Car theft and joyriding are becoming increasingly common in regional areas of Australia. Whilst the consequences of these actions can be fatal, surprisingly little is known about the motivations for young people who engage in these behaviours. This exploratory study draws from 17 in-depth interviews that were conducted as part of a pilot intervention program aimed at adolescents either already engaged or susceptible to car theft and joyriding behaviours. The findings suggest that joyriding predominately attracts disenfranchised youth who lack opportunities for education, employment, and leisure. For these adolescents, joyriding emerges to counterbalance disadvantage and alleviate the monotony in their lives. Interviewees also provide suggestions for deterring joyriding, such as providing young people with occupational opportunities that allow them to serve as role models to themselves and others and adrenaline-based activities to replace the thrills of joyriding. This article, titled "Implementation of video-assisted anal fistula treatment (VAAFT) for complex fistulas - A Video Vignette," discusses the use of video-assisted anal fistula treatment (VAAFT) as a minimally invasive technique for treating complex anal fistulas. Young people with mental illness are significantly more likely to encounter the police than their counterparts who do not identify as having a mental illness. Yet little is known regarding how this cohort perceives the police and whether they believe the police to be a fair, trustworthy, and legitimate service. Research suggests that young people and other vulnerable groups (such as adults with mental illness) value procedural justice policing as a technique for nurturing fair and trustworthy policing, which in turn, increases satisfaction with police interactions and willingness to cooperate with police. This study uses procedural justice as a lens for analysing the perceptions of young people with mental illnesses regarding the police. Drawing upon survey data from a sample of 3147 Australian participants aged between 14 and 25 years old – a third of which identified as having a mental health condition – results demonstrate that young people with a mental illness offered significantly lower perceptions of the police in relation to procedural justice. Young people identifying as not heterosexual or as trans* or gender diverse, and those who report that they are not seen as Australian, also offered significantly lower perceptions of police procedural justice. A theoretical explanation is offered for why these marginalised young groups perceive the police to be procedurally unjust. The rising incidence combined with pregnancy-related physiological changes make gallbladder and biliary pathology high on the differential for pregnant patients presenting with right upper abdominal pain. Imaging plays a crucial role in determining surgical versus non-surgical management in pregnant patients with biliary or gallbladder pathology. Ultrasound (first-line) and magnetic resonance with magnetic resonance cholangiopancreatography (second-line) are the imaging techniques of choice in pregnant patients with suspected biliary pathology due to their lack of ionizing radiation. MRI/MRCP offers an excellent non-invasive imaging option, providing detailed anatomical detail without known harmful fetal side effects. Liver transplantation is a potentially curative treatment for patients with acute liver failure, end-stage liver disease, and primary hepatic malignancy. Despite tremendous advancements in surgical techniques and immunosuppressive management, there remains a high rate of post-transplant complications, with one of the main complications being biliary complications. In addition to anastomotic leak and stricture, numerous additional biliary complications are encountered, including ischemic cholangiopathy due to the sole arterial supply of the bile ducts, recurrence of primary biliary disease, infections, biliary obstruction from stones, cast, or hemobilia, and less commonly cystic duct remnant mucocele, vanishing duct syndrome, duct discrepancy and kinking, post-transplant lymphoproliferative disorder, retained stent, and ampullary dysfunction. This article presents an overview of biliary anatomy and surgical techniques in liver transplantation, followed by a detailed review of post-transplant biliary complications with their corresponding imaging findings on multiple modalities with emphasis on magnetic resonance imaging and MR cholangiopancreatography. Primary sclerosing cholangitis is a rare chronic inflammatory disease affecting the bile ducts, which can eventually result in bile duct strictures, cholestasis and cirrhosis. Pathologies of the biliary tree include a wide-spectrum of benign and malignant processes. The differential for benign disease includes congenital and acquired disease with variable prognosis and management pathways. Given the ability to mimic malignancy, benign processes are difficult to diagnose by imaging. Direct cholangiography techniques with tissue sampling are the gold standards for the diagnosis of benign and malignant biliary pathologies. Non-invasive imaging with ultrasound offers a first-line diagnostic tool while MRI/MRCP offers higher specificity for identifying underlying pathology and distinguishing from malignant disease. In this review, we focus on the imaging appearance of dilatation, cystic anomalies obstruction, inflammation, ischemia, strictures, pneumobilia, and hemobilia to help construct a differential for benign processes. Primary biliary cholangitis is a rare progressive chronic inflammation of the medium and small bile ducts that abdominal radiologists may encounter, particularly if working in a tertiary setting or at a transplant center. This brief review covers current thinking about the pathophysiology and presentation of the disease, as well as the current diagnostic criteria in use by hepatologists. Imaging strategies for diagnosis will be