-of-hospital cardiorespiratory arrest while being transported by the Helicopter Emergency Service, refractory to standard, electrical and medical treatment. The patient recovered spontaneous circulation after 58 minutes with heart arrest and 31 delivered shocks. The reanimation manoeuvres were monitored with capnography (titrated over 20 mmHg EtCO2). Upon arrival at the hospital, the patient went directly to the haemodynamic laboratory where a percutaneous coronary intervention was performed, with a stent in the right coronary artery. The patient was discharged after 8 days without any neurological handicap (cerebral performance category 1.)

In this work, we developed the first study in Colombia to obtain and evaluate syngas compositions derived from asphaltenes gasification. These asphaltenes came from the implementation of a Deasphalting process to San Fernando crude oil, with the purpose of looking for technological options for their utilization. We performed the design, installation and commissioning of facilities for the gasification of asphaltenes at laboratory scale, it following an experimental methodology, performing nine tests and considering temperature and agent gasification quantity (oxygen) as independent variables. The syngas derived from gasification was analyzed by two chromatographic techniques, which reported the presence of refinery gases and sulfur. We evidenced a growth tendency of CO, H2 and sulfur composition and a decrease in CH4 and CO2 composition with temperature. The composition of the syngas was evaluated with different quantities of gasification agent (33%, 40% and 47% the amount of oxygen theoretically required for complete combustion) at each temperature levels operated. It was established that when using a 40% of gasification agent, you get greater average content of CO and H2, which are the interest gases in the gasification process. En este trabajo se desarrolló el primer estudio en el país de la obtención y evaluación de las composiciones del syngas a partir de la gasificación de asfaltenos provenientes de la aplicación del proceso de Desasfaltado al crudo San Fernando, con el objeto de evaluar opciones tecnológicas para su utilización. Para ello se realizó el diseño y puesta en marcha de un sistema de gasificación de asfaltenos a escala de laboratorio, donde se empleó una metodología experimental, realizando nueve pruebas y tomando como variables independientes la temperatura y el agente gasificante (oxígeno). El syngas obtenido de la gasificación fue analizado por dos técnicas de cromatografía, las cuales reportaron la presencia de gases de refinería y azufrados. De estos resultados experimentales se evidencia una tendencia de crecimiento en la composición del CO, H2 y azufrados, y una disminución en la composición del CH4 y CO2 con la temperatura. Se evaluó la composición del syngas con la variación del agente gasificante (33%, 40% y 47% del oxígeno teórico requerido en una combustión total) en cada uno de los niveles de temperatura operados y se estableció que al utilizar un 40% de agente gasificante, se obtienen en promedio los mejores resultados en las composiciones del CO y H4, siendo estos los gases de interés en el proceso de gasificación. Neste trabalho desenvolveuse o primeiro estudo no país da obtenção e avaliação das composições do syngas a partir da gaseificação de asfaltenos provenientes da aplicação do processo de Desasfaltado ao cru São Fernando, com o objeto de avaliar opções tecnológicas para a sua utilização. Para tal realizou-se o desenho e posta em marcha de um sistema de gaseificação de asfaltenos a escala de laboratório, onde se empregou uma metodologia experimental, realizando nove provas e tomando como variáveis independentes a temperatura e o agente gaseificante (oxigênio). O syngas obtido da gaseificação foi analisado por duas técnicas de cromatografia, as quais reportaram a presença de gases de refinaria e enxofrados. Destes resultados experimentais evidenciase uma tendência de crescimento na composição do CO, H2 e enxofrados, e uma diminuição na composição do CH4 e CO2 com a temperatura. Avaliouse a composição do syngas com a variação do agente gaseificante (33%, 40% e 47% do oxigênio teórico requerido em uma combustão total) em cada um dos níveis de temperatura operados e estabeleceuse que ao utilizar um 40% de agente gaseificante, obtém-se em média os melhores resultados nas composições do CO e H4, sendo estes os gases de interesse no processo de gaseificação.

