Abstract

BackgroundMethodResultsDiscussionPeople experiencing psychosis in acute crisis should be offered Cognitive Behavioural Therapy for psychosis (CBTp) however there are no protocols developed to inform delivery specifically for people in acute crisis receiving inpatient mental health care. This paper narratively describes a CBTp-informed crisis-focused therapy protocol to inform the delivery of therapy in inpatient settings.This study draws on relevant systematic reviews, qualitative interview studies with stakeholders, Delphi studies, and coproduction to develop the protocol. It draws upon crisis theories and CBTp theories to underpin it.This paper outlines a modularised approach to working with people with psychosis and in crisis. It outlines the key values underpinning the protocol, and the key modules of: engagement, assessment and identifying priorities; formulation of the crisis, stabilisation, safety, and problem solving, crisis plans and crisis cards, change strategy work focusing on crisis appraisals, and discharge and relapse planning.A crisis-focused cognitive behavioural therapy protocol is presented which can be used to inform therapy for people experiencing psychosis and in crisis. Abstract

Objectives: Hearing voices is associated with public stigma and this can influence readiness to identify as a voice hearer (VH) and psychological wellbeing. In this study, we investigated the relationships between a VH social identity, the integration of that identity with other important social identities and wellbeing. Design: Cross‐sectional study, with a subset of longitudinal data across three time points. Methods: People who self‐identified as voice hearers completed questionnaires (VH social identity, identity integration, wellbeing and perceptions of in‐group and out‐group empathy) at three time points, spaced at 3‐monthly intervals. The final sample comprised 182 participants at T1, 91 at T2 and 75 at T3. Hierarchical linear multiple regression analyses were used to test all hypotheses. Results: The integration of a VH social identity was strongly associated with better psychological wellbeing at T1. Identity integration was also associated with static wellbeing scores at 6 months. Effects on wellbeing were not accounted for by either severity of voice‐hearing or paranoia. Whilst perceptions of in‐group empathy were associated with VH social identification, perceptions of outgroup empathy were important for identity integration. Conclusions: Integrating a VH social identity with other important identities into a coherent sense of self is important for wellbeing in voice hearers; perceived in‐group and outgroup empathy are important in this process. Abstract

As access to psychological therapy for voice-hearing continues to increase, it is important to understand what treatment outcomes may be particularly valued by those who coordinate, administer, and engage with such interventions. Self-report cross-sectional questionnaires were distributed amongst samples of transdiagnostic voice-hearers and multidisciplinary mental health staff to identify and contrast opinions on the importance of different treatment outcomes for therapeutic work with voices. Responses were received by 89 service-users and 176 staff members. Both groups showed many similarities in their view of desirable treatment goals, although service-users were more likely to prioritise a clinically focussed view of recovery (voice cessation), while staff emphasised a more psychosocial view (to live the life one wants to while hearing voices). Study limitations and implications for clinical practice are discussed, including the value of collaborative goal setting when supporting clients who experience distressing voices. 47. Extreme Appraisals of Internal States and Bipolar Symptoms: The Hypomanic Attitudes and Positive Predictions Inventory

Abstract

The Hypomanic Attitudes and Positive Predictions Inventory (HAPPI; W. Mansell, 2006) was developed to assess multiple, extreme, self-relevant appraisals of internal states. The present study aimed to validate the HAPPI in a clinical sample. Participants (N = 50) with a diagnosis of bipolar disorder (confirmed by a structured clinical interview) completed a series of questionnaires pertaining to clinical factors, reward motivation, cognitive style, symptoms, and functioning at baseline, in addition to the HAPPI. Over the following 4 weeks, participants were asked to complete self-report measures of symptoms (activation, perceived conflict, depression, and well-being), as well as work and social functioning, twice weekly. The authors hypothesized that the HAPPI would be associated with prospective bipolar symptoms and functioning, when controlling for baseline symptoms and potentially confounding measures. The HAPPI was positively, independently associated with activation and conflict after 4 weeks. Furthermore, individual HAPPI factors were associated with activation, conflict, and depression. The results provided preliminary support for the predictive validity of the HAPPI in a clinical sample. The HAPPI could be used in the future as a tool in cognitive behavioral therapy for bipolar disorder to identify problematic beliefs and guide formulation.