reviewed as well as current treatment strategies and the use of imaging in monitoring response to treatment, including image-guided elastography. Naegleria fowleri has been detected in drinking water distribution systems (DWDS) in Australia, Pakistan and the United States and is the causative agent of the highly fatal disease primary amoebic meningoencephalitis. Previous small scale field studies have shown that Meiothermus may be a potential biomarker for N. fowleri. However, correlations between predictive biomarkers in small sample sizes often breakdown when applied to larger more representative datasets. This study represents one of the largest and most rigorous temporal investigations of Naegleria fowleri colonisation in an operational DWDS in the world and measured the association of Meiothermus and N. fowleri over a significantly larger space and time in the DWDS. A total of 232 samples were collected from five sites over three-years (2016–2018), which contained 29 positive N. fowleri samples. Two specific operational taxonomic units assigned to M. chliarophilus and M. hypogaeus, were significantly associated with N. fowleri presence. Furthermore, inoculation experiments demonstrated that Meiothermus was required to support N. fowleri growth in field-collected biofilms. MicroRNAs expressed by germ cell tumors represent a novel approach to detection of metastatic disease during staging, surveillance, and recurrence post-therapy. It has particular promise in settings of equivocal imaging, such as clinical stage I GCT, tumor marker negative stage IIA, or after chemotherapy. Multi-covariate Imaging of Sub-resolution Targets (MIST) is an estimation-based method of imaging the statistics of diffuse scattering targets. MIST estimates the contributions of a set of covariance models to the echo data covariance matrix. Models are defined based on a spatial decomposition of the theoretical transmit intensity distribution into ON-axis and OFF-axis contributions, delineated by a user-specified spatial cutoff. We define this cutoff as the region of interest width (ROI width). In our previous work, we selected the ROI width as the first zero crossing separating the mainlobe from the sidelobe regions. This article explores the effects of varying two key parameters on MIST image quality: 1) ROI width and 2) the degree of spatial averaging of the measured echo data covariance matrix. These results demonstrate a fundamental tradeoff between resolution and speckle texture. We characterize MIST imaging performance across these tunable parameters in a number of simulated, phantom, and in vivo liver applications. We consider performance in noise, fidelity to native contrast, resolution, and speckle texture. MIST is also compared with varying levels of spatial and frequency compounding, demonstrating quantitative improvements in image quality at comparable levels of speckle reduction. In an in vivo example, optimized MIST images demonstrated 20.2% and 13.4% improvements in contrast-to-noise ratio over optimized spatial and frequency compounding images, respectively. Nosocomial transmission of COVID-19 puts patients with other medical problems at risk of severe illness and death. Of 662 inpatients with COVID-19 at an NHS Trust in South London, 45 (6.8%) were likely to have acquired COVID-19 in hospital. These patients had no evidence of respiratory or influenza-like illness on admission and developed symptoms, with positive SARS-CoV-2 PCR test results, more than 7 days after admission (>14 days for 38 [5.7%] patients). Forty (88.9%) of these patients had shared a ward with a confirmed COVID-19 case prior to testing positive. Implementation of a triage system combining clinical assessment with rapid SARS-CoV-2 testing facilitated cohorting so that fewer susceptible patients were exposed to COVID-19 on shared wards. With hospital service resumption alongside the possibility of future waves of COVID-19 related admissions, strategies to prevent nosocomial transmission are essential. Coherence-based imaging methods suffer from reduced image quality outside the depth of field for focused ultrasound transmissions. Synthetic aperture methods can extend the depth of field by coherently compounding time-delayed echo data from multiple transmit events. Recently, our group has presented the Multi-covariate Imaging of Sub-resolution Targets (MIST), an estimation-based method to image the statistical properties of diffuse targets. MIST has demonstrated improved image quality over conventional delay-and-sum, but like many coherence-based imaging methods, suffers from limited depth of field artifacts. This article applies synthetic aperture focusing to MIST, which is evaluated using focused, plane-wave, and diverging-wave transmit geometries. Synthetic aperture MIST is evaluated in simulation, phantom, and in vivo applications, demonstrating consistent improvements in contrast-to-noise ratio (CNR) over conventional dynamic receive MIST outside the transmit depth of field, with approximately equivalent results between synthetic transmit geometries. In vivo synthetic aperture MIST images demonstrated 16.8 dB and 16.6% improvements in contrast and CNR, respectively, over dynamic receive MIST images, as well as 17.4 dB and 32.3% improvements over synthetic aperture B-Mode. MIST performance is characterized in the space of plane-wave imaging, where the total plane-wave count is reduced through coarse angular sampling or total angular span. Objective: The Philadelphia Mindfulness Scale's (PHLMS) psychometric properties, theoretical assumptions, convergent validity, and relationships to symptom, happiness and social desirability measures were investigated in novel samples. Methods: Internet‐recruited general public samples of experienced meditators (n = 67, 21 male, 46 female, average age 50) and nonmeditators (n = 61, 28 male, 32 female, and 1 transgender; average age 41.9) were used. Results: Meditators scored higher than nonmeditators on both