Background: The vein of Marshall (VOM) ethanol infusion improves rhythm control in atrial fibrillation (AF). The identification and cannulation of the VOM can be technically challenging. This study aimed to assess the angiographic morphology of the VOM and investigate its value in the VOM ethanol infusion. Methods: Patients with AF (n = 162) scheduled for combined catheter ablation and VOM ethanol infusion were enrolled. The VOM morphologic features in the right anterior oblique (RAO), the left anterior oblique (LAO), and the LAO cranial views were analyzed. The impact of morphology on the identification and cannulation of the VOM was investigated. Results: The VOM was identified in 159 (98.1 %) and cannulated in 150 (92.6 %) patients. The VOM identification rate in the RAO and LAO/LAO cranial view was 97.3 % and 89.3 %, respectively. Of 134 patients with VOM identification in the LAO/LAO cranial view, 104 (77.6 %) had a VOM ostium clock location (VOMoClock) of ≤3 and 3–4 o'clock. The VOM cannulation success rate in the ≤3, 3–4, 4–5, and 5–6 o'clock groups was 100 %, 92.6 %, 88.5 %, and 77.8 %, respectively (p = 0.032). A review of recent publications reveals an increased interest in the so-called self-assembled carriers and their applicability in drug delivery via various routes of administration. Self-assembly denotes the process of rather spontaneous formation of ordered aggregates (sometimes under specific conditions-e.g., pH, temperature, ionic strength), via diverse interactions. This process, seen in many naturally occurring substances (polysaccharides, proteins, lipids), has inspired researchers to synthetize innovative self-assembling materials or combinations of existing ones. This paper provides a review of the recently investigated self-assembling materials and the carriers they form, often belonging to the sphere of pharmaceutical nanotechnology. Self-assembled carriers may provide enhanced stability, more efficient encapsulation and/or controlled delivery of active pharmaceutical ingredients. However, the diversity of geometries obtained (spheres, polyhedrals, ellipses, discs, porous structures, etc.) presents a significant characterization challenge, often requiring the application of several complementary techniques for proper evaluation of carrier size and morphology. Commonly utilized characterization techniques for investigating physico-chemical and certain biopharmaceutical properties are discussed, along with their advantages and disadvantages. Finally, the authors offer their critical opinion on the outlook of self-assembled drug carriers.

Identifying the precise topography of cancer for targeted biopsy in colonoscopic examination is a challenge in current diagnostic practice. For the first time we demonstrate the use of compression optical coherence elastography (C-OCE) technology as a new functional OCT modality for differentiating between cancerous and non-cancerous tissues in colon and detecting their morphological features on the basis of measurement of tissue elastic properties. The method uses pre-determined stiffness values (Young's modulus) to distinguish between different morphological structures of normal (mucosa and submucosa), benign tumor (adenoma) and malignant tumor tissue (including cancer cells, gland-like structures, cribriform gland-like structures, stromal fibers, extracellular mucin). After analyzing in excess of fifty tissue samples, a threshold stiffness value of 520 kPa was suggested above which areas of colorectal cancer were detected invariably. A high Pearson correlation (r =0.98; p <0.05), and a negligible bias (0.22) by good agreement of the segmentation results of C-OCE and histological (reference standard) images was demonstrated, indicating the efficiency of C-OCE to identify the precise localization of colorectal cancer and the possibility to perform targeted biopsy. Furthermore, we demonstrated the ability of C-OCE to differentiate morphological subtypes of colorectal cancer - low-grade and high-grade colorectal adenocarcinomas, mucinous adenocarcinoma, and cribriform patterns. The obtained ex vivo results highlight prospects of C-OCE for highlevel colon malignancy detection. The future endoscopic use of C-OCE will allow targeted biopsy sampling and simultaneous rapid analysis of the heterogeneous morphology of colon tumors. [ABSTRACT FROM AUTHOR]