A reduction in radiation doses at the catheterization laboratory, maintaining the quality of procedures is essential. Our objective was to analyze the results of a simple radiation reduction protocol at a high-volume interventional cardiology unit.We analyzed 1160 consecutive procedures: 580 performed before the implementation of the protocol and 580 after it. The protocol consisted in: the reduction of the number of ventriculographies and aortographies, the optimization of the collimation and the geometry of the X ray tube-patient-receptor, the use of low dose-rate fluoroscopy and the reduction of the number of cine sequences using the software "last fluoroscopy hold".There were no significant differences in clinical baseline features or in the procedural characteristics with the exception of a higher percentage of radial approach (30.7% vs 69.6%; p0.001) and of percutaneous coronary interventions of chronic total occlusions after the implementation of the protocol (2.1% vs 6.7%; p=0,001). Angiographic success was similar during both periods (98.3% vs 99.2%; p=0.2). There were no significant differences between both periods regarding the overall duration of the procedures (26.9 vs 29.6min; p=0.14), or the fluoroscopy time (13.3 vs 13.2min; p=0.8). We observed a reduction in the percentage of procedures with ventriculography (80.9% vs 7.1%; p0.0001) or aortography (15.4% vs 4.4%; p0.0001), the cine runs (21.8 vs 6.9; p0.0001) and the dose-area product (165 vs 71 Gyxcm(2); p0.0001).With the implementation of a simple radiation reduction protocol, a 57% reduction of dose-area product was observed without a reduction in the quality or the complexity of procedures.

An 84 years-old male was admitted for stable angina and an early positive treadmill test. He presented an angiographic severe ostial stenosis of the left main (LM) with no other significant lesions. Intravascular ultrasound image (IVUS) showed a fibro-calcified plaque with a minimal luminal area (MLA) of 3.9 mm2 (Figure1A-asterisk) and a reference diameter of 5.8 mm. A percutaneous coronary intervention (PCI) was scheduled the following day.

El presente informe describe a profundidad el resultado del ejercicio de práctica profesional que dentro del marco formativo de la Maestría en Estudios Sociales, en la línea de profundización o Análisis y Gestión de proyectos de Intervención Social. Se elabora un diagnóstico situacional y contextual sobre un ejercicio investigativo realizado por una firma consultora de proyectos antropológicos, que se ejecutó bajo contrato y por petición de una compañía petrolera con operación en varios departamentos de Colombia. Es preciso aclarar que no se cuenta con la información de una línea base, ya que ésta no fue suministrada por la empresa para quien se elaboró el diagnóstico. La investigación fue desarrollada durante veinticinco días en los que se visitaron tres campos, ubicados en dos departamentos diferentes. Durante este tiempo, el equipo investigador vivió bajo las mismas condiciones que los trabajadores, empleando técnicas de investigación cualitativa como la observación, la entrevista a profundidad y el grupo focal; esto atendiendo a las características del contexto y la disponibilidad de informantes clave, que estaban sujetas a la no alteración de la vida cotidiana laboral. Entre los hallazgos más relevantes aportados por la información recabada, se encuentran: la caracterización del relacionamiento y discursos que refuerzan la discriminación de género, las condiciones laborales en términos de estatus y remuneración, los usos y apropiación del espacio, todo esto analizado principalmente desde un abordaje Bourdieusano, y la pertinencia que en este contexto puede tener el abordaje del fenómeno desde la perspectiva del trabajo decente como lo propone la OIT (2005). A partir de esto, se señalan los aspectos fundamentales a tener en cuenta para una futura estructuración de estrategias de intervención encaminadas a la modificación del contexto y la dignificación del trabajo en campo, demostrando la pertinencia y el aporte que tiene la formación aplicada en cualquier tipo de contexto social. This report describes in depth the results of an exercise during my internship within the educational framework of the Master of Social Studies. A situational and contextual diagnosis is made, based on a research exercise conducted by a consulting firm of anthropological projects, implemented under contract and at the request of an oil company with operations in several departments of Colombia. It should be clarified there is not baseline information, as this was not provided by the company for whom the diagnosis was made. The research was made for twenty days in three different fields, located in two different departaments. During this time, the research team lived under the same conditions as workers, using qualitative research techniques such as observation, depth interviews and focus group; this, according to the characteristics of the context and the availability of key informants. Among the most important findings provided by the information collected they are: the characterization of the relationship and speeches that reinforce gender discrimination, working conditions in terms of status and remuneration, uses and appropriation of space, all analyzed mainly from a Bourdieu approach and the relevance in this context may be the approach of the phenomenon from the perspective of decent work as proposed by the ILO (2005). From this, the fundamental aspects to consider for future structuring of intervention strategies aimed at modifying the context and the dignity of work in the field are identified, demonstrating the relevance and contribution that has applied in any training social context.