PHLMS Acceptance, t(126) = 2.77, p <.01, d = 0.49, and Awareness t(126) = 4.18, p <.01, d = 0.74. Higher PHLMS Acceptance subscale scores were associated with decreased scores on all symptom measures in nonmeditators, but only in select measures among meditators. Generally, PHLMS Awareness subscale scores were not related to symptoms. Awareness interacted with meditator status on outcome measures. The electrophilic borylation of 2,5-diarylpyrazines results in the formation of boron–nitrogen doped dihydroindeno[1,2-b]fluorene which can be synthesized using standard Schlenk techniques and worked up and handled readily under atmospheric conditions. Through transmetallation via diarylzinc reagents a series of derivatives were synthesized which show broad visible to near-IR light absorption profiles that highlight the versatility of this BN substituted core for use in optoelectronic devices. The synthesis is efficient, scalable and allows for tuning through changes in substituents on the planar heterocyclic core and at boron. The van Cittert-Zernike (VCZ) theorem describes the propagation of spatial covariance from an incoherent source distribution, such as backscatter from stochastic targets in pulse-echo imaging. These stochastic targets are typically assumed statistically stationary and spatially incoherent with uniform scattering strength. In this work, the VCZ theorem is applied to a piecewise-stationary scattering model. Under this framework, the spatial covariance of the received echo data is demonstrated as the linear superposition of covariances from distinct spatial regions. This theory is analytically derived from fundamental physical principles, and validated through simulation studies demonstrating superposition and scaling. Simulations show that linearity is preserved over various depths and transmit apodizations, and in the presence of noise. These results provide a general framework to decompose spatial covariance into contributions from distinct regions of interest, which may be applied to advanced imaging methods. While the simulation tools used for validation are specific to ultrasound, this analysis is generally applicable to other coherent imaging applications involving stochastic targets. Conventional B-mode ultrasound imaging assumes that targets consist of collections of point scatterers. Diffraction, however, presents a fundamental limit on a scanner’s ability to resolve individual scatterers in most clinical imaging environments. Well-known optics and ultrasound literature has characterized these diffuse scattering targets as spatially incoherent and statistically stationary. In this paper, we apply a piecewise-stationary statistical model to diffuse scattering targets, in which the covariance of backscattered echoes can be described as the linear superposition of constituent components corresponding to echoes from distinct spatial regions in the field. Using this framework, we present Multi-covariate Imaging of Sub-resolution Targets (MIST), a novel estimation-based method to image the statistical properties of diffuse scattering targets, based on a decomposition of aperture domain spatial covariance. The mathematical foundations of the estimator are analytically derived, and MIST is evaluated in phantom, simulation, and in vivo studies, demonstrating consistent improvements in contrast-to-noise ratio and speckle statistics across imaging targets, without an apparent loss in resolution. Short-lag spatial coherence (SLSC) imaging has demonstrated improved performance over conventional B-mode ultrasound imaging. Previous work has evaluated the performance of SLSC using 2-D matrix arrays in simulation and in vivo studies across various levels of subaperture beamforming, demonstrating improved contrast-to-noise ratio (CNR) and speckle signal-to-noise ratio (SNR) over 1-D arrays. This work explores the application of SLSC imaging in 1.5-D and 1.75-D arrays to quantify the impacts of elevation element count, mirroring, and Fresnel element spacing on SLSC image quality. Through simulation and in vivo studies, increased elevation element count was shown to improve CNR and speckle SNR relative to 1-D SLSC and B-mode images. Elevation mirroring (1.5-D) was shown to force the inclusion of long lags into the SLSC calculation, introducing additional decorrelation and reducing image quality relative to 1.75-D arrays with individually connected elements. A human rights approach to the policing of mental ill-health raises fundamental questions about the vulnerability of people in the care of the police, the appropriateness of police interventions, and how societies define and delineate the role and function of the police and health sectors. It is the challenge of understanding and interpreting the police–health nexus and its associated points of intervention that this article addresses. The article uses a human rights framework to explore the challenges that emerge when policing mental ill-health through the use of Section 136 of the Mental Health Act 1983 and recent experimental use of mental health triage in England and Wales. The article explores the potential of triage to alleviate some of the human rights concerns associate with the use of Section 136 through interviews with police officers involved with the triage pilots. The final discussion situates experiments with mental health triage against a backcloth of mental health's increasingly prominent position on the global public policy agenda. The article concludes with call for a reassessment and realignment of thinking about the police–health nexus that aligns with the United Nations' sustainable development goals for 2030. Purpose: To compare frequency of new and recurrent urothelial cell carcinoma (UCC) among patients with and without pseudodiverticulosis on imaging.Methods: This retrospective case-control study compared all 113 sequential patients with ureteral pseudodiverticulosis on radiographic urography between 1/1/2002 and 12/31/2012. Six patients were lost to follow-up. 107 patients without pseudodiverticulosis were matched by imaging modality, clinical indication, and tumor grade. Known UCC and primary outcome of new or recurrent UCC were determined through pathology on cystoscopy or clinical follow-up.Results: Nearly half of patients with pseudodiverticulosis had known UCC at the time of imaging (49/107, 46%). Mean cystoscopy follow-up was 7.0 and 4.6 years for pseudodiverticulosis cases with and without known UCC, respectively, and 7.5 and 7.3 years for controls, respectively. Mean clinic follow-up was 7.5 and 6.0 years for pseudodiverticulosis cases with and without known UCC, respectively, and 6.4 and 7.6 years for controls, respectively. Among patients with known UCC at the time of imaging, similar rates of recurrent UCC were demonstrated on follow-up among patients with pseudodiverticulosis (6/49, 12%) and without (7/49, 14%). Among patients with no known history of UCC at the time of imaging, no patients with pseudodiverticulosis developed UCC on follow-up and 5% (3/58) of patients without pseudodiverticulosis developed UCC.Conclusion: Although half of patients with ureteral pseudodiverticulosis have a known diagnosis of UCC, the presence of pseudodiverticulosis did not signify an increased likelihood of developing new or recurrent UCC over the follow-up period. Purpose: This article reviews current prostatectomy techniques and then techniques for fluoroscopic voiding cystourethrography (VCUG) evaluation of the prostate and bladder after prostatectomy. Both normal post-prostatectomy appearances and complications will be reviewed.Conclusion: VCUG is a useful evaluation for both common and uncommon post-prostatectomy complications and appropriate interpretation of the VCUG complications is important to aid the urologist’s clinical decision making. Ultrasound imaging has indications across many areas of medicine, but the need for training and the variability in skill and acquired image quality among 2-D ultrasound users have limited its wider adoption and utilization. Low-cost volumetric ultrasound with a known frame of reference has the potential to lower these operator-dependent barriers and enhance the clinical utility of ultrasound imaging. In this paper, we improve upon our previous research-scanner-based prototype to implement a versatile volumetric imaging platform for existing clinical 2-D ultrasound systems. We present improved data acquisition and image reconstruction schemes to increase quality, streamline workflow, and provide real-time visual feedback. The amoeba Naegleria fowleri is the causative agent of the highly fatal disease, primary amoebic meningoencephalitis, and estimated to cause 16 deaths per year in the United States alone. Colonisation of drinking water distribution systems (DWDSs) by the N. fowleri is a significant public health issue. Understanding the factors which enable this pathogen to colonise and thrive in DWDSs is critical for proper management. The microbial ecology within DWDSs may influence the ability of N. fowleri to colonise DWDSs by facilitating the availability of an appropriate food source. Using biofilm samples obtained from operational DWDSs, 16S rRNA amplicon metabarcoding was combined with genus-specific PCR and Sanger sequencing of intracellular associated bacteria from isolated amoeba and their parental biofilms to identify Meiothermus chliarophilus as a potential food source for N. fowleri . Meiothermus was confirmed as a food source for N. fowleri following successful serial culturing of axenic N. fowleri with M. chliarophilus or M. ruber as the sole food source. The ability to identify environmental and ecological conditions favourable to N. fowleri colonisation, including the detection of appropriate food sources such as Meiothermus , could provide water utilities with a predictive tool for managing N. fowleri colonisation within the DWDS. Objective: Local control is a major challenge in treating canine nasal tumors, and cytoreduction following radiation therapy has been recommended to extend survival and to delay local recurrence. Our objective was to compare the effect of definitive radiotherapy on the tumor volume of intranasal carcinomas compared to sarcomas. We evaluated 15 dogs that received radiotherapy within 1 month of initial CT scan, and post radiation CT scans performed within 3 months of completing full course definitive megavoltage radiation. Tumor reduction volume based on CT scans were obtained and compared between carcinoma and sarcoma groups. Results: The following tumor types were treated; carcinoma (8/15), sarcoma (7/15). The mean nasal tumor size before radiation therapy was 24.5 cm³ and tumor size after radiation therapy was 13.5 cm³ resulting in a mean reduction of 55.1% reduction in tumor size for both carcinomas and sarcomas. The carcinoma group displayed a volume reduction of 67.1% (SD ± 16.9) and the sarcoma group displayed a volume reduction of 21.3% (SD ± 39.7). The reactions of 1-bora-7a-azaindenide anions, prepared in moderate to excellent yields by reduction of the appropriate 1-bora-7a-azaindenyl chlorides with KC8 in THF, with alkyl halides and carbon dioxide were studied. With alkyl halides (CH2Cl2, CH3I and BrCH(D)CH(D)tBu), the anions behave as boron anions, alkylating the boron centre via a classic SN2 mechanism. This was established with DFT methods and via experiments utilizing the neo-hexyl stereoprobe BrCH(D)CH(D)tBu. These reactions were in part driven by a re-aromatization of the six membered pyridyl ring upon formation of the product. Conversely, in the reaction of the 1-bora-7a-azaindenide anions with CO2, a novel carboxylation of the C-2 carbon alpha to boron was observed. Computations indicated that while carboxylation of the boron centre was kinetically feasible, the products of B-carboxylation were not thermodynamically favored relative to the observed C-2 carboxylated species, which were formed preferably due to the generation of both C–C and B–O bonds. This paper argues that the way NATO conducts its interventions has altered since the Global Financial Crisis. The pressures of the Global Financial Crisis have forced NATO to shift from the sustained peacekeeping operation in Afghanistan to what I refer to as the trans-scalar space of intervention in Libya. Trans-scalar spaces of intervention are spaces where violence is temporarily concentrated by a transnational military apparatus to eliminate a perceived threat as efficiently as possible. The threats targeted are contained within a particular national locale and are unable to manifest themselves transnationally; their capacity is therefore several orders of magnitude below the force projection abilities of the transnational military force seeking its destruction. I examine how NATO’s creation of these new types of spaces has impacted global security and undermined territorial-based forms of governance. A method for the preparation of 3-bora-9-azaindene heterocycles based on zirconocene-mediated functionalization of the o-CH bonds of pyridines has been developed and used to make two such compounds. Unlike other methods, the boron center in these heterocycles remains functionalized with a chloride ligand, and so the compounds can be further elaborated through halide abstraction and reduction. The utility of the method was further demonstrated by applying it toward the preparation of 1,5-dibora-4a,8a-diaza BN analogues of the intriguing hydrocarbon s-indacene starting from 2,5-dimethylpyrazine. Gram quantities of one such compound were prepared and fully characterized, and both experimental and computational data are presented to compare its properties to those of the parent hydrocarbon s-indacene. Global incidence of primary amoebic meningoencephalitis cases associated with domestic drinking water is increasing. The need for understanding disinfectant regimes capable of eliminating the causative microorganism, Naegleria fowleri, from bulk water and pipe wall biofilms is critical. This field study demonstrated the successful elimination of N. fowleri from the bulk water and pipe wall biofilm of a persistently colonised operational drinking water distribution system (DWDS), and the prevention of further re-colonisation. A new chlorination unit was installed along the pipe line to boost the free chlorine residual to combat the persistence of N. fowleri . Biofilm and bulk water were monitored prior to and after re-chlorination (RCl), pre-rechlorination (pre-RCl) and post-rechlorination (post-RCl), respectively, for one year. A constant free chlorine concentration of > 1 mg/L resulted in the elimination of N. fowleri from both the bulk water and biofilm at the post-RCl site. Other amoeba species were detected during the first two months of chlorination, but all amoebae were eliminated from both the bulk water and biofilm at post-RCl after 60 days of chlorination with free chlorine concentrations > 1 mg/L. In addition, a dynamic change in the biofilm community composition and a four log reduction in biofilm cell density occurred post-RCl. The pre-RCl site continued to be seasonally colonised by N. fowleri , but the constant free chlorine residual of > 1 mg/L prevented N. fowleri from recolonising the bulk and pipe wall biofilm at the post-RCl site. To our knowledge, this is the first study to demonstrate successful removal of N. fowleri from both the bulk and pipe wall biofilm and prevention of re-colonisation of N. fowleri in an operational DWDS. Polycyclic aromatic hydrocarbons in which one or more CC units have been replaced by isoelectronic BN units have attracted interest as potentially improved organic materials in various devices. This promise has been hampered by a lack of access to gram quantities of these materials. However, the exploitation of keystone reactions such as ring closing metathesis, borylative cyclization of amino styrenes and electrophilic borylation has lead to strategies for access to workable amounts of material. These strategies can be augmented by judicious postfunctionalization reactions to diversify the library of materials available. Free-living amoebae, such as Naegleria fowleri, Acanthamoeba spp., and Vermamoeba spp., have been identified as organisms of concern due to their role as hosts for pathogenic bacteria and as agents of human disease. In particular, N. fowleri is known to cause the disease primary amoebic meningoencephalitis (PAM) and can be found in drinking water systems in many countries. Understanding the temporal dynamics in relation to environmental and biological factors is vital for developing management tools for mitigating the risks of PAM. Characterizing drinking water systems in Western Australia with a combination of physical, chemical and biological measurements over the course of a year showed a close association of N. fowleri with free chlorine and distance from treatment over the course of a year. Background: The current body of literature surrounding anterior cruciate ligament (ACL) survival and the variables contributing to further ACL injuries after primary ACL reconstruction in children and adolescents is limited, with no long-term evidence examining the incidence and contributing factors of further ACL injuries in this younger patient population. Purpose: To determine the long-term survival of the ACL graft and the contralateral ACL (CACL) after primary reconstruction in patients aged ≤18 years and to identify the factors that increase the odds of subsequent ACL injuries. Study Design: Case series; Level of evidence, 4. Methods: Patients having undergone primary ACL reconstruction at age ≤18 years between 1993 and 1998 who were included in a prospective database by a single surgeon were considered for this study. Single-incision endoscopic ACL reconstruction was performed with either an autologous bone–patellar tendon–bone graft or a hamstring tendon graft. At a minimum of 15 years after ACL reconstruction, patients completed a subjective survey involving the International Knee Documentation Committee (IKDC) questionnaire in addition to questions regarding current symptoms, further ACL injuries, family history of ACL injury, and current level of activity. Results: A total of 288 adolescents (age range, 13-18 years) met the inclusion criteria, of whom 242 (84%) were reviewed at a mean of 16 years and 6 months after ACL reconstruction. Of these patients, 75 (31%) sustained a further ACL injury: 27 (11.2%) suffered an ACL graft rupture, 33 suffered a CACL injury (13.6%), and 15 sustained both an ACL graft rupture and a CACL injury (6.2%) over 15 years. Survival of the ACL graft was 95%, 92%, 88%, 85%, and 83% at 1, 2, 5, 10, and 15 years, respectively, and survival of the CACL was 99%, 98%, 90%, 83%, and 81%, respectively. Survival of the ACL graft was less favorable in those with a family history of ACL injury than in those without a family history (69% vs 90%, respectively; hazard ratio [HR], 3.6; P = .001). Survival of the CACL was less favorable in male patients than in female patients (75% vs 88%, respectively; HR, 2.1; P = .03) and in those who returned to competitive team ball sports than in those who did not (78% vs 89%, respectively; HR, 2.3; P = .05). Conclusion: After ACL reconstruction in patients aged ≤18 years, a further ACL injury occurred in 1 in 3 patients over 15 years. The 15-year survival rate of the ACL graft was 83%, and the 15-year survival rate of the CACL was 81%. The ACL graft and CACL were most vulnerable within the first 5 years after index surgery. A family history of ACL rupture significantly increased the risk for ACL graft ruptures, and a CACL injury was more common in male patients and those who returned to team ball sports. Sequence alignment is just as much a part of phylogenetics as is tree building, although it is often viewed solely as a necessary tool to construct trees. However, alignment for the purpose of phylogenetic inference is primarily about homology, as it is the procedure that expresses homology relationships among the characters, rather than the historical relationships of the taxa. Molecular homology is rather vaguely defined and understood, despite its importance in the molecular age. Indeed, homology has rarely been evaluated with respect to nucleotide sequence alignments, in spite of the fact that nucleotides are the only data that directly represent genotype. All other molecular data represent phenotype, just as do morphology and anatomy. Thus, efforts to improve sequence alignment for phylogenetic purposes should involve a more refined use of the homology concept at a molecular level. To this end, we present examples of molecular-data levels at which homology might be considered, and arrange them in a hierarchy. The concept that we propose has many levels, which link directly to the developmental and morphological components of homology. Of note, there is no simple relationship between gene homology and nucleotide homology. We also propose terminology with which to better describe and discuss molecular homology at these levels. Our over-arching conceptual framework is then used to shed light on the multitude of automated procedures that have been created for multiple-sequence alignment. Sequence alignment needs to be based on aligning homologous nucleotides, without necessary reference to homology at any other level of the hierarchy. In particular, inference of nucleotide homology involves deriving a plausible scenario for molecular change among the set of sequences. Our clarifications should allow the development of a procedure that specifically addresses homology, which is required when performing alignment for phylogenetic purposes, but which does not yet exist. The design, fabrication, and measurement of a coax to double-ridged waveguide launcher and horn antenna is presented. The novel launcher design employs two symmetric field probes across the ridge gap to minimize spreading inductance in the transition, and achieves better than 15 dB return loss over a 10:1 bandwidth. The aperture-matched horn uses a half-cosine transition into a linear taper for the outer waveguide dimensions and ridge width, and a power-law scaled gap to realize monotonically varying cutoff frequencies, thus avoiding the appearance of trapped mode resonances. This paper expands on the previously described reflectionless filters—i.e., filters having, in principle, identically zero reflection coefficient at all frequencies—by introducing a wide variety of new reflectionless structures that were not part of the original publication. In addition to extending the lumped-element derivation to include transmission line filters, this is achieved by the introduction of a novel method wherein the left- and right-hand-side stopband terminations are coupled to each other using a two-port sub-network. Specific examples of the sub-network are given, which increase the stopband attenuation per filter cell and steepen the cutoff response without disrupting the reflectionless property or increasing the insertion loss in the passband. It is noteworthy that a common feature of all the structures derived by these methods is that most, if not all, of the reactive elements are of equal normalized value. This greatly simplifies the tuning requirements, and has facilitated their implementation as monolithic microwave integrated circuits (MMICs). In response to the uncertain geopolitical environment which confronted it after the end of the Cold War, NATO adopted a strategy of risk management. Two institutions – the Science and Technology Office and Allied Command Transformation – were responsible for translating this policy into practice. By promoting knowledge development, each institution sought to make NATO a more reflexive and responsive organization. Knowledge development became the foundation for a new model of command and control created by two Science and Technology Office working groups. In the Colorado Piedmont, talus flatiron chronosequences are associated with buttes that consist of erodible arkosic sandstone and resistant caprock. Following the removal of the caprock, some buttes evolve into crater-like relict slope rings resulting from the differential erosion of the soft bedrock in the core of the hills. These unique landforms are only documented in the Colorado Piedmont. Their development is attributed to the unusually high erodibility contrast between the low-cohesion sandy bedrock and the bouldery colluvial armor. The talus flatiron sequences and relict slope rings mapped in the three studied areas record alternating periods of accumulation and incision in the slopes that are likely controlled by changes in moisture availability and vegetation cover density. The obtained OSL dates place slope accumulation phases at > 124 ka, and ca. 73 ka, 50–40 ka, 15 ka, and 10–6 ka. A comparison of these geochronological data with paleoclimatic records from the region suggests that colluvium deposition occurred during periods of denser vegetation cover, which are controlled by climate changes. Further investigations, including additional and more accurate geochrological data from the relict slopes, will help to better understand the paleoclimatic significance of these largely unknown morphostratigraphic features. Background Restoration of bowel continuity following Hartmann's procedure may be performed using a laparoscopic or open technique. This study is the first of its kind comparing laparoscopic with open reversal of Hartmann's procedure in Australasia. Methods This is a retrospective review of 107 patients who underwent either a laparoscopic ( n = 43) or open ( n = 64) reversal of Hartmann's procedure between 2001 and 2012. Outcome measures were perioperative clinical outcomes and post-operative complications. Results Patients in the two groups were comparable in age, body mass index, American Society of Anesthesiologists score and number of previous operations. The most common indication for the original Hartmann's operation in both groups was diverticular disease. Total theatre time was longer for the laparoscopic group (276.4 versus 242.0 min; P = 0.02). Three patients in the laparoscopic group required conversion to laparotomy (7%). Laparoscopic reversal of Hartmann's procedure was associated with shorter time to passage of flatus (2.8 versus 4.0 days; P < 0.001) and faeces (4.2 versus 5.6 days; P = 0.002), and shorter overall length of hospital stay (6.7 versus 10.8 days; P < 0.001). There were fewer patients in the laparoscopic group who had post-operative complications (14% versus 31%; P = 0.04), including fewer cases of post-operative ileus (2% versus 17%; P = 0.02). There were no cases of anastomotic leak or in-hospital mortality in either group. Conclusion Laparoscopic reversal of Hartmann's procedure is a safe and feasible alternative to open Hartmann's reversal and may be associated with significantly faster recovery time and fewer post-operative complications. Persistent pulmonary hypertension of the newborn (PPHN) is a clinical syndrome that is characterized by high pulmonary vascular resistance due to changes in lung vascular growth, structure, and tone. PPHN has been primarily considered as a disease of the small pulmonary arteries (PA), but proximal vascular stiffness has been shown to be an important predictor of morbidity and mortality in other diseases associated with pulmonary hypertension (PH). The objective of this study is to characterize main PA (MPA) stiffness in experimental PPHN and to determine the relationship of altered biomechanics of the MPA with changes in extracellular matrix (ECM) content and orientation of collagen and elastin fibers. MPAs were isolated from control and PPHN fetal sheep model and were tested by planar biaxial testing to measure stiffness in circumferential and axial vessel orientations. Test specimens were fixed for histological assessments of the vascular wall ECM constituents collagen and elastin. MPAs from PPHN sheep had increased mechanical stiffness (P < 0.05) and altered ECM remodeling compared with control MPA. A constitutive mathematical model and histology demonstrated that PPHN vessels have a smaller contribution of elastin and a greater role for collagen fiber engagement compared with the control arteries. We conclude that exposure to chronic hemodynamic stress in late-gestation fetal sheep increases proximal PA stiffness and alters ECM remodeling. We speculate that proximal PA stiffness further contributes to increased right ventricular impedance in experimental PPHN, which contributes to abnormal transition of the pulmonary circulation at birth. Many plant pathogens are microscopic, cryptic, and difficult to diagnose. The new approach of ecometagenetics, involving ultrasequencing, bioinformatics, and biostatistics, has the potential to improve diagnoses of plant pathogens such as nematodes from the complex mixtures found in many agricultural and biosecurity situations. We tested this approach on a gradient of complexity ranging from a few individuals from a few species of known nematode pathogens in a relatively defined substrate to a complex and poorly known suite of nematode pathogens in a complex forest soil, including its associated biota of unknown protists, fungi, and other microscopic eukaryotes. We added three known but contrasting species (Pratylenchus neglectus, the closely related P. thornei, and Heterodera avenae) to half the set of substrates, leaving the other half without them. We then tested whether all nematode pathogens-known and unknown, indigenous, and experimentally added-were detected consistently present or absent. We always detected the Pratylenchus spp. correctly and with the number of sequence reads proportional to the numbers added. However, a single cyst of H. avenae was only identified approximately half the time it was present. Other plant-parasitic nematodes and nematodes from other trophic groups were detected well but other eukaryotes were detected less consistently. DNA sampling errors or informatic errors or both were involved in misidentification of H. avenae; however, the proportions of each varied in the different bioinformatic pipelines and with different parameters used. To a large extent, false-positive and false-negative errors were complementary: pipelines and parameters with the highest false-positive rates had the lowest false-negative rates and vice versa. Sources of error identified included assumptions in the bioinformatic pipelines, slight differences in primer regions, the number of sequence reads regarded as the minimum threshold for inclusion in analysis, and inaccessible DNA in resistant life stages. Research in Caucasian populations has begun to examine the broad associations between physical and mental health in dementia caregivers. However, the examination of this relationship in Latin America is largely absent from the literature despite the fact that the region will see a major increase in dementia cases over the next 20 years. The current study examined the associations between health-related quality of life (HRQOL) and mental health in 90 dementia caregivers from Colombia, South America. A canonical correlation found that higher caregiver HRQOL was related to better mental health, as expected. Caregivers with high vitality and low role limitations due to physical problems tended to have low depression and high satisfaction with life. Follow-up multiple regressions found that caregiver role limitations due to physical problems was uniquely associated with satisfaction with life, whereas vitality, role limitations due to physical problems, and pain were uniquely associated with burden (although the pain effect was likely error due to a suppressor effect). Additionally, vitality and social functioning were uniquely negatively related to depression. Because of the extremely high overlap between these two sets of variables, dementia interventions are needed in Latin America that target both caregiver mental and physical health, as both likely operate in unison and influence each other. Accurate estimation of biological diversity in environmental DNA samples using high-throughput amplicon pyrosequencing must account for errors generated by PCR and sequencing. We describe a novel approach to distinguish the underlying sequence diversity in environmental DNA samples from errors that uses information on the abundance distribution of similar sequences across independent samples, as well as the frequency and diversity of sequences within individual samples. We have further refined this approach into a bioinformatics pipeline, Amplicon Pyrosequence Denoising Program (APDP) that is able to process raw sequence datasets into a set of validated sequences in formats compatible with commonly used downstream analyses packages. We demonstrate, by sequencing complex environmental samples and mock communities, that APDP is effective for removing errors from deeply sequenced datasets comprising biological and technical replicates, and can efficiently denoise single-sample datasets. APDP provides more conservative diversity estimates for complex datasets than other approaches; however, for some applications this may provide a more accurate and appropriate level of resolution, and result in greater confidence that returned sequences reflect the diversity of the underlying sample. Estimated phylogenies of evolutionarily diverse taxa will be well supported and more likely to be historically accurate when the analysis contains large amounts of data–many genes sequenced across many taxa. Inferring such phylogenies for non-model organisms is challenging given limited resources for whole-genome sequencing. We take advantage of genomic data from a single species to test the limits of hybridization-based enrichment of hundreds of exons across frog species that diverged up to 250 million years ago. Enrichment success for a given species depends greatly on the divergence time between it and the reference species, and the resulting alignment contains a significant proportion of missing data. However, our alignment generates a well-supported phylogeny of frogs, suggesting that this technique is a practical solution towards resolving relationships across deep evolutionary time. Objective Expanded populations of CD4+CD28− T cells with a cytotoxic phenotype have been repeatedly reported in patients with granulomatosis with polyangiitis (Wegener's) (GPA). In healthy individuals expansion of this T cell population follows cytomegalovirus (CMV) infection. We undertook this study to investigate whether CMV infection may be responsible for driving the expansion of CD4+CD28− T cells in GPA patients and how this might relate to clinical features. Methods Forty-eight GPA patients and 38 age-matched healthy donors were included in the study. CMV-specific IgG in serum was detected by enzyme-linked immunosorbent assay. Flow cytometric analysis was used to study T cell populations and phenotype. The presence of CMV in renal biopsy tissue from GPA patients was investigated by immunohistochemistry and polymerase chain reaction (PCR). Clinical information was obtained from patient records. Results Populations of CD4+CD28− T cells were only expanded in CMV-seropositive GPA patients and controls. In CMV-seropositive GPA patients we observed negative correlations between the percentages of CD4+CD28− T cells and both the percentage of naive T cells and the glomerular filtration rate at presentation. There was a significant association between the percentage of CD4+CD28− T cells and risk of infection and mortality. CMV could not be detected in renal tissue by PCR or immunohistochemistry. CMV seropositivity itself was not a risk factor for infection in a cohort of 182 patients with antineutrophil cytoplasmic antibody-associated vasculitis who had been recruited into clinical trials performed by the European Vasculitis Study Group.
